What I like about Horikoshi is that he makes plot twists that seem obvious, but then he turns them on their head.
Remember the theory about Dabi, everyone knew he was Touya, but no one knew what kind of problems he really had with his father. While everyone thought that he was forcing Touya to train like Shoto, in reality it turned out that Endeavor, on the contrary, forbade it, and Touya did not listen to him.
Horikoshi is doing it again now. Everyone knew that AFO was responsible for Tenko's Decay and that he was keeping an eye on the Shimura family, but no one assumed that he was a close friend of this family.
AFO pretended to work for a construction company and then befriended Kotaro to keep an eye on how Kotaro's children were doing.
It's a shame that the flashback with Kotaro ends so abruptly, because I think we would have found out that it was either the AFO who offered to give Tenko a quirk (or pretended to know someone who could do it), or Kotaro himself who asked him about it.

Controversial take, but the crush plotline being introduced at this time in this way makes sense and helps move the plot forward. (The problems with this plotline are more that it overstays its welcome and takes up too much of Ochako's screen time)
It's logical that Aoyama would pick up on Ochako's crush because he was keeping an eye on Izuku. He doesn't know how Izuku is involved in the All for One/One for All conflict, but because one wrong move could get himself and his family killed, he has to pay attention to Izuku.
And it makes sense Aoyama would bring up the crush here; he is trying to sabotage Ochako's exam and he needs a distraction. Pointing out her high school crush out of the blue is an effective way to accomplish that.
Beyond that, this crush plotline needed to be in play before Toga - Ochako's narrative foil - was introduced.
Anyway, regardless of your thoughts of Izuocha, for a Shounen Jump romantic subplot, it's pretty intertwined with the main storyline.

thinkin’ about my predictions for the manga
i think the tenko vs izuku fight is gonna be put on pause soon.
there’s been two stages of it so far:
1) tomura shigaraki vs. hero deku
2) tenko shimura vs. (is it right to even say “versus”?) izuku midoriya
and i think there’s going to be a third:
3) izuku midoriya vs. himself
by that, i mean i think izuku is going to learn something new about himself via his interactions with tenko in the next few chapters. we’re going to get left on a cliffhanger and then directed towards the aizawa/mic/shirakumo conflict.
my hope is that the third phase of the final battle will reveal the missing piece of izuku’s origin—whether that be the identity of izuku’s father, and/or the explanation for garaki’s involvement in izuku’s “quirkless” diagnosis. i think the cliffhanger we’ll get will be related to that.
what i can say with confidence is that i don’t think katsuki can show up in any way, shape, or form in this last conflict without totally derailing it and losing the war for the heroes and killing izuku. it’s not katsuki’s moment rn. he’s done his thing. and by god i hope he sits his ass down and rests for a minute.
if we know one thing about our new and improved katsuki, it’s that he is izuku’s greatest weakness. katsuki has always held this special, undefined place in izuku’s heart, but there is promise for a future in which he and izuku are equals, and i know izuku holds katsuki closer now more than ever.
if katsuki shows up, it’s game over. even if he just appears in a shared memory. especially if he appears in a shared memory.
shigaraki has no memory of katsuki that won’t immediately set izuku off. he kidnapped katsuki. he killed katsuki. izuku cannot see him like that again. he can’t afford to lose control of his heart again. it will kill him.
even izuku’s own memories of katsuki just don’t belong right now. this is izuku’s moment to finally connect with baby tenko and understand the pain that birthed tomura shigaraki.
bkdk will have their moment to talk—as all might stated like 130+ chapters ago—and that moment will be even more intense if izuku loses OFA.
but i really don’t think we’ll see katsuki until the war’s end.

The removing of the mask symbolism is back - Aoyama's mask is sucked up by Thirteen immediately before he drops his façade and starts acting more underhanded here.
"I am what I am." Aoyama knew he would be forced to give away the location of the training camp and that those who fail would not be permitted to go. This is Aoyama accepting that he is the kind of person who would sabotage Ochako's exam in order to prevent something worse because he recognizes that he isn't brave enough to try to come forward.
