Terra passed the Braveheart Keyblade to Riku, not the Kingdom Key
The Timeline
[see: some proof the timeline isn't what you initially assume is Ventus receiving his new Keyblade for the FIRST time at Destiny Islands and wielding it at the Badlands]
In BBS, each individual is only hours behind or before the other. Sometimes only minutes, some of them even cross paths. At Destiny Islands, people have argued it's days. I argue hours because days all of a sudden simply doesn't make sense unless it's Ventus who would be there the next day...which would STILL be only hours later.
Take a look at the timing of each encounter.
With Terra's entrance, they are already about to leave to go home.
But with Aqua's entrance, they are still playing and having fun.
And thus, this order is my understanding of the timeline.
The Narrative
Now, let's see if the writing backs me up here.
What did Terra say in the passing of power again?
Yeah, that seems about right. It fits quite well.
Riku has only truly reached this point at the end of KH3D.
And it carries over quite beautifully in KH3.
So when Way to the Dawn breaks because he's outgrown it, he's able to summon a brand new Keyblade that Terra left for him.
He has become the person Terra hoped he would.
Terra was Riku's second chance.
As to why Terra didn't notice Riku already had a Keyblade in BBS, it could be unintentional negligence or simply fate's will.
The Kingdom Key
Okay but...who gave Riku the Kingdom Key? It's a good question because I also have no idea! Was it even a "person"?
I think what we need to find out first is where the Kingdom Key originated in the first place. I'm hoping KHML might fill in these blanks a little but that doesn't mean it'll happen.
And I hesitate to believe it was the Master of Masters that passed the Kingdom Key because...I feel like Way to the Dawn is more his style considering how long he watched over Riku.
That's only my assumption, however.
Xigbar and others sure made it sound like it was a true honor to be chosen by the Kingdom Key. Maybe it's something you just get chosen for? Not by a person but by fate itself and Aqua simply assumed it was a person who passed it to Riku.

As cheesy as that sounds, Kingdom Hearts can be quite the cheesy series and I love it all the same.
