Countdown Begins for TB Mukt Bharat.
24th March is observed as World Tuberculosis Day in the memory of Dr. Robert Koch.
In 1883, Dr Robert Koch discovered the bacteria that causes TB. However, it took 60 years since then for the discovery of its’ cure and another 6 years to actually test it on a human patient. Since then, there have been many scientific breakthroughs to detect and cure this disease, however, it continues to haunt many developing nations as a major public health challenge. WHO has recognized TB as one of the top 10 causes of morbidity and mortality in 2019. Even though it is prevalent in Asia, Africa, Caribbean countries etc, India alone accounts for over one-third of deaths in the world. COVID-19 pandemic has only made it worse.
In 2018, Prime Minister Narendra Modi announced to make India free of Tuberculosis in 2025, which is 5 years before the SDG 2030 timelines. This was a huge commitment. To bring this to fruition, multiple stakeholders, especially the Ministry of Health, Ministry of Women and Child, ICMR, National Institute of Research in Tuberculosis (NIRT) and the States have engaged and planned to eliminate this communicable disease, but how close are we from the intended goal?
In the recent report released by NIRT, Chennai – ICMR on 25th March 2022, there are revelations and insights that policymakers working in this area must actively look into. The report indicates that a mammoth nationwide survey, targeting over 3.5 lakh people was conducted before and during the COVID pandemic. NIRT aimed to cover 625 clusters across all states of which only 448 could be covered. The BIMARU states along with Haryana, which have higher incidences than the national average, defaulted the most from the protocol with each targeting less than 60% of the intended clusters. But, to give credit where its’ due, the will to complete this survey showed the commitment of all stakeholders to eliminate TB from India.
The report contains a study on the ‘Health Seeking Behavior’ of over 3 Lakh symptomatic participants across India, which clearly shows that of the 14k people showing symptoms eligible for a sputum test, 65% did not consult for TB symptoms due to various reasons, the most alarming being “Ignored symptoms – 68%” and “Could not afford to seek care – 11%”. Even though Government of India under various programmes and schemes is provisioning for free-of-cost diagnosis and cure along with awareness campaigns in all mediums, the impact is clearly questionable.
India has done well, but time is ticking away. 2025 is not far and we are way behind in achieve the goal of eliminating TB from India. If we only talk about pulmonary TB cases, the policymakers must consider recommendations given by NIRT with respect to including X-Ray and Molecular testing for more effective detection of TB. Given the current inflation (both urban and rural) in India because of various global phenomena, Nikshay Poshan Abhiyaan scheme to give direct cash transfers to patients of Rs. 500 for 6 months each, might not meet its intended goal of improving nutrition levels. Also, research shows that the duration DOT treatment can be reduced from 6 months to 4 months in cases where the severity of infection is lower. Making detection, escalation and treatment more robust is imperative.
We need a targeted approach for eliminating TB. The targeting must be done to cater to the most infringed and untouched parts of the populations in tribal areas, as well as remote parts of India, who have a deviant lifestyle with respect to prescriptions for TB, owing to traditions, social interactions or education levels. Women and Children must be given special attention to this approach as per their nutrition needs. States which are performing ineffectively, must catalyze their efforts and align with the national mission and create real-time dashboards for transparency and public involvement. Every state must trickle down district based targets for each day.
Lastly, the most vital means is awareness. Effective messaging should be given utmost importance to hammer the gravity of the situation in the minds of all sections and sub-sections of India, especially the states where the curative health infrastructure is in tatters. Early detection for better response should be one of the mantras going forward. Simultaneous capacity building for health infrastructure and awareness programmes to nudge people to take the tests will catalyze detection and give a better picture for conceiving subsequent mitigation strategies.
If we wish to make India ‘TB Mukt’ as early as possible, we take the right steps in the right direction. This cannot be done by Government alone as seen in the reports as well, support from private sector and Civil Soceity Organizations are a must. With ongoing research for easier solutions and a potential one-time vaccination for TB elimination underway, it is not an impossible dream but a tedious one at that.

Before #consumerforumchennaisouth
Complainant-A Rajagopalan took car loan for an amount of Rs.7,09,288/- from #IDFCFirstBankAnnaSalaiTeynampetChennai. To realize the amount an #agreement was entered between the #complainant and the #OppositePartyFirstBank.
Accordingly, Petitioner signed Electronic Clearing System (#ECS)for the payment of loan for 60 months for an amount of Rs.17,730/- from his saving Bank account with Union Bank Of India West Mambalam Branch, Chennai & this amount was to be deducted on the second day of every month.
The deduction process began from January 2,2019 and regular clearing was made till March 2020.
Then came the Lockdown due to Covid 19 & a #moratorium on #loan for #bankandfinancialinstitutions was announced by Union and State Govnt. from1.3.20 to 31.8.20 & the IDFC Bank didn't presented for EMI with the Union Bank of India.
However, on 19.06.2020 the Opposite Party presented ECS for Rs.8,865/- two times in the same day which amounts to Rs.17,730/- and this amount was cleared by the Complainant's bank from his account.
Due to #deduction of #EMI, Complainant was #left with only Rs.2185/- in his saving bank account.
Complainant got worried how he will manage with Rs.2185/- during the lock down period.
He sent two #notices on 20 June & 28 July 2020 as to why without any intimation the amount was deducted when moratorium was going on.
Complainant alleges no reply was received from the OP Bank.
On #02OCTOBER2020 the ECS amount of Rs.17,730/- was #presented by the Opposite Party after the moratorium period was over & it was #cleared by the Complainant.
On #17OCTOBER2020 the Complainant was having a balance of Rs.18,126/-. to his #dismay on 17.10.2020 the Opposite Party withdrew one more EMI of Rs.17,730/- from his account as such a #balance amount after the withdrawal on 17.10.2020 was just Rs. 396/-. And the question before him was how to survive for the rest of the month of October 2020.
To all this averment of the Complainant the contention of the OP-IDFC First Bank was that the Complainant neither communicated any letters nor contacted the customer care services of the Opposite Parties bank with respect to the moratorium.
Hence, they deducted as scheduled.
It was observed by the District Consumer Dispute Redressal Commission South Chennai that OP-IDFC Bank should have taken one stand that since the Complainant had not opted for Moratorium and hence they had realized the EMIs or suo moto applied for the whole moratorium period.
But in this case once they suo moto applied for moratorium and next time they presented for deduction of EMI.
This amounts to deficiency in Service on the part of the OP - Bank as such Bank was found by the Commission liable to pay compensation of Rs.50,000/- for the #deficiencyinservice and #mentalagony & Rs.5,000/- towards the #litigationcost & accordingly directed on #19July2023.

Combating Water Scarcity With Rooftop Rainwater Harvesting With Water Organizations

The management of water and sanitation services, whose safe and dependable availability proved to be the first line of defense against the scourges of the novel coronavirus disease (COVID-19), is particularly under pressure as a result of India's rapid urbanization, which is putting pressure on its already deteriorating base of public service arrangements. It wouldn't be incorrect to say that many more cities will soon meet the requirements to join this notorious list if we had a look at the portfolio of water resources management with water organizations for other cities right now. Rooftop rainwater harvesting is a method for collecting rainwater from residential rooftop catchments and storing it in a specific reservoir. These reservoirs may be used to store collected rainwater for future home usage, while subterranean groundwater reservoirs can be used to store any excess water for extra purposes like irrigation. In general, rainwater is free from the majority of chemical and bacterial pollution, with the exception of contaminants gathered from the atmosphere. Because of this, it is a cheap alternative for consumption in households with little treatment.

The soil profile and days with the most rainfall should be taken into consideration when designing the gutter and recharge pit.
Depending on the quantity of contaminants, rainwater should be sent through a single, specially constructed filter.
Communities with the help of water organizations store some of the rainwater in masonry storage tanks once their consumption demands have been satisfied. To maintain the water safe for the upcoming water-scarce season, we periodically administer the necessary coagulants.

96.4% of Americans had COVID-19 antibodies in their blood by fall 2022

Antibodies to SARS-CoV-2, the virus that causes COVID-19, were present in the blood of 96.4% of Americans over the age of 16 by September 2022. That’s according to a serosurvey – an analysis testing for the presence of these immune defense molecules – conducted on samples from blood donors.
A serosurvey like this one helps researchers estimate how many people have been exposed to any part of the coronavirus, whether via vaccination or infection. Both can trigger the generation of antibodies to SARS-CoV-2. And by identifying which kind of antibodies someone has in their blood, researchers can break down the 96.4% into different types of immunity: infection-derived, vaccine-derived and hybrid.
COVID-19 vaccines used in the United States are based on only one part of the virus – the spike, or S, protein. Researchers can tell that a person has been vaccinated and has not been infected if their blood has only anti-S antibodies that target that spike protein. If someone has anti-N antibodies, which target the virus’s nucleocapsid protein, it’s a sign that they’ve been infected by SARS-CoV-2. To reliably identify someone with hybrid immunity, a researcher would need to match someone who has anti-N antibodies to an official vaccination database.
What about the 3.6% without antibodies?
Immunologists know that antibody levels decrease in the months after a COVID-19 infection or vaccination, and this is true for many pathogens. It’s possible some people did have antibodies at one point, but they’re no longer detectable. And not every infection leads to a detectable antibody response, particularly if the case was mild or asymptomatic.
Another factor is the accuracy of the antibody test. No test is perfect, so a small percentage of people who truly have antibodies might come up negative.
Together, these considerations mean that the 96.4% number is likely an underestimate. It seems reasonable to conclude that almost no one in this population has neither been infected by SARS-CoV-2 nor received a COVID-19 vaccine.
A clearer picture of a virus’s spread
Serosurveys are useful for understanding how likely different types of people – of varying ages or races, for example – were to have been infected. For this purpose, a serosurvey can be much more reliable than using data on people who received a positive PCR test, or who report having had a positive rapid antigen test, because getting a positive test is heavily influenced by access to care, health care behavior and how severe your illness is. These are sources of what is called bias.
This bias has two effects: It leads to large underestimation of the proportion of the overall population infected, and it can lead to spurious differences between groups. For example, people with mild symptoms are less likely to get tested and are also likely to be younger. Researchers might draw the wrong conclusion that because they’re not getting tested these people aren’t actually catching the virus.
Looking at antibodies as a marker of infection is not biased by such behavioral factors. Many serosurveys, including ones that we worked on in Chennai, India, and Salvador, Brazil, found similar or even higher seroprevalence in children compared with young adults, contradicting an early narrative that children were less susceptible to the virus. Instead, our results suggested that infections in children were less likely to be detected.
What does this statistic mean for future waves?
Antibodies are not just a marker of previous infection; part of their job is to help prevent future infection with the same pathogen. So, serosurveys can be used to understand levels of immunity in the population.
For some diseases, like measles, immunity is essentially lifelong, and having antibodies means you are protected. However, for SARS-CoV-2 this is not the case, because the virus has continually evolved new variants that are able to reinfect people despite their antibodies.
Nevertheless, many studies have shown that individuals with hybrid immunity will be more protected against future infection and variants than those with vaccine- or infection-derived immunity alone. It may be useful to know the proportion of the population with single-source immunity in order to target certain groups with vaccination campaigns.

TN minister Senthil Balaji hospitalised; breaks down after ED arrest under PMLA
It is learnt that while he was being taken into custody at 2 am, the minister complained of chest pain and was taken to the multi-speciality hospital in Omandurar Estate.

CHENNAI: The Enforcement Directorate (ED) on Wednesday arrested Tamil Nadu Electricity Minister V Senthil Balaji under the Prevention of Money Laundering Act (PMLA) after a long session of questioning.
However, the Tamil Nadu minister has been hospitalised in the ICU of a government hospital here early on Wednesday after he complained of uneasiness during ED raids at his premises on June 13.
Initially, Senthil Balaji was taken into custody by the ED after 18 hours of questioning by the Central agency. It is learnt that while he was being taken into custody at 2 am, the minister complained of chest pain and was taken to the multi-speciality hospital in Omandurar Estate. TV visuals showed Balaji being uneasy while being brought to the hospital in the wee hours of this morning.
The arrest sparked concern in the DMK camp as ministers, PK Sekar Babu, and Udayanidi Stalin rushed to the hospital to meet the minister. Sekar Babu claimed there were 'symptoms' Balaji has been 'tortured.'
After visiting Senthil Balaji at the hospital, HR and CE minister Sekar Babu said that the electricity minister was unconscious. "He didn't respond despite calling his name several times. There is swelling on his leg. The state he was brought to the hospital shows there are human rights violations," Sekar Babu said.
He also alleged that the enforcement directorate, CBI and the income tax department are acting at the behest of the central government against those opposing the bad politics of the central government.
"Several leaders, including chief ministers of other states and the parliamentary opposition leader, condemning the union government shows there is opposition centrally. DMK government, led by MK Stalin, will face the problem legally. More than a minister, Senthil Balaji is a DMK worker. The CM will fight for every DMK worker to save him and all actions will be taken based on his guidance," the DMK minister told media persons.
Law minister S Reghupathy, who visited the hospital, also said there was no clarity if Balaji has been arrested. He questioned the need for the ED raids at Balaji's house running non-stop for hours.
DMK advocates, who visited the hospital, demanded the ED officially make it clear immediately whether Balaji has been arrested or not.
Meanwhile, V Ananda Kumar, the nodal officer at the Omandurar hospital, said that Senthil Balaji is being treated for blood pressure and variations in the ECG and added that there were no external injuries to the Electricity Minister. "After Covid-19, several patients come with the complaint and there is a standard operating procedure for such cases. While we are investigating if he has been tortured, there are no external injuries," he added.
