I would love to, thank you so much for asking!
It's 99% because of story reasons, 1% because I feel that deaths are fairly meaningless when they can be immediately undone.
In my homebrew world, Winyul, there are no gods but entities called Titans. They are the manifestations of wants, desires, hopes, memories, experiences, knowledge, and thoughts. Titans form around a community, be it a city, a mercenary band, even between lovers. The prime Titans are those who oversee the entirety of a region and all other Titans in that region are considered part of that prime Titan. [I have a bit more information on Titans in my little packets - link!]
The Titans are the arbiters of life and death on Winyul. There are no spells of resurrection that have ever worked, as no magic can find the threads of the person lost. Upon death, souls are weaved into their respective Titans, their pattern becoming a part of the Titan itself. To resurrect someone, their pattern must be ripped out of the Titan.
The Titan which holds the persons pattern, their soul, must be sought out and pleaded with, in the hopes that they might ravage their own weave. Resurrection has always been historically rare, invoked only in times of great need. Typically, those pleading on behalf of a resurrection will give up magical items to the Titan. That magical energy helps a Titan repair the damage being done by tearing out the soul. During the False Wisp crisis now, the Four of Four are the only ones the Titans have been resurrecting.
Resurrection also comes with its own risk. The more death one experiences, the tighter ones soul is stitched into the Titan. Multiple deaths and resurrections risks leaving behind threads that could not be undone. This means that, past a certain point, those being resurrected begin to forget things - as their memory, those threads, have been left behind in the Titan. It can work the other way as well, where some threads which don't belong come along during the resurrection. Die and be resurrected often enough, and eventually you're going to end up an entirely different person.
It's led to some amazing moments in campaign - from Adra, Ilsa, and Talee kneeling for hours in the snow over Loam's body begging for his resurrection, to the playful ribbing of having to haul Ilsa's corpse up a ladder which took eight hours to fully climb. The current death count in campaign is Ilsa leading with three deaths, Loam in second with two, Talee with one, and Adra still untouched by death (not for my lack of trying huhu).

Okay, I've been cooking up something, and it's not done yet but it's getting pretty close! I have been doing a sorta homebrew type situation putting minecraft into d&d terms. And I know that this has been done before, and by way more talented people than I but I'm pretty proud of what I, and a friend of mine have made so far.
We've got the classes done, except for you know buffs debunks all the stuff anyone needs to actually know, but let me explain our thought process with them a bit. There are three main classes they are: Explorer, Enchanter, and Builder. And don't worry there's more to it than that. The idea is you pick a main clas and for the first like 2 levels you get general proficiencys in different things. Like strength for builder, dex for explorer, and wisdom for enchanter. Stuff like that.
Than at level three you pick a subclass, and side note with how I'm doing this I don't think it'd be possible to multiclass. There are three subclasses for each main class. And to add a little twist to it, y'all know d&d alignments? When you pick a subclass your also picking 1/2 of your alignment. That's right, each subclass is designated as either chaotic, neutral, and lawful while you, the player pick the second part of good, neutral, or evil!
Explorer: Speedrunner (chaotic), Fighter (neutral), and PvPer (lawful)
Enchanter: Prankster (chaotic), Witch (neutral), and Redstoner (lawful)
Builder: Miner (chaotic), Farmer (neutral), and Gatherer (lawful)
Some of the builder alignments might be changed but as of right now this is how it's going to be. I'm pretty proud of the work and thought my friend and I put into this. I also have some stuff done for what potions, Armour, stats, and other things will look like but I need to go over them again. But to give you some kind of perspective of what the classes are like let's get some real world example based on vibes.
I'm going to use some minecrafters as examples. So Grian for example would be of the Prankster subclass, while Mumbo would be of the Redstoner subclass. GoodTimesWithScar as I'm thinking about him would fit well with the Prankster or Witch subclasses.
Tommyinnit would be a fighter subclass, possibly a pvper the same with Philza Minecrafter. I feel like Martyn InTheLittleWood would be more of a speed runner, but he could easily be a fighter subclass as well.
I'll admit I don't have many examples of any builder subclass minecrafters off the top of my head, but their out there!

Expanded Optional Class Features!
Over my time playing 5e there's a few small tweaks I've wanted to make to the core classes, below are just a few of the tweaks I personally use at my tables!
Below are optional class features for Barbarians, Clerics, Monks, Paladins, Rangers and Warlocks, with reasoning given for them all

Barbarian
Ancestral Rage
Rage stops barbarians casting spells, but that leaves races that get spells a little high and dry, not able to use their spells in most fights, this is intended to fix that. Nothing super crazy, but tiefling barbarians will have just a little more fun.
Unending Rage
The latest barbarian playtests have let barbarians use bonus actions to keep raging, and let them rage for 10 minutes. While I think this doesn't really work thematically, it made me realise Rage probably shouldn't be as limited either. Will this come up much, not really, but it's a little less tracking and it's more thematic.
Unshakeable Rage
Force damage is becoming a lot more common on higher CR monsters, so having resistance to it for barbarians is pretty important. It's a fix to the main problem I have with barbarians, like I spoke about in THIS tumblr post.
Cleric
Channel Divinity: Spirit Guardians
Once again this is a fix to my biggest problem with the class, I don't really think turn undead is thematic for all clerics, but I think the spirit guardians spell both is pretty thematic for all clerics (or can easily be reflavoured to be), and is such a must pick for the class, it should just be built in, so this does! Scales with your proficiency bonus so it'll stay useful at all levels, and frees up your third level slots for something else

Monk
Overflowing Ki
Monks need more ki points, and while this might look like it's giving them less at first, the recharge on this is a lot better, you might have less ki points overall, but you'll have all of them for all every combat
Perfect Flow
Similarly to overflowing ki, monks deserve a better capstone, so this is it, a constant flow of ki points, so you can always do what you need in a fight
Paladin
Swear Your Oath
Again a fix to my biggest problem with paladins. The source of sorcerers power is their subclass, so they get it at level 1, same with clerics and warlocks, so why not paladins? This feature, while not doing anything mechanical, helps the thematics of paladins immensely
Ranger
Mark Foe
Potentially the first part of a full ranger rework, rangers need a better feature than Favoured Enemy, and this is it. In an ideal world rangers would have a core class feature like this that subclasses could build off...... maybe....
Survivalist's Spellcasting
Another fix, rangers are the masters of the wilderness, they should be able to prepare for any situation.... but no, they're known casters, well no longer
Warlock
Eldritch Blast
The final fix I've spoken about before, it's such a must-have, that it just needs to be a class feature. Plus having it be a class feature means it doesn't scale with multiclassing which helps a lot (still not quite enough to stop hexblades, but I've got another fix there
Cunning Contract
Warlocks were int based in the D&DNext playtests, and they still absolutely make sense to be, so this just does that!

So, here's something a little bit different. I recently ended an eight-year TTRPG campaign. To celebrate, I picked up a before-after meme I did at roughly the half-point of the story (2018, I think) and added a third panel, marking the players' progression (thus the style discrepancy between the first two columns and the third one).
I will be uploading these here with short explanations about what happened to these characters. The first two are the demon Korwin and the knight Sev.
Korwin was quite sneaky, treacherous and distrustful of others at the beginning but eventually warmed up toward the party and became the responsible voice of reason. So responsible, in fact, that, after providing invaluable strategic help to one of the countries, was awarded a fief. Unfortunately, Korwin perished during the final battle, sacrificing herself to save her friends and the country she served.
Sev, on the contrary, started out as a cunning but friendly guy, who was unfortunately the victim of a chain of unfortunate events. As a result, he became bitter, angsty and extremely violent, preferring punches to negotiations. While he never had the chance to truly open up to someone again, his fiery angry eventually died out, transforming him into a cold-blooded and somewhat unemotional leader of a squad of ex-criminals whom he turned into an auxiliary army. After the end of the big conflict, he used his amassed wealth to open up a cabaret club in the capital.

The Nightstealer (purple one) was actually pretty nice and the party managed to do some bartering for a few items of questionable use!
The Nightswoopers though had the party sledding down a mountain to nope away from em.

Behold, the Meat-Wizard Mutophage v2.5
I am once again rolling out an update for the mutophage, class. While not as drastic an overhaul as the last time, this update includes a lot of changes to help balance out what was an overpowered mess in the last release. As well as some incredible new cover art by the wonderful @booogerbox, of thier own Character, Addie!
Long story short, I was going through some rough times when I released v2, and I let myself be pressured into adding or changing a lot of things in the class to make others happy, rather than sticking to what I wanted the class to be. It became a brick shithouse of barbarian, with all the added versatility of a wizard - and besides that being incredibly broken, it's never what the mutophage was meant to be.
V2.5 is no longer an inherently tanky class: yes, you still get a double d6 hit dice, but you aren't throwing out roided-up multiattacks every round, and you can't mutate yourself more resistances than a barbarian. The core idea behind my mutophage has always been to create a flexible alternative to the wizard, that didn't rely on magic. The Mutophage can be a brick wall of shear muscle and DPS, but it is not inherently so. It can also be built as a stealthy infiltrator, or a clever utility. But it cannot do all those things at once. It's not a one-man party anymore.
Some people will probably be grumpy that their god-build is no longer feasible. But those people are more than welcome to just stick to the older version. For the rest of us, this update provides a well-needed nerf and rebalance, to make the class playable alongside core content.

More Marks!
Hunter's Mark is a great spell for Rangers, but what if they had more!
Over on my Discord Server, peple voted for what Deity's Domain they wanted next for priest, and Hunt was the winner, giving me the opportunity to try out some design space I've wanted to for a while.
EDIT: These are all now available in JayPea's Spell Compendium on itch, they're also all under CC, so feel free to use them in your games or in your own homebrew!
Ethereal Mark
Hunter's Mark helps you track, so surely these other marks should also help somehow with finding people. First up is ethereal mark, letting you see creatures that turn invisible, or punch and grapple ghosts.

Guiding Mark
Next up is Guiding Mark, this isn't really useful on melee rangers, and damage-wise isn't too much better than ethereal mark, but for ranged rangers, you can now shoot around corners the arrow or bolt magically finding it's mark

Seer's Mark
Lastly we have Seer's Mark, the logical endpoint of hunter's mark, as long as you've seen your mark, for the next day, you can find them wherever they are, or at least know pretty much exactly where they are.
