Okay that would take forever so instead you're getting a general overview of my warrior cats fan universe :3
So the main clans are PaleClan, DustClan, CloudClan, and OasisClan. I have tribe equivalents too but I'm keeping this simple.
Timeline - Start of Arc/Series 2
PaleClan are the Quiet and Thoughtful. Their first leader was Palestar, or Air Of Pale Skies, or Palesky. Their current leader is Ravenstar, formerly Ravenpounce. PaleClan live in the desert, and their neighbouring Clans are DustClan and OasisClan. They do not border the Twolegplace. They have an average territory size, and the most cats. Their camp lies in a dried-up lake.
CloudClan are the Prideful and Sneaky. Their first leader was Cloudstar, or Sunlit Cloud, or Cloudlight. Their current leader is Whitestar, formerly Whitedrop. CloudClan lives in two parts, cats with thicker fur in the mountains, and thinner in the desert below, and their neighbouring Clans are DustClan and OasisClan. They also border the Twolegplace. They have the largest territory, and the second most cats. Their camp sits in the centre of a ring of mountains. Additionally, they host the Mooncave, the StarClan communication thing in this universe.
DustClan are the Intelligent and Caring. Their first leader was Duststar, or Dust Storms, or Duststorm. Their current leader is Wrenstar, formerly Wrenclaw. DustClan lives in the dusty deserts, and their neighbouring Clans are PaleClan and CloudClan. They do not border the Twolegplace. They have the smallest territory, and the second least cats. Their camp lies in the heart of the dust storms that tend to swirl around their territory.
OasisClan are the Calm and Quick. Their first leader was Oasisstar, or Oasis Of Life And Death, or Oasislife. Their current leader is Sandstar, formerly Sandtwist. Oasisclan lives around the oasis within the deserts, and their neighbouring Clans are CloudClan and PaleClan. They also border the Twolegplace. They have an average territory size, and the least cats. Their camp lies on an island in one of their lakes. Additionally, they hold a StarClan-blessed pool of water that always stays cool and clear, which they share with the other Clans to not die out.
The Gathering is held under the shade of the Starslates, a small ditch with rocks that jut out and shade any cat underneath it, lying in the heart of all four Clans, during Sunhigh on a full moon. If needed, it can be down at Moonrise instead, but due to the cold of the desert at night, the cats prefer Sunhigh.
I've accidentally gotten too hyperfocused and I will answer more questions/make a sideblog for this maybe to explain the full universe (which is STILL unfinished actually! I have done the most work on Arc Two characters and I still have no idea who I want to be alive during Arc One! Haha, the CloudClan family tree hasn't even been organised for the living cats! I am STRUGGLING!)

wait okay so what’s the deal with cloudclan? why do they live separated like that/how did that start? are they separated at birth or after the warrior ceremony/apprentice ceremony… how does it all work?
When Cloudstar originally split off from the cats who arrived in the desert, she didn't expect to find much. All the cats who had followed her had done so in hopes of something more familiar to the grove. Despite the fires that made them leave, it was still a home. When she had run into the mountains, the towering stone reminded her of the cliff faces around the grove. So she had lead her cats up the mountain range, finding a glacier-melt stream to drink from, and all seemed well.
But the thinner-furred cats were struggling with the cold, the similar stone and prey when layered with the unfamiliar climate left them lethargic and unstable. Cloudstar had been trusted to lead these cats to a new home, she couldn't let her members suffer! Besides, new kits born into this place during whatever the desert's Cold Season was would likely freeze to death.
So, despite her own thick fur, she gathered her thin-furred cats to set out back into the desert to find somewhere warmer. Of course, she had learned a lesson from the journey to the mountains that she shouldn't try find somewhere in one trip. Besides, Cloudstar's cats were counting on her. She couldn't just leave them. So, over the course of two moons, she ran short scouting sessions around the desert, mapping out the place and finding somewhere all her cats would thrive.
When enough of these trips occurred, [unnamed early CloudClan cat] realised they had been living on one of a ring of mountains that could shelter a camp that would work for all cats. They quickly told Cloudstar, and she saw what they were going for. A camp placed in the centre of the mountains, where a stretch of desert still lay, but a cooler one with a convenient path that lead up to a higher point in the mountains, close to their glacier river and with some interesting looking unexplored caves.
So, Cloudstar went out with her closest friend (future deputy), [unnamed cat], and [future medicine cat] to check out the place more thoroughly. With half their cats in the desert cradle, and the other half set up higher in the mountains, it would be a perfect damp set up. The caves gave them something to do as they settled as well.
CloudClan were the first to find their modern-Clan home, with OasisClan next, DustClan after, and PaleClan being the last, for they moved camp ~half way through the Clan's existence.
That's the origins of the mountain split, as for how things work, it's not a strict split? I see it more like past WindClan, with tunnel diggers and moor runners or whatever they were called.
As mentioned, the mountains are cold, so kits are born in a half-way point between the desert and the mountains. Rather temperate. When they're apprenticed, it's always to someone with similar fur-thickness, so training and patrols and hunts can be done in the same place. So, separation is just about what's comfortable really, and it's not like you have to live all separate, you can still visit and stuff, and some stubborn cats live with their littermates or friends. It's not exactly strict but if a medicine cat deems it to be detrimental to their health they are pushed back to a better climate.
examples of stubborn cats include- Mintpaw and Bellpaw, who stay in the mountains with their mother Dewpetal, and Blueflip, who lives in the desert with his mother, Riverclaw (until he moves to the mountains with his mate Snowflight)
like the separation is mostly who sleeps where, hunting, and training, when you have no tasks you can literally just dip anywhere you want

Yesterday I was playing clangen and a cat named omlette joined the clan, and in the same time skip was found dead outside a fox den. She was pretty too and Paleclan is criplingly small with only 3 kits ever born to the clan at like 80 moons, and we keep losing cats to wolverines.

Ever wonder why the heck they're called Paleclan? A) Clans are named after their first leader, and B) They get a lot of snow. In fact I usually think of their territory as being snowy, so I don't know why I've drawn it un-snowy up to this point XD
