I assume the armor on his back can provide a certain angle for him to grab the sword with right hand perhaps?

NEED HELP!
Recently I'm playing Baldur's Gate 3, and romancing Astarion as the dark urge. and during act 2 I encountered a line regarding Astarion, saying"He is so afraid, so so afraid. Of everyone, besides you, who he ought to fear most."
I'm not a native English speaker, so I have this language problem...is Astarion afraid of the dark urge or not? In Chinese translation, it says"He is afraid of everyone, and among them he fears you most", because it used "besides" instead of "beside", and "besides" means "other than everyone, you too".
But I feel there's something wrong with the translation……I don't think it's consistent with the following lines "you should have done much better my lord". It indicated the dark urge failed to make Astarion fear him, right?
So please tell me how should I understand this line, thank you sooooo much
