Team MLV vs the Ultra Brothers: Who will win?

Either way, let’s also set up 3 basic rules. The first concerns both sides, the second concerns only the Ultra Brothers, and the last only concerns only Team MLV, because if they have this, they WILL win.

—1: No enhanced forms or fusions.

—2: No reality manipulation or dimensional manipulation or whatever you were talking about in my last qn. I don’t know where Taro and Ace showcased this, but even if it was in their shows, it’s clearly considered non-canon now. I mean, why wouldn’t they have just used that to find the Kingdom in UGF3 if it was canon?

—3: No elemental abilities, ‘cause if Team MLV has this, even if Vylcan’s the only one using them, the Ultra Brothers have no shot, especially given the other rules. I mean it, if their elemental abilities were allowed, Vylcan could probably just amplify himself enough to destroy any one of the Ultra Brothers with only a few punches.

Anyway, I’ve sent you a doc with all of the abilities of each member of Team MLV. (Btw, recall that Ultraman Adam is my name for the OG Ultraman).

Point 2 is gonna rub some feathers tbh cause the reason why they dont use those abilities anymore is the same reason why main series ultras dont just show up and instantly annihilate Tier 2 kaiju even tho theyve shown very clearly that they could, same reason why Tartarus dosent also instantly destroy US Zero since it was clearly shown in UGF3 ep 10 that even the upgraded US Zero barely makes him flinch (recall that Seven was also involved in that fight thus I can say that AT's damage was due to Seven rather than him, consistent with the fact that he took a hit from Shining Ultimate Zero and was still standing AFTER getting a beatdown from the Leo brothers), same reason why PI Belial didnt also instantly dissipate Geed and Z as well despite the vast difference in power and why can Deathcium Ray Burst knock down PI Belial as well

Its all to make a good show, otherwise fights would end in under a few seconds, and the entire displayed conflict between the Ultras and the Absolutians would also end in a single ep since I pointed out that there simply isnt enough Elite Absolutians to handle even the entire cast of UB-level Ultras

Otherwise by your rationale, the infamous Ginga S episode 12 meme where the Zoa Muruchi that solo'ed both Ginga and Victory would be universal level

There are fans that are for and against those feats still being considered canon or nah. Personally, considering the other ridiculous feats of power we've seen beings other than Ultras have protrayed, as well as how powerful Ultras get as they grow older, Im of the opinion that those hax are canon, and they only reason why they dont showcase those abilities anymore is as said, to have a more interesting fight sequence

(Additionally as I also answered before, TsuPro wouldnt want to have to address a situation of "Hax vs Hax" regularly cause it inevitably introduces an extremely convoluted system, iirc i talked in depth about this in the Chaos Darkness vs Sphere ask)

And with good reason too, as the franchise has tried its best to shift away from delving into higher and higher powerscales beyond New Gen Ultras (Movie form++ like US Zero is the current cap) or else theyll eventually encounter the same problem like Dragonball by writing themselves into a hole whereby the in-universe/multiverse/omniverse scaling cannot keep up with its inhabitants (unless they would have a do a metaversal level of retconning every few years just to avoid this issue, which is an absolute pain in the ass to do due to its implications like what Marvel and DC comics do from what I can understand), which was something they alr did up till Taro which was why u saw that they toned it down so much up till now, with the exception of rare occurences of Legendary Ultras and High Tier fusions obviously

(Note that we're currently not even vibing at Multiversal scales mind you, low multi-universal at best with the current UGF plot, expect things to stay this way for the foreseeable future until TsuPro decides to shift their target audiences to young adults)

Thus Im vibin with this canon, and by removing those abilities, youre bascially nerfing them as far as Im concerned

But again, this has too many characters in a single ask, recall I told u max 2v2 yo, unless u can generalize 2 groups in a short summary via numbers or sth within the ask itself (or summarize and highlight the main attributes like base power, multipliers or sth, not just a reference scale to yet another OC which we dk the scaling for) cause I sure as hell aint gonna read all that in the link u sent just for an AU ask

why there was never an episode of doctor who where the doc and his new companion/s on their first adventure together accidentally go to the same place and time one of the previous doctors were in, saving the place and people from some historical disaster... it would be so much fun the new companions trying to save the same people, not listening to doctor and doctor not wanting to ruin their first adventure together, not telling them there’s actually old him/her here so they don’t have to explain about regenerations but still trying to stop the new companion from actually making it harder for the old doc and companion to save the day...

Synopsis; Where the team discusses the question ‘do you kiss after head’, you find out Spencer has too little experience to answer the question so you help him out

a/n; LMAO im so sorry for disappearing again life has been actually kicking my ass but anyways lately i’ve been thinking about subby early season spence so here we go,, hope you enjoy!

Another Friday night and the team was out bar crawling after an easy case. But this time all members were there as it reached 11pm which was rare. Usually Hotch and JJ would have been home by 10:30 and Spencer wouldn’t have been there at all. But there was something light in the air which had all parties concerned sitting packed in a booth, laughing after each sip of their drinks.

Since it wasn’t your first rodeo together you knew how the night went. It started off with Rossi offering to buy the first few rounds, always whiskey but he made an exception for Penelope. Then again who would deny her anything.

Once the drinks were flowing and lips got a little loose, the questions would start popping in at the top of your heads. However these were not your run of the mill, ‘hows so and so doing?’ ‘done your taxes yet?’ oh no. The name of the game was discuss where you would all think of a question which would help you dig just a tiny bit deeper into your coworkers sex lives.

Maybe if you were all sober then you’d avoid thinking of each other in such positions, pun intended, yet in this state your prying minds were open and your stomachs were ready to grow abs from bending over in laughter.

You raised the margarita glass up clinking it with a fork to get the tables attention. Everyone including Aaron had a smile on their face, ready to hear the intrusive question for the night.

“Ok my fellow profilers, doctor, and tech genius,” you added pointing at Spencer then Pen, “Do you kiss your partner after they give you head? Discuss!” you finished in your most formal voice.

Right as you took a swig of your drink the mixed responses of yes and no filled your small space.

“Why wouldn’t you? You guys especially, if someones willingly trying to swallow then you damn well owe them a kiss,” Emily finished earning nods and ‘exactly’s from JJ, Pen, and yourself.

“Ok but thats weird. I just can’t explain it but its a no go for me,” Morgan finished. This only gained him a scoff and raised voices, “Hotch man help me out here,” he said looking over to the man hiding his smirk behind the amber liquid.

“I have to agree with the ladies here Derek,” he said curtly.

The girls yelped and hooted at Hotch for siding with them while Morgan sat with his arms crossed being the singular person left out as even Rossi agreed. Meanwhile you noticed the presence next to you had shrunk back and wasn’t too active in the conversation.

“So Spence do you kiss your partner after they,” you trailed off shaking your fist by your cheek and poking your tongue in the side.

He coughed as he instantly sat up quicker. Even under the dim lights of the bar you could still see the blush creeping up from his neck to his ears and the slightest tint on his cheeks.

“Oh I uh- I never-” he said looking anywhere but your eyes.

“You don’t kiss them?” you said raising your brows.

“No! I-i mean yes. I would I think b-but I haven’t had the chance to actually partake in such.. activities,” he finished finally taking a look into your eyes.

You could tell he was waiting for you to laugh in his face for being so inexperienced but you felt far from it. If anything you wished you could be the one to show him things.

That sweet boy had you wrapped around his finger and he didn’t even know it. Maybe it was his naivety considering how exceptionally smart he was. Or maybe it was the cute sweater vests he wore and now he nervously tucked his hair behind his ears. All you knew was that you wanted Spencer Reid and tonight was your night to make it happen.

You hummed taking in the information, “Well that’s not a bad thing Spence. Everything takes time,” you said putting your hand on his arm for comfort and giving him a smile.

Going to turn back to face the table you almost didn’t hear Spencer go to speak again, “Do you?”

Got him.

“Why don’t you find out pretty boy,” you said with a wink as you downed the rest of your marg. In the corner of your eye you could see Spencer shifting in his seat, subtly moving his bag to cover the slowly growing tent in his slacks.

As the night went by you couldn’t help but really give him a show. You had popped open a button or two on the long sleeve you had on, since it was getting stuffy in the booth. Though when you leaned forward and jutted your chest out, the soft inhale of a breath from the man next to you was just serving as motivation to get bolder.

For the last hour you called it quits on the alcohol and drank a few glasses of water before you drove home. Spencer had been uncharacteristically quiet ever since your little interactions.

The team had all gotten up to say their goodbyes. Rossi going by and giving everyone a kiss on each cheek. Derek having to quite literally rangle Penelope from talking to passing by groups on their way out. Then there were two.

You turned to the side where Spencer was nursing on his coke, “Hey pretty boy, it’s late, let me give you a ride home,” you said grabbing your belongings.

“Y-yeah ok. Thanks Y/n,” he said getting up. You’d noticed how he still had the burnt orange bag over his crotch. He couldn’t still be hard could he? Well you’d love to find out.

As gentlemanly as he was, Spencer opened the door for you to exit the building first. The whip of fresh night air cooling on your exposed chest and legs under your skirt.

You unlocked your car and stepped in, Spencer waiting to hear the little beep signaling his side was open. As he sat down you heard him let out a little whimper. Your head shot over to look at him, you could tell from the flush on his cheeks he didn’t mean to let the noise out.

Holding in your chuckle you started the ignition and pulled out of the lot, “Can I put on some music?”

“Yeah I don’t mind,” he said looking over at you with his lips in a line. If it was anyone else, they’d probably think he was uncomfortable but you loved his tiny awkward smiles.

The ride to his apartment was mostly silent besides a rare quip from Spencer about paper work or fact about an older building you had passed by. It fascinated you to no end hearing him talk. Spencer was a hand speaker, meaning he always used his hands waving them around and making gestures. The pale digits had you captivated. Probably a driving hazard but you couldn’t help but let your mind wander.

You pulled up into one of the visitor spots and put the car in park. You looked over to see Spencer almost contemplating something. You’d seen the look on his face before when he was looking over puzzles.

“Somethin on your mind Doc?” you said with a small smile. As cute as he looked when he was nervous, you’d never want him to feel uncomfortable around you.

“Would you-,” he cleared his throat, “Wo- Would you maybe want to c-come inside?”

“Of course Spence I’d love to,” you finished with a reassuring nod.

As he led you upstairs you were giddy with anticipation. So what if nothing happened. He was your friend first and you were glad he was letting you into his personal space. Even if you wanted nothing more than to have him writhi-

“Y/n?”

The door closing snapped you out of your thoughts. You didn’t even realize you were in his living room. The dark green walls and shelves bursting with books put a grin on your face, “Sorry Doc, just caught up in my thoughts. What did you say hun?”

His brows practically raised to his hairline from hearing the pet name. While he was used to the names coming from Garcia they took a whole different light coming from your lips.

“I was asking if you wanted water or something,” he said fiddling with the keys in his hands. Eyes darting everywhere but your face so you wouldn’t be able to see the flush rising on his cheeks.

“No I’m fine thanks for asking though,” you said taking a seat on the worn leather couch.

You reached for the tv remote making a face at Spencer to ask for permission. He nodded and you settled back turning on an old sitcom that played late at night.

As the episode ended you both sat in silence. Again you didn’t mind but you could practically hear the cogs moving in Spencer’s brain.

You were about to speak when he cut you off before you could even get a word out, “What did you mean by ‘why don’t you find out’.”

Gaining confidence you moved closer to where he was on the couch, slow enough for him to stop you in case he wanted to back out.

“Well you have options pretty boy,” you said moving a leg to straddle him. Your hands instinctively going to his brown locks. You could’ve sworn you heard a little moan leave his chapped lips. Noted.

“W-what are the options,” lust blown eyes looked up to yours.

“One, you can put that mouth to good use on me,” you said trailing your finger over his bottom lip, “and let me cum over that pretty face.”

Hiiii maybe LL!Ren for the character headcannon thing?

hey!! yes!! im really glad i get to talk abt ren bc he lives in my head rent free At All Times :) ty very much for requesting him

Realistic: While Ren did want to win, i mean thats kind of the whole point of the game, he never was truly passionate about it? Like, he saw how it was a hollow victory last season and being a natural two lifer, he didn't see his chances as too great. The driving force for making alliances and gearing up was mostly survival instinct. He would try, of course, but he woudn't go as far as betraying allies or doing anything else crazy like that, this game wasn't worth it.

Well, he used to think so. But BigB saying that if they win, a part of his queen wins with them? That stuck with him, for some reason. He is willing to go far futher, now. A far lot futher.

He needs to win, after all.

Funny: He still is very much a werewolf, but he does not transform on full moons (bc server power supression, regulation to make the game more fair etc. etc. whatever, he just naturally is a bit dogboy anyways). Instead, he just kinda,, goes crazy goes stupid? think this post without the transforming basically.

It is especially incredibly inconvinient as this is Very Likely To Happen during the mysterious and very important meetings with Martyn/the Shadows. Sometimes this just makes him get into their weeb roleplay even more, more likely than not it ends up with furniture being nibbled on and everybody else incredibly tired with how much affection he demands. Usually Martyn has to console him bc he is very embarrassed afterwards. This is a routine they have developed in Dogwards. They Are Used To This.

(And yes, this is the reason he set himself on fire and we didnt see it in his episode. Fuck You if that wasnt even a full moon. It's also why Martyn insists on teaming with him afterwards, he clearly needs somebody)

Sad: He made a grave for his queen. Well, not a grave grave, her body is with the Joel and the other reds, and Ren doesn't think he even has half the claim to it as they do, but as close to closure as he could get. He gathered every trinked and item he could find that felt connected to her (I headcanon time to be much slower for them, so the recordings take a few days and there are days of down time between the recordings. You would be suprised how much Stuff piles up when you live with somebody for a few weeks) and burried it in the ashes of the fairy fort. Despite everything that happens late game, he always makes sure to go there and take care of it a bit, restore parts of the fairy fort maybe. He feels a bit silly, doing so in a death game, but he feels like its part of his duty to her. Like he owes it for not being able to protect her.

Canon who: OK SO this isnt entirely original, but!!!! do u know the post thats like "3rd life happend in a millisecond on the hermitcraft server, and when everybody returned they werent even sure it actually happend? and ren didnt even know if martyn actually exists?"

PLEASE if somebody knows where to find that post send it to me, its a banger and i wanna credit it properly

So take this and extend it to Last Life. Imagine Ren waking up after being killed, having watched first Lizzie, then BigB, then Martyn die, die, die. Imagine him opening his eyes in a panic, ready to scream for help from his allies, only to see... Doc? Looking concerned at him? Asking "Are you alright, man? You blanked out for a second there."

He is in a meeting. Doc looks expectantly at him to explain the new developments of the octagon. His whole life has just changed.

But it has been just a second. And every other second that passes, his memory gets more blurry. Did he really imagine that? Was he going crazy? Did he make them up? Make up three whole new people important to him? After all, most of the other players in that game had been hermits, except the ones closest to him, for some reason.

But no matter how much time passed, he could clearly remember their faces, and you know how they say that you can't make a new face up in a dream? He hung onto that.

Weeks later, and he doesn't remember why, but he metioned it to Pearl. And she told him about a little server she was on once called Evo, and that she had known Martyn and BigB there. She admits not knowing where they are now, but it is enough to make Ren determined to find them.

She also mentions another server, one she still frequents today, and while it doesn't usually have visitors, she promises to arrange something.

Time passes again, then Ren finds both Gem and Pearl at his front door and they take him on a journey to a world of empires.

It's where he gets to see his queen again, and this time, she is more royal then she has ever been before.

Grif

For the character ask! Sorry this took me a while, I can't copy and paste on mobile, so I waited till I got on desktop tumblr! Thank you so much for asking about him, I could go on forever lol so this is gonna be fucking LONG. Please, anyone, feel free to ask about any other character, too. These are fun.

Why I like them: Grif is, for lack of a better word, complex. On one hand, it's like, okay he's just a funny comedy character. But on the other hand, he's like every all of my favorite archetypes of a character. He's crafty but stupid, selfish but selfless, antisocial but friendly, the straight man but the dramatic guy, he's snarky but oddly caring: the reluctant hero. It's kind of paradoxical, and I feel like not only do I relate to him, but I just really enjoy him overall. Whether it's because he's a good brother, being a complete dickbag because he doesn't know how to handle emotions, being a complete cynic on the battlefield, or being hyperactive and snappy, it's just... ah. He. Everything in his life just happens, and he has to deal with it, and yeah he'll kick and pout and probably eat everything but in the end, he's gonna choose his family and he cares deeply... even when he can't really show it.

Why I don’t: I see this a lot in myself too - the fact that he's impulsive and inherently negative when he speaks specifically. his words don't match his actions. He often doesn't hesitate when it comes to making negative comments because they are easier than saying something nice, but what his intentions are are completely different. in other words, he's a jerk, lol. Especially in season 15. I know people felt bad for him because he was partially right and went insane on Iris, but he lowkey deserved it. He said he hated his friends, and even his closest friend. He didn't want to admit that he was a good person just because he didn't want to help. yeah, it was valid, but he needs to learn how to make a case without fucking everyone emotionally and being so clammed up. >:/ sometimes it feels like he regresses in character, as much as he's matured. i guess that's realistic and just the writers making comedy, but also the way he handles Doc specifically irks me. so mean spirited for no reason, as funny as it is.

Favorite episode (scene if movie): OH MY GOD, okay, literally, every single episode with Grif starring as a main is fucking gold. I think for this I'm going to say, uh, This One Goes to Eleven. Even though it's not Grif-centric, it's the episode that's my favorite overall because it introduced me to RvB and made me like Grif right away, simply because he was attacked so much and I felt so bad for him. Another great one that sticks out right now in my sleep deprived state, is Grif does a Rescue. Augh. And the episode where Grif and Simmons get stuck underground in the caves.

Favorite season/movie: Season 8 (shotgun!!!, hyperactive ai grif), Season 11 (hanging in the canyon with simmons), Season 4 (the tank and blue simmons w/ grif), Season 5/6 (kai and rat's nest), Season 12/13 (the recruits, grif building the snowmen), Season 14 (backstory with simmons, Room Zero), Season 15 (you know why)

Favorite line: OK, don't make me choose. There are SO many that are good!!! I think one of my most favorite things that Grif says is "yoink!" It's so adorable! I also really enjoy "BLUEEE TEAAAAM SUCKKKKKS" with his epsilon double, the whole "invisible nap" scene, "what are we, on a date?"/"I can tell you what we weren't doing", "no one made me, I made me", "WERE GONNA FUCKING DIE" when charging at the meta, "that's a figure of speech?" [when carolina says im so hungry i could eat a horse is a figure of speech], "dexta grif he who shall not be messed with!"... I'm sure I'm missing a lot, he has SO many snarky funny lines, but these are some off the top of my head.

Favorite outfit: LOL THIS IS SUCH A FUNNY QUESTION BECAUSE THIS IS RVB. HAHAHA. Uh. Season 6 probably. I just like Halo 3 Graphics. Also s14 Room Zero because THEY DREW HIM FAT CANONICALLY. THANK YOU.

OTP: I'm with the majority of people in the fandom who like Grimmons! I think Grimmons is the only ship that I really vocally ship with my whole heart besides OC ships. I just very much enjoy their dynamic -- it's very angsty, dialogue full, intimate yet so unspoken. It's just a really good pair to write about and see the development of through canon. And, not to mention... season 15... hrk...

Brotp: I really REALLY want grif/tucker, grif/church, grif/locus BROTP. SO SO SO BAD. I've always seen grif and tucker as bros, grif and church are HILARIOUS together and we were ROBBED of more time together, and grif and locus are fucking adorable.

Head Canon: I have a lot of headcanons about Grif, but one of my favorite ones is that he has half-lidded eyes, like he's always sleepy. I also headcanon him as bisexual, though I think that is a popular headcanon!

Unpopular opinion: I dunno if I have super unpopular opinions about Grif... maybe that I think that his labryinth wasn't as bad as it seems at first? A lot of people seemed to think that though, yknow. Like if you look deeply into it, it's actually kinda fucked. But I feel like we should have gotten the Hawaii scene anyways. Hm. I also didn't like how they altered the canon so that Grif wasn't drafted. I think it does add something to his char that he chose to go, but I always really liked the aspect that Grif didn't control that, and yet he still did this on purpose. He was good on purpose.

An oh-god-please-dont-ever-happen: Have him betray the reds -- i think every other red has betrayed the team at some point. please god for the love of god dont do this to grif. it really adds and says something about his character that he doesn't ever betray his team.

The stuff that Wash said about owning up to your mistakes... that was good. Focusing on what you do in the present instead of what you couldve done better in the past is SO hard sometimes

I loved everyone's introductions this episode, yunno the "im tucker, breaker of head boards!", I also loved how every except caboose got a sword lol

And caboose getting Genkins' golf club was fun 😂

Oh and also when they were suggesting hammer names and even Wash, Lopez and Atlus were joining in 😂😂

And the pink guy Donut did in fact steal the hammer!!

Honestly good for him. He should team up with Doc/omalley, they both deserve to be a little evil imo

SPOILER FOR 4.01 AND POSSIBLE SPOLIERS FOR PROMO FOR 4.02. READ AT YOUR OWN RISK.

So now that I've slept on it (or tried to at least) I definitely think the Nicole in the Garden isn't our Nicole. I've seen a lot of people say something along the lines of "shes dead and shes gone to heaven". I'm not religious and I know this version is far from traditional but I don't think the Garden is supposed to be heaven?

Others have thought that Nicole may have fallen into a portal/door or something similar. While yeah, it's possible, feel like thats too simple. That whole part felt kind of like a red herring. We're supposed to think that Nicole's gotten into the Garden.

I stand by my previous theory of the Nicole in the Garden being another version of her. The books (I think they were books?) that Waverly had to choose from had names. While she was losing her memory by the campfire with Doc, it was Nicoles name she was repeating. So I think, if their memories are compromised, Waverly would pick the one that stood out most to her- Nicoles.

Another thing: the Nicole we see in the Garden is reminiscent of season 1 Nicole. Braid and all. When Waverly was trying to remember her, she mentioned the stetson, which we haven't seen Nicole wear since season one. I think that, if Waverly did choose Nicole's, the Garden is trying to figure out who would be best to convince Waverly to take the throne (or something along those lines) and using her remaining memories to conjure a version of her.

Furthermore, that promo for the next episode? It just doesn't seem like Nicole.

1. There's no way in hell that Nicole would just stand there with her tatas out while Doc looked on.

2. Reunion. While I know they wouldn't put a wayhaught reunion in the promo, from what we have seen it didn't seem right. I don't feel like Nicole would just stand there after worrying about Waverly so much. The "Its really you" and "who else would I be?" seems suspicious to me. Not to mention the "I have got to get out of this cage". I think whatever is feasting on blood is using Nicole's image.

3. Wayhaught. We see in the promo that Waverly takes the throne (or tries to) as Wynonna tries to stop her. If Nicole was really there I pretty sure she would be right beside Wynonna trying to stop her kind of fiancé from possibly turning to stone.

No. I think that Nicole fell into the basement, but not into a portal or to her death. There was a spoliery article last week (I don't remember which one) that mentioned both Nedley getting injured at the stairs AND Nicole getting hurt trying to open another portal. I think Nicole might find the portal before Wynonna does, but is possibly too injured to go through it- so instead stands guard with Rachel while Wynonna goes in (cuz you know- Zombies).

In regards to Nicole's seemingly longer hair during the rest of the season- I've not really got a proper theory for it but atm I'm thinking something to do with time. Waverly said something in passing in the Garden. Something along the lines of "How long have we been here?" It might have just been a throw away line, but what if time does works differently there? We see that Wynonna ends up in there. What if when they get back out, they realise that time was moving slower there? It would possibly explain why- assuming still that the real Nicole is outside of the garden- Her hair may have grown with time.

Maybe the new portal that they find closes after Wynonna goes through it and Nicole has to find another one? Maybe she tries to find out where the government dudes took the rest of the town in the process? We know Jeremy gets all MIB wherever he is. Maybe he helps find another portal, or opens the old one. Who knows...

Edit 2:

Some folks have pointed out that in the story of Adam & Eve they were all nakie too. Maybe if the Garden created this version of Nicole, it wouldn't have known to put clothes on her?

Gotta remember that this is Wynonna Earp. I don't think Emily and the writing team would have made things easy to figure out. Things are rarely as they seem on this show and they're definitely not straight forward.

Is it possible that Im over thinking? Absolutely. But what I know for sure? I've missed this show so fucking much. And the live tweets. I've missed it all.

Watching House as a Physician. Season 2 Episode 3. Infectious diseases & Respiratory.

Welcome to another episode of medicine done badly. I’ve been watching House on Amazon prime. Got the subscription during the pandemic, as like everyone else, I’ve garnered an online shopping habit now.

Alright. In the opening scene a young roof worker falls off the roof presumably due to acute shortness of breath. i.e. trouble breathing. (why do we use the term shortness of breath? it’s the english version of the greek term dyspnoea - the actual preferred language of Western doctors. Fuck do I know why we like Greek and Latin so much. Moving on.) Then cut to Dr. Cuddy examining him in the back of the ambulance.

This would never happen in real life. Yes you can be on the scene and handover to the paramedics or EMT when they arrive as a doctor. But they would take over. I personally wouldn’t have the balls to look after a patient in a different environment, different resources and field I’m not familiar with. You can have field Emergency docs - but requires different training.

Also, ethically, you’re not meant to treat family or friends. Dr. Cuddy later in the episode gets a bit emotionally involved - this is why we don’t treat people close to us. We lose objectivity. We make mistakes. And you see later see Cuddy do some pretty bad ones.

I feel like much of this episode is not really IM. THere’s less differential diagnoses being made. More side tracks into trauma, emergency, intensive care or vascular surgery.

Anyhoo. Trauma and emergency would manage the fall and post fall traumatic injuries. And the trauma protocol was either not shown or completely off in this episode. Surgeons don’t seem to exist in House, at least not very much. Similarly, no other doctors exist except surgeons in Grey’s anatomy. Also you can’t clear a C Spine clinically, which is what Dr. Cuddy does in the back of the ambulance. You’d need a CT first and clearance both radiological (by a radiologist) and a clinician.

Aaaanddd, you can’t just listen to the chest and go no pneumothorax (air in lung or collapsed lung) - yes it’s reassuring, but again you’d need imaging to confirm this, given how serious a condition this is. It is realistic to consider in the setting of a fall, particularly if there are rib fractures that can puncture the lung.

Once the more critical injuries are managed, we would look after the IM side to things.

So. Finally.. differential diagnoses.

Takes what seems and feels like days before they finally sit down and go through differentials. Really not much on that white board. Dark fingers, broken ribs, fever and lung infiltrates. Time line’s not clear on when he developed the fever.

Presenting complaint isn’t really addressed. It could be: - Dyspnoea, leading to the fall, he’s requiring O2 via nasal prongs, which suggests that he’s hypoxic (this is definitely odd in a young guy who’s normally very physical fit if he works as labourer). so much to unpack here, but they never get into this well. Post fall, Cuddy notices his ring and pinky finger becoming dusky, which becomes very central in this episode. Very few things would cause this. pains me that they do no differentials on a white board for this alone.

Then a lot of throwing around medical terms.

PTT prolonged and Fibrinogen off. These are markers of your coagulation pathway and signs that you’re not forming the clots the way you should if you have an injury. DIC is also thrown around. What is DIC? Disseminated intravascular coagulopathy. Certainly severe sepsis and trauma can cause this and lead to severe bleeding. It will throw off your coagulation pathways (things that stop bleeding). It’s not common. I’ve treated it once, while I was rotating in ICU, it is not standard ward medicine practice. Standard therapy is fresh frozen plasma (FFP) and even large metropolitan hospitals only have a limited supply. It’s a huge concern for surgery and post-op (as you patient will just not stop bleeding after you cut them open, and if not treated, potentially bleed to death). Cuddy mentions ARDS. Acute respiratory distress syndrome, it could be a complication, but it’s not a cause. Again, falls more into the realm of critical care (a la ICU). However, patient had SOB prior to the fall. Finally HOuse makes the observation. of “what if he was sick before he had his run in with gravity...” Everyone jumps to Pneumonia. And this is where it gets confusing. If he was unwell, the minute he entered the emergency department with a fever and hypoxia, they would have worked him up for any garden variety pneumonia, bacterial or viral. Cultures would have been sent and imaging. Any young hypoxic patient would prompt a closer look at the chest. And no one waits that long to start antibiotics - “sepsis kills” is a slogan often used around hospitals. You have to initiate empirical therapy within 30 mins, to reduce mortality and morbiditiy.

Ordering an Echocardiogram (USS of the Heart) also makes no sense in the context of a lung infection. I would order one, but not to look at the lungs.

Then there’s the most unrealistic thing about this series. Doctors breaking into patient homes.

It is however, a good way to showcase social history. It’d be boring to watch a doctor ask the patient outright about their living situation etc, but it’s far more interesting to see exactly how they live. We try as much as possible to illustrate to each other and ourselves what the living environment and working environment of our patients are like.

In the context of infection, a good social history can point out exposure. As they exemplify by showing dead rodents and mould. This leads to 2 further differentials: Rat bite fever (caused by streptobacillus, something you’d see in the US, but probably not anywhere else), it’s an unrealistic differential in general. And the 2nd is aspergillosis. Okay.. So aspergillus is a mould commonly found in our environment. In fact it’s everywhere around us.

THere’s few times when it’s an issue. It is a concern in respiratory syndromes like asthma or bronchiectasis. And also as an opportunistic infection in immunocompromised individuals. in the context of asthma, it’s not so much the aspergillus itself that causes issue, it’s our body’s over reaction. It’s a hypersensitivity issue that causes inflammation in the lungs or a pneumonitis. We even gave it a name. Allergic bronchopulmonary aspergillosis. It’s still badness, but it doesn’t happen that quickly. We also have specific tests for this, which were obviously not considered in this episode of medicine done badly. In the immunocompromised host (steroid therapy in transplant patients or those on chemo, etc.), you can get the invasive mould as an opportunistic infection. I don’t really understand why they think it would be the case here. Also, killing the bug with heavy duty anti fungals will only give more issues rather than do anything. They start him on amphotericin. this is not standard practice. And now it flips to why amphotericin is not standard practice or first line treatment for invasive aspergillosis. The patient has now become anuric (not making any urine). (First line drug by the way is voraconazole, superior efficacy in trials with a lower mortality rate and ADRs) Also, note that they have just jumped straight to dire renal failure from the amphotericin. No work up. That said, heavy drugs like amphotericin are often a cause, but It’s often temporary with the appropriate supportive measures (stop insulting agents, give hydration, monitor fluid balance), reversible, even if you require temporary dialysis or haemofiltration. Anyways, would get into AKI another day, that’s a whole other post in and of itself. Then his hand is apparently “dying.” There’s pain on light touch, but it’s not a cold, pulseless limb. Or discoloured. doesn’t add up. This now enters vascular surgeon territory. Again. It’s interesting that there’s never any referrals to any other teams. If he has good circulation, I would imagine they would try to save the hand and consider other differentials.

The only time I can think of an emergency amputation in this situation is necrotising fascitiis. That’s the only thing that would occur that rapidly AND necessitate losing tissue or limb. With a young person who’s this ill, there’s often multiple subspecialties involved by this point. I’m also surprised he’s not in ICU.

Then there’s a buncha filler scenes of the cast of house getting emotional. Ho my god, they’ve taken the hand of a young 20 something physical labourer. Indeed, this is badness. Unlike House, we actually are trained to always consider how a patient’s illness impacts their activities of daily living and livelihood.

I find the general population assumes that we practice medicine in a vacuum, we merely treat the clinical illness and ignore everything else. They imagine that we all must be like house.

Actually we try to put things in perspective as much as possible and knowing our limitations in this area, we often enlist the help of friends - physiotherapists, occupational therapists and social workers. They never exist on TV or on the movies. Ever. Unless it’s to portray how terrible it is to be a social worker. From time to time in this episode, Cuddy laments that being chief of medicine is too administrative and she hasn’t been a doctor in years. That also doesn’t happen in real life. If you’re chief you’re still a doctor. You have admin shit to do deal with yes, but you still practice. It’s like being chief resident, in all the TV shows with one of these, you still seem them working as residents, be it scrubs or grey’s anatomy.

Back to the differential. They finally get to endocarditis. Culture negative to be precise. That indeed would explain the bilateral dusky fingers that led to unnecessary amputation. Septic emboli.

Going to stop here, more out of exhaustion now. I’ve created quite a lengthy post. Happy to reblog thoughts on culture negative endocarditis on request later. This is a worthy topic to study up on for students or residents. At least review Duke’s criteria and think about your clinical features like Roth Spots and Janeway lesions or Ouch Osler’s nodes.

The ending is also a far fetched connection to make, but is one that we would consider. In fact, we would ask in detail every time from day one - have you had any exposure to animals. It’s very rare to see someone so young be that sick out of the blue when you’re immunocompetent and have no underlying predisposing conditions. If there’s no focal source, then we would even ask about injectable recreational drugs, exotic travels, sexual health.

Most of the time, patients that sick are honest to their doctors.

But what about..

Frankly, much as we lie as humans, when our lives our on the line, we’re generally pretty honest (sometimes too honest) with the people we want to save us.

Any patient who is young and comes to hospital requiring inpatient admission, they’d be investigated by subspecialties with expertise in certain areas such as infectious disease. The dept of infectious disease would either be home team, or all over this patient as they special in the realm of both common and rare infectious diseases, culture negative endocarditis would have been considered before a hand amputation.

The term, “department of diagnostic medicine is laughable,” particularly when they consider it the only department in the world in the show.

In actuality, it’s a department that is universal and exists everywhere. it’s Internal medicine. Dr. Vivek Murthy, the next surgeon general (and also the last one under Obama) is an internal medicine physician. Ken Jeong of Community and the Hangover fame is also a physician of internal medicine.

Beginning to get the sense that most episodes are going to end with a diagnosis that is either infectious disease, rheumatology or haematology. But generally those tend to be most interesting and give the most plot twists or meaty differentials V.s. a stroke or acute myocardial infarction is fairly straightforward to diagnose.

This is a very twisty episode in all the wrong directions.

Dyspnoea is a very common presenting complaint. There’s a properly done approach to this in the podcast by the Curbsiders by the way.

Conversation between Quintis following Pregnancy Announcement after 3.07

H - I’m Pregnant

Toby relaxed in Cabe’s hold and stared at Happy, not quite believing what she had just said, first he found out she is married to Walter and if that secret wasn’t enough now she was dropping the bombshell she was pregnant. Toby just stood there staring at her as she looked back at him he could read nervousness, apprehension, fear in her face but also a tiny flicker of excitement.

C - Lets give these kids sometime to talk things through, if i let you go Doc are you gonna launch yourself at Walter again?

Toby just looked at Cabe and very slowly shook his head, Cabe slowly released his hold on him and nodded to the others who were all stood staring at Happy and Toby. Paige went to start to speak but no words came out and Cabe shook his head at her telling her to leave it. The team gathered their belongings and left the garage, Walter was the last to leave and turned back looking like to he was going to speak but he just stared, he did then speak

W - I’ll stay with Cabe tonight guys so take as much time as you need to talk things through here if you want

Toby suddenly snapped out of staring at Happy

T - so very kind of you Walt, give us chance to fix the mess you made. Its the lease you could do

H - Doc!

Walter just nodded and left shutting the door behind him quietly.

Happy looked at Toby, she wasn’t sure what reaction she was expecting, she was still processing the news herself but she had expected something but Toby just stood there staring not saying anything.

H - Come on Toby say something

Toby opened his mouth and then shut it again, obviously at a loss of what to say

H - Come on Doc, lets sit on the sofa and I will fill you in and then maybe you could say some words you are usually full of them

Toby just nodded and walked over to the sofa and sat down crossing his leg over his knee and resting his one arm along the back of the sofa and then rubbed his other hand across his face. Happy came and sat on the sofa with both her legs crossed facing Toby just close enough her knees where touching his leg.

H - right, well i guess i’ll start. I’ve been feeling sick the last week or so and then the last few morning I have been sick a few times, coupled with my period being 3 weeks late i knew something was up and then I remembered that day a couple of months back you know after girl with the heart problem, when we were on the hike we went on to stay awake and well you remember what happened in that secluded beach and we thought we would be ok but i guess we weren’t

Toby looked at Happy with the small smile on his face

T - How could I forget that day sweetheart, it’s when I decided I would spend the rest of my life trying to make you feel as good as i did on that day

Happy smiled at him and continued

H - well i took a pregnancy test this morning before work and well you know the rest. I’m not going to lie and tell you it wasn’t a shock and it’s the right time or i’m ready but the more it thought about it today the more I realized that a baby would be pretty amazing and he fact that it is yours makes it so much more special.

Happy paused looking at Toby who had some colour back in his cheeks and wasn’t looking so spaced out anymore

H - I’m scared Toby I need to know we are ok and you are with me on this, I cant do this without you

Toby looked at Happy she looked so vulnerable a look he was not used to seeing on her, he was still so mad over the marriage with Walter but first of all he needed to reassure her he wasn’t going anywhere.

T - Listen Hap I love you, of course i’m with you 100%. It was such a shock especially after finding out about Walter. I just needed to few minutes to let it absorb. You will never be alone again even if we fight or i’m upset i would never leave you you don’t need to worry about that.

Happy leaned over and placed a gentle kiss on Toby’s lips

H - Thanks Doc

T - Look we have a lot to talk about, i need to find the best pregnancy papers and books to read though. My Gynae rotation was only short so we need to research whats happening to your body and our little bun in your oven is going to needs lots of care and extra nourishment to make sure we give him or her the best brain development possible, oh I know I read a study that playing classical music enhances their brain development let me download some on your phone so you can put it on your belly when you go to sleep

H - Doc calm down we have plenty of time to sort all this out im sure one night without listening to the works of Bach won’t do the baby any harm. Now I know we are good and you are on bored with the baby business we do need to talk about Walter

Toby scowled

T - I suppose so, I can't believe you married Walter and kept it from me for years, how did I not pick up on this and how did Collins know

Happy sighed

H - look you know I love you and I will only say this once, I'm sorry I kept this from you but we couldn't risk it getting out and Walt losing his green card

T - I understand the risk Hap but when your in a relationship one which I thought was leading to marriage you don't keep secrets this big. It makes me worry what else your hiding or will hide especially as I couldn't read that you were keeping a secret. Maybe I'm not the world class behaviourist I think I am

H - first you know your an amazing Doctor you weren't looking for deception in me, my bad. Secondly I have no more big secrets and I promise you here and now there won't be any in the future. And thirdly Walter told Collins I wasn't aware he did until you mentioned it the other week trust me I ran him the riot act about that.

T - I need to know Happy is there any part of you that wants the marriage with Walt, I know you need a family but if you have any feelings like that towards him tell me now

H - are you freaking kidding me Doc, Walt is a mate nothing more, he's never seen me naked, never seen me cry, never seen how I doubt myself, doesn't know what it does to me when someone kisses me behind my ear, doesn't know what happened to me in the care home when I was 13 or what happened with my first boyfriend.... do you know who does or shall I go on

Toby held his hands up in defence

T - ok your right I'm sorry, I know Walts just a mate and nothing more. I'm still hurt though Hap howcelild you feel if you learnt I was already married

H - I episode be gutted but I would try to understand why

T - I am trying sweetie and I'm not pissed at you but Walt he knew how much it destroyed me the night I found out he was here and yet he never said anything

H - I asked him not too,I text him and told him it needed to come from me. How are we going to move on from this

T - We look at getting you divorced and we move on,cwe have our little bambino to think of now. I'm going to let this go my love for you is far stronger then any anger I felt.

H - Walters looked into it and is looking at his options. We haven't spent a night together all night since before you got kidnapped you always go home after we have sex, how do you feel about staying over tonight, doing some pregnancy research. I've missed waking up with you.

T - you are a softie really, I would love too and I'm sorry I've been distant lately its been a lot to process.

H - I get it but we need to move on so let's start now

Toby smiled at Happy and nodded, he then pulled her over to him running his hand through her hair and along her leg and pulled her in for a kiss.

Happy sighed into the kiss pulling Toby closer to her by grabbing his shirt and running her fingers along his stubble. She slowly pulled away and smiled she felt closer to him then she had for weeks.

H - come on Doc let's get out of here.

deku vs kacchan part 2 is my favorite ep of the entire series actually. i’ve watch that ep so many times as well it’s just perfect. i got into them when i watched that episode actually! the voice acting god..... literally shivers. now that i think about it it might even be my favorite anime ep of all time help. i just love how bakugou lets it all out and we get to see that side of him.. the insecure scared child he tries to hide. god i could ramble about it forever idk just love it. OH YOU WATCHED THE MOVE RIGHT AFTER? a scene with bakugou and todoroki is guaranteed a good time.. and they work so well together. haha that’s such a nice coincidence tho their dynamic is great in that movie even if it’s mostly kiribaku sjsksjk. what do you think about kiribaku btw?

i just go in and use her skill then heal a bit and switch right away fhsdkjfsd it going alright! and then i go ham with my other 3 charas and switch back to her to heal again fhskfjd. OMGGG that’s so cute please... i miss diluc too... come back!! i wish we had a way to replay the old quests even if we get nothing out of them like i just wanna experience it again ya know.

I have no work so pre-NaNo ramblings under the jump as I try to sort out if I’m starting anything new for it, feel free to click through or ignore entirely.

All fic, because I have no idea what Im doing with any original stuff right now. Most of it is TUA fic. Because I am currently Having An Obsession.

- Speaking of Having An Obsession, primary goal is definitely to get turpentine and patches (the pre-S1, “Diego and Vanya and pregnancy and dealing with trauma by being determined to be better parents than Reginald and finding something else to live for, technically a kidfic I guess even though it probably ends right before or right after the kid if born” fic) finished. Or close to it? Just... to work on it.

(I’m stalled right now because I’ve written through what my brain termed The First Part, and I have an idea for The Third Or Fourth-ish Part, but need to figure out what The Second Part is going to be in a way that’s interesting to write about. Right now in my head it’s just “?????? Dealing? Somewhere in here we deal with the Question of Doctors, but not The Question of Medication, they need to be closer for that. also in here is where Diego effectively moves in even if they don’t really register that fact til later.” I may write the attached one-shot to The Third-ish Part in my head first, see where the emotional states land, and write towards that? That might work. Or maybe I’ll just put Braille on repeat and go to town.)

But I am not Proper NaNoing -- for one thing I don’t think the above is going to be 50k, especially not on top of the ~7k I have already? I could be wrong, but we’ll see. So other thoughts:

- I would like to get some more words on the Allison-is-born-in-Night-Vale story(/ies) done, because right now that exists as a decontextualized collection of paragraphs, some from the base fic, some from the series of spin-offs where she meets the various Hargreeves (mostly the one where Luther starts getting her show on the moon and falls in love with her from afar). And it’s just fun. I think I need a WTNV refresher, so I may make a point of listening to some of my favorites this week. It’s also good Halloween noise and I can work to it, so.

(Should not just listen to Desert Bluffs episodes. But may do some of those too. Because I love Kevin more than is probably healthy for me. One day I’ll go as Kevin for a con somewhere and have way too much fun.)

- I appear to have lost the bare: a pop opera/TUA crossover file somewhere in my docs? I need to dig it out again and go back to that as well, but I need a refresher on what bits I have scrambled down, because I think that’s also pretty piecemeal.

- ..... I don’t have that much oomph for my other existing WIPs right now, which is dissapointing. Some part of me DOES want to turn the series of Abigail/63!Will ficlets into somethign more than A Series of Kinky Moments, but I don’t know what to do with that, so it’s probably not happening yet.

I may make an effort to work on the eldritch-Sigyn-time-loop fic though. Because I love it and i feel like it’s going to end up just languishing. Or maybe that will be my wind-up to NaNo, since I don’t know what to write lately?

- Things I’d like to start but need to do research for:

-- That fic where Abigail survives but is mute, the metaphor becomes text and in her lack of voice becoming text she finds a way to reclaim her voice and her power, because I still have STRONG FEELINGS about Abigail Hobbs apparently. Like. A lot of them. Like, it’s almost a problem, but not surprising.

However, to write it properly I’d really want to research throat wounds and healing, and I am A Chicken about gory images, so I have to figure out how to research it without risking running into pictures.

-- I ALSO need to research throat wounds and healing for the fic where Diego crash-lands into the 60s a couple of months after Allison, they’re assumed to be married, and they just kind of roll with it. Also, racism in the 60s, and specifically how it affects marriages that are different races but nobody involved is white? Because literally every source I found on my first go-round was only about interracial marriage when one part was.

(That, or I need to throw up my hands and choose to declare this fic not reality complaint, but I feel like this one I should definitely at least TRY to research first. But goddammit I just want to write fake-married-and-worried-about-everyone-else turning into actual-relationship(-and-still-worrying) [and also somewhere it developed a plot where Diego tries to get into boxing because he has no ability to keep jobs and ends up running into Luther and possibly evolves into threesomes and also collecting their other siblings several months before Five shows up? I don’t know how but it did].)

- Shorter contenders if I need to find something else to think on for a bit include:

-- The fic where Ben slowly becomes possessed by the Horror.

-- The series of AUs where each member of the Academy ended up being the last one left behind.

-- Luther viewing Vanya as their father’s left-hand team leader to his personal right-hand team leader and coming to her to keep her in the loop and ask advice, which eventually morphs as they get older into a habit of going to Vanya to confess everything he feels he’s done wrong and her giving him a kind of penance

... Do I even have anything else that’s not TUA left? I feel like I should but I Have A Fixation right now, it’s a problem. I need to go back to the Magicians and see if that creeps in. Or maybe something else, but this will work for NaNo, I think. altogether it’s definitely more than 50k, though I’m not planning to write it all at once this coming month, just, it’ll be easier if there’s a plan for me to look at while I’m going ‘but what do I wriiiiiiiite’ than if there’s nothing.

Not even sure I’m really going for 50k, but just. Writing in a Substantial Amount would be nice. For once I think I can do it! It’s been 14 fucking years since I’ve won (or even gone over 10k I believe)! So let’s see how it goes!
