Quite a haul from the gastroenterology appointment this afternoon!
Tumblr media
We've now got quite a supply of disinfectant stuff and "glideslime", plus a fistful of gauze swabs to use with them.
Tumblr media
Plus that (useless for me) box of individual packs of lidocaine goop.
Tumblr media
They also threw in about half a pack they had handy of hospital barf bags. Can't forget the barf bags, apparently! 🙃
Tumblr media
Not to mention the centerpiece, looking like the world's crappiest hose to beat people with.
Which I am intended to snake down my throat on a regular basis, with the tape line as a guide of how far to go. Which I did in office already, a couple of times. That was fun. At least I did show the foresight to skip lunch beforehand, is all I can say.
Tumblr media
They pulled out what looked for all the world like some particularly large pool cue case, but full of a graduated set of tapers rather than cues. (Small blessings? 🥴) I halfway expected to get sent home with that, but no today they decided to just cram that smallish one in a plastic bag.
I am actually trying this shit voluntarily, btw.
Home self-torture was mentioned as a possible option not that long after we got here--I think partly to hopefully cut down on how often I needed the gastroscopy treatments while we were still having to pay completely out of pocket before I was covered by the local system. Then it didn't come up again. I actually brought it up myself pretty recently, after they decided to switch me over onto the much more involved (and totally fucking miserable) full outpatient day surgery under general anesthesia plan for the procedures.
I would really like to continue being able to swallow and, like, live. But, I truly do not know how much longer I can force myself to do that on a monthly basis. So yeah, I will gladly snake out my own damn throat like a balky drain if it might hopefully mean that I can keep going without the standing appointments in Medical Trauma Hell* which are also physically much harder on me than the previous arrangement.
Tumblr media
Anyway, they were really not sure about this, especially since the worst of the damage to the esophagus is way down the pipe, almost to my stomach. These drain snake hoses are apparently what they were relying on themselves for dilation treatments before endoscopes were a thing, so it (thankfully) wasn't necessarily a question of how effective that approach might be. More how well the person might be able to tolerate reaming out their own throat that far.
Their department has evidently only had two other patients taking the DIY approach, both of them with strictures much higher up. (Though I was reassured that both of them did manage to go at least several years between needing further professional help.)
But yeah, I am highly motivated and pretty damned stubborn. I do appreciate that they are actually willing to talk to me, and seeming to consider comfort as a somewhat important consideration. All of which is one hell of a lot better than I could say for the clowns in London during actual procedures.
I did try the next size up in the office too, but that was enough trickier today with several people watching me deepthroat plastic plumbing snakes that for now they just sent me home with the small 5mm hose. I am scheduled to go back next week for another round in Day Surgery Hell anyway, and they might give me more then. It should also be easier to manage with bigger ones once they've reamed the esophagus out again.
_________
* They have been treating me like an actual person at that hospital, consistently. But, PTSD just being stuck in a situation like that for most of a day. No wonder medical professionals keep not being happy at what my blood pressure is doing.

I'm trying not to worry too much, but that's been going about as well as you might expect.
Tomorrow I've got an intake appointment set up for endoscopy, because they apparently decided to pass me over to a different team. No communication or anything, I just didn't hear anything about the next regular appointment--and then I got this new patient appointment letter for a week after my usual appointment schedule has been running.
That was confusing as hell, but Mr. C called to see if there had been some kind of mixup. Nope, evidently somebody just decided to send me to another team there after around 2 years with absolutely no communication about it. And I am apparently supposed to do a full new intake, with forms and blood tests and all.
There was also a procedure set for the 19th, or two weeks longer than they have been leaving between procedures lately. Thankfully, I am still eating fine right now, and here's hoping that will hold out for another week. At least they are evidently not planning to immediately stop doing treatments entirely, since they did give me that appointment date already.
But yeah, I have no idea wtf is even happening here until hopefully finding out something tomorrow, and it's making me nervous. I am actually at the point where I pretty much trust the team I've been working with so far here, after horrible and generally traumatic experiences dealing with the NHS before. And this is kind of a life-threatening issue they've been treating.
I do of course worry that they're basically just trying to dump me as a patient, since it hasn't been completely solved after 2+ years of them treating it (largely thanks to the delays and assorted fuckups when the damaged esophagus was still more easily fixable). I am also concerned that whatever new people I end up with won't actually try to keep me able to swallow as non-traumatically as possible, and I'll basically end up back in an NHS-level mess. The team I've been dealing with did actually seem to gaf if I could eat--and tried to keep me out of severe pain while they were reaming my throat out on a schedule informed by it staying open or not!--but yeah who knows.
We'll have to see what they have to say tomorrow. And I'm glad I do have some kind of backup, going in to who the hell knows what.

more of my sci-fi oc shit
ANYWAY I have lost my notes on him that were full of stuff about his condition and how it worked but i tried to remember
Jagger works as a staff sergeant in the A.E.C. pilot program to train and oversee future Self pilots.
He grew up in the northern parts of East Cross, where UK and Scotland used to be. The dangerous gasses the plague the warmer parts of the Earth, tend to travel north along the Gulf-stream and because of it he was exposed to it as a child.
At the age of 20 he joined a military college and graduated four years later on the top of his class. During that time he met a woman who would become his future wife and the mother of his child. They both ended up working for A.E.C. and Jagger went on to train pilots. At the age of 27 he was moved to work near the front lines and to pilot a Self android for 2 years. After that he moved back to training future pilots.
The disease Jagger suffers from targets his esophagus. It is caused by allergies and most likely the dangerous gasses near where he grew up (esophageal stricture is the main "inspiration" for his disease). The cells in his esophagus react abnormally to themselves and eradicate the cells near them causing scarring and built up material in the throat, which results in the tightening and eventual closing of the esophagus. He went on to a surgery that tried to remove parts of the scarring from his throat to ease the pain, but the abnormal cells reacted more aggressively after the surgery and made the progress worse.
The symptoms are difficulties swallowing, burning sensations in the throat area and high probability of choking. The disease develops slowly but at the age of 25 he no longer could swallow solid foods and had to be put on a full liquid diet. But then, in his thirties he no longer could swallow liquids without pain and moved to a IV diet, where he got his daily needs.
Inevitable death overseeing his future, Jagger was ready to leave his life behind too soon. But he was offered an opportunity in a small WBE (whole brain emulation) test that could have a chance at saving his life. Moving his consciousness to a machine body.

if anyone is curious Enzo has been doing well lately. Still holding out to see if he has any delayed reaction to the lepto vaccine but so far so good. The only real symptom hes having consistently now is the water drinking cough. It's annoying since it has gone away twice but after a flare up, esp if he had some v*mitting associated with t, there's usually a recurrence of it. I'm treating it now with some medicine (carafate) but it's still lingering :/ mostly it just seems like some sort of mechanical issue with his swallowing. which either is caused by reflux (ie irritation and damage to the esophagus) or it could be what has caused his reflux in the past (ie anatomically speaking he may have issues with motility and his esophageal stricture)
in jan we have an appointment with an internist, so hopefully at we can start working to get some answers. Only way to know for sure what is going on is to do an endoscopy and possibly floroscopy unfortunately :/
but to that end there may not be much that can actually be done about it besides symptom treatment. There's probably not a big enough issue for surgery to be particularly warranted in his case, especially weighing risks
otherwise he's been in decently good health lately and his appetite has been frankly increased when compared to previously which is nice to see. weight has been good and his gut seems to be working as good as one could hope for
if he's still doing well in a week or 2 I will most likely start slowly introducing a novel protein diet to him (rabbit) since I think beyond just seeing if a diet change can further improve symptoms im hoping to see if i can actually give my dude some actual meat. it would be so awesome to have high value treats in our arsenal again

I just got another professional throat reaming yesterday, and I can't help but be relieved in a way that it's still feeling sore enough for the home drain snaking not to sound like a great idea tonight. Probably better to give it another day to heal up.
They did send me home with another one ofa little larger diameter yesterday. And actually asked me to bring at least the smallest one back next time, since apparently those aren't even made anymore and they really don't want to lose any of this set. Guess that's not really something they could reasonably just pick up used off eBay or whatever, either!
As miserable as the DIY experience still is, at least it really did seem to make a difference--as everyone was hoping. It kept things open to the point that I wasn't even having to be more careful about what I was eating the last week or so before they worked on it again. I was really hoping that they would be willing to increase the interval between gastroscopy sessions now, but they still want me back in a month. Hopefully once they see if it continues that way, though.
It'll apparently get a decent test over the summer, at least, since the doctor said they wouldn't have access to the OR they've been using for some reason during July. They can apparently use another one elsewhere in the hospital if necessary, but it sounded like that guy wouldn't be sorry to skip July either. (At least I guess both of us can be glad it is gastroscopy, and not necessary from the other end! 😩 Still not a lot of fun for anybody involved, probably.)
But yeah, even if they're not willing to call a longer time between the professional dilation yet, it is at least a HUGE relief to feel a lot more secure that, worst case? I am extremely unlikely to get so that I can't even swallow liquids again. And there is something I can personally do about it if my throat does try to close up like that again.
Being completely dependent on people who kinda seemed to be hoping I would go ahead and die already--just as long as it stopped being their responsibility--had to be one of the roughest, and frankly most traumatic, parts of this whole extended shitshow. Thankfully not the vibe I have ever gotten here at all, but it's still a big relief to have some way of helping myself. Even if it does involve gagging myself with plastic tubes.

Restoring Digestive Health: Insights into the Gastrointestinal Stent Market
The gastrointestinal system plays a pivotal role in our overall health and well-being, responsible for the digestion and absorption of nutrients essential for bodily functions. However, conditions such as gastrointestinal cancers, strictures, and obstructions can disrupt this delicate balance, leading to discomfort, pain, and complications. In such cases, gastrointestinal stents have emerged as vital medical devices, offering a minimally invasive solution to restore digestive health and alleviate symptoms. In this article, we explore the significance, trends, and impact of gastrointestinal stents in modern healthcare.
Gastrointestinal stents are specialized medical implants designed to open blocked or narrowed sections of the gastrointestinal tract, allowing for the passage of food, fluids, and bodily waste. These flexible, tubular devices are typically made from biocompatible materials such as nitinol, stainless steel, or silicone, and come in various shapes and sizes to accommodate different anatomical structures and conditions.
One of the primary applications of gastrointestinal stents is in the management of gastrointestinal cancers, including esophageal, gastric, and colorectal cancers. In patients with malignant strictures or obstructions, gastrointestinal stents provide immediate relief by restoring the patency of the affected area, relieving symptoms such as dysphagia, nausea, and vomiting, and improving quality of life. Moreover, stent placement can serve as a palliative measure to alleviate symptoms in patients with advanced or inoperable tumors, enabling them to eat and drink more comfortably and maintain nutritional intake.
Request the sample copy of report @ https://www.globalinsightservices.com/request-sample/GIS10289
In addition to cancer management, gastrointestinal stents are also used in the treatment of benign conditions such as strictures, fistulas, and leaks, as well as in the management of complications following gastrointestinal surgeries. In cases of benign strictures or strictures related to inflammatory bowel disease (IBD), stent placement can help alleviate symptoms and avoid the need for repeat surgeries. Similarly, in patients with gastrointestinal fistulas or leaks, stents can provide a temporary barrier to facilitate healing and prevent further complications.
Technological advancements and innovations in gastrointestinal stent design and delivery techniques have significantly improved patient outcomes and expanded the scope of gastrointestinal interventions. The development of self-expandable metal stents (SEMS) and covered stents, for example, has enhanced stent durability, flexibility, and biocompatibility, reducing the risk of migration, perforation, and tissue ingrowth. Moreover, the advent of endoscopic ultrasound (EUS)-guided stent placement and fluoroscopic-guided stent deployment has enabled precise positioning and improved procedural success rates, minimizing complications and optimizing clinical outcomes.
Furthermore, the growing prevalence of gastrointestinal disorders, the aging population, and the increasing adoption of minimally invasive endoscopic procedures have fueled demand for gastrointestinal stents worldwide. According to recent studies, the global gastrointestinal stent market is projected to experience steady growth in the coming years, driven by advancements in stent technology, increasing awareness among healthcare providers, and rising patient demand for less invasive treatment options.
In conclusion, gastrointestinal stents have revolutionized the management of a wide range of gastrointestinal conditions, offering patients a minimally invasive alternative to traditional surgical interventions. With ongoing technological advancements, expanding indications, and growing adoption in clinical practice, gastrointestinal stents are poised to play an increasingly important role in restoring digestive health and improving quality of life for patients worldwide. As healthcare providers continue to embrace innovation and evidence-based practices, the future of gastrointestinal stent therapy looks promising, with potential for further advancements in patient care and clinical outcomes.

Heartburn vs. GERD vs. APD: Understanding the Differences
Digestive issues are common, but distinguishing between similar symptoms can be challenging. Heartburn, GERD (Gastroesophageal Reflux Disease), and APD (Acid Peptic Disorders) are often confused due to overlapping symptoms. Here, we break down the key differences to help you understand and manage these conditions effectively.
1. Heartburn:
Definition: Heartburn is a burning sensation in the chest, often behind the breastbone, caused by stomach acid refluxing into the esophagus.
Symptoms: Burning chest pain, often after eating or at night, regurgitation of acidic or bitter-tasting fluid into the throat.
Causes: Weakening of the lower esophageal sphincter (LES), which normally prevents stomach acid from flowing back into the esophagus.
Triggers: Spicy or fatty foods, caffeine, alcohol, smoking, obesity, pregnancy.
Treatment: Antacids for immediate relief, lifestyle changes (elevating the head of the bed, avoiding trigger foods), medications to reduce acid production (H2 blockers, proton pump inhibitors).
Complications: Occasional heartburn is common and usually harmless, but frequent or severe heartburn can indicate GERD.
2. GERD (Gastroesophageal Reflux Disease):
Definition: GERD is a chronic condition where stomach acid frequently flows back into the esophagus, leading to complications.
Symptoms: Persistent heartburn, regurgitation, chest pain, difficulty swallowing, chronic cough, hoarseness, asthma-like symptoms.
Diagnosis: Based on symptoms, medical history, and tests such as endoscopy, pH monitoring, or esophageal manometry.
Complications: Esophagitis (inflammation of the esophagus), Barrett's esophagus (precancerous changes in the esophagus), strictures (narrowing of the esophagus), respiratory problems.
Treatment: Lifestyle changes (diet modification, weight loss, avoiding trigger foods), medications (proton pump inhibitors, H2 blockers), surgery (fundoplication) in severe cases.
3. APD (Acid Peptic Disorders):
Definition: APD is a group of conditions characterized by an imbalance between acid production and the protective mechanisms of the stomach and duodenum.
Types: Peptic ulcer disease (ulcers in the stomach or duodenum), gastritis (inflammation of the stomach lining), gastroesophageal reflux disease (GERD).
Symptoms: Abdominal pain, bloating, belching, nausea, vomiting, heartburn.
Causes: H. pylori infection, NSAIDs (nonsteroidal anti-inflammatory drugs), stress, smoking, alcohol.
Diagnosis: Based on symptoms, medical history, endoscopy, stool tests, or breath tests for H. pylori.
Treatment: Antibiotics for H. pylori eradication, acid-suppressing medications, antacids, lifestyle modifications.
In Conclusion: Heartburn, GERD, and APD share common symptoms but differ in terms of severity, frequency, and potential complications. While occasional heartburn is normal, persistent symptoms should not be ignored. Consult a healthcare professional for an accurate diagnosis and appropriate management plan tailored to your condition. Managing these conditions effectively can improve your quality of life and prevent complications.
