Fractured Relationships
Break a bone and a frenzy of activity ensues to repair it. Bone-forming cells (osteoblasts) and their precursors (osteoprogenitors) take charge. However, fractures don’t always heal well. Consequently, growth factors are sometimes given to promote healing, specifically BMP which promotes bone formation and PDGF-BB which maintains osteoprogenitor numbers. Researchers investigate how they work in a mouse bone fracture model. MicroCT of fractured bones (pictured) revealed that adding high dose BMP, low dose BMP or PDGF-BB (top, left to right) promoted bone regeneration compared with no treatment (bottom left) or combining different BMP doses with PDGF-BB (bottom middle and right). Analysing the cell types revealed that BMP2 alone increased osteoblast and osteoprogenitor numbers but failed to do so when combined with PDGF-BB. While separately BMP2 and PDGF-BB promote bone healing, together they have the opposite effect. Further unpicking their interactions may help advance bone healing treatments.

Trứng cá là một bệnh da viêm với cơ chế bệnh sinh phức tạp. Bên cạnh bốn cơ chế chính được chấp thuận rộng rãi, nhiều yếu tố khác cũng có ảnh hưởng đến bệnh như yếu tố di truyền, hormone, stress, ô nhiễm môi trường, chế độ ăn và lối sống.
Mối liên quan giữa chế độ ăn và trứng cá là mối quan tâm của nhiều bệnh nhân, cũng là câu hỏi thường xuyên được đặt cho bác sĩ da liễu trong thực hành lâm sàng, tuy nhiên các quan điểm về vấn đề này trong y văn vẫn còn chưa thống nhất.

+ Cơ chế bệnh sinh của trứng cá gồm 4 yếu tố chính:
- Tăng sản xuất bã nhờn
- Tăng sinh vi khuẩn Cutibacterium acnes (C. acnes, trước đây được gọi là Propionibacterium acnes)
- Tăng sừng hóa cổ nang lông
- Phản ứng viêm ở khu vực nang lông- tuyến bã.
+ Sản xuất bã nhờn quá mức xảy ra do sự gia tăng hoạt động của hormone androgen và yếu tố tăng trưởng giống insulin-1(IGF-1). IGF-1 đã được chứng minh là làm giảm nồng độ trong nhân của yếu tố phiên mã FoxO1, dẫn đến kích hoạt con đường tín hiệu của phức hợp rapamycin 1 ở động vật có vú (mTORC1). Phức hợp này có liên quan đến chuyển hóa và tăng sinh tế bào. Trong mụn trứng cá, mTORC1 kích thích tăng sinh tuyến bã, tổng hợp lipid và tăng sản tế bào sừng. Leucine, một axit amin phổ biến trong thịt và protein từ sữa, cũng kích hoạt mTORC1. Ngoài ra, IGF-1 làm tăng nồng độ androgen, androgen lại có tác dụng làm tăng sản xuất IGF-1 nội sinh, tạo thành một vòng điều hòa ngược dương tính càng làm tăng sản xuất bã nhờn. Tăng insulin máu làm tăng mức lưu hành của IGF-1 và protein gắn với IGF, tác động trực tiếp đến tăng sản tế bào sừng và quá trình chết tế bào theo chương trình. Hormone tăng trưởng và chất trung gian gây viêm cũng có liên quan đến sự phát triển của mụn trứng cá.
- Nhiều nghiên cứu đã chứng minh rằng các sản phẩm sữa ảnh hưởng đến mụn trứng cá thông qua tăng nồng độ yếu tố tăng trưởng giống insulin-1 (IGF-1) trong huyết thanh. IGF-1 kích thích tổng hợp nội tiết tố androgen từ buồng trứng ở nữ và tinh hoàn ở nam, đồng thời ức chế gan tổng hợp globulin liên kết hormone giới tính, dẫn đến tăng nồng độ của androgen tự do.
- Ngoài ra, sữa bò công nghiệp đã được báo cáo là có nhiều yếu tố tăng trưởng khác, như tiền chất của testosterone, chất này tăng sinh nhân mụn thông qua kích thích sản xuất bã nhờn và tăng sừng hóa của đơn vị nang lông-tuyến bã.
- Trên thị trường, sữa bò có các mức chất béo khác nhau: sữa nguyên chất, chứa khoảng 3,5% chất béo, sữa ít béo chứa khoảng 2% chất béo và sữa tách béo. Trong đó, sữa tách béo có nguy cơ gây mụn cao nhất, do quá trình chế biến sữa tách béo làm tăng nồng độ của các chất có hoạt tính sinh nhân mụn, đồng thời loại bỏ bớt estrogen, một thành phần được chứng minh là làm giảm mụn trứng cá. Tuy nhiên, chưa có đủ bằng chứng về mối liên hệ giữa hàm lượng chất béo trong sữa và trứng cá.
- Đạm whey là protein huyết thanh sữa được sử dụng như chất bổ sung đường uống để tăng khối lượng cơ. Một số bài báo báo cáo mối liên quan tiềm ẩn của những protein này và mụn trứng cá. Meninjau Teknik Kutukan “Blood Manipulation” Choso Menurut Sudut Pandang Biologi: Part 1
Choso merupakan seorang karakter antagonis pendukung di seri manga Jujutsu Kaisen oleh Gege Akutami yang kini menjadi salah satu manga yang paling digandrungi oleh para penikmat karya anime. Dalam dunia persihiran Jujutsu Kaisen, ia memiliki kemampuan fisik dan teknik kutukan yang hebat dan bahkan dapat membuat karakter utama dalam seri ini, Yuji, kewalahan. Teknik kutukan sendiri merujuk pada berbagai macam kemampuan magis unik dalam karakteristik serta aplikasinya yang dimiliki oleh karakter-karakter dalam seri ini. Teknik kutukan milik Choso yang bernama “blood manipulation” memungkinkannya untuk memanipulasi arah gerak, kepadatan, dan koagulasi darahnya sendiri di dalam maupun di luar tubuhnya. Dengan mengulas penggunaan teknik kutukan tersebut serta strategi karakter utama untuk mengunggulinya, karakteristik darah beserta komponen penyusunnya secara umum dapat dipahami. Tidak seluruh kemampuan teknik kutukan Choso akan dibahas pada artikel ini, tapi kini kita akan fokus membahas perihal skill manipulasi koagulasi darah miliknya.
Seperti yang telah disinggung sedikit sebelumnya, teknik kekuatan Choso dapat digunakan untuk mengontrol koagulasi darahnya. Untuk menghadapi musuhnya dalam jarak yang jauh, Choso mengompres cairan darahnya di dalam tangannya lalu menembakkannya bagaikan semburan selang air yang terkonsentrasi ke satu titik (Gambar 1). Untuk menambah keefektifan serangan tersebut, ia harus memperlambat koagulasi darah yang ia tembakkan. Dalam serial anime ini, tidak dijelaskan secara detail mengenai teknis kerja dari bagaimana ia melakukannya, tapi kita dapat mengajukan suatu hipotesis dari sudut pandang biologi.

Pada saat tubuh terluka dan darah mengalir keluar, baik itu di luar kulit maupun di pembuluh darah dalam kulit, tubuh akan melakukan mekanisme koagulasi atau pembekuan darah (blood clotting) untuk mengehentikan kehilangan darah dari pembuluh darah. Darah sendiri terdiri atas beberapa komponen, yakni plasma yang menempati 55% dari total volume darah dan mengandung protein plasma, sel darah merah, sel darah putih, serta platelet/keping darah. Proses pembekuan darah berkaitan erat dengan kehadiran keping darah, suatu sel yang berbentuk seperti cakram terfragmentasi. Fungsi platelet dalam tubuh ialah melepaskan senyawa kimia untuk menginisiasi proses clotting, membentuk tambalan sementara pada dinding pembuluh darah yang rusak, dan mereduksi area kerusakan pada pembuluh darah seiring berjalannya waktu.
Pada tubuh, pembekuan darah termasuk ke dalam rangkaian proses hemostasis. Hemostasis atau pemberhentian pendarahan sendiri memiliki tiga tahap sekuensial, yaitu fase vaskular, fase platelet, dan fase koagulasi. Fase vaskular dimulai ketika pembuluh darah mengalami kejang dan kontraksi setelah terjadinya pendarahan. Penyempitan pembuluh darah yang diakibatkannya akan memperlambat atau bahkan memberhentikan pendarahan pada dinding pembuluh darah. Pada fase ini pula, sel endotel akan melepaskan faktor kimiawi pembuluh dan hormon lokal, salah satunya adalah hormon peptida endotelin. Hormon lokal tersebut dapat menstimulasi kontraksi otot polos serta pembelahan sel endotel, sel otot polos, dan fibroblas untuk mempercepat proses perbaikan. Selanjutnya, membran plasma sel endotel tersebut akan menjadi “lengket” bagi platelet untuk menempel di sana sehingga hemostasis dapat berlanjut ke fase platelet.
Pada permulaan fase platelet, platelet yang menempel akan melepaskan senyawa adenosin difosfat yang dapat menstimulasi agregasi dan sekresi platelet, tromboksan A2 dan serotonin yang menstimulasi kejang vaskular, faktor clotting, platelet-derived growth factor (PDGF) yang berperan dalam perbaikan pembuluh, serta ion kalsium yang dibutuhkan untuk agregasi platelet dan beberapa tahap dalam proses clotting. Platelet yang terus menempel dan menumpuk akan menjadi platelet plug yang dapat menutup pendarahan secara langsung apabila luka tidak parah. Akumulasi platelet selanjutnya menandai fase koagulasi, yakni pembekuan darah, hingga luka dan lendarahan dapat tertutup secara menyeluruh.
Dalam dunia Jujutsu Kaisen, hal-hal magis seperti sihir yang di luar nalar sudah menjadi hal wajar. Seseorang dapat mengasumsikan bahwa kemampuan Choso untuk menghambat koagulasi darah merupakan hasil dari aktivitas sihir yang membuat segala hal menjadi mungkin terjadi. Namun, kita juga dapat mencoba menjelaskan proses tersebut dengan pendekatan saintifik. Penghambat alami yang dapat mencegah koagulasi darah adalah antitrombin III, protein S, dan protein C. Setelah diaktifkan, ketiga protein tersebut akan menonaktifkan faktor clotting tertentu sehingga menyediakan mekanisme regulasi yang dapat mengontrol respon koagulasi dan membatasi ekstensi penggumpalan darah. Dengan demikian, dari sudut pandang biologi, dapat dikatakan bahwa salah satu kemampuan Choso adalah melakukan overexpression terhadap ketiga protein inhibitor agar molekul tersebut dapat diproduksi secara masif.

Silymarin – Chiết xuất Cây Kế Sữa | Cao khô Kế Sữa (Milk Thistle extract) là hoạt chất chính được chiết xuất từ cây kế sữa, hay còn được gọi là cây cúc gai đã được chứng minh lâm sàn là hoạt chất có tác dụng cho gan tôm tốt nhất trong số tất cả những hoạt chất có chức năng tương tự.
1. Silymarin – Chiết xuất cây kế sữa | Cao khô kế sữa (cây cúc gai) là gì?
Silymarin được chiết xuất từ Cây Kế sữa (Milk Thistle) (dân gian còn gọi là cây cúc gai) hay còn được biết đến với tên khoa học là Silybum Marianum (L.) Gaertn). Cây kế sữa là một loại cây có nguồn gốc từ châu Âu và được thực dân đầu tiên đưa đến Bắc Mỹ. Cây kế sữa hiện được tìm thấy trên khắp miền đông Hoa Kỳ, California, Nam Mỹ, Châu Phi, Úc và Châu Á.
Cây kế sữa được đặt tên dựa trên nhựa c��a cây như sữa chảy ra từ lá khi chúng bị bẻ gãy. Tất cả các bộ phận lộ trên mặt đất và hạt giống đều được sử dụng để làm thuốc. Silymarin là thành phần hoạt chất chính trong cây kế sữa, đây là chất vừa chống viêm, chống oxy hóa và có tác dụng hạ đường huyết. Hạt giống cây kế sữa có thể bảo vệ các tế bào gan khỏi các hóa chất và thuốc độc hại.
Hợp chất này đã được chứng minh lâm sàng và được công nhận là có hiệu quả vượt trội dành riêng cho gan, là hoạt chất có tác dụng cho gan tốt nhất trong số tất cả những hoạt chất có chức năng tương tự.
Silymarin nguyên liệu có dạng bột có màu nâu và được sử dụng như là một chất bổ sung vào trong rất nhiều các loại thực phẩm, đồ ăn và thức uống khác nhau. Các nhà nghiên cứu khuyến cáo nên sử dụng hợp chất này với liều lượng khoảng 80mg/ngày.
2. Phân biệt Silymarin thường và Silymarin Phytosome®
Silymarin là một Flavonoid (chất chuyển hóa trung gian của thực vật), là một nhóm các thành phần được chiết xuất từ cây kế sữa (cây cúc gai). Trong số các thành phần đó, Silybin là hoạt chất chính, đã được chứng minh lâm sàn là có tác dụng vượt trội dành cho gan, và là hoạt chất có tác dụng cho gan tốt nhất trong số tất cả những hoạt chất có chức năng tương tự. Hiện nay, Silymarin ngoài thị trường sẽ có hai loại chính:
