It does make sense and even funnier that Arata would be confused on how Yuji and Aoi could be related when they look nothing alike because he and Akari (his older sister) actually do look related.
And even still, Yuji doesn't look like any of his actual blood related brothers!

Interpretations of all the Death Painting Wombs in fetus form based on what little we've seen from the anime + Death Painting Yuji :)
In this AU Yuji would take on a more humanoid appearance even as a fetus. Since he would have been produced from a bit of Sukuna's DNA plus Jin Itadori's (who I hc as being a descendant of Heian Era Sukuna's brother) while he is in fetal form he would have more of Heian Sukuna's features. Those would fade or fall off as he ages, and rather than incarnating like his brothers he would grow like a normal human. This could be rapid growth or gradual, I haven't decided yet. I want the cast to interact w/ Yuji in his little ugly baby form so I'm leaning towards rapid aging.
For how he'd look when he's more grown up, I think that he would be able to pass as a human. If he eats Sukuna's fingers in this AU it would activate the dormant transformation process and he would start to look more like Heian Sukuna. That way Yuji would be forced to look less and less human with every finger he ingests in exchange for power.
