“Aren’t you just the cutest little thing on earth?” you squeal in delight as you spot a tiny black kitten outside the grocery store.

Bending down, you plop yourself on the edge of the pavement and scoop up the kitten, cradling it gently in your arms.

“Meow,” it mewls, its big round eyes looking at you with an innocent gaze. It has shaggy, fluffy black fur and beautiful hazel eyes, reminding you of someone very dear to you.

“You don’t seem to have an owner, do you? Hm, I see no collar and no one is around. Tell me, kitty, ya hungry?” you coo at it, tilting your head inquisitively, and the kitten mimics the movement in a cute, almost cartoonish way.

Realizing the kitten might not respond to you, you nod your head in determination.

“Don’t worry, kitty cat, you can come home with me. I’ll just get you some food first.”

You glance into your grocery bags and then back at the kitten sheepishly.

“I guess you can’t eat ice cream and sushi, can you?” you say with a giggle, realizing your mistake.

Looking around to make sure no one is watching, you carefully pick up the kitten and gently tuck it into your tote bag. With a nonchalant demeanor, you stroll into the store, doing your best not to attract attention to your feline stowaway.

“Hello, Ms. L/N! Back for some more?” Vanessa, the cashier asks as she notices your quick return.

You freeze, subconsciously clutching your bag as you look around. “Hey, V! I just forgot a little something. I’ll be in and out,” you say nervously, just as your bag meows. You both look at each other in surprise.

“D-Did your bag just meow?” Vanessa asks, raising an eyebrow.

“N-no, um, that was me! I was just bored. Okay, bye V, see ya,” you stammer, trying to play it off as a joke before hurrying further into the store.

You can’t help yourself from getting every single cat toy on the planet, along with every type of food you think the kitten might like. Now, the trunk of your car is filled with toys as you strap the kitten into the passenger seat of your rose Porsche Taycan.

“Okay, kitty. I’ll put on some music. And I don’t know what kind of music you dig, so you’ll just have to listen to Frank Ocean. He’s a really good artist!” you chirp cheerfully, adjusting the volume knob as you drove, hoping the kitten will enjoy Pink+White playing.

Driving into your garage, you park your car and bring your newest housemate inside.

There, you play with him, squealing in happiness at the way he jumps cutely. You praise him and clap your hands whenever he does a cool trick. He even liked the mini bat wings you got for him. The kitten looked so stinkin’ cute in them you just had to take a million photos!

After a while, you get his bowls, fill one with food and the other with water as you sit down next to him, eating your sushi.

“I have to name you something, kitty. I mean, ‘kitty cat’ is adorable, but you need a new name. But just what?” You pout and furrowed your brows, thinking really hard as you lay on the plushy fluffy carpet, the kitten resting on your chest, tired after hours of playing.

As you pat his fur, you wonder what name would suit him. He has black fur and hazel eyes, is calm and collected, just like… Suddenly, your eyes widen, and you gasp.

“Oh my gods, Azzie! Your name is totally Azzie!”

Excitedly jumping up from the carpet, you practically skip over to your laptop, eager to make your new housemate’s name official. After a quick search, you find the perfect pink collar with a heart-shaped tag that says “Azzie.” Without a second thought, you click “Add to Cart” and eagerly wait for it to arrive. After all, Azzie deserved nothing but the best!

The next time Azriel comes over, he is greeted by a furry surprise.

As he steps into your home, his eyes widen in astonishment as he spots a tiny kitten sitting on the plush carpet, its hazel eyes mirroring his own.

For a moment, they both freeze in their tracks, assessing each other with a mix of curiosity and wonder. Slowly, Azriel puts down the math books on the counter before he approaches the kitten, his gaze never leaving its adorable face. Kneeling down, he extends his hand tentatively, unsure of how the little creature will react.

To his amazement, the kitten responds by rubbing its nose against his hand, nuzzling into his palm with a happy purr. Azriel can't help but smile, his heart melting at the sight of the affectionate gesture from his miniature doppelganger.

As the cat jumps into his lap, the jingle of its collar catches Azriel's attention. He glances down and notices the pink collar with a heart-shaped tag that read "Azzie" in delicate letters.

A surprised chuckle escape his lips as he gently pets the kitten, realizing that it has been named after him. The gesture warms his heart, and he can’t help but feel a newfound fondness for the furry little companion.

You walk into the room and are met with the heartwarming sight of Azriel playing with your kitten. A smile spreads across your face as you join in, the three of you engaging in a playful game of chase and pounce. Laughter fills the air as you watch Azriel's eyes light up with joy, his usual stoic demeanor melting away in the presence of the adorable feline.

“Oh, Azzie, you two are so freakin’ cute together, I think I might die,” you gush, clapping your hands in delight.

Azriel looks up, a shy smile gracing his lips. “Yeah, I guess he's not so bad,” he replies, his nerdy, shy charm shining through.

You giggle, scooping up the kitten and holding it close. “He's the best! And he's just like you, Azzie. Quiet, calm and sweet like candy!”

Azriel's cheeks flush slightly at the comparison, but he can't hide the fondness in his eyes as he kisses your cheek before he continues to play with the kitten. Together, the three of you get to know each other better, happy with the new addition to your life.

Do you know how our understanding and treatment of diabetes has changed through history?

Oooh good question, anon!

As you may guess, diabetes mellitus is not new.

We've known about it since at least the Ebers Papyrus (1550 BCE) when the disease and a treatment was first described. This treatment was: "a liquid extract of bones, grain, grit, wheat, green lead and earth." I did not look these up, but I would guess they did not do a whole lot for the treatment of diabetes.

Later during the 6th century BCE it was first given a name when it was described by Hindu physician Sushruta as madhumeh or "honey urine."

Honey urine is a very apt descriptor for diabetes. In any type, one of the most measurable symptoms is that the person urinates a lot, and the urine tastes sweet (or, if one didn't feel like tasting, that it ferments, or that it attracts ants). This was also the first test for diabetes.

The reason for the sweetness of the urine (as well as a lot of other general info about diabetes) is spelled out more clearly in my "Don't Be That Guy Who Wrote Hansel and Gretel: Witch Hunters" post.

A Greek physician Apolonius of Memphis named it Diabetes, meaning "to siphon" (referring to the large amount of urine lost).

Roman physician Aretaeus later made the first precise description of diabetes. This included the classic symptoms of incessant thirst, copious urination, and constant hunger leading to emaciation and death. He also notes that if deprived of water, the patient will continue to urinate until they become so dehydrated that they die.

The term "Mellitus" was not added until the 1600s by an English physician Thomas Willis. This was again due to the sweetness of the expressed urine. Willis prescribed a diet of "slimy vegetables, rice, and white starch. He also suggested a milk drink which was distilled with cypress tops and egg whites, two powders (a mixture of gum arabic and gum dragant), rhubarb and cinnamon". Supposedly his patients improved if they kept to this diet, though few managed it long term. I honestly don't know how it would have worked, even temporarily.

A major breakthrough came in 1889 when it was discovered that if you removed the pancreas from a dog, the dog would become diabetic (particularly, that it would urinate large quantities of sweet urine). Up until this point it was thought that diabetes stemmed from the kidneys and bladder, or perhaps the lungs. This was the first time it had been shown experimentally that the pancreas was the problem.

Speaking of this, this was also part of a series of experiments where an English physician named Merkowski implanted a small amount of pancreas in the pancreas-less dog's fat, which reversed the diabetes temporarily. This proved that the pancreas was making something that helped regulate blood (and thus urine) sugar.

What this was wasn't figured out until 1921, when Canadian scientists Banting and Best (with help from McLeod and Collip) isolated something they called insletin (after the islets of langerhans, where the substance was being produced). It's important to note that all of these scientists hated each other so much they almost refused a Nobel Prize over it. Later, Collip would refine the substance and McLeod would rename it insulin.

Prior to insulin existing there was basically 1 vaguely useful treatment for diabetes. Unfortunately, that was starvation. So you could either die a slow and painful death by diabetes or you could die a slightly less slow but still painful death due to eating about 500 calories per day. Either way, diabetes was fatal, usually within a couple of years of diagnosis.

By 1923, the first commercial insulin product, Iletin, had been developed. Iletin was a U10 insulin (10 units per 1 milliliter- less potent than today's U100 and U500 insulins) and was made from pork pancreases. It took nearly a ton of pork pancreas to make 1oz of insulin. Fortunately, as a byproduct of the meat industry, pancreases were readily available.

Now, you might be thinking- no one has mentioned type 1 or type 2 yet in this entire post!

Well, you would be right, because diabetes wouldn't be split into 2 forms (insulin-dependent and non-insulin dependent) until 1979, and wouldn't be classified as types 1 and 2 until 1995. That's right- some of you were alive when there was only one kind of diabetes out there.

Now, there's more about the types in the Hansel and Gretel post, but essentially type 1 diabetes occurs when the pancreas itself stops producing insulin, usually in childhood. When this happens, the body stops being able to use sugar (insulin, a hormone, acts as a "key" to let sugar into cells for use). Without replacing that insulin, the person dies because their cells starve.

Type 2 diabetes occurs when the pancreas still produces insulin, but the cells stop responding to it correctly. This causes high sugar levels in the blood, which causes longer-term complications (infections, ulcers, blindness, neuropathy, heart and kidney disease, hyperosmolar syndrome, etc..) which eventually lead to death.

We started discovering oral drugs that worked on what would later become type 2 in the 1950s. Particularly those that worked by increasing the insulin output of the pancreas, but only when the pancreas was still producing some insulin.

Predicting which diabetics would benefit from oral therapies was challenging, but it was recognized that when the onset of diabetes was slow and came on in adulthood, the oral agents would work, while if it came on suddenly in childhood, the oral agents wouldn't. Terms like "adult onset" and "maturity onset" were common:

(Side note: if you have ever read Alas, Babylon (1955) there is a diabetic character who by today's standards clearly has type 1 diabetes, but wants to switch to the "new oral pill" (called "orinase" in the book, though they are likely referring to diabinese pictured above).)

From 1923 into the 1980s, insulin was given once or twice per day, and not particularly titrated to blood sugar. This was probably just because we didn't have a great way to measure blood sugar in real time. Pre-1970s, there was no way to test blood sugar outside of a lab setting.

Urine testing was common starting in the 1940s, but was cumbersome as it required a flame for heating the urine. By the 1950s, a test had been developed that didn't require a flame, but was still not practical for home use. In the 1960s, paper strips were developed that changed color for different amounts of sugar in the urine. The problem with this was that the strips couldn't change color until there was sugar in the urine- a blood sugar level of over 200 by today's measurements. Low blood sugar readings were impossible at this time, and had to be treated based on symptoms.

In the 1970s, blood sugar could finally be measured by putting a drop of blood on a test strip, wiping it off, and matching the color of the test strip to a chart. While less cumbersome than urine tests, this was still something that would generally only be done at a doctor's office.

In 1983, the first home blood glucometer is developed. Finally, it was practical to take one's sugar multiple times per day, and it becomes possible to experiment with "sliding scale" insulin injections that keep tighter control of blood sugar. By the late 90s, continuous glucose monitors became available- though unlike today's CGMs that allow readings in real time on a smartphone or monitor, these had to be downloaded to a computer at regular intervals.

The 1980s were the first decade where insulin pumps become widely available. The very first pump was large and had to be carried in a backpack, but it represented a huge step forward in glucose control, as it more closely mimicked the function of a working pancreas than once-daily injections.

For the next 30 or so years you really had to work to qualify for an insulin pump, but recently it's been found that pumps greatly improve compliance with blood glucose control whether or not the person had good compliance before getting the pumps, and insurance has gotten better about covering them (though CGMs are still a pain to get insurance to cover).

The 1980s was also the decade that recombinant human insulin (insulin made by genetically modified bacteria) was first used. Up until that point the only insulins were pork and beef insulins, which some people had allergic reactions to. Recombinant insulin was closer to regular human insulin than beef or pork, and represented a big change in how insulin was made.

Today for people who take insulin to manage their diabetes, insulin is usually given as a single injection of a long-acting basal insulin, coupled with smaller doses of ultra-short-acting insulins with meals or snacks. This is the closest we've gotten to mimicking the way a pancreas would work in the wild, and keeps very tight control of blood sugar. This can be done by fingerstick blood sugar tests and individual injections of insulin, or it can be done with a CGM and pump- it just depends on the resources available to the person and their personal preference.

Summary : In a remarkable twist of fate, two strangers miraculously survive fatal accidents, only to realize they are impervious to the passage of time. Unbeknownst to each other, they lead separate lives in a world where everyone else ages. By chance, their paths intersect, and a magnetic attraction ignites, so what happens when these two individuals fall in love?

The Reach 1935

Upon graduating from high school, she and her friends made a joyful decision to visit the lake and have a picnic together, celebrating their long-awaited achievement. It was a time of shared happiness, as they marked the end of their educational journey and embraced the start of a new chapter in their lives.

"Darling, are you absolutely certain about proceeding with this plan?" Her mother inquired, her voice laced with concern. "I heard from the radio that a heavy downpour is expected today!"

"Mommy, it'll be fine, I promise to return before the rain starts," she reassured, slipping on her shoes. With a sudden burst of affection, she rushed to her mother and planted a swift kiss on her cheek. "I love you, Mommy” A gentle smile graced her mother's face as she replied, "Take care, my dear. I love you too."

"Don't worry, I'll be careful!" she declared, dashing out the door, eager to embark on her outing.

Before meeting up with her friends, she made a stop at the bakery to pick up a luscious chocolate cake and some mouthwatering raspberry tarts that she knew the twins adored. With the delectable treats in hand, she headed towards their designated meeting spot by the lake.

As she approached, her friends were engaged in a lively conversation, their laughter echoing through the air. Joining in their joyful banter, she playfully teased, "I hope you two are ready to share those raspberry tarts with me !"

The twins exchanged mischievous glances, feigning offense. "Well, because we love you, we'll let you have a tiny piece " Rhaena replied, winking at her.

Amidst their light-hearted chatter, the topic took a turn, transitioning to a more serious tone. Reflecting on their newfound status as graduates, they pondered the path that lay ahead.

Baela sighed and said, "Now that we've graduated, what do we do next? It feels both exciting and overwhelming."

Sadly, in 1930s, societal expectations greatly limited the opportunities available to women. They were primarily seen as homemakers, tasked with marrying, tending to their husbands, and raising children. She always wanted to become a doctor, to help people heal and become better. Despite graduating with exceptional academic achievements, she confronted the harsh reality that medical school doors remained closed to her simply because she was a woman “ I have no idea “ she sighed.

Rhaena noticed that the mood changed from joyful to depressed and dull, so she tried to lift the mood "How about we take a refreshing swim in the lake?" she suggested, her voice filled with enthusiasm.

The girls exchanged glances, their faces lighting up with wide smiles. They squealed with delight and dashed towards the lake, eagerly wading into the water and splashing each other. Laughter echoed through the air as they reveled in the carefree moments, oblivious to the dark clouds forming overhead.

As the rain began to pour down upon them, she realized the promise she had made to her mother. "Girls, we need to leave! I promised my mother we'd be back before the rain started!" Baela growled in protest, her eyes shifting to Rhaena, who had a pout on her face "Please? Just five more minutes!"She exhaled and reluctantly agreed. "Okay, just five more minutes." The twins grinned mischievously and resumed their splashing, laughter rekindling in the air. The five minutes swiftly passed, signaling the time to bid farewell to their watery playground.

As she prepared to leave the water, she noticed her earrings were missing. She urgently beckoned Baela and Rhaena to continue on without her, assuring them she would catch up as soon as she find her earrings. Her heart raced as she searched the water, desperately seeking her lost earrings. After a few tense seconds, she finally spotted them. With a sigh of relief, she reached down to retrieve them.

Suddenly, a lightning bolt crackled through the sky, striking the water with a deafening crash. The world around her became a blur. The next thing she knew, she awoke in a hospital bed. The doctor's voice filled the room, emphasizing how lucky she was to have survived such a terrifying event.

King’s landing 1935

Aemond's voice carried the weight of frustration as he complained to his sister about their father's unwelcome decree. "I told him I don't want to join the army, but he forcibly dragged me from my bed to sign up!" His words dripped with resentment and disappointment, the echoes of shattered dreams reverberating in the air.

From a tender age, Aemond had harbored a profound passion for history, yearning to become a revered historian and share his knowledge as a professor. However, his older brother's warnings had proven prophetic. Just as his sibling had foretold, their father imposed his will, ensuring that Aemond would follow in his brother's footsteps, consigned to a fate not of his choosing.

Helaena, his empathetic sister, sought to provide solace amidst the storm of Aemond's dashed aspirations. "It's only for four years, Aemond," she consoled, her voice laced with compassion. "Once your service is complete, you can move out, just like Aegon did, and pursue your dreams at college."

Looking up at his sister, Aemond endeavored to change the subject, if only momentarily. "How are the twins? I haven't seen them since I returned," he inquired, longing to find solace in the comforting presence of his beloved niece and nephew. “ They are with Jace. I believe they all fell asleep before your arrival," Helaena responded, her tone reflecting a mixture of weariness and love. "Come, let's wake them up . Dinner will be ready in an hour."

The two siblings made their way to Helaena's old room, where she, her husband Jace, and their twins slept during their visits. Aemond descended the stairs to lend his mother a helping hand in preparing dinner. The sight of Alicent, his caring mother, filled him with warmth, a flicker of gratitude amidst his trials."Thank the gods you're awake," Alicent greeted him with relief as she spotted him approaching the kitchen. "Sweet boy, could you run to the store and fetch some salt? We've run out, and your father, as always, forgot to bring it."

"Of course, Mother," Aemond replied, planting a tender kiss on her cheek before heading out the door. The heavy rain poured down, he was grateful for the proximity of the store, He ran as swiftly as his legs would carry him, desperate to return home quickly.

Finally reaching the store, he burst through the entrance, the bell chiming overhead. His eyes darted through the aisles, searching for the much-needed salt. Grasping the familiar cylindrical container, he swiftly made his way to the counter, his hands slightly trembling.

As he embarked on the journey back home, the weight of exhaustion pressed upon him. The sound of raindrops intensified, resonating like a symphony around him. Nearing their house, he spotted a solitary lamppost, its soft glow struggling against the gloom. Seeking a moment's respite, he leaned against it, the cool metal against his weary frame.

Suddenly, as if the heavens unleashed their fury upon the world, a dazzling bolt of lightning cleaved through the darkened sky. The crackling surge raced downward, striking the very pole against which Aemond sought shelter. The world erupted in a blinding flash, and his consciousness faded into obscurity.

With a dazed mind and blurred vision, Aemond slowly stirred from his slumber. The sterile hospital room encompassed him, its unfamiliar sights and antiseptic scents assaulting his senses. As his vision cleared, the doctor's voice reached his ears, recounting the harrowing events that had transpired. Words of disbelief mingled with gratitude, as the doctor emphasized the extraordinary fortune that had safeguarded him from the destructive power of the lightning strike.

The Reach 1946

After World War II came to an end and Aemond returned, he made a firm decision to leave King's Landing behind. His desire to escape his father's control led him to choose the Reach as his new home, seeking a place far away from the struggles of the past.

Enrolling at Oldtown University, Aemond's acceptance was swiftly granted, courtesy of his great uncle's influence as a faculty member. Moving to his grandfather's ancestral home, he found solace in a stunning Victorian house that boasted a breathtaking view of the surrounding landscape.

Another reason that drew him to the Reach was its tranquil nature. The region was known for its peaceful ambiance, where the residents tended to keep to themselves and respect each other's privacy. The quietude of the area provided the ideal environment for Aemond, who sought solitude and a chance to carve out his own path without undue interference.

He found himself compelled to visit the library in order to conduct research on the First Men invasion for his history class. It was there, amidst the lofty shelves and the scent of aged parchment, that he first laid eyes upon her. Behind the reception desk, she exuded a captivating beauty that enraptured his heart from the very first glance.

As their eyes met, time seemed to slow. Aemond found himself entranced by the gentle curve of her lips, the sparkle in her eyes, and the graceful arch of her brow. He couldn't help but be captured by her radiant charm. Her beauty was a beacon that drew him closer, awakening an unfamiliar yearning within his soul.

Breaking free from the trance, she acknowledged his presence with a gentle inquiry, "May I help you?" Aemond's heart raced, his voice betraying a hint of nervousness as he cleared his throat. Stepping forward, he extended a trembling hand, offering her a slip of paper that detailed his need for specific books related to the history of the First Men.

Their fingers brushed for an ephemeral moment, electrifying the air between them. Her touch sent a jolt of excitement through his veins, igniting a desire to know her more deeply. Her eyes sparkled with curiosity and warmth as she accepted the paper, their connection intensifying with each passing second.

Following her lead, Aemond trailed in her wake, scarcely aware of the words exchanged about the books. His focus remained fixed upon her every movement—the graceful sway of her hips, the delicate tilt of her head, and the subtle flush that adorned her cheeks. The library seemed to fade into the background as he became wholly captivated by the enigmatic enchantress before him.

Time slipped away as she selected the books he sought, her fingers expertly tracing the spines. With each book she placed in his hands, she shared a brief summary, her voice like a gentle melody that caressed his ears. Yet, Aemond found it difficult to concentrate on the words, for his gaze remained fixed upon her, enchanted by the depths of her eyes and the subtle curves of her lips.

As their task neared completion, she smiled warmly, filling the room with a radiant glow. "I believe that's everything," Her words snapped him back to reality, her voice a soft lullaby that stirred his soul "Good luck with your research, Mr...?" Her voice trailed off, an unspoken invitation for him to reveal his name.

"Targaryen," he replied, his voice infused with a mixture of vulnerability and newfound hope. "Aemond Targaryen." As he spoke his name, he extended his hand towards her, his eyes fixed on hers with an intensity that mirrored the storm of emotions within his heart.

Accepting his gesture, she placed her delicate hand within his, her touch radiating warmth and understanding. "Nice to meet you, Mr. Targaryen," she offered. Aemond, entranced by her presence, reciprocated her sentiment, his voice carrying a hint of anticipation. "The pleasure is all mine," he replied, his heart fluttering as their worlds merged in that fleeting moment.

Little did they know that this chance encounter would set in motion a journey of profound connection and shared dreams, intertwining their paths in a tale of love, discovery, and the pursuit of a future beyond the bounds of their wildest imagination.
