* chronic heart failure.

* a hemorrhage in the brain.

* low blood pressure.

* liver problems.

* moderate to severe kidney impairment.

* chronic idiopathic constipation.

Who should not take thiazides ?

* Hypotension ( in low bp)

* Allergy to sulphur-containing medications.

* Gout.

* Kidney failure.

* Lithium therapy.

* Hypokalemia (low level of potassium)

* May worsen diabetes.

Dose of thiazide ( chlorothiazide ) for hypertension

* Starting: 500 - 1000 mg/day

* Maintenance: 500 - 1500 mg/day

* Max: 2000 mg/day

* May be given in one or two divided doses

* May take without regard to food

Dose of nicardipine (cardene) for hypertension

* Starting: 20 mg three times a day

* Maintenance: 20 - 40 mg three times a day

* Max: 120 mg a day

* Increase dose at intervals of ≥ 3 days

* May take without regard to food

Dose of labetalol for pregnancy

Tablet

* 100mg

* 200mg

* 300mg

Injection

100 mg PO q12hr initially; increased by 100 mg q12hr every 2-3 days

Usual dosage range: 200-400 mg PO q12hr; not to exceed 2400 mg/day

Other hypertension drugs in pregnancy

Methyldopa

Clonidine

Hydralazine

Prazosin
