Putting the Omnitrix in DnD

The original purpose of the Omnitrix was to give the user access to the DNA of aliens from various worlds to preserve the DNA of a species in the event of a genocide or mass extinction. The watch fused to one person, was extremely hard to remove, and its default setting gave the user access to the DNA of 10 different species they could choose between. Aside from Humanoid, there are 13 total creature types: Aberrations, Beasts, Celestials, Constructs, Dragons, Elementals, Fey, Fiends, Giants, Monstrosities, Oozes, Plants, and Undead. In the spirit of Ben 10, I’ll make a list of 1 creature from each creature type that the watch can turn a player into. Playable races vary a little. Kenku, Grung, and Drow are all CR 1/4, but Changeling, Satyr, and Hobgoblin are 1/2, and Bugbears are CR 1 creatures. So, I’ll be considering everything within that scope. Some do go beyond that, such as Centaurs and Minotaurs, but most are within the window of CR 1/4 and CR 1. To give the DM and Player some variety, I’ll pick one option from each, if applicable. Celestials had to be eliminated because the lowest CR Celestial is CR 2 Pegasus. In-Universe, it was likely created by Mordenkainen.
ITEM:
BRACELET OF POLYMORPH
VERY RARE | ATTUNEMENT REQUIRED
The DM chooses any 10 possible transformations from the list below, only choosing 1 from each creature type. As a bonus action, you can activate the bracelet, becoming one of the creatures chosen by the DM. You use that creature’s stat block instead of your own, except you retain all languages you know, and your INT, WIS, and CHA are not affected unless the creature’s are higher than your own. If the creature you turn into does not speak any languages, you cannot communicate verbally while polymorphed by this item. Depending on the CR of the creature you turn into, the recharge rate of this item varies. For a CR 1/4 creature, it recharges on a short rest, on a CR 1/2, it recharges at dawn, and for a CR 1 creature, it recharges after 1d4 days. You can stay in the form of a CR 1/4 creature for up to 8 hours, a CR 1/2 creature for up to 4 hours, and a CR 1 creature for up to 1 hour. If your hit points in this form are reduced to 0, you return to your original form with your current hit points at the moment you transformed. The DM decides what the bracelet polymorphs the player into, and may ask the player for a contested INT check against a DC 15 to coax the bracelet into cooperation. Otherwise, the bracelet may choose to polymorph the attuned into a different creature than the one they wanted to turn into.

Magic Item: Ring of Turn into Muppet
Upon equipping and attuning to this item, you become a Small felt puppet. While a puppet, you have the following attributes:
-Immune to Bludgeoning, Force, and Poison damage (but vulnerable to Fire damage)
-Ability to summon useful items from Hammerspace
-Mechanic similar to Wild Magic Surges, during which you can cause any effect you want as long as you can convince the DM that it would be funny.

A glamored dress that appears as one of two designs depending on who observes it. The first time a creature views the dress, flip a coin. On a heads, the dress appears to be black and blue; on a tails, the dress appears to be white and gold. Once a creature has viewed the dress once, it will always appear to be the selected colors whenever that creature views the dress in the future. The effect is not illusory such that truesight or any spell would reveal any “true” color; it has two simultaneous true color combinations that differ by observer.

Fyren's Quarry (longbow, made for a gloomstalker ranger.)
Moriata (pepperbox/revolver, made for a gunslinger who also has one or two levels in warlock.)

the arcane gate spell really made me think of portal (the game) and i want to make a portal gun that basically casts arcane gate 4 times a day (and every times you do use it you roll a d4 and if it rolls on a 4 it recharges one charge) and all charges are recharged the next dawn. also the change with this gun is that instead of you having to cast it on the ground, as long as the surface is flat, or a round surface is large enough that would be close to round like the side of a large circular tower for example as long as whatever curved circle's diameter is equal to or greater than 100 ft, you can cast that version of arcane gate to create a gate there. basically the portal gun from portal and you can do the whole manipulating inertia and stuff like that like in the game
the question is should this be a rare item or a very rare item?

Homebrew Magical Instruments
A random collection of magical instruments. Not all of them for bards, either, though obviously they’ll get the most use out of them. Inspired in three cases by fairytales/folk songs (The Singing Bone, The Pied Piper of Hamelin, The Devil Went Down to Georgia), and the Vox Ethereal is literally just a theremin made with fantasy magic instead of RL magic (and electricity). Heart’s Rhythm is the only one without a direct inspiration, it just feels like a thing drums could do. Beat the right rhythm on it and you should be able to restart someone’s heart.
VOX ETHEREAL
Wonderous Item, Instrument, Common
This strange instrument is formed of a slab of wood carved with arcane runes, from which two slender rods of silver protrude on one side. Ethereal, hauntingly beautiful music can be created by mere gestures of the player’s hand in the air next to the rods.
THE SINGING BONE
Wonderous Item, Instrument, Uncommon
The Singing Bone is a flute carved from a single long humanoid bone, worn smooth and yellow by time and use. If used as a spellcasting focus by a bard, the flute grants a +1 bonus to spell attack rolls and spell save DCs. Once per day, the Singing Bone can be used to cast the Speak with Dead spell. When cast from the flute, the spell does not require the corpse to have a mouth, as the flute will instead sing with the voice the corpse had in life.
PIPER’S CALL
Wonderous Item, Instrument, Uncommon, requires attunement by a bard, druid, ranger or warlock
Piper’s Call is a slender wooden pipe with a flared end that once belonged to a mysterious archfey known only as the Piper. You gain a +1 bonus to your spell attacks and spell save DCs while holding this instrument. Once per day, you can use an action to play the pipe and summon up to 8 swarms of rats, as if by the Conjure Animals spell.
HEARTS’ RHYTHM
Wonderous Item, Instrument, Uncommon, requires attunement by a bard
Hearts’ Rhythm is a magical drum made from an unknown pale hide stretched over a frame of rich red wood. You gain a +1 bonus to your spell attacks and spell save DCs while holding this instrument. In addition, if a creature that you can see within 30ft of you drops to 0 hit points while you are holding this instrument, you can use your reaction to magically cause them to drop to 1 hit point instead. Once used, this property of the drum cannot be used again until the next dawn.
AUREAS
Wonderous Item, Instrument, Very Rare, requires attunement by a bard
Aureas is a sentient magical viol, made of an incomparably beautiful golden wood and stringed in pure gold. It is believed to have had an otherworldly origin, though whether it was fey, fiends or even gods that built this instrument no one knows, and Aureas itself will not say. While holding this viol, you gain a +3 bonus to spell attacks and spell save DCs.
Spells. You can use an action to play the viol and cast one of its spells. Once the instrument has been used to cast a spell, it can't be used to cast that spell again until the next dawn. The spells use your spellcasting ability and spell save DC. You can cast the following spells from Aureas: Mass Suggestion, Synaptic Static, Confusion, Compelled Duel.
Sentience. Aureas is a neutral evil sentient instrument with an Intelligence of 12, a Wisdom of 10, and a Charisma of 16. It has darkvision and can see and hear out to a range of 120ft. The viol speaks telepathically to its attuned bearer and can speak, read and understand all humanoid languages, as well as Infernal, Abyssal and Celestial. Aureas considers itself the most beautiful and beautiful-sounding instrument in creation, and urges its player to challenge even fiends and gods to prove as much, without care or consideration for consequences. It holds itself to have had no maker, and to have been formed from the raw essence of music itself. Above all else, Aureas is prideful, and encourages both pride and competition in anyone attuned to it, urging them to demean and crush those who would challenge, defy or insult them.

While spellcasters can easily extract the magic contained within arcane crystals, forging said power into something less volatile requires a great deal of skill. Some artificers have managed such a feat, though, and have built weapons out of them. Magical blades are often far more effective than most non-enchanted metals.
Second sword for Swordtember! This time I got to mix holographic (as in hologram) and some iridiscent materials, such as titanium quartz and tarnished chalcopyrite. I also pushed that cyberpunky vibe here. This one now belongs to @rbillustration, but if you're interested in others, check out this post!

Someone in Twitch Chat, "Make a magic potato!" "Where does this fit lore-wise in a world?" I imagine an elderly, retired adventurer grandma who has turned bounty hunter. She travels around now, offering people some of her famous potato salad. But she never eats it herself, no one ever notices until it's too late and she's taking them in as a spud.
Magic Potato
Wondrous Item, uncommon
“It’s just a normal potato. Nothing unusual about this at all…unless?”
If a creature takes a bite of the magic potato they must make a DC 12 Charisma saving throw or be turned into a sentient potato. While in this form they retain all of their mental stats, and can communicate telepathically to a range of 20 feet. They have an AC of 8 and only 1 hit point. If they take any damage, they revert back to their original form. The effects of the magic potato last for 24 hours.

Hey folks! It took WAY too long to get this out, but it's finally done. Well, technically it isn't done, but I'll get to that in a bit. One weird thing that needs explanation at a time.
So, as one can tell, I didn't just make statblocks for the behemoths, write some lore, and call it a day. I've discovered that I really don't like making statblocks and dealing with all the nonsense that comes with it (CR is a joke, and I'm not laughing). So, in typical Foundry fashion, I tried doing something weird that requires more work and ultimately still required me to make statblocks anyway. Yeah, I'm not smart. 
So I made them templates instead. And while I was tempted to give up and just do the obvious thing since I was just going to end up with stats anyway, making them templates makes a certain amount of sense from a lore perspective and I genuinely think is an interesting idea worth pursuing. Quick lore tidbit, behemoths are likely the result of normal creatures becoming mutated by aether (it's not certain, but there are signs of this origin from what I've read), so a template makes logical sense. So, as long as I pick appropriate creatures for the template examples, the end result will get you pretty close to the behemoths in game. Sure, they're not perfect, but it would be easy to tweak them to better suit your game.
So the other behemoth in the room is there are only 3 behemoths here. I had intended on releasing them all at once. Turns out there's like 30+ behemoths AND 5 basically legendary behemoths. So, I'm splitting them up into their elemental categories, and the legendaries by themselves. I already have the Blaze behemoths written out, so those won't take nearly as long. As I complete each category, I'll update this post and make y'all aware of the additions. By the end, I'll have one document with all of them in it, a brew to rival Flesh & Bone.
