I was looking up possible errors for my orgo lab and found this website of "rookie mistakes" and some of these are too funny

Took a good sniff of liquid NH3 to see if it smelled much different - nearly passed out

Added reaction to a sep funnel without closing the stopcock, then tried to catch the DCM in your hands

Injected self with syringe containing BF3.Et2O

Didn't read the part of the instructions that said add concentrated sulfuric acid dropwise - had to leave lab for a few minutes while the acid cloud dispersed

Dropped dry ice into a LiAlH4 reaction to "cool it down"

Melted shoe to the ground with HCl

Flooded the lab. Tried to clean up the water with kitty litter.
