He's talking about his gills coming in and how his family dealt with that.

My aunt--lovely woman, rest in power Aunt Loretta--had three nipples, so we thought the gills were some genetic deformity.

How do you get "he fucked his aunt" from the fact that he knew his aunt had three nips? If you've got a family member with something weird like that, everyone in the family will know, whether they've seen it or not. Second, this is something that would have come up during a doctor visit, trying to figure out what the deal with the gills was.
