At Yale’s Long COVID Clinic, Lisa Sanders Is Trying It All

Since the beginning of the pandemic, she — together with colleagues in the pulmonary and neurology departments — had been seeing long-COVID patients at Yale but often in an ad hoc way. Some of the doctors had become so flooded with people seeking help that they were having difficulty scheduling and treating their regular patients who came to them for everything else ...
Long-COVID patients, generally speaking, have been very miserable for a very long time, and because the illness attacks their brains, their hearts, their lungs, their guts, their joints — sometimes simultaneously, sometimes intermittently, and sometimes in a chain reaction — they bounce from specialist to specialist, none of whom has the bandwidth to hear their whole frustrating ordeal together with the expertise to address all of their complaints: the nonspecific pain, the perpetual exhaustion, the bewildering test results, the one-off treatments. “These are people who have not been able to tell their story to anybody but their spouse and their mom — for years sometimes,” Sanders tells me. “And they are, in some ways, every doctor’s worst nightmare.”

Long COVID has been pushing the limits of hospital systems everywhere, not just at Yale. As Americans emerged from the most acute phase of the pandemic, as mask and vaccine mandates lifted and life returned to a semblance of normal for the people who had contracted COVID and recovered, primary-care physicians started to say, “‘I’m not interested in long COVID,’ or ‘I don’t treat long COVID. Let me refer you to a specialist,’” said David Putrino, who runs the new chronic-illness recovery clinic at Mount Sinai. For their part, Putrino added, the specialists were saying, “This is not what my practice is. This is not an emergency anymore.” Patients all over the country reported monthslong waiting times for appointments at long-COVID clinics. All the while, scientists and pundits heaped skepticism on the very notion of long COVID, arguing that infection made people stronger, that new variants posed no threats, that the danger of long COVID was overblown — implying that what patients were suffering from was all in their heads.
Forgotten in this debate are the 65 million people worldwide for whom the pandemic remains a torturous everyday reality.

In the late 1990s, patients with a galaxy of unexplained chronic symptoms — including fatigue, sore throat, joint pain, insomnia, dizziness, brain fog, and depression — began to gather into activist and identity groups, calling themselves sufferers of “chronic Lyme.” They had something, they argued, related to a previous infection from the bite of a deer tick, but their doctors were dismissing them as whiners and neurotics. This was in an era when the medical Establishment was rolling its collective eyes at patients who were querying whether diagnoses of “chronic fatigue syndrome” or “fibromyalgia” might fit their symptoms — ones that looked very much like those linked to chronic Lyme.
Sanders joined the chorus of debunkers. These patients had real symptoms and real ailments, she asserted in Every Patient Tells a Story. But the collection of symptoms was “hopelessly broad and overinclusive,” she wrote. “These are some of the most common symptoms of patients presenting to a primary care office.” She concluded that chronic Lyme was a “phantom diagnosis.” 
“I completely regret that chapter,” she says now. “I would like to rewrite it.” Sanders explains that she was reacting to the doctors who were preying on suffering people by prescribing interminable courses of antibiotics that were not helping them: “But I completely misunderstood it. The patients were making the connection between their symptoms and Lyme disease.”

“We’re not paying enough attention,” she said. “We’re not. Doctors are still dismissing this disease as something that’s in your head. I have seen the exchanges on Twitter: ‘Long COVID is exaggerated, not real.’” Women are diagnosed with long COVID at roughly twice the rate of men, Iwasaki pointed out: “I think if the situation was reversed, we’d pay even more attention to this disease.” - Akiko Iwasaki, immunologist at Yale [also known for her work nasal vaccine & long covid research]

But even Sanders was not prepared for how little doctors and scientists know about long COVID. There is no blood test. Health officials can’t even agree on how to define it. The CDC describes long COVID as “signs, symptoms, and conditions that continue or develop after acute COVID-19 infection” — or, in Sanders’s paraphrase, “You got COVID and then something bad happened.” Under the CDC definition, patients have long COVID if they are symptomatic at least four weeks after initial infection. The WHO defines it similarly but with a different time frame: occurring or lasting at least three months after initial infection. This discrepancy matters to Sanders because, as much as possible, she wants to identify patients who have long COVID and not those who may take a little longer to recover from their original illness. In her clinic, she uses the WHO definition.

Sanders, more than ever before, is dependent on the patient’s account — on detailed specifics — to establish her diagnosis. It’s a process of elimination and deduction. So she has “learned just to shut the fuck up and listen.”

The commander from 2, a middle-aged woman named Lyme, takes us on a virtual tour of the Nut, its interior and fortifications, and recounts the failed attempts to seize it. I've crossed paths with her briefly a couple of times since my arrival, and was dogged by the feeling I'd met her before. She's memorable enough, standing over six feet tall and heavily muscled. But it's only when I see a clip of her in the field, leading a raid on the main entrance of the Nut, that something clicks and I realize I'm in the presence of another victor. Lyme, the tribute from District 2, who won her Hunger Games over a generation ago. Effie sent us her tape, among others, to prepare for the Quarter Quell. I've probably caught glimpses of her during the Games over the years, but she's kept a low profile. With my newfound knowledge of Haymitch's and Finnick's treatment, all I can think is: What did the Capitol do to her after she won?

apparently if you're already disabled and you get hit by a fucking car while walking home from a college class then the dumbass's insurance wants to use your disability to give you less compensation money

if anything the fact that I was already disabled makes it worse? becasue it further disabled me?!!! It made my already painful body even more painful
what the actual ever living fuck
i knew insurance was a fucking scam but holy shit
anyways now I have to wait another 2 years for this lawsuit to go through so I can actually get my medical bills covered and get compensation for pain and suffering
fuck insurance and fuck this capitalist hellscape that allows insurance companies to take advantage of people like this

This is my OC, Lyme, illustrated by @cirenk. Lyme is a minor antagonist in Plasmasonic: Deadtides. She's a half-human, half-slime girl who fights alongside the O.S.O. (Occultural Surveillance Organization). In Lyme's past, Leah Locklear was captured and experimented on giving her the abilities that she now possesses. The experiments were traumatic and the mad doctors were shrouded in mystery. Her mission in El Gouna, Egypt was the last mission she was required to go on before the O.S.O. handed her information on who transformed her so that she could take revenge and stop them from experimenting on others. Lyme, alongside the OSO, teams up with Tomi and Jaaden to stop Captain Lalh and Xesyn.

I know people really want to see Haymitch and Finnick's games. I'd read those books for sure. (Although a novel than ends with a 14-year-old getting sex trafficked is tough to make a satisfying conclusion.) And I've made my pitch for Mags.
I'd also love to see someone like Lyme. Someone who was presumably a volunteer and got what they wanted ("won") only to realize the Capitol is the enemy and fight for the revolution. We know Lyme dies during the Rebellion but we don't know how or anything else about her life.
Because I think it would be interesting to see the scales fall from a Career tribute's eyes as it were.

"Mockingjay", Chapter 26
Part 3: The Assassin
Chapter 26: Paylor is still there. Katniss takes her rose and internally debates whether Snow is lying or not. She notes that most people she would trust with this matter are dead (Cinna, Boggs, Finnick and Prim). Peeta wouldn't know, and she can't ask Gale. So she turns to Haymitch. She goes to his room and throws water on him (like in CF!) but he hurts her feelings and she flees. The next day, she is bathed when once again Octavia yells "surprise!" The Preps have arrived (just like in CF!) and so has Effie (who we have not seen since CF). Then Gale who doesn't know if it was his and Beetee's bomb. Coin calls the meeting of the remaining Victors. Katniss votes yes and Haymitch votes "with the Mockingjay." Katniss shoots Coin.
-- Last chapter I didn't discuss the fact that Paylor's people (presumably from 8) were the ones to imprison Snow. Do we think that means they captured him? That they were the rebels on the front line? I am curious as to how many soldiers from 13 actually did anything. Certainly those in Squad 451 did. I just wonder if Coin held her troops back so they wouldn't be the ones risking as much since 13 is supposed to be small.
-- RIP to Lyme who must have died sometime between her appearance in 2 and here, or she would be at the meeting. I hope she died in some glorious battle. I imagine the people of 2 really respect military victories and it would suck if she just choked on a corn chip or something.
Children are precious to 13, or so it has always seemed. Well, not me, maybe. Once I had outlived my usefulness, I was expendable. Although I think it's been a long time since I've been considered a child in this war. 
But while I think Haymitch might gamble with my life in the arena, I don't think he'd rat me out to Coin.
One thing I have noticed in this reread is that Haymitch and Katniss are not tattletales. He eats her dinner when he first shows up and she says she's going to report him but doesn't, she doesn't report Johanna for stealing her drugs, so this reads as a bit of an acknowledgement of how similar they are.
“They say Plutarch and Haymitch had a hard time keeping her alive,” comments Venia under her breath. “She was imprisoned after your escape, so that helps.”
Effie is back. I wonder if she was held wherever Enobaria was held? It says all the Victors are wearing 13 jumpsuits which doesn't really make sense? Enobaria was never in 13 as far as we know. I guess Coin is making everyone wear them now? We should kill her for that alone. I don't look good in a jumpsuit; I'm too petite.

Being disabled in this society sucks so bad most of the time especially when cannabis is the only thing that helps
Like I can’t go anywhere and do things I enjoy because of pain and fatigue so I micro-dose (cannabis) to keep myself functional and ease the pain but micro-dosing means I can’t go anywhere unless someone else drives and there isn’t anywhere near enough public transport available
Also cannabis is so fucking expensive and when you’re disabled you can’t really work full time and insurance sure as hell won’t help help even though cannabis is the best option since over the counter pain meds don’t do shit and prescription pain meds are incredibly addictive (to me not worth the risk even if a Doctor would prescribe them)
And the state I live in still hasnt legalized it yet and the legal delta 9/8/10 thc-a/v etc edibles tend to not be gluten free (fucking celiac) so my options are to either cross state boarders and be upcharged by dispensaries near the boarder or to buy legal flower/ pre rolls …but as we all know smoking is kinda bad for your lungs and I really hate it but like… wtf am I supposed to do?
(Luckily in Sep there’s gonna be a bill to legalize medical marijuana in Wisconsin but it’ll still be slow progress getting the industry to catch up)
Anyways being disabled in America just means being fucked over constantly at every turn and being so isolated most of the time
And I’m incredibly privileged in that my husband makes decent money so we aren’t living paycheck to paycheck and we both have strong family & friend support systems to fall back on if shit hits the wall
Disabled people who don’t have those things suffer even more and are super fucked by the system
Being disabled is so fucking expensive and it��s exhausting
I just wanna fucking live without dealing with all of this every moment of the day
