Dear Me,
Yesterday was eventful. I woke up feeling like complete shit. My entire body had been hurting all day. But, I still had things to do.
My doctor messaged me back. They switched my medicine from Methotrexate to Hydroxychloroquine. I know I should probably stick with the Methotrexate. However, my stomach issues say differently. I'll start my new medicine later tonight. I'm sure I'll blog about it. I really hope it helps.
I had to pick up a rental car yesterday. I'm taking my Jeep to the body shop later today. Is it weird that the drive itself is already stressing me out and causing me to flare? It's silly, I know.
Other than how busy and how much pain I was in all day, I actually had a decent day. I honestly have no idea what a pain-free day would be like. I'm crossing my fingers that this new medicine introduces me to one really soon.
Side note: Remember how I mentioned my parents and my little sister all passed away? All I have left from my childhood family is my brother. Guess who I rarely hear from? Who I think doesn't give one solid shit about me? Yeah... It's okay, though. I still love him. I just wish he didn't act like we always have tomorrow because we obviously don't. To be fair, I did miss a few calls from him when I was having a procedure done. But, there was no response when I texted him back. I figured he was probably just wanting to blame me for something I don't even know about. I love him & I miss him. I just don't ever know where I stand with him. I wish you all the love and light, Bubba. Even if I'm not included.

ANA is sensitive for SLE. Anti-dsDNA Abs are specific for SLE. You should get ANA first. It's unlikely that someone with lupus will have a negative ANA (thus ANA is a sensitive test--it rarely misses people who have lupus). If the ANA is positive, then test for more specific markers like anti-dsDNA.
But a lot of disorders have positive ANA, so just because ANA is positive doesn't mean the pt has lupus. That's why you would then test for anti-dsDNA.
Hydroxychloroquine is the initial treatment of choice for SLE. Other medications are generally added depending on patient severity. This medication is associated with retinopathy, and routine ophthalmological exams are recommended.
Hydroxyurea is for sickle cell disease. I often mix it up with hydroxychloroquine, which is for SLE.

In the early days of the COVID-19 pandemic, Mehmet Oz—aka Dr. Oz—repeatedly emailed top-level Trump administration officials, urging them to push the ineffective malaria drug, hydroxychloroquine, to treat COVID-19 based on scant, sketchy data from a now-disgraced French researcher.
Emails from the notorious celebrity doctor were revealed for the first time Wednesday in a report from the House Select Subcommittee on the coronavirus crisis. The report, titled "A 'Knife Fight' with the FDA," delved into how the Trump administration worked to undermine, pressure, and bully the Food and Drug Administration during the pandemic.
Specifically, it unearthed how the Trump White House pressured the FDA to bend safety standards so that COVID-19 vaccines could be released before election day. It also revealed the tenacious efforts and subterfuge by top Trump trade adviser Peter Navarro and adviser Steven Hatfill to pressure the FDA into supporting the use of the debunked malaria drug. The report's title stems from a direct quote from Hatfill that the White House had a "knife fight scheduled with the FDA" over hydroxychloroquine.
Both efforts were ultimately unsuccessful—the FDA did not authorize vaccines before the election, nor did it re-authorize hydroxychloroquine after it was clear it was not effective. But the administration's actions caused lasting damage to the FDA, the country's pandemic response, and public trust, the report concludes.
"As today's report makes clear, senior Trump Administration officials undermined public health experts because they believed doing so would benefit the former President politically—plotting covertly with known conspiracy theorists to dangerously push a disproven coronavirus treatment, bullying FDA to change its vaccine guidance, and advocating for federal investigations into those who stood in their way," Select Subcommittee chair Rep. James Clyburn (D-S.C.) said in a press statement. "These assaults on our nation's public health institutions undermined the nation's coronavirus response—and are precisely why we must never again settle for leaders who prioritize politics over keeping Americans safe."
OZ’S ROLE
But before Navarro and Hatfill spearheaded efforts and "work from the shadows" to champion a junk COVID-19 remedy, there was Dr. Oz, who was equally eager to promote the unproven treatment.
Oz, who has a long history of peddling unproven treatments and health scams, was quick to jump on the hydroxychloroquine train. Days after the small, dubious French study—led by a now-disgraced microbiologist—suggested that hydroxychloroquine was 100% effective at treating COVID-19, Oz sent emails to Trump White House Coronavirus Response Coordinator Deborah Birx and Trump's son-in-law and White House senior adviser, Jared Kushner, pushing for hydroxychloroquine.
On March 22, Oz sent Birx a series of emails complaining about a "coronavirus drug shortage" and prodding her to help expand access to hydroxychloroquine. He also claimed that the drug had "confirmed clinical benefits." The next day, Oz emailed Kushner, saying that Trump should "push academic centers to move more expeditiously" on hydroxychloroquine trials. Kushner responded, "What do u[sic] recommend to speed it up?"
In those emails, Oz mentioned plans to run a clinical trial of hydroxychloroquine with his own money. Earlier this year, a representative for Oz confirmed to the New York Post that Oz spent nearly $9,000 of his money to buy more than 2,000 hydroxychloroquine tablets in 2020. And he was reportedly prepared to spend $250,000 to fund a clinical trial at Columbia University.
On March 28, Oz emailed Birx again about the French study. Birx forwarded his message to then-FDA Commissioner Stephen Hahn with the note, "We should talk." That same day, the FDA granted a controversial emergency use authorization for hydroxychloroquine and chloroquine, allowing use against COVID-19.
In June, the FDA reversed the decision, sending Navarro and Hatfill on an unsuccessful mission to get hydroxychloroquine reauthorized. And numerous studies have subsequently found the drug ineffective and potentially harmful for treating COVID-19 patients.
Oz, a crudité aficionado who is now running as a Republican Senate candidate for Pennsylvania, ultimately scrapped the hydroxychloroquine clinical trial idea and reportedly donated the pills to an unnamed hospital.

Edit: I just heard about the hydroxychloroquine stock. That’s not why I hate him but that doesn’t make him look any better, even if the rumors are false.
I’ll just say that the reason involves animals.

Kinda frustrated.
My rheumatologist got back to me about going on Hydroxychloroquine/Plaquenil (which my orthopedic dr recommended and sent to her for approval)… after 2 weeks. I was approved!!! So all should be good right? Nope.
According to my pharmacy, they send an ICD-10 code that didn’t match the prescription, so my pharmacy faxed the rheum for clarification this morning. It’s now 4:14 PM and still nothing back from the dr… Ik that they’re busy! I just wish it wasn’t consistently so slow :(( Especially because I’m in a lot of pain all the time, and this medication can take 1-2 ish months to take effect. I would appreciate it being correctly prescribed sooner than later.
Anyways, I AM grateful and lucky to have the privilege to get access to specialists and medication. I just am a cranky old 23 y/o. /lh

Hello!
I just got a working diagnosis of "undifferentiated inflammatory arthritis" and all but AS and nonradiographic AS have been all but ruled out for me, and my father has AS and his whole lower spine is fused (partially surgically from a fracture).
The question I have is: have you ever tried hydroxychloroquine? Or anyone who follows you with AS/inflammatory arthritis? I would like to know how it went if so, how long it took to have an effect especially.

in my non-professional opinion if that was me i’d definitely assume i had AS until proven otherwise, like honestly with the family history i’m shocked you haven’t received that diagnosis yet? but anyway! i haven’t been on hydroxychloroquine, i’ve done prednisone (ranging from 5-40mg, longest duration at 20mg every other day) and humira/adalimumab (40mg every other week and now weekly).
i’m actually hoping to get on hydroxychloroquine soon to potentially treat what is most likely lupus so i will definitely update you if that happens! i actually haven’t heard of hydroxychloroquine being used for AS (mainly for lupus and sjögren’s) and neither had my gf who’s in med school, i did some googling and found this:
Treatment recommendations for AS recommend against using SAARDs like hydroxychloroquine to manage AS, as there is little to no evidence for the effectiveness of SAARDs in treating AS and newer medications like TNF (tumor necrosis factor) inhibitors have demonstrated more effectiveness.

i’m definitely not ruling out that you could experience some relief, especially if something else is going on like joint pain from sjögren’s, but personally i would recommend taking it very seriously if you don’t feel your symptoms are being improved and to be totally honest that does not inspire confidence in your medical providers for me
