What's with your pauses, readers?

I have an audiobook problem, in that I can't seem to get things done without them, but they also frequently irritate me the entire time. I'm curious what producing one is actually like, because if I was producing such a thing and someone paused at weird spots in the sentence, or didn't pause at all, there'd be some serious reader/director discussion.

I just finished one with a very clear reader, lovely voice. She reads the dialogue fairly naturally, but has exaggerated pauses and odd emphasis within narration sentences. I feel like maybe she's being deliberately slow for clarity (which is fine), but just slowing down and enunciated doesn't give you a line that sounds like

Tremain folded, her arms smiling.

Which is obviously fundamentally different and weirder than

Tremain folded her arms, smiling.

...which is what I assume the author intended. I keep having to back up because sentences parse entirely wrong and I feel I've lost something, and it turns out it was just one odd adverb or something.

Then there's the narrator who had no pauses between sentences (also reading a book by Martha Wells, oddly). His pauses and emphasis within a sentence made sense, but there was no space between sentences — was that his choice, or bizarre audio engineering?

The way we emphasize things changes meaning; that doesn't go away when you're reading fiction.
