While firing on ex-USS Miantonomoh (BM-5) during a battle practice on September 21, 1916, a shell exploded in the port 12 inch gun barrel of turret no. 2 of USS Michigan (BB-27) and sent fragments through the ship.
Luckily only one crewman was injured.
