As many doctors were referring to CT scans to determine covid positivity instead of RT PCR test, the government has ordered that CT scans shouldn't be done on covid suspects before subjecting them to RT PCR, and only if the patient tests positive can a CT scan be referred.

Health Minister Dinesh Gundu Rao said, "The covid sub-committee has taken this decision to stop fleecing patients as many doctors were asking for CT scans for every such case."

Reacting to this, city doctors said that the Reverse Transcriptase Polymerase Chain Reaction (RT-PCR) was often considered the 'gold standard' test for the detection of covid cases. However, during the first and second waves, there were a lot of false negatives with RT-PCR, and hence experts were depending on the CT thorax scan as it gave them a clear picture of the viral infection.

A city-based doctor said, "Whenever people visit me with fever, I recommend that they take a RT- PCR test. However, many are refusing to take the test due to the stigma attached to it.

Hence, I am asking them immediately to go for a scan, which can give the correct picture. There are ample examples where the patient might have been tested negative..."

Another medical expert said that in the case of a rapid antigen test (RAT), people who are having fever, cough, or any other covid-19 symptoms will test positive in the RAT only after a period of five days to one week. The sensitivity of this test ranges from 50% to 60%. There are possibilities of missing positive cases, which are also known as 'false negative' results. Therefore, if someone has gone for a test earlier, they may test negative.

The second test that is currently available and is considered the 'Gold Standard' for covid-19 testing is RT-PCR, which has a sensitivity of 60% to 70%. This test depends on the prevalence of covid infection in a person.

If the prevalence is high, then the person will definitely test positive for covid, and chances are that their RT-PCR will show a sensitivity of 80%.

However, there are chances that the remaining 20% of patients may walk out freely with a "negative" report in hand, as the prevalence of their virus may not be much in their bodies.

Hence, the CT thorax scan was recommended, which can provide a test report with a diagnostic value of more than 90%-95% accuracy. Even though it does not provide a 100% positive report, it can help clinicians understand the pattern of the diseases based on the scan report. It is true that symptomatic patients may show a negative RT-PCR result due to irregular and intermittent shedding of viruses or low or nil viral loads in the nasal swab area.

Dr. Mansukh Mandaviya, Union Minister of Health and Family Welfare today chaired a high-level meeting to review the COVID-19 situation in India and preparedness of public health system for surveillance, containment and management of COVID-19 in view of the recent spike in cases of COVID-19 in some states. He was joined by Prof. S P Singh Baghel and Dr Bharati Pravin Pawar, Union Ministers of State for Health and Family Welfare. Dr V K Paul, Member (Health), NITI Aayog was also present.

State Ministers who joined the meeting include Shri Alo Libang, Chief Minister and Minister of Health (Aruachal Pradesh); Shri Brajesh Pathak, Deputy Chief Minister and Health Minister (Uttar Pradesh); Shri Dhan Singh Rawat, Health Minister (Uttarakhand) who was present physically; Shri Dinesh Gundu Rao, Health Minister (Karnataka); Shri Anil Vij, Health Minister (Haryana); Smt. Veena George, Health Minister (Kerala), Shri Vishwajit Pratapsingh Rane, Health Minister (Goa); Shri Keshab Mahanta, Health Minister (Assam), Shri Banna Gupta, Health Minister (Jharkhand); Dr Balbir Singh, Health Minister (Punjab); Shri Saurabh Bharadwaj, Health Minister (Delhi); Dr. (Col.) Dhani Ram Shandil, Health Minister (Himachal Pradesh); Prof. Dr. Tanajirao Sawant, Health Minister (Maharashtra); Shri Damodar Rajanarasimha, Health Minister (Telangana); Dr Sapam Ranjan, Health Minister (Manipur); Shri Niranjan Pujari, Health Minister (Odisha); Shri Rangaswamy, Administrator (Puducherry); among others.

Underlining the challenge posed by the increasing number of COVID-19 cases in some countries across the world such as China, Brazil, Germany and United States, Union Health Minister noted the importance of being prepared and remaining alert against new and emerging strains of COVID-19, especially in view of the upcoming festive season. Underlying and reiterating that COVID is not over yet, he requested states to monitor emerging evidences of COVID-19 cases, symptoms and case severity to plan appropriate public health response.

Dr Mandaviya stressed the need for collective efforts between the Centre and the States to deal with the emerging situation in the spirit of “Whole of Government” approach. He directed for strengthening the surveillance system for whole genome sequencing of positive case samples to track the variants through Indian SARS-CoV-2 Genomics Consortium (INSACOG) network to ensure timely detection of newer variants, circulating in the country. This would facilitate undertaking of appropriate public health measures in a timely manner, he noted. States/UTs have been requested to ramp up testing and refer large number of samples of COVID-19 positive cases and pneumonia-like illness to INSACOG Genome Sequencing Laboratories (IGSLs) on a daily basis, for sequencing, to track new variants, if any.

The Union Health Minister urged all the states to remain alert, increase surveillance and ensure adequate stock of medicines, oxygen cylinders and concentrators, ventilators and vaccines, He asked officials to undertake mock drills every three months at both the Central and State levels to assess functionality of PSA plants, Oxygen concentrators and cylinders, ventilators, etc., and encouraged the sharing of best practices. He also urged states to create awareness on respiratory hygiene and ensure dissemination of factually correct information and counter fake news to manage infodemic and mitigate any panic. The Union Health Minister urged States/UTs to share information on cases, tests, positivity etc., in real time on the COVID portal to enable timely monitoring and prompt public health measures. He assured the states of all support from the Centre.

Through a presentation, Shri Sudhansh Pant, Secretary, Union Health Ministry briefed the Union Health Minister on the global COVID19 situation and the domestic scenario. It was informed that while active COVID cases in India are significantly less compared to the global scenario, in the last two weeks there has been a steep rise in active cases from 115 on 6th Dec 2023 to 614 on date. It was also noted that 92.8% of the cases are home isolated, indicating mild illness. No increase in hospitalization rates have been witnessed due to COVID-19, the cases that are hospitalized are due to other medical conditions – COVID-19 is an incidental finding. A rise in daily positivity rate has been observed in a few states like Kerala, Maharashtra, Jharkhand and Karnataka.

On the new JN.1 variant of SARS-CoV-2, it was informed that the variant is currently under intense scientific scrutiny, but not a cause of immediate concern. No clustering of cases has been observed in India due to JN.1 and all the cases were found to be mild and all of them have recovered without any complications.

Dr V K Paul reiterated the need for a whole-of-government approach to tackle with the challenge posed by the surge in COVID cases and the emergence of a new variant. He informed that the scientific community in India is closely investigating the new variant but stressed on the need for states to ramp up testing and strengthen their surveillance systems.

Dr Rajiv Bahl, Secretary, Department of Health Research and DG, ICMR informed that ICMR is currently working on genome sequencing of the new JN.1 variant. He urged states to monitor the COVID-19 scenario and increase RT-PCR tests but noted that there is no cause for panic.

State Health Ministers appreciated the support and guidance received from the Centre. They assured to increase testing and surveillance measures in view of the rising cases in some states.

Karnataka Health Minister Dinesh Gundu Rao calls Delhi Mohalla Clinics overhyped

Hours after praising Mohalla Clinics of Delhi, Karnataka Health Minister Dinesh Gundu Rao Friday backtracked from his statement and said they are overhyped.

Rao visited a Mohalla Clinic in Delhi during his day-long trip to the national capital. The Karnataka health minister was accompanied by his Delhi counterpart during his visit to the local clinic.

Rao said, “Visited a Mohalla Clinic in Delhi with hardly any people there. Our clinics in Karnataka have more facilities including a laboratory to do immediate tests for patients. I guess it is overhyped and I came back feeling disappointed.”

Jul 13, 2022 09:18 IST Porvorim (Goa) , July 13 (Always First): Amid the ongoing crisis within the Congress Goa unit, former Chief Minister Digambar Kamat has called for introspection within the party citing losses in elections and defections. Kamat, responding to the "anti-party" charge against him, said that he will face it stating that he has worked for the party with sincerity. The remarks of the Congress leader came after allegations were levelled against him of hatching a conspiracy to split the party. "I worked for the party with sincerity. Congress has to do a lot of introspection as we are losing so many polls and leaders are leaving us," he said while speaking to ANI on Tuesday. Kamat further said that he has opportunities in the past "if he wanted to take action", however, he stayed with the party, yet was rewarded with the disqualification petition. "I don't know what crisis is going on in the Congress party... I saw his (Dinesh Gundu Rao) video and was shocked, and surprised to see the way he was talking. If I wanted to take any action, I had a lot of opportunities in 2017 and 2022," he said. "I stayed with the party in 2017 when we had a clear mandate, still party didn't ask me whether I can form the government and we lost the opportunity... Now, this is the reward I get. They are filing a disqualification petition against me for anti-party activities. I don't understand what anti-parties activities? However, I will face it. The party should find out those who are doing this," the Congress leader added. Earlier on Monday, he made it clear that he is still in Congress. "If I wanted to go, I'd have gone. No one can stop me. But still, I led the party from the front, I went all out. And suddenly, they changed me from LoP when the results came in. I don't know why he made such a statement. I'm in Congress, you can see my performance," Kamat said. Notably, Congress on Sunday also sacked Michael Lobo as the Leader of Opposition in the Goa Assembly for alleged conspiracy to engineer defections. Congress said that the BJP was trying for a 2/3rd split and offered huge amounts of money to party legislators. Congress alleged that Lobo along with Digambar Kamat hatched a conspiracy to engineer defections in "total coordination" with the BJP. "This conspiracy was led by two of our own leaders, Leaders of Opposition Michael Lobo and Digambar Kamat. Both these people had been working in total coordination with BJP. One person- Digambar Kamat- did it to safeguard his own skin because so many cases are against him and the other person- Michael Lobo- for the sake of power and position.
