2. Not Interested...
If you would’ve told Alex Karev a few years ago that he would be a pediatric surgeon instead of a plastic surgeon he would have laughed, but then again, the same would go if you have told him he would’ve accepted his little needs, only to get adopted by the two most overprotective caregivers the world could’ve seen, plus a big sister who was annoying as hell when she was in her teenage stage.
He snorted watching the picture of the photo session his dad convinced his mom to get once he and Amelia got officially adopted, he was frowning -no pouting, he didn’t pout, despite what his parents said- while Amelia was giving a weird smile, Meredith and Derek smiled, the first one almost rolling her eyes in the picture, the last one was over the moon. It was a funny picture, and one that to this day he really liked, because it reflected them all perfectly.
Yeah, he wouldn’t have believed this would be his life when he finished his residency, but he was happy, even if that meant he had to take naps everyday and have a bedtime, even when he wasn’t in his little version.
His pager went off, and he stood up fast to get out of his office and run to the emergency room, he frowned when he saw one of the new interns following him but didn’t really pay attention as he arrived at his destination, to find Dr. Torres.
“Talk to me, what do we got?”

AN: So I finished this months ago and was like ‘oh I'll save it for Halloween’ and I got so into the Jolex Halloween fic that I forgot to post this! Anyway better late than never I hope you all enjoy because I've always said that this is how this scene should have been!

Jo looked through Alex’s books on the shelf behind his desk. She hadn’t gotten the chance to see her husband’s new office yet. It was very much like the one at Grey-Slone, but smaller and dirtier. He had a few wrestling magazines, some good management books, and Ped’s medical journals. She ran her fingers along the spines as she read the titles.
“Let's get out of here,” Alex said, coming into the room and letting the folder drop onto his desk. “Today was a nightmare.”
“Hey,” Jo said, reaching up and holding his face as he looked at her. “You are amazing. You are turning a house of horrors into a legit hospital, and I am sorry if those investors can't see that.”
“Well, don't be,” Alex said, raising his eyebrows and giving her a slight smile. “I'm getting a new research facility.”
“They gave you the funding?”
“Yeah, they said they were impressed with my ability to handle chaos.”
“They don't even know the half of it,” Jo said as she wrapped her arms around his neck. “They should visit your childhood.”
“Hey, you know what?” Ale said, as he pulled back from another kiss.
“Yeah?” 
“We should do something to celebrate,” Alex said, not telling her his plans just yet.
“Great. I am absolutely dressed for that,” Jo said, holding out the skirt of her dress before she looked up at him, seeing that familiar glint of trouble in his eyes. “What?”
“What time does the courthouse close?” Alex said with a smile as Jo tilted her head.
He held out his hand and Jo let him pull her away.

They were standing in the courthouse together. Jo can hardly believe that they were doing this, but she was so giddy. She still loved standing there in her white dress, holding his hands and loving every word he said to her. She was so wrapped up in this moment. It was the perfect wedding and she couldn't even believe the courthouse was open this late much less on Halloween. Although she was possibly elated more so about the fact that she got Alex to actually wear the vampire teeth. She knew he did it only for her. 
It's not their wedding on the ferry boat at sunset. She didn't have the perfect white dress and they didn't have their friends and family, but it was an intimate moment that they would have forever. 
Finally, it was her turn to say her vows. They had the traditional vows as she repeated after the judge. She's been trying to think of the right thing to say to make her vows a little unique for him. The second the idea popped up in her head, she knew she had to do it. It would also surprise him and possibly scare him a little bit, but hopefully in a good way.
“For better or worse.” 
“For better or worse.” Jo repeated after him.
“For richer or poorer.”
“For richer… or pregnant,” Jo said in a single breath as she watched Alex take in her words. 
For a second he just stood there, gobsmacked. His eyes were wide and his lips parted. She wasn’t sure if he actually heard her. Although, judging by the look on his face he did. 
“What?” Alex asked, his voice muffled by the vampire teeth.
“I'm pregnant,” Jo said, confirming it to him with a nervous little smile. 
“Are you serious,” Alex asked, still surprised and Jo suddenly got nervous about telling him like this. 
“Yes,” Jo said, biting her lip as she nodded. “Why do you think I dressed up and have been following you around at work all day? I’ve been trying to tell you all day, but it’s been one catastrophe after another at the hospital. I didn't want to stress you out at work so I just figured I'd wait to tell you at home, but then you wanted to get legally married….”
Before Jo could say anything more, Alex cut her off with a kiss. He pressed his lips into hers and she could feel the excitement on his lips. He smiled against her lips and she pulled back to see him with a broad smile.
“You’re really pregnant?” Alex asked, sure of himself, but more so asking her to say it again.
“Yes, I’m pregnant,” Jo said with a matching smile that started to hurt her cheeks.
He looked down at her stomach and let go of one of her hands to reach down to hold her belly, rubbing his thumb in a little circle. There's nothing there quite yet, just the tiniest curve of her belly where her uterus was a little more prominent. She wasn’t sure if he could even feel it yet. 
“We're gonna have a baby,” Alex said, his voice giddy with his large smile.
“Yeah we're going to have a baby,” Jo said sighing in contentment as he leaned in and kissed her again. 
The officiant let them have their moment, but after their second kiss, he cleared his throat. They reluctantly parted, but Alex still smiled wide and happy and he kept his hand on her belly. Jo felt like she was floating on air. She was so happy at this moment and she can barely contain it. 
Jo looked over at him and smiled as she repeated her vows. “For richer or poor. “
Alex leaned in and she did too as their noses touch and they smile at each other. He gave her a little smile and held her belly through the rest of the preceding until they were finally officially and legally married. Alex leaned in and kissed her with lips full of passion. Jo melted into him with a little moan. As she let herself be consumed by his lips. She wrapped her arms around him and put one hand on his cheek, holding him against her for as long as possible. They only break apart when they're desperate for air. 
After they got out of the court, having filed all the correct documents and making sure everything was in order, Alex kissed her again as he led her out to the car, holding her hand. He pulled her against him as he kissed her against the car. He fumbled with the door handle until they fell into the backseat. Jo sat up with a laugh as he undid the ties on her dress and forced it open. 
Instead of his hands going to more intimate places, he smiled as he leaned down and stared at her belly. Jo easily smiled as she put her hand on his cheek, his eyes glance up at her before he looked back down at their baby. He put one hand over her bump, gently gracing her skin before he held the little bump. 
“Hey you,” Alex said as he leaned down and kissed her bump. “I'm your Daddy, it's nice to meet you.”
Jo reached up to tangle her hand in his hair as he leaned down to kiss her bump. “You're finally gonna be a dad.”
“And you're gonna be a mom,” Alex said with a smile as he came up and kissed her lips. “And don't worry because you're going to be an amazing mom, Jo.”
Jo smiled and sighed, she knew he was right and she was finally ready. She was going to be a mom and he was going to be a dad and they were going to have the family they always wanted.

im so glad you asked let me tell you (these are in no particular order unless we want them to be, in which case i say i am indecisive)
1. obviously, the bear. cannot go wrong with rewatching this and creating fake scenarios in my head.
2. greys anatomy, i’ve been falling in love with jolex and slexie for some reason recently. the medical aspect / the love stories / the cinematography sometimes is.. chefs kiss
3. peacemaker is one ive started watching recently but i have really liked it so far. i’m still trying to get used to the idea of like potentially getting to write adrian chase x reader
4. criminal minds. i will always watch criminal minds, any chance i get. long road trip? criminal minds. it is.. yes. its a show i can rewatch and never grow tired of.
5. shameless. i have been religiously rewatching this and specifically like lip gallagher scenes on youtube because i need .. inspiration for a fanfic i have.
now if we’re talking movies ugh i could rant about those all day long too

I basically blocked out all things Grey’s Anatomy after 16x16 because it made me so angry (and I still haven’t watched a single episode after that) but I remembered how much I love Jolex recently and how much I miss them. Rereading all of your fanfics has been so nice and god it just makes me with the whole storyline never happened. Will you be accepting any prompt requests?

Did you get scared while releasing DTI and getting that much backlash for it?
Did you ever think to stop sharing the story?
I think it's an amazing story and I'm honestly so impressed by you. For your writing and for having the courage to go through that.
To come into a Fandom and say "I'm going to take the main couple and end their love story", (like Shonda with Merder and Slexie and Jolex 😭) and in their prime time when the Fandom was bigger, must have taken a lot of guts.

You know when they say ignorance is bliss? lol Well...
With the exception of an alternate ending to the canon series, I hadn't written anything else when I entered the fandom. DTI was my start (I like to dive in head first. 😂 And, while I read several fanfic writers before I joined Tumblr, I wasn't on Tumblr, so I really didn't understand the discourse, politics... and fucking lunacy... until I was here. And I'm glad about that because if I had, I don't know if I would have done it, and I'm quite glad I did.
But no, I never considered stopping writing or sharing it. If people didn't like it, no one was forcing them to read.
Now, a little history. My initial intent was to have three separate endings, and each reader could choose their own... Choices... after all. But here is what happened.
I needed to make the Ethan/Casey break up quite cruel and painful in order to make it believable that Casey would attempt to move on. And, I believe it was delivered. It delivered to the point that I no longer thought it was believable for Casey to return to Ethan without giving up her dignity. Yes, his actions stemmed from a place of great pain; however, he had hurt her - an innocent bystander who simply loved him - not once but twice. Casey is not a fool.
I was already stressed over that decision, then I started getting a ton of DMs and anons from people. Some told me about their past relationships, where they truly loved someone, but they were no longer safe for them. About their struggles of moving on and the pain they endured. Some had moved on to have happier, healthier relationships as a result.
I also got very disturbing messages from women that basically said Casey was a horrible villain. That she should take whatever abuse was heaped upon her by Ethan because he had suffered in the past. I was like... let's slam on the breaks RIGHT THERE. NO. HARD NO. I don't care what the situation is, no one is justified in hurting anyone else because of what happened to them. Broken people cut others until they heal themselves, but that doesn't mean the others don't bleed.
Casey bled once, and she forgave him and allowed him a second chance. Now, she was hurt again... and she knew where this was stemming from, but I HAD to show her having enough self-respect to walk away and protect herself above all others. It became about much more than a silly story about fictional characters. I lost sleep. If ONE young woman took away that it's OK to be treated that way because "you love someone," I couldn't live with that. In real life, I have worked with survivors of domestic abuse, and I could not do it.
So, I scrapped the other 2 endings (one where she chose Ethan, one where she chose no one) and moved forward.
I'm very happy with the ending because I believe everyone got a happy ending. As a loyal reader said to me once, "The only reason you're getting grief is that it's Ethan & Casey. Change Ethan to any other name, the people who are persecuting you for your choices would have been demanding he suffer and pay." She was spot on.
I do think there was one path to them reconciling. If things didn't work with Tobias & Casey and Ethan & Sonia. If each of them had gone on years (and it would have to be YEARS), and it never worked with anyone else. If Casey saw SUSTAINED PROOF that Ethan had truly changed, and that strong love, platonic love, that had replaced what they once shared continued, they could have made their way back to each other. But it would have been a minimum of a decade down the line. I've toyed with writing that, but I'm not inclined to do it.
Wow, that became excessively long. Of course, if you read DTI, this should come as no shock to you! lol

The second he saw her there in their bed, pretending to be asleep, he knew she was not ok. The fresh stain of tears under her puffy eyes, the wet spot on her pillow, and her red nose let Alex know immediately that she wasn't sleeping. But he still let her be. He figured quickly that whatever she discovered during her trip to Pittsburgh wasn't the closure and peace she had hoped for. Otherwise she wouldn't lie there pretending to be asleep. But he let her be. If he knows one thing about his wife than it's that pressuring her never helped. So he will wait until she's ready. Just like she does whenever he needs his time - she leaves him alone until he's ready to talk about it. And now is his turn to let her process and digest before she's willing to talk to him about it.
After kissing her forehead and a last look at her he quickly resumes his nightly routine in the bathroom and changes before he joins his wife in bed.
Normally he would scoot closer and wrap his arms around Jo but he's not sure if that's what she really needs right now; instead he pulls the cover closer to his chest and stares at the ceiling, trying to guess what discovery his wife made to return this broken. Every possible scenario plays in his head, preventing him to close his eyes and fall asleep.
Even though the loft was dark except from the little moonlight shining through their windows, he turns his head to look at Jo.
Her back is still turned towards him, her head is buried in her pillow while her hands are still holding tightly onto it.
All he wants to do is hold her close, showing her that no matter what had happened he is here for her, will keep her safe and just loves her, but he wasn't sure if his closeness wouldn't cross her boundaries right now.
But he also can't just lay there - he at least needs to let her know that he's here. So he probs up on his left arm and comes a little closer.
"Jo." He starts softly, "Whatever happened in Pittsburgh I want you to know that I love you and that I'm here for you whenever you're ready."
He carefully brushes the stray hair behind her ear and places another kiss against her temple before he scoots backwards to lay back down. But before his head hits the pillow again her shaky voice fills the silence.
"C- can you- hold me?" She quietly asks between now loud sobs.
That's all he needs to hear to pull her close in his arms, tightly holding her while she cries into her pillow.

Even though he got only three hours of sleep that night, he gets up the next morning to make breakfast. He knows that Jo didn't get any sleep either, so he called the hospital to call both of them sick for the day. Luckily Bailey didn't ask any further questions and Alex continued preparing them something to eat.
It breaks his heart to see his wife like this. Even though she's sleeping right now her hand is still holding tightly onto his pillow while her red puffy face looks anything but relaxed.
He misses her smile, her laugh. But this is what he promised her - for better, for worse. And he would keep this promise - always.
The smell of smoke rips him out of his thoughts. He quickly turns the waffle iron off before he puts the last waffle on the little pile he already made. He pours himself a coffee and takes it with him back to bed. He doesn't know why but he feels like he needs to stay close to Jo; even though she's sleeping he doesn't want her to wake up alone, which is actually bull shit considering they are living in one big room - he still wants to be close for her.
Sipping on his coffee he falls back into his thoughts. What the hell happened in Pittsburgh? What could Jo have discovered to push her this hard off the road? He hopes she will talk to him once she's awake. He really wants to help her through this like she did so many times for him. Like when his dad showed up and later died; she was the only one by his side. Or when he started working for Lebackes she was his only cheerleader. And even when they were broken up and she had every right to hate him for what he did to DeLuca, she came to see him at Mer's house and hugged him without a word just to be sure he was safe.
She really is the one person that was always there for him, that always supported him and always made sure he was ok. And he could never put into words how much he loves her for being this person in his life. It wasn't fair back then when he told her that Mer was the only one the could count on because it simply wasn't true.
He often finds himself wondering how he out of all the people is the lucky one to have this wonderful woman as his wife. He doesn't know how he deserves her.
From the corner of his eye he sees her eyes open slowly. Giving her the space she needs,
he keeps sipping on his coffee until it's empty. Putting the empty mug on his nightstand he feels her flinch.
"I'm sorry." He whispers, watching her stare at the kitchen island.
"You should go. You have work." Her voice is quiet.
"I'm not going anywhere." He answers, making sure she knows that he's here for her.
"There are sick kids who need you."
"You are more important right now." He tries to make it clear to her that he is here for her to lean onto if she needs to.
"No." It's nothing but a whisper, "I'm not worth your love." Her voice is cold but he knows how to take it.
"That's the dumbest thing I've ever heard."
He turns carefully so he could look at her with still enough space between them.
"You are worth every single bit of my love, Jo. I love you so much it hurts sometimes. I would do anything for you. Every morning when I wake up I look at you and wonder what I did to deserve you. I've said it before but I'll say it again, nothing has ever surprised me more than getting you to love and marry me. I'm in on this. For better for worse. In sickness and in health. I can only promise you again that I'm not going anywhere. Not today, not ever." He assures her softly, not taking his eyes off her.
"Can you hold me?" She asks, new tears building in her eyes.
"Always." He doesn't even hesitate, putting his arms around Jo.
It's silent for a while. He can see her tears streaming down her face but she's not making a sound. He strokes her hair and holds her close. He can feel her opening and closing her mouth from time to time but not saying anything.
"I- I met her. In a diner and we talked. He raped her-" Jo breaks the silence with a sob, "My father."
He doesn't know what to say. He pulls her even closer and lets his body say what his mouth can't form.
"She hated being pregnant. When I was born she loved me but I reminded her of him so she left me at the fire station. She was young. She didn't tell anyone. No one knew she was pregnant. No one knew I was born. I'm only here because he raped her. I should have never existed in the first place."
It's again silent before he finds the right words, "But I'm glad you do. You make me a better man every day. If it wasn't for you I would still be this douchebag attending who sleeps around the hospital. Paul would still be out there hurting people. Mer would have never started her mini liver study. You help and heal so many people every day." It's almost impossible but he pulls her even closer and kisses her head before he continues, "I'm so grateful every day that you exist. You make me and my life so much better. I love you, please don't ever doubt that." He whispers as his head lays on hers.
"There's more I need to tell you but I'm not ready yet." She answers quietly, relaxing a little in his strong, protective arms.
"And that's ok. I can wait." He kisses her head again.
"Thank you." She whispers before they fall into an silence again.
"I think I wanna try therapy."
"Ok." Alex answers, "Are you hungry?"
She replies with a small nod.
"You need to let me go so I can serve you breakfast in bed." He jokes as she lets him go.
They lock eyes for a second and her beautiful smile appears for the first time again.
She knows it's gonna get better because he is there.

“Dr. Bailey! Oh thank God, I need your help!” Jo squeaked as she came to a halt in front of the woman in question.
“What did you do, Wilson?” Bailey sighed and looked up from the tablet in her hand.
“Karev.” Jo stated, eliciting a confused expression from the woman in front of her. “It’s Karev now.”
“Oh yes. Karev.” Bailey rolled her eyes. “What can I do for you, Karev?”
“I need someone to take my two o’clock surgery.” Jo grimaced. “I wouldn’t ask you unless I absolutely had to. I tried to get Meredith to take it but she’s in the OR doing a transplant that is going to last another four hours. It’s an ex-lap.”
“And why is it that you can’t perform your own surgery? You’re a fellow now Wilson—I mean Karev. You shouldn’t be carting off a simple ex-lap to another surgeon. You’ve done plenty of them before.” Bailey’s frown made it known that she was unimpressed with Jo’s request. “So you better have a pretty damn good reason why you need me to do it.”
“I—uh,” Jo paused and considered whether she should tell Bailey the truth. She had tried keeping her suspicions to herself for the past few days, but Jo was practically bursting to tell someone. “I have an appointment to see Carina DeLuca.”
Jo watched as Bailey’s eyes grew wide.
“Wilson, are you pregnant?”
“Karev. And could you keep your voice down?” Jo shushed her superior and looked around to make sure no one was listening. “I haven’t told anyone. Not even Alex. I took a test this morning because I had been feeling strange and the line was really light but it was there so I just made an appointment to be sure. I don’t want to say anything until after the appointment. I mean, we just got married and he’s interim chief and I just started a fellowship, so this just might be the worst, most stressful time to make a baby, but of course, this is when he decides to knock me up!”
Bailey stood there dumbfounded for a few seconds before speaking. “Alex Karev is having a baby?”
“Maybe?” Jo shrugged. “I don’t know. You see. This is why I really need you to take my ex-lap so that I can make my 1:45 appointment and find out if my entire life is about to change.”
“Um… yes of course.” Bailey nodded. “Just let me rearrange some things. I can get one of the senior residents to take my appy.”
“Thank you so much Bailey. You’re amazing!” Jo clapped her hands together in gratitude.
“Yeah, uh huh…” Bailey waved her hand as Jo skittered away. “You better tell me as soon as you find out.”
“Yes ma’am!”
The next few hours seemed to drag on as Jo waited for 1:45 to roll around. She spent the entirety of the lumpectomy she was performing mulling over the faintest little line that had showed up on the stick that morning.
“Dr. Karev? Dr. Karev? Jo?”
Jo looked up to see Andrew trying to capture her attention.
“Jo. Are you okay? You froze for a second there.” Andrew asked, concerned. “Did you feel like you missed something? Because, you already finished the hardest part of the procedure and you did it flawlessly. All that’s left is to close.”
“Thanks. And no it’s not that.” Jo shook her head. “Sorry, I just got a lot on my mind right now. Do you think you can close?”
“Uh, yeah of course!” Andrew’s eyebrows shot up. “Are you sure you don’t want to do it?”
“I trust you. You have great suturing technique. You barely leave a scar. Go ahead. Finish.” Jo insisted. “I’m going to head out. I have an appointment to get to. When you’re done, make sure to talk to the family and assure them that everything went smoothly. I’ll come back to check on her as soon as I'm done.”
“Okay…” Andrew grinned under his mask. “I won’t let you down!”
As soon as Jo stepped out of the OR, she braced herself against the nearest wall. She took some slow deep breaths to help keep the ever present nausea and dizziness at bay. Come on, Karev. Get yourself together. You don’t even know for sure yet. That line was so light it was basically nonexistent. Your results could come back negative. Yes. They could be negative. That’s what Jo needed to remind herself of. They could also be positive, though. That would mean you’re having a baby. That would mean that you’re growing a human, right now, at this very moment.
Jo’s heart pounded the entire way down to the OBGYN clinic offices. When she finally rounded the corner, she was surprised to see Carina waiting for her already.
“Jo. I was surprised to see your name added last minute to my schedule. Let’s get you into the room quickly.” Carina led her past the front desk as discreetly as possible and into an exam. “I’m assuming you want to keep this from as many people as possible since your husband is the chief, no?”
“You would be correct.” Jo nodded.
“I figured that was the case.” Carina smiled. “I normally wouldn’t do these things myself but I had a nurse come set up everything for me so I could do all of your examinations myself. I promise everything will be done with the utmost discretion.”
“Thank you so much Carina.”
“So you said that you think you might be pregnant? Tell me, have you taken a test yet?” Carina washed her hands and donned some gloves.
“I took one this morning because I had been having some of the same symptoms as last time and wanted to check. So, I took the the test, but the line was really light so I’m not sure if it was a false positive or if it’s just too early.”
“You said you’re having the same symptoms as last time. I thought you and Dr. Karev didn’t have any children. Have you been pregnant in the past?”
Jo cursed herself for the slip. She’d never had this conversation out loud with another soul and was terrified to do it now, but it seemed as good a time as any. “I was pregnant about ten years ago. It was during a very difficult time in my life. I was in an abusive relationship, so… I had an abortion.”
“I’m sorry about your situation.” Carina placed a sympathetic hand on Jo’s shoulder. “But these circumstances are very different from the last one. So, no matter what your results say and what you decide to do going forward, it will not be like the last time.”
“You’re right.” Jo took a deep breath. “My life is different now. It’s good. I'm happy. I’m in a loving relationship with a man who would die before ever laying a hand on me. That won’t change depending on if I’m pregnant or not.”
“Well, we are still going to find out. I don’t need you running around stressed out about whether or not you’re pregnant.” Carina took the needle in her hand and began to draw some blood. “I'm going to do this first and run the labs so that hopefully it’ll be in soon.”
“Do you think I’m pregnant? They say OB’s have like a sixth sense about these kinds of things. Well, they also say that about peds surgeons and Alex hasn’t said anything to me so maybe they’re wrong. Maybe a special pregnancy sixth sense doesn’t exist.” Jo rambled as Carina put a bandaid on Jo’s arm.
“Oh, it exists. Trust me.” Carina’s eyes glimmered as she dropped a couple vials of blood into a specimen bag. “But, I will not tell you what I suspect until the results come in.”
“Come on, that’s no fun.” Jo pouted.
Carina walked over to the door and opened it slightly, blocking Jo from view. She motioned to one of the nurses to come over.
“Hi, Vivi. Could you run these labs for me? I need them back as soon as possible.” Carina smiled and handed them over to the nurse.
“Yes, of course.” Vivi reached for the bag and took a look at the vials. “There’s no name on these.”
“Yes. I know.” Carina nodded. “Just put it down as Jane Doe or something like that. Remember to tell them that it is urgent!”
As soon as she closed the door, Carina turned back towards Jo. “Okay, I am going to go check on some of my other patients while we wait for those results to come. I will be back soon and then we will know if we’re going to have a little bambino to care for.”
“Sounds good.”
Minutes never felt this long in the past. The ticking of the clock seemed exponentially louder than it ever had before. The last time she’d been pregnant, Jo never gave herself the time to think of whether she could be a mom. She never allowed herself to picture her pregnancy as more than just another thing that Paul could beat her for. She refused to allow herself those thoughts in the past out of fear of getting attached to something that could never happen. Now that she was no longer in the same position, Jo couldn’t help but think of everything. The passing minutes were full of countless thoughts ranging from fear and anxiety inducing ones, to happiness and excitement, to uncertainty and trying to calm herself down.
By the time Carina returned, Jo was sure of one thing and one thing only. She needed to know the results.
“Okay, Jo. Let’s see what this says.” Carina clicked on the screen of the iPad in her hands and waited for the results to load. When they showed up on the screen, she gave an almost imperceptible nod. “You ready?”
“As I’ll ever be.”
“You’re pregnant.” Carina beamed. “Most likely very early on based on your hCG levels. I’d estimate somewhere around five weeks. Two of those weeks are counted from your last period which means you conceived approximately three weeks ago… So right around the second week of June.”
As she did the mental math, Jo burst out into laughter. “Oh my God.”
“What’s going on?” Carina asked, a confused expression making its way on her face.
“I know when I got pregnant.”
“You do?” Carina’s eyebrows raised. “Well, if you can remember the exact date, that would be very helpful in tracking the development of your baby.”
“It was on my wedding day, June ninth.” Jo laughed again. “Alex and I snuck away and had sex in a shed before the ceremony. I forgot to take my pill the night before and didn’t bring them with me to the venue. I thought I’d be okay because I had Meredith bring them to me so I could double up the next day.”
“It must’ve been enough to cause you to ovulate.” Carina chuckled. “But besides the nausea and dizziness, how do you feel? Do we need to talk about options or have you decided?”
“I’m keeping it.” Jo said without hesitation. She gasped immediately after. “I can’t believe I said that. I’m going to have a baby. I’m going to be a mom.”
“Yes, you are.” Carina laughed. “My sixth sense never fails me.”
The remainder of the appointment went smoothly. When Jo and Carina finally parted ways, Jo took a second to gather herself before walking back over towards the surgical wing. She entered the gallery of OR 4 and waited for Bailey to notice her presence.
After a few minutes, Bailey’s head turned in Jo’s direction. “Well, Wilson? Sorry, I mean Karev. What’s the verdict?”
Sure that no one would understand the context of their conversation, Jo smiled. “Yes.”
“Ah!” Bailey let out a little shout of excitement. “Okay, we’ll talk as soon as I get out of here. For now though, I’d appreciate it if you popped in on Baldwin in OR 6. Make sure that he isn’t screwing up my appy.”
After checking in on the residents, a consult in the pit, and a quick hernia repair, Jo was back in the lab with Bailey. She was grateful for the older woman’s care and support. Bailey had been so helpful and understanding as soon as she learned about Jo’s pregnancy.
“I’m really happy for you, Jo. I think this is going to be wonderful.” Bailey squeezed Jo’s hand lightly. “Can you believe how far you’ve come?”
“It’s still surreal.” Jo confessed. “Sometimes I wonder if I’m going to wake up as Brooke, living in Paul’s prison of a townhouse hoping that he’ll be in a good mood today and decide not to hit me… But then I realize that this is my life. I’m Jo Karev now. I have a loving husband. I’m a doctor. I have friends who care about me. And I’m going to be a mother.”
“Alex Karev is going to be a father.” Bailey snorted. “I never thought I would live to see the day.” She shook her head. “Is it selfish of me to hope that the baby is a girl? You know, as payback for all of the headaches he gave me with every one of his indiscretions?”
“Not at all.”

“Hey. You’re home.” Jo said as Alex walked in the door.
“Yes, finally.” Alex sighed as he put his stuff down. He walked over to where Jo was seated on the couch and kissed her. “I missed you today. I usually see you at least once but I didn’t see you at all today.”
“I was uh, busy.”
Yeah, real busy avoiding him to avoid slipping up and revealing their baby news.
“I got something for you.” Alex pulled away to retrieve something from his bag. He handed her a small box.
Jo looked at the box and her eyes widened. “Is this a pregnancy test?”
Alex shrugged. “Look, call me crazy, but I’m a peds surgeon. I’ve been enough pregnant ladies that I can practically sniff it out. I’m not saying you look different or anything, I don’t know… I just have a feeling.”
“And you want me to take this?” Jo stared at him in shock.
“I know it’s ridiculous and it’ll probably be negative, but I don't know. Humor me.” Alex cracked a grin.
“I don’t need to take this test.” Jo shook her head.
“Come on, Jo. It’s not that hard. All you gotta do is pee on the stick and give me a little peace of mind.”
“No, you don’t get it.” Jo put the box down beside her. “I don’t need to take this test because I already took one.” She inhaled deeply as if trying to muster up the courage to say it. “Alex, I’m pregnant.”
“Hold on, what?”
“I’m pregnant.” Jo beamed. “We’re gonna have a baby.”
“We’re having a baby?” Alex’s jaw dropped. “You’re pregnant? Like, right now?”
“Yes.” Jo laughed as Alex swept her into his arms.
“Holy crap. We’re going to be parents.” Alex’s eyes widened in shock and excitement.
“Yeah, we are.” Jo felt a couple tears prickle at her eyes. “I love you.”
“I love you.” Alex kissed her. “I love you so much, Jo. More than you could comprehend.”
After a few tender moments just reveling in their joy and impending parenthood, Jo sat up. “So, I figured out when we conceived this baby.”
“When? Because I’ve been trying to figure it out myself. We haven’t exactly had a ton of sex since I got this job.”
“We made a baby in that shed next to that corpse on our wedding day.”
Alex stared at Jo in disbelief and laughed. “You’re joking right?”
“I wish I was.” Jo giggled softly.
“No.”
“Yes.”
“Jo, I—” Alex's laughs filled the room. “Well, you have to admit that was some of my best work.”
“Yeah it was.” Jo joined his laughter.
“You’re happy about this right? I know it’s not ideal and I certainly didn’t mean to knock you up right at the beginning of your fellowship, but you’re okay with it, right?” Alex asked quietly.
“Alex, I don’t care if it isn’t ideal. I love you so much and I already love this kid so much and they’re nowhere close to being here yet. I’m happier than I’ve ever been and that’s thanks to you.” Jo pressed her forehead against his. “You’re giving me everything I thought I’d never have.”
“You’re everything to me.” Alex kissed her again. “I can’t wait to experience this with you.”
“Me neither.”

The opposite of loving me
FROM STRANGERS...
Alex grunts in annoyance as he runs into a blue blur who walks past him to get a gown in the emergency room. "Be careful," he mutters. "Sorry" she replies simply, wrinkling her eyebrows. He walks toward the ambulance bay, and she follows him. "Why are you following me?" "I'm your intern for the day" she replies. "Hello intern, do you have a name?" he asks, slipping on his gloves. "Jo Wilson." He looks at her with a small smirk, the thing that interested him. "Cool. I like girls with boys' names."

...TO FRIENDS...
"Come with me," she whispers in his ear, taking his hand and dragging him into a guard room. She looks back down the hallway before closing the door. "What are we doing here?" he asks, an edge of curiosity in his voice. "Wait here," she says simply, giving him a sly smile, before going behind a shelf with supplies. When she re-emerges, she has a pink box in her hands. She opens it, effectively revealing the donuts. "Where did you get them?" "Nurses' room, they're all no sugar," she mutters, making a small grimace as she hands him a donut, making him smile. She then grabs one and bites into it, letting out a small groan. "What?" she asks, catching him staring at her. "They're with powdered sugar, do you like it." "Yeah, yeah..." he murmurs, before mimicking her and smiling at her weird faces.

...INTO LOVERS...
"You came... you're here," Jo smiles, watching him as he walks towards her outside the rehab center. "Yeah... look, what I'm about to do is kind of cheesy, but I'm gonna..." he says, before kneeling in front of her, taking her hands between his. "I don't want an out. Do you hear me? You offered me a way out, and you were right. I was scared. So, I thought about it, and what I know for sure now is that I'm a better man with you.I want you to be my wife." Jo smiles, and then looks down at the ground, sighing. "I've done the work here, but I'm not magically cured. I can't promise it won't happen again." "I don't care. I love you. I want to grow old with you, no matter what. For better or for worse. In sickness and in health." "Marry me again, please." "Yes." "Yes?" "Yes!"

...AND STRANGERS AGAIN 
"Jo?" she gasps when she hears her name said, by a voice she hasn't heard in a long time, yet still so familiar. She's sitting at the bar of the hotel she's in for a conference when she looks up from her drink, shifting it to the figure to her right. His hair is a little longer, grayer. He's changed, but it's still him. "Alex?" "Jo, hi," he says, is can very well hear the embarrassment and guilt in his voice. "Hi." "How are you?" he asks after a few seconds. "I'm good, fine." "Izzie and your kids?" She asks freely. "Jo, I..." "I know" she says, closing her eyes. All these years and the wound still hasn't fully healed. "But it's passed. I've moved on, I've gotten over it." "How are you?" he asks after a few awkward minutes of silence. "Alex-" "Mom!" suddenly a voice rings out from behind the two of them. Jo immediately turns around when she hears it. "Hey, Lulu," she smiles, reaching down to grab her. "Weren't you with your uncle?" She asks her, looking behind her for Link. "I wanted to see you" she says simply, before turning her attention back to the person she was talking to "Who are you?" Alex is caught off guard, by the question and the situation. "Hum, I-" "He's an old friend of mine, Alex" Jo replies instead, looking at him. "Hi Alex, I'm Luna," she says cheerfully, extending her hand to him. Jo giggles, shaking her head and looking away. "Hi Luna, it's nice to meet you" he smiles, squeezing her hand gently in response for a couple of seconds. He stares in bewilderment at the little girl, who he assumes to be about 6 years old, her blonde hair pulled back into a pair of messy braids and her blue eyes staring at him. "How do you two know each other?" "Okay Luna, I think it's time to go to Uncle Link's, okay?" says Jo, setting her down and taking her hand to grab the bag on the stool. "Hi Alex" greets Luna, showing off her toothless smile. "Hi Luna" he says, smiling at her, before turning his attention back to Jo. "Goodbye Alex" she says, looking at her ex-husband. "Goodbye Jo" and with that, he sees her disappear among the rest of the people, thinking about how much it actually hurt to say that. Last time it was a bye. Now a goodbye, and suddenly they were done. Their relationship, the letter. They were done. And they were nothing anymore, not lovers, not friends. Just two people at a conference, both with happy lives they don't have together, though.

3. Doomed.
Jo’s night shift was going great, she was finally getting the hang of the internship, she was in Yang’s service, the woman was cold, but also a great teacher, that morning she observed as Yang sew a heart back together, all while she was allowed to apply suction and put her finger in to stop a small bleeding while doctor Yang got the instruments to fix it.
She made her way to the cafeteria, were she sat down with her fellow interns, she laughed and made jokes with them, they talked about cases and gave each other ideas on possible diagnoses for a thirty-year-old who was admitted with weird symptoms and had Stephanie’s head spinning around as most of the results were inconclusive.
She took a nap while she could in one of the on-call rooms, she was exhausted, and she knew that if she didn’t get at least a little bit of rest she was risking her little persona getting out and ruining her amazing last few hours.
That was not an option.

Nina wrote a fic that @cicinicole-14 didn’t start?! SAY IT AINT SO?!
no but really this fic has been sitting in my docs folder for weeks. but it’s here now so shut up. also I am actively taking title rebranding suggestions for this fic.

It feels surreal, sitting on the floor of the loft bathroom holding a positive pregnancy test. Jo has to blink a few times to make sure she’s reading the results correctly. But there’s no denying the two lines, one much darker than the other, telling Jo that her vomiting wasn’t the flu and her weight gain wasn’t all in her head. She was definitely pregnant.
“Hello? Jo?”
Jo pops her head out of the bathroom after having shoved the two positive tests into her box of tampons the second she heard the door unlock. She throws on her most convincing smile, walking over and settling herself in Alex’s arms. She stays longer than she normally would, letting the news of their impending parenthood sink in as she squeezes Alex tightly.
“Hey there, I missed you today,” Alex presses a kiss to Jo’s forehead as she buries her face in his chest. “You okay?”
Jo nods, pulling back from Alex and pressing a kiss to his lips, “Yeah, just tired. I didn’t sleep well.”
Alex nods and Jo is thankful he doesn’t prod any further, “I have to be at the airport in two hours. You sure you don’t want to come?”
As soon as Alex had discovered that his mom wasn’t caching his checks, he’d booked a plane ticket to Iowa. Jo knew that this was a trip Alex needed to make on his own. Now though, she was grateful for the extra time to process the news of her unexpected pregnancy.
“You need to focus on your mom. But I’m always a phone call away,” Jo grins as she grabs Alex’s packed duffle bag. “You’re lucky I went through this, you somehow managed to forget socks and underwear.”
Alex grabs her waist from behind, pressing a kiss to her shoulder, “What would I do without you?”
“You’d stink, that's for sure.”

Alex had texted Jo every day since leaving for Iowa a week ago. Usually just an update or a photo of something that reminded him of her. In the time since he’d left, Jo’s pregnancy had become something she couldn’t ignore anymore. Her cravings for cheese went from cute to obnoxious overnight and her stomach had popped out just enough that her scrub pants were beginning to feel tight.
She’s just walking through the door of the loft when her phone rings out, a smile appearing on her face as she checks the caller ID, “Hey stranger, how are you?”
Alex doesn’t respond right away and all Jo can hear is heavy breathing, “Hey beautiful.”
In an instant, she knows something is wrong just from Alex’s labored breathing and the way it sounds like it’s taking everything in him to speak.
“Alex? What’s wrong?”
“Nothing. I just got into a little fender bender.”
Jo’s heart sinks to her stomach, knowing Alex was downplaying things to keep her calm. She can hear sirens in the background of his call as he lets out a loud cough. She picks her purse up just as quickly as she’d dropped it before locking the door and running out of the loft.
“I’m leaving right now, I’m going to get on the first flight to Iowa, and I’ll be there as soon as I can,” Jo doesn’t realize she’s crying until her voice cracks. Then, she pauses, leaning against her car and willing herself not to vomit. “Are you okay?”
“I’m fine. Maybe a couple broken ribs, definitely a concussion,” Alex coughs again as the sirens in the background grow louder. “I’ll see you soon?”
“Please don’t- just wait for me, okay?”
“I love you Jo,” his words sound so finite that Jo can’t help the cry that she lets out as she cradles the curve of her stomach. “I love you so much.”
“I love you too Alex,” Jo wipes her damp cheeks, taking a deep breath as she climbs into her car. “I’ll be there soon.”

She’s not sure how she got there so quickly, but it feels like the blink of an eye as Jo walks through the doors of Des Moines Medical Center. She’d spent the majority of her flight throwing up or crying, much to the displeasure of her seatmate. Now though, all she can think about is getting to Alex.
“Hi, I’m looking for Alex Karev, he’s a patient.”
“Name and relationship, please.”
“Josephine Wilson, he’s my fiancé.”
The nurse looks up from the desk with an unamused expression, “Family only, I can’t let you in to see him.”
“But-“
“No exceptions.”
Jo is about to scream at the receptionist or start crying uncontrollably or something in between when a voice sounds out, “Jo?”
Standing behind her is a tall and lanky woman, grey hair styled impeccably and a knit scarf around her neck. She’s about to ask how she knows her when they meet eyes. Jo instantly recognizes the warm expression as one she sees on Alex, “Helen. Hi.”
Despite never having met before, Helen moves forward and wraps Jo in a tight hug, the action calming her nerves ever so slightly. When she pulls back, Helen looks her over with a smile, “You’re absolutely glowing. The photos Alex showed me don’t do you justice.”
Jo tries not to read too far into Helen’s words as she thanks her, fiddling with her engagement ring to distract herself.
“Is he okay? I mean, what happened? Alex is so careful, I just….” Jo takes a deep breath before meeting Helen’s gaze again. “I’m sorry for rambling. I've just been a nervous wreck the whole trip here.”
“He’s okay, a truck ran a red light and side-swiped him. They said he spun out and hit a light pole, but he’s fine,” Helen grabs Jo’s hand and squeezes it. “I’ll let him explain all that medical terminology to you, I was never good at remembering that. Let me walk you up, the nurses seem to like me.”
Helen leads Jo past the reception desk and into the elevators. She closes her eyes and leans against the back wall in an attempt to slow her mind down. There’s a brief silence before Helen speaks up, “How far along are you?”
For a moment, Jo freezes, wondering if she should lie to her future mother-in-law, but she knows it’s a futile thought. She thought that she’d done a good job hiding the slight curve of her belly with her oversized sweater but Helen has three children of her own, it probably wasn’t hard to spot the signs in her.
“I’m not sure, I found out right before Alex left and I didn’t want to go to an appointment without him,” Jo swallows back the lump in her throat, trying her best not to cry again. “Maybe ten weeks if I had to guess.”
“He’s going to be fine dear,” Helen’s soft voice is comforting, something Jo hasn’t realized she needed. “And I’m sure he’s going to be thrilled.”
Jo nods, watching the numbers silently climb as she twists her engagement ring once again. She and Alex were a month and a half out from their wedding, they should be planning and celebrating, but instead, Jo is stuck wondering if he’ll be okay enough for their big day.
“He’s in room 412, down there on the left,” Jo is snapped out of her thoughts by Helen guiding her out of the elevator and pointing down the hallway in front of them. “I’ll let you go, sweetie. You can come back and find me when you’re done.”
Her anxiety mounts to a new high as Jo walks down the hallway, reading the room numbers slowly to make sure she doesn’t miss her destination. Finally, she comes to a stop outside of room 412, only hesitating for a moment before stepping inside.
Alex is sitting up in his hospital bed, stitches across his forehead and a frown as he reads the magazine in front of him. Jo is so relieved to see him that she doesn’t think twice about rushing forward and bringing him into a hug. Alex is confused for a moment but quickly reciprocates the hug, pulling her closer to him.
“You had me worried sick, I thought you were dying or something,” Jo doesn’t make an attempt to stifle her tears as she holds Alex close. “Never do that again, that was awful!”
When Alex doesn’t respond right away, Jo thinks she might have shocked him with her entrance. However, when she pulls away, he has tears welling in his eyes. She cups his face, wiping at his wet cheeks, “Alex? What’s wrong?”
“You’re pregnant.”
“What? How...?”
Alex slides his hand to cover Jo’s baby bump and she has to stop the gasp she so desperately wants to let out. Having him feel the small swell that had grown in made everything feel real.
“We’re having a baby?”
Jo can only nod, overwhelmed by the fact that Alex was okay, that they were going to get married, that they were having a baby together. She cups his cheek, meeting his eyes with a serious expression.
“You’re not allowed to drive anywhere until the baby is born,” Jo takes on a stern tone but Alex still chuckles at the request. “I’m serious Alex! What happens if you die? It won’t just be me that’s alone, we have someone else to worry about now.”
“Nothing bad is going to happen to me, I promise,” Alex presses a kiss to Jo’s lips before grinning at her. “Looks like we might need to move the wedding up though. How far along are you?”
“Maybe ten weeks, I haven’t gone in yet,” Alex laughs at the statement and Jo furrows her eyebrows. “What’s so funny?”
“Jo there’s no way you’re only ten weeks. You have a bump already,” Jo leans back from Alex, hand coming to protectively cradle her stomach. “It’s not a bad thing, I’m just saying. Either you’re further along than you thought or there’s more than one baby in there.”
“Don’t you dare even say that Alex.”

Not for the Faint of Heart 
Chapter: One of Three
Words: 3,042
Summary: When Jo is rushed into an emergency c-section Alex has a hard time seeing his wife in surgery and ends up on the OR floor. However, it's Jo’s life that’s at risk.

Jo stared up at the lights in the cold OR. She took a deep breath in and smiled. The OR had always been the place where she felt most comfortable. It was the place where she had learned and grown into the surgeon that she was. Where she fell in love with her husband. In her therapy sessions, it was her safe place, where she would go when things got too intense. The OR was the place where nothing bad could happen to her. Even now, as Carina prepped her for a crash C-section, Jo was calm. But then again, she had lost a lot of blood and was on a lot of drugs.
“Are you seriously smiling right now?” Alex asked as his hand cupped her face and his thumb rubbed across her cheek, careful not to move the nasal cannula tubes. 
Alex was sitting next to her and focused completely on her. While everyone else scurried around and focused on where Carina was cutting her open. 
Jo smiled up at him, he was wearing his familiar brown scrub cap. She loved that scrub cap. “Our baby’s going to be born in an OR, I think it's kind of fitting?”
“You're scaring the shit out of me, I hope you know that,” Alex said with a nervous laugh as his eyebrows knitted together in worry, but Jo just smiled.
“Everything is going to be fine,” Jo said as she looked over at him with a smile.
Her labor had started out normally. She had felt a few contractions earlier that morning, but hadn't registered that they were anything more than a few Braxton Hicks. So she hadn't bothered to count them. As she went on with her day though, Alex insisted on keeping an absurdly close eye on her. Although he had done this throughout her pregnancy. Still, she knew he suspected she was in labor before she did. Finally, around lunch, she admitted that she might be in labor. She allowed him to pull her over to the maternity ward as a patient rather than a doctor. 
Carina quickly set her up on the monitors and things were going smoothly. Jo was handling her labor well. The contractions were pretty far apart. She just hated that she was in a hospital gown and confined to the maternity ward. Alex had vetoed any suggestions she had made on what to do while she labored except walking the halls. So they walked around a little bit and it did help. 
After a few circles around the hospital, Jo had started to feel a little light-headed and before she knew it, she collapsed in Alex's arms. She awoke back in her hospital bed with the alarms going off and several nurses around her. She had begun to bleed internally and a few minutes later, when her water broke, it all came gushing out. Carina quickly ran in and Jo was then rushed to the OR with a trail of blood behind her. 
Jo knew that she was in danger. That she could lose her life. That if Schmitt didn't get back with the blood soon, there would be dire consequences. She also knew that Carina was the best and her baby's heart rate was still good. It was her life that was on the line and she was so tired. 
“Hey, stay with me Jo,” Alex pleaded with her as her heavy eyelids closed just for a moment. “You gotta stay awake and meet our baby.”
“Just a few more minutes then they’ll be in your arms,” Carina assured her, glancing over at her before looking back to where she was cutting her open.
“I'm trying, but I’m really tired. I just wanna close my eyes for a second,” Jo complained, her eyelids fluttering again. 
She was so tired, she felt like she'd drift off if she closed her eyes. Especially as Alex squeezed her hand and rubbed his thumb over her cheek. It would be okay. She'd wake up with renewed strength and she'd get to meet her baby. 
“Have you two decided on the name yet?” Meredith asked, peeking over the divider from where she was assisting Carina.
“No,” they both replied as Jo opened her eyes to look up at Meredith.
“Didn't the two of you agree on the name Kaitlin, for Kate?” Hayes asked, his heavy accent changing the pronunciation as he looked down at her from where he was next to Meredith, ready to receive their baby. 
“I thought you said it was gonna be a boy?” Meredith asked, looking between the two of them with raised eyebrows.
“We don’t know for sure,” Alex clarified, looking over at Meredith for a second.
“It’s a boy, I know it,” Jo said, confidently as she smiled. 
“Yeah, only because you peeked at them, on the ultrasound,” Alex said, trying to be mad at her, but failing miserably. 
“I didn't, I just know,” Jo smirked as she looked up at him. 
She thought about how even now, he joked around and played with her. He refused to stay mad at her, even though he thought she had broken their rules and peaked at the baby's gender. But worry overtook his face and his eyes betrayed him as he looked down at her. She didn't want him to worry about her. She understood him worrying about the baby. She was worried too, but she didn't want him to worry about her. 
Jo would be all right. If she lived she would be fine and if she didn't well… She had lived long enough and she finally found happiness and that had been enough for her. If that's all she got in this life, it was enough.
“You're going to be… a, a good dad,” Jo whispered, the words coming out of her slowly as she used the last of her breath to speak and a few tears escaped from the corners of her eyes. 
“And you're going to be a good mom too,” Alex said, begging her to stay in his own way as he brushed the tears from her cheek with his thumb.
She thought about never watching him be a father to their child. To watch him hold them for the first time. To watch him play with them and goof around. To watch him teach them to ride a bike and help them do their homework. To watch him teach them to drive and send them off to college. To watch him give them advice as they found their way in the world. 
She didn't want to miss it, but she feared she would. She wanted to stay too. She wanted to watch her baby grow up and do all of that with them. She wanted to be a mom more than anything. She wanted to be there for them because her mom wasn't there for her. To love them because she wasn't loved. Jo loved her baby so much more than words could ever express. More than she ever thought she could love her child. 
Yet, she was so tired. She felt like if she could just close her eyes and get some rest, then everything would be okay. But she wasn't sure if she would wake up if she did. Jo fought her heavy eyelids with everything inside of her but it felt like a losing battle. She was just too tired.
“Please, please, Jo, stay with me. I need you here,” Alex said as he leaned closer as the desperation spilled over and his eyes filled with tears.
“I'm right here,” Jo whispered again, smiling up at him. She promised him she was there, taking in this moment with him. 
“Please stay,” Alex begged her, but she couldn't promise she'd stay and changed the subject.
“As soon as this is over, I'm going to take a nap for a week and you can take care of the baby,” Jo said with a smile as he chuckled. It helped relieve some of the tension and his eyes smiled as he looked down at her. 
“When we get out of here, you can nap for as long as you want and I'll take care of the baby and you, I promise,” Alex said, smiling as he looked down at her still brushing his thumb over her cheek and she knew he would keep his promise.
“I'm here, I'm here, I have the blood,” Schmitt said, running into the OR as Jo turned her head to see the blood bags. 
“Great, let's get them hung,” Meredith said immediately, moving to help Schmitt hang the bags of blood. There was a lot, but Jo knew she needed all of it. She could still feel the blood dripping between her legs and she knew she was soaking the table underneath her. 
“Dr. Karev,” Carina said, looking over at him. “Do you want to come over here and cut the cord, I’m almost ready to pull your baby out.
“Yes,” Alex said eagerly, but he looked back at Jo.
“Go watch our baby boy be born,” Jo said with a smile.
“Our baby boy or girl,” Alex reminded her with a chuckle.
“It's a boy,” Jo said for sure. 
Alex just leaned down and placed a kiss on her forehead through his mask as Jo smiled. Then he stood up and looked over to where Carina was working inside of her. Jo smiled but she watched Alex freeze as he looked over the curtain to her open belly.
“Alex?” Jo asked, staring at him as his eyes glanced over at her. Then back to her belly, then back to her face and as the color drained from his cheeks. 
“Alex?” Meredith asked as well, looking over at him. 
“Alex!” Jo said again more insistently as he just stared at her.
But he gave them no warning before he fell to the floor.
“Alex!” Jo screamed again, picking up her head to look over at him, despite how dizzy it made her feel.
The nurses ran to him as Jo watched them swarm around where Alex had fallen, although she couldn't see him on the floor. Then Meredith appeared in front of her. “It's okay Jo, we’ve got him.”
“He's all right, he's just fainted,” one of the nurses said with a shake of her head. 
Jo sighed in relief as she laid her head back down. Alex was okay.
The next thing she knew, the cries of her child filled the room. It was a loud screaming cry, but it was the best sound Jo had ever heard. Her baby was here and they were alive and screaming. Jo looked up at Carina, but she couldn't see anything except her masked face as the curtain was too high. 
“Your baby boy is here,” Carina said, excitement filling her voice as she finally held up the baby for Jo to see. 
She looked up and saw her big baby boy. He was clenching his fists and his eyes were shut as he wailed with an open mouth. His lips were full like hers and he had her round cheeks, but he was his father's son. He was huge with a chubby stomach and chubby arms that he swung around. He also had a large Karev sized head.
“He's beautiful,” Jo said with a cry as the tears streamed out of the corners of her eyes in full force, but she smiled. 
She was so unbelievably happy and all she could focus on was her baby. She watched as Carina handed him over to Hayes, who took her baby boy over to the warmer. Jo felt a sting of sadness that Alex didn't get to cut the cord as she remembered her husband.
“Is Alex okay?” Jo asked, watching Hayes look over her baby in the warmer as his strong cry still filled the room.
“He's going to be fine, Jo,” Amelia said, although she couldn't see where they were.
The room was a buzz as nurses and doctors moved around her and Carina shouted orders for medications although Jo could barely hear them as she focused on her son. She watched Hayes wrap him up in a hospital blanket and look over at her.
“Do you want to meet your son, Jo?” Hayes asked, his eyes crinkling under his mask as he smiled.
Jo eagerly nodded as he brought the baby up to her, so they were cheek to cheek. She nearly cried in relief as she got to meet her son. Someone unstrapped one of her arms and Jo held her baby boy. 
“Hi my baby,” Jo said as she turned to kiss his cheek. 
The moment Hayes brought him over, his cry had softened and after she kissed him, he fell silent. Jo knew his eyes were closed, but she couldn't see his face. Her son, she had the baby boy she knew she would. And she loved him so much. He snuggled up next to her and she kissed him again. She couldn't wait to actually hold him in her arms and look down at his face.
“Is he okay,” Jo asked, looking up at Hayes, who nodded.
“He's absolutely perfect Jo,” Hayes said with a reassuring nod. “His APGAR scores are a perfect 10.” 
“That's good, that's good,” Jo said briefly, closing her eyes again as she felt like she could rest now. Hayes kept the baby on her cheek and she relished this moment. 
“Is that him?” Alex asked as Jo looked over to see that he was getting settled in a wheelchair, his eyes still a little foggy as he stared at them. 
“It is,” Jo said with a large beaming smile. 
“What happened?” Alex asked, rubbing the back of his head.
“You fainted,” Jo said of the little laugh. “What went wrong? I saw you staring at me and then at my belly and then you just passed out.” 
Alex just took his head, unable to take his eyes off their son. He looked down at their beautiful boy as Meredith wheeled him closer so they were both together. Alex reached out to run a finger across his cheek as well. As they both stared at the little miracle of their child.
“He's perfect,” Alex said, his eyes crinkling as he stared at their baby boy and she knew he was smiling under his mask. 
“Yeah, he's perfect,” Jo agreed, looking back at their son.
Jo closed her eyes as she felt her baby boy's hand against her cheek. She relished in this little moment together. She felt her vision going dark as sleep overtook her. But she was desperate to fight it and stay with her son. He needed her. Jo quickly opened her eyes again to see someone standing over her. 
“Stay with me, Jo,” Meredith said sternly as she looked at her.
“Where's my son?” Jo asked, turning her head to look for him. 
“He's right over there,” Meredith said, pointing to where Hayes had the newborn on the warmer again.
Jo didn't even remember him leaving her cheek. She must have slipped out of consciousness. Jo felt so tired, she just wanted to fall asleep as her eyes fluttered. Still, she mustered up the strength to glance up at Meredith again. 
“Meredith.” 
“Yeah, I'm right here Jo.” 
“Meredith,” Jo said, looking up at her again. “You have to promise me that Alex won't name our son Alex Jr. or some male version of Josephine like Joe or something.” 
“AJ is a good name, though,” Alex countered and Jo smiled, but she shook her head. 
Although it made her dizzy, she heard everybody shouting around her. She looked up to see that Schmidt was hanging more bags of blood and starting to squeeze them. She was still so tired and her legs felt very wet.
“What are you talking about, Jo? You're going to be here to pick out a name with him,” Meredith said, trying to sound lighthearted, but failing.
Jo closed her eyes before opening them again. They were so heavy and all she wanted to do was sleep. But she had to do this. “Promised me.” 
“Okay, I promise,” Meredith said with a nod.
“Where's Alex? I want Alex,” Jo demanded, she didn't want to be without him right now. 
“I'm right here, Jo,” Alex said as she looked over to see him sitting on a stool across the room, although he couldn't meet her eyes as Amelia was pointing a flashlight in them to check him.
“I want Alex,” Jo said again, losing the fight to keep her eyes open as they closed.
“Jo, Jo! Alex, get over here,” Meredith shouted, as she gripped Jo's hand and gave it a squeeze. 
“Jo, I'm here, I’m right here,” Alex said, his voice pleading with her as he held her cheek.
She managed to open her eyes and smile up at him. As Alex glanced down at her, his eyes filled with tears but still he put on a smile for her. 
“Can I hold our baby again?”
“Of course,” Alex said, glancing back at Hayes.
He had heard her request and was already bringing their baby back to her. He handed the baby to Alex who held him against her cheek. 
She smiled and looked over at Alex again. “I like the name, Leo, Leonard.”
“I like that one too,” Alex said with a smile as the tears spilled over onto his cheeks. “And for a middle name, we should do Michael or Wilson for you.”
“I like Wilson,” Jo said, glancing over at her baby as he snuggled against her cheek.
“Yeah, I like it. Leonard Wilson Karev,” Alex said, rubbing a finger against Leo's cheek.
“It's kind of a mouthful,” Jo chuckled as she looked up at him.
“Yeah, it is,” Alex agreed, but he smiled too.
“I love you and tell Luna and Leo that I love them, to the moon and… and…” Jo couldn’t finish her sentence as she was too sleepy and had to close her eyes. 
“No, Jo, please, please, don't go,” Alex begged her, but she had already closed her eyes and before she knew it she had drifted off into the darkness.

summary: years after they last saw each other, jo and alex reunite at a conference, and while she is still struggling from her heartache, he seems to be mr. perfectly fine.

A/N: guess who finished this just now? me. that's who. I was actually working on this one long before the announcement for this event came out, but it gave me the motivation to continue this! Alex is unintentionally an ass in this and for that, i’m sorry. Anyways… hope you enjoy! (first part's pretty crappy, but after that it gets better.)

The absolute last thing Jo wanted right now was to attend a medical conference in Salt Lake City, Utah. There were multiple reasons why she should’ve said no. One; she had just started her second year of her OB GYN residency a few weeks prior, and there were a very limited number of lectures on her new line of study where she was headed. Two; she now held an absolute hatred for the Seattle Tacoma International Airport, since it was the place where her ex-husband stood before her, placed a soft kiss on her lips and promised to see her in a couple of weeks, lying to her face about seeing his mother in Iowa, when in reality he was going to see his ex-wife and kids, never really planning on returning home.
It was ironic really; how not even two years ago, she was standing in front of gate 48, saying goodbye to him, and now she stood in the same spot, in front of the exact same gate, with him nowhere near her side as she glared up at the same exact entrance she last saw him at.
(She also now hated the number forty-eight)
But, since Meredith had been hounding her non-stop about attending this event, she gave in. Because how was she supposed to say no to the woman who had barely survived the pandemic? Along with a plethora of other things she was too lazy to memorize. (The answer was that she couldn’t. She felt guilty for leaving Meredith behind for OB, so she now did whatever her former mentor wanted, even if more than two years had passed) If Meredith Grey asked for something, Meredith Grey got it. She was turning back into the intern who cried fake tears and would do anything to earn her approval, and Jo was not a fan.
“It’ll be good for you Wilson. You need to leave that loft of your’s and go somewhere Wilson, blah blah blah,” she mutters to herself as she shows the attendant her ticket, not even bothering to return the smile, settling on brushing a loose strand of hair behind her ear instead. The messy bun on the top of her head bounced in place, and she felt like a college student after a night of studying, but rather than books tucked under her arm it was a neck pillow and a carry-on.
She shuffles into her seat, thankful that it was an aisle one before letting her eyes shut, trying to catch a wink of sleep in an attempt to make up for the less than few hours she had gotten the night before. It proves to be unsuccessful, because the businessman sitting next to her ate his chips so loud she had to restrain herself from coiling into herself every time a chip crunched beneath his teeth, and the toddler behind her continuously managed to kick her seat, no matter how many times his mother told him to stop.
After three hours of loud chewing, seat kicking, and crying babies, she lands in Salt Lake, taking in a deep breath when she finally escapes the crowded airport and is met with humid air. She picks up her rental car, and arrives at her hotel not long after, immediately tossing her bags into the corner and sprawling onto the bed. Eventually, she moves to unpack her clothes and other necessities, casting a glance at the clock which read 10:37.
Taking a look around the room, she decided that nothing more needs to be done; all of her makeup is laid out for the morning, and what she chose to wear is already hanging up in the closet, freshly ironed and wrinkle-free. She yawns, crawling under the covers and flicking off the light, sleep coming easily.
When she wakes up the next morning to her alarm clock going off, she’s freshly rested and in a better mood than she normally was when she had to wake up before nine. The early morning sun beats down onto her face, and she knows that if she wanted to get to the first lecture on time, she needed to start getting ready sooner rather than later.
With a sigh, she begrudgingly gets out of bed, hopping in a quick shower and changing into her clothes. She adds some soft waves to her hair, in the hopes that it made her look more put together than she felt. It didn’t seem that just because she got hours of sleep made up for the fact that she would rather be curled up in the soft sheets of her bed at home than the stiff ones the hotel provided.
It doesn’t take long until she’s arrived in the lobby, where doctors all around her were dressed in firmly pressed suits and skirts. Tables advertising different events throughout the day were lined up and down the hallways, fellow surgeons and mentees passing out little pamphlets with information about their courses littering the pages.
Jo passes by a few stands displaying lectures on the OBGYN field, mentally making a list of the ones she would like to attend.
A quick glance to her watch tells her that the first item on her list was starting soon, and with that she dumps the now-empty coffee cup she had ordered a few minutes prior as a pick-me-up, and shuffles into the seat of the auditorium, the chairs around her filling up quicker than she expects.
It’s only a few minutes later Dr. Maria Cavanaugh steps out onto the stage, silencing the crowd after a round of applause. Jo leans back into her seat, feeling grateful that Meredith had pushed her to go to this conference.
If her whole weekend is spent listening to experts talk and teach about her new specialty, she thinks she’s going to enjoy it.

It��s hours and a multitude of lectures later when she exits another grand room, fellow surgeons beside her chatting about the new information they’d acquired. She feels a small smile grace her lips as she realizes that the excited squeals could only come from the med school students that had been invited from the nearby college.
She pulls out her phone, seeing a text from Link and Meredith, both along the lines of that they hoped she arrived safely and that she enjoyed her time away from home. She responds, pocketing her phone and making her way to the hotel bar, settling into one of the few empty seats and ordering a whiskey sour, thanking the bartender when it's placed in front of her not even a minute later.
She sips the drink slowly, feeling some leftover tenseness from sitting all day leave her body as the alcohol starts to flow through her veins. She was far from drunk, but even saying tipsy felt like too much of a stretch.
Pleasantly buzzed, she decides on, taking another sip of her drink, making a brief second of eye contact with a man on the other end of the bar. He raises her glass with a slight nod of his head, and she does the same. She’s tempted to continue; take use of the way the man’s making subtle glances to the seat next to him when her ears pick up another conversion from a few feet away, freezing her in place, glass nearly dropping to the floor.
“Do you want anything?”
“Just a water’s fine. I’m gonna go talk to Dr. Conwell about some of the research her and her team were talking about.”
She turns in her seat, against her better judgment. She watches as he kisses her cheek before leaning against the bar’s counter a few feet away.
She feels as if ice had just been dropped down her back —frozen in place, unable to move as she watches Alex drum his fingers on the dark wood, head nodding along to the soft tune playing through the speakers. He grabs his drinks from the bartender; two waters, one with ice and one without, about to walk towards the woman he was with earlier when he spots her.
“Jo?” he questions, blinking multiple times as if to see if it was really her, and not a trick of the light.
She wants to run when he makes his way over to her. She wants to run, and scream, and throw her drink in his face. But instead, she places a tight, thin smile on her lips and acts as if nothing was wrong.
“Alex, hi!” She turns to face him, her voice too high for her enthusiasm to be true, but if he knows it he doesn’t make any move to show it.
He runs a hand through his hair —it's longer than the last time she saw it, a bit of grey speckled in so lightly that it wouldn’t be noticeable to someone who wasn’t looking.”I uh, I didn't know you’d be here.”
She wants to roll her eyes, but refrains. “Well,” she spins her drink with the straw, watching as the ice cubes clink against the glass “The feeling’s mutual.”
“How are you?” He asks after a few beats of silence, and she wants nothing more than to yell at him that he couldn’t talk to her like this —like they were old friends who had lost contact after a while, and were due for a catch up. Because they weren’t old friends. They were so far from friends that someone could build a bridge with the amount of space they had between them.
“Fine,” she lies through her teeth, her heart nearly beating out of her chest in nervousness. She was worried that she was going to break down at any second, spew the words that she had said to Carly in anger at him if he looked at her any longer as if things were okay between them.
“You?” she questions, not interested in the answer, sipping her drink similarly to how she had the night of Bailey’s wedding, the unwelcome memory of the start of their relationship blossoming to the front of her mind.
He sips his water, “Good. I’ve been good.”
With his hand clasped around the glass she catches sight of the pristine, shiny gold ring that sits on his fourth finger, and she has to fight back the growing lump in her throat that threatens to escape.
“Congrats,” she says instead, eyebrows pinched together as she nods towards the ring, a tell that always told whether or not her words rang true.
He seems to have forgotten that, and takes her words as a compliment. He gives her a small smile, “Thanks.”
Jo wants to laugh, because this was so far from the Alex Karev she had known, the man in front of her unfamiliar and so, so different from the one she married years ago. The Alex she knew would’ve never ordered a water instead of a beer, or act as if everything was okay between them when it was so far from it.
He’d changed, and she couldn’t say that she liked it.
“How are the kids?” she asks, and she can tell that she’s surprised him. He looks taken aback for a moment, before smiling gently and reaching into his pocket for his phone. “They’re great,” he says, pulling up a photo of them, the twins smiling brightly into the camera.
“That was at their birthday party last week,” he explains, and she notices the large 7 balloons that are floating in the background.
“He looks like you,” she comments about the boy. He grins, and she wants nothing more than to slap him for not being able to see that she’s so obviously hurting, that her heart is breaking inside of her chest. That she selfishly wants him to say that he made a mistake, that he was in love with her, not Izzie. But her life wasn’t a trashy romance novel, and even if it was, Alex was never one to admit to his faults, his pride and ego too large to have it bruised in such a way.
She had always wondered what he had been up to after he had left. If he was okay. She’d imagined different futures for him, some including Izzie and the kids, some of them not. She’s even naively envisioned ones with her and the children they had always talked about having one day.
Somedays, when she was feeling worse for wear, she had wished that he was miserable —that he and Izzie weren’t together and as far as they went communication-wise was a phone call to see who had the kids that week; that he was as alone and bitter as she was.
But instead, here he was, Mr. Perfectly Fine. The man who had looked her in the eyes and told her he would never go away.
Here he was, Mr. Perfectly Fine. Married to the woman he had left her for, a shiny gold ring on his finger instead of neat, polished wood.
For the past two years she’d been picking up the pieces of herself that he had shattered and left behind, only just starting to put herself back together, and here he was, Mr. Perfectly Fine. While she was piecing together her shattered heart, he was picking up her, the woman he had left her for.
She had thought that he was different from the rest; different from everyone who had ever left her. But he wasn’t —the man she had thought was so different was so exactly the same as everyone else.
“I’m sorry, Jo.” he says, and she has the urge to laugh —laugh because he wasn’t sorry, she could tell. He was sorry that he left her and caused her pain, sure. But he wasn’t sorry that he had done it, that he had married Izzie again and lived with her and his kids on a farm in the middle of Kansas. He wasn’t sorry about that at all.
Mr. Insincere Apology so he didn’t look like the bad guy.
She acts as if she believes him, giving him another tight smile, and lies through her teeth once more. “It’s fine Alex. It’s in the past.”
Jo checks phone, making a face when she ‘realizes’ the time, eager to get out of there as soon as she could. “I need to go, early morning,” she fibs, and she internally rolls her eyes at how he believes her. While he seemed to have forgotten everything about her, she still remembers every detail about him.
“It was nice seeing you again Jo,” he’s playing with the sleeves of his jacket and his eyes meet hers, and for less than a second it’s almost enough to break her resolve.
Another tight smile graces her lips, and she begins to walk away when her body fights her mind and she finds herself leaning down, her lips against his ear.
“I’m glad it was worth it, Alex.”
Her voice is harsh and bitter, yet broken, and she doesn’t need to look at him to know that his face has fallen, and he’s realized that everything she’d told him that night was a lie. She turns and lets the tears fall silently, making her way back up to her room with a head full of thoughts about her ex-husband, about Alex Karev.
