Epley maneuver - treats BPPV. You turn the pt's head towards the affected side and lay them down quickly; stay laying down in that position for at least a minute (this will reproduce the vertigo sumptoms; it's also the Dix-Hallpike maneuver, which is what you used to disgnose BPPV). Then turn the pt's head towards the unaffected side. Then roll pt onto the unaffected side so they're on their shoulder. Have pt tuck chin down and towards the unaffected side. Then sit up with chin still pointed down towards unaffected side. Slowly straighten head. Sleep on the side that is not affected.

I had a pt in clinic the other day who had BPPV. Her L ear was affected. I remember one of the attendings telling me once that the Epley maneuver (which fixes BPPV) is like the Dix-Hallpike maneuver (which diagnoses BPPV). I told this other attending that and he said I was wrong. But I'm not. At least the beginning of the Epley maneuver is the Dix-Hallpike maneuver. This is a great video that explains the Epley maneuver:

Anyway, Dix-Hallpike maneuver is when you turn your head to one side and quickly lay back. Nystagmus will be seen when the pt does this. The Epley maneuve starts the same way as the Dix-Hallpike maneuver: you turn your head to the affected side and then quickly lay back--then you stay in that position until the vertigo stops. After that, you turn your head to the unaffected side and hold this position for 1 minute or until the vertigo stops. Then you completely roll onto the unaffected side so you're on your shoulder of the unaffected side. When you do this, tuck your chin down and turn your head to the unaffected side. Then finally, you sit up with your head still pointed down and chin is tucked.

The BD exercise is a movement/habituation-based vestibular rehabilitation treatment and includes a sequence of rapid lateral head/trunk tilts repeated serially. Most clinics treating dizziness customarily perform diverse maneuvers for treating PC-BPPV-cu, such as the Epley maneuver, Brandt and Daroff (BD) exercise, vibratory stimulation, and head-shaking maneuver. Since effective treatment of PC-BPPV-cu has not been validated, a recent clinical guideline did not recommend specific treatment options based on the subtypes of PC-BPPV (canalolithiasis or cupulolithiasis) ( 5, 6). PC-BPPV-cu generates upward and ipsitorsional nystagmus, but the duration of symptoms and positional nystagmus are longer (over 1 min) than experienced with canalolithiasis of PC-BPPV (PC-BPPV-ca) ( 1– 4). Based on his theory, Barany's society formulated the diagnostic criteria of PC-BPPV-cu on 2015 ( 2).

He suggested that half Hallpike maneuver can provoke persistent up and ipsitorsional nystagmus because the cupula of PC may be oriented along earth-horizontal axis, and thus the weighted cupula has maximal propensity to be deflected earthward ( 1). Epley previously described nystagmus characteristics and his clinical experience of diagnostic posture ( 1). Clear classification of PC-BPPV should be required at the time of different pathology and different treatment response.Ĭupulolithiasis of benign paroxysmal positional vertigo involving the posterior semicircular canal (PC-BPPV-cu) is a rare form of BPPV. 36%, p = 0.436) and the decrease of mSPV and DHI.Ĭonclusion: Neither the Epley maneuver nor BD exercise has an immediate therapeutic effect in treating PC-BPPV-cu. The Epley maneuver and BD exercise had an equivalent effect at 1 week in treating PC-BPPV-cu in terms of resolving positional nystagmus (48 vs. Results: Immediate resolution occurred in none of 29 patients in the Epley maneuver group and only 1 of 33 patients in the BD exercise group. Secondary outcomes included the resolution of positional nystagmus at 1 week, the change of maximal slow phase velocity (mSPV) of positional nystagmus, and dizziness handicap inventory (DHI) immediately and at 1 week. The primary outcome was an immediate resolution of positional nystagmus within 1 h after a single treatment of each maneuver on the visit day. Patients were randomly assigned to undergo the Epley maneuver ( n = 29) or BD exercise ( n = 33).

We got a double chapter today for some reason, with 280 focusing on Tasha and Wisha, and 281 about Cale and Lock. Both were battle chapters, but the vivid descriptions made the chapters enjoyable.
Cale making his Fire AP thunderbolts fall down like a typhoon was awesome! 🤩 Purifier Cale is back!!! I think a fourth of chapter 281 was just about Cale and his Fire AP.
The other protagonist was Lock, with him pretending to fight Ryan, but his real goal was to snatch the divine item. Ryan thought that the divine item had been fully corrupted, but he was wrong because its essence, a small blue flame, was still present.
However, that blue flame suddenly flew into Lock's mouth, shocking Ryan, Cale, Lock, and Super Rock. But Super Rock, why are you asking if it was okay to eat a divine item? Haven't you made Cale eat your rock to gain your power? 😂
Ending Remarks
I was quite surprised by the quality of the chapters today, as the author spent time to vividly describe the scenes from the perspective of others and not just Cale. Next chapter is Lock's power up. Will the Blue Wolf god descend in him since he ate the essence? Or will he get a new power up?
