it's the way ice pick joe got to be feral and REAL from the start, in a way most of the other characters had to fight for through the entire film. never hiding who he was or what he wanted. you saw ice pick joe and you knew what you were getting. fucking ice picked. it's the way he was punished for it as a cautionary tale to the viewer but also a lesson to the other characters. One we know they refused to learn from. Especially katya. Katya saw the cost of what she wanted in what happened to ice pick joe, and she chose to go for it anyway.
