Most important warning: self-administered syringes and injection discussions of legal medications (Depo-Provera) approved of by professionals and properly researched. P.S. this may sound rather Karen-like but I would never do this to someone’s face. Online ranting and acknowledging where I could do better is not the same as screaming in public for bossy requests or comps, etc. Ew.

Another ‘warning’… pharmacists being kick-ass allies and giving a damn about their patients.

I’m really annoyed because (and I know healthcare and scheduling is a clusterfuck right now, but…) for over a month now I’ve been trying to get an appointment in person to get this injectable medication that is, yes, birth control, but is also used for endometriosis in my case. And I have severe endometriosis (exacerbated severely by fibromyalgia, siiiiigh) to the point I bleed enough and lose so much I have to go to the hospital when my care is not properly preventative… like in this case, and the pain is unbelievably severe also to the point I’ve spent time in the hospital, including my 11th Christmas Eve and Day. I started this injectable medication at 13 because it was the only thing that came close to helping reduce my endometrial tissue. Even a hysterectomy wouldn’t help as much, unless they decided to go the super invasive route and remove all the organs (or parts of them) that had become ‘infected’ by the tissue. Again, tissue where it’s not supposed to be, and it causes extreme pain as the tissue tries to flush out of my body each period, even if it’s attached to, like, my pancreas. Just no. That does not work at all. No. That is not fun.

SO. I’m 31, nearing 32, and the doctor’s office knows this. I’ve had the same doctor since I was 10. Been on this medication nearly non-stop for just shy of two decades (with appropriate precautions such as bone density tests) because of the absolute severity of the pain and my inability to function when it hits… which can be months at a time of non-stop bleeding and morning sickness-level nausea and vomiting, migraines and the occasional complete inability to move—in other words, it’s debilitating.

My doctor (even the nurses, as it’s in large print at the top of my file in the system) knows all about this. They’re supposed to call me if I’m overdue by a certain margin (I get they’re busy but months and months???). But my doc’s also a bit of an airhead (albeit a smart one when he focuses) and takes forever to reply to anything on time, even when it’s a severe issue, but not severe enough to go to the hospital. But it’s gotten to the point where the nurses say to go to the ER and then the ER nurses and doctors there get SUPER pissed off (AT ME AND SOMEHOW NOT AT MY DOCTOR/NURSES AND THEIR ORDERS) at the ‘waste of time’, and it’s just a clusterfuck.

Oh yeah, and that ER visit while I was overdue for my injection? Internal intestinal bleeding along with a lovely, even if small, perforation in my fucking uterus from the growth of endometrial tissue. I MEAN COME ON — WHAT IN THE HELL. Totally preventable if they fit me in when I called literally over a month ago.

But I will not change my doctor (the other docs at the practice know what is going on and have offered to take me on, but they don’t have the experience with myself and my conditions or the history, but they can do little else because of professional conduct—it’s between myself and my doc) because he is the only one who treats me with humanity and understands fibromyalgia, endometriosis, pre-MS and pre-RhA/PsA, endo-related IBS, (ulcerative) colitis, and other neurological conditions with any degree of empathy. (See, I told you I’m a mess!) There is no way I’m switching offices in the perpetual shortage of doctors in Canada moving elsewhere for m o n e y (plus Covid-19 being a teen hooligan and constantly coming back to wreck more goddamn shit, including everyone’s sanity, then setting things on fire like the real hooligans in my village have been doing this summer — I mean… what in the hell!?!?), so with all that in mind I actually thank my lucky stars. So I put up with a lot of this shit because he treats me, besides him being an airhead, like an actual human being deserving of compassion and care and quality of life despite my severe disabilities and pain. So.

I’m usually treated really well (even if they often think I’m a nuisance for daring to be severely chronically ill/in pain all the time) so I try to be patient and good and understanding when I can.

But his STAFF (I know they’re busy and I’ve been patient but they’ve been so awful honestly to the point I cried hard enough my dad noticed my red eyes and frustration-tear fracks on my face)! And the doc himself’s inability to reply to notes on time even when urgent and when he knows the circumstances (I admit I am a bit of a hard patient so I can understand if he just kinda ignores me sometimes, honestly). But in this case I was THREE DAMN MONTHS LATE for my injection and they’ve always called in the past when I was coming due if it looked like I hadn’t scheduled an injection, so that I was all on time and squared away and didn’t risk severe pain and damage to my already-fucked hormonal system (learning I couldn’t have kids was absolutely heartbreaking, let me tell you, but even a hysterectomy in that case would solve nothing — this is by far the easiest option, especially considering how my fibromyalgia would fuck with my post-surgery recovery and leave me with lasting pain for years if not decades; sigh).

Anyway. So. After some ridiculous levels of back and forth and some truly remarkable levels of lack of compassion (she kept giving me the exact same, word for word response in a bored tone UGH) considering the severe pain I was in (I was told, in front of OTHER PATIENTS AND STAFF, that I could just wait until I talk to the doctor myself at my next phone appointment and then schedule my injection for my next MONTHLY followup — 4.5 months overdue at that point, it would’ve been — because, and I quote, ‘am used to dealing with pain because of my fibromyalgia and years of dealing with it and other conditions’ which they named in front of others!!!!!!!! what. the. fuck. But I kept my cool because I know all these people, my mom taught their kids music, they’re a fixture of the community, etc. and I refuse to be a Karen…. At least externally.

But here comes the nice part that makes me love our new (okay, he’s been here like 5 years but still, in a small town that’s pretty new lmao) pharmacist that much more. Rasik was aware of my frustration with the doctor and nurses and was even the one who brought to my attention that, at the time, I was 2 months late for my injection and he was a bit concerned since he’s privy to how much pain I exist in without throwing in one or more knives directly into my womb, ovaries, tummy, hips, and other areas my endometrial tissue has taken root. He’s such a sweetheart and he really does care for his patients— the work he does with my father’s diabetes (the tricky one where you’re not obese) management is above and beyond the call of a pharmacist and I will forever be grateful for that alone, never mind how he cares for me.

So I went in today to pick up another medication, after yet another frustrating stop-over at the nurses’ desks, and he suggested I ask for my injectable medication (it’s Depo-Provera, by the way) and the syringe plus the two tips necessary — I’m actually familiar with this since I had to learn epinephrine injections from an early age (not Epipen) and how to give testosterone daily to my ex-husband (sorry not sorry, dude, but congrats on your first kid *grouchy thumbs up*). But yeah! Legally he’s not allowed to suggest I give it to myself, but he was getting super fed up with the nurses and doctors dragging their feet and ‘being assholes with little empathy’ in his own words, so I took the hint and requested my vial plus syringe, as well as the drawing and injection gauge needles…. which he gleefully filled for me, and I reiterated that it was ‘fully my idea, not yours, Rasik, because everyone knows I’m dumb and would never think it’s you if something happened’ (I’m not dumb and I’ve given injections to others many times looool).

Long story short: HERE’S TO PHARMACISTS AROUND THE WORLD, BEING AMAZING AND CARING FOR THEIR PATIENTS AND ‘BENDING BUT NOT REALLY BENDING’ THE RULES TO MAKE SURE THEIR CLIENTS ARE CARED FOR PROPERLY. They are amazing and deserve every last bit of your courtesy, especially when they pull double duty every. single. day. because of Covid and their subsequent boosters. (i.e. boosters in the form of humans who are fucking stupid if they have no medical reason not to get the vaccine… I mean JFC.)

Rasik? You are amazing and I am 100% going to find you some Indian-Canadian (or North Indian; I believe that’s where he’s from originally) treats or desserts or make some myself after slyly asking his assistant what he leans toward liking.

Be kind to one another, yeah, but… my goodness: be kind to those who can truly make a difference in your health, sanity, and even life or death.

P.S. … now I just gotta stab myself intramuscularly after making sure there’s no air bubbles and etc., and swap out to the proper gauge needle (different, smaller, to draw from the vial, larger to inject so that it goes in more quickly and, oddly enough, hurts less haha). I don’t think air bubbles are as much of an issue as when injecting intravenously (ummm I have a doctor uncle and grandma nurse and nurse friends, so shush 😆). But I’ve done this for others and animals so I should be good! :)

I’m a smart enough cookie even if I’ve lost a few nibble-size pieces around the edges. 😉😘 buahaha

Misfortune's Intentions

(Liam x OC* Elisse Mallin) (Drake x Riley) in a Choices The Royal Heir fan fiction

A/N This idea came to me one night when I couldn't sleep. In this version from The Royal Heir, it has been nearly three years since Riley married Drake. Their infant daughter, Hope, was named heir to Cordonia's crown. Liam considers the possibility of dating after pining for Riley all these years. Elisse is a noble and first cousin to Queen Amalas of Monterisso. She will eventually become an ambassador, settling in Cordonia to continue Amalas's alliance. She also has a small bit of significant history coinciding with Liam's social season. This first part though will take us through the past for our main characters.

Prologue

Two years ago, Cordonia...

"To keep people's attention away from your brother's," Constantine's voice hardened, "decision to abdicate," his fist clinched for a moment, "we will have you participate in a season for a potential bride."

Liam swallowed uncomfortably. He knew there was a possibility his father would insist on this particular custom. He had hoped though that like Leo, he would be given a few years reprieve before he had to choose a bride.

Constantine fought through the chest pain he hid from everyone as he continued to explain what the next few months would be like for his only obedient son.

"We will have the usual bevy of ladies from Cordonia's noble houses. Perhaps sprinkle a few from other countries to appease the people."

"Yes sir." Liam mumbled when his father seemed to silently demand his agreement. "Whatever you think is the best way to move forward."

"Good." Constantine waved toward the door. "You may go. We will make our announcement tomorrow."

Liam bowed his head in acceptance. Still in a state of disbelief as he made his way to his chambers, he didn't hear his best friend calling his name.

"Liam?" Drake jogged to catch up to him. "I heard about Leo. Is it true?"

The new crown prince waited to answer until they were behind closed doors.

"It is." He rubbed a hand down his face. "Leo abdicated."

Drake poured them both a drink. He observed the slight tremble in Liam's hand as he took a healthy gulp of whiskey. Eyes closed briefly as he gathered his thoughts.

"I--I'm going to have to get married."

"What?" Drake breathed in disbelief. "Why?"

"All part of being king." Liam's lips twisted in a bitter smile.

He rolled his glass between his hands as he stared down at its contents. "I have to choose one during this year's social season."

"You have to go through all that Leo did?" Drake asked.

"Yes, but unlike his, I won't know who I'm supposed to choose." Liam grimaced as he swallowed the rest of his drink. "Madeleine had been chosen years ago by Father and Regina. The competition was mostly for the public's entertainment than to actually choose a queen."

Drake slumped back in his chair. "I'm sorry, Liam."

"It's..." He got up to pace, "it's fine. It's an honor to have a chance to serve my country. To marry and have a family...perhaps life will be better."

Perhaps this palace will feel like a home, he thought.

"When will it be announced?"

"Tomorrow."

"So soon." Drake murmured.

Liam briskly nodded.

His closest friend held his glass up in a toast. "Here's hoping, Liam."

Monterisso Royal Palace, a week later...

"Elisse." Amalas hugged her cousin. "Thank you for coming so quickly."

"Of course." Elisse took the offered chair. Her eyes dropped down to the baby bump showing. "How are you feeling?"

"Good." Amalas gently patted her tummy. "This baby has finally decided I'm not the enemy and has agreed that food is definitely a good thing."

Elisse laughed, shaking her head at her cousin's humor.

"But enough about the spoiled future king or queen I’m having." Amalas handed over a letter with Cordonia's royal seal. "Read this."

Elisse scanned the invitation, eyes widening slightly. "Prince Leo abdicated? Do we know why?"

The spy queen chuckled. "You know I do." She leaned over and grabbed a file off her table. "Let's just say that the former crown prince did not want the responsibilities that went along with the crown."

Elisse opened the file and snorted. "I've heard that Lady Madeleine can be difficult to deal with."

"That along with wanting his freedom to wander the earth like a rich playboy are his reasons." Amalas pointed out a photograph one of her agents snapped recently on a cruise ship. "This woman, Katie, is an even bigger reason."

"They're engaged?" Elisse whispered in disbelief. "He didn't waste any time, did he?"

"No, he did not." Amalas reclined in her chair. "Elisse? What would you think of Monterisso gaining an alliance with Cordonia?"

"I think it would benefit our country as well as theirs." She replied, setting Leo's folder down.

Another folder was handed to her. Her lips curved into a soft smile at the pictures of Cordonia's new crown prince. Each aspect her cousin found on Liam had made him a man she thought she would enjoy meeting.

"Marriage is the easiest and strongest alliance one can have with a foreign power."

Elisse looked up. "You're suggesting  I take part in Prince Liam's search."

"Exactly." Amalas smiled at her. "I can't think of a better queen for Cordonia than you." She reached over and grasped her hand. "But feel free to say no. I won't force you to do this if you do not wish to."

"I would like to meet him." Elisse admitted. She lowered his eyes to his picture. "He is very handsome."

"You always did like blue eyes." Amalas teased.

Elisse bit back a smile. "I do."

"Would you like to attend Cordonia's social season?"

Elisse slowly nodded. "Yes, I think I would."

Amalas picked up the letter from Constantine. "I'll go ahead and schedule your health exam."

"My what?" Elisse blinked in surprise.

"Cordonia's king isn't taking any chances that his last possible heir marries a woman unable to have children." Amalas grinned at her cousin. "Lucky for him that our family is notorious for our reproduction abilities."

Elisse laughed. "Our mothers certainly came from a large family." Her smile turned tender. "Whenever I think of my future, I always see myself with three or four children."

A few weeks later, Cordonia, the King's study...

"Have you made a list of those that are participating?" Regina asked her husband.

"I have." He handed her a sheet of paper.

The Queen scanned the names. "What happened to the Monterisso viscountess? I thought from Queen Amalas's letter that they were hoping to make a match."

Constantine's lips firmed in a disappointed frown. "She cannot have children."

Regina's countenance fell. "How tragic."

"Indeed." He replied. "It would have been a profitable match for us." He released a resigned sigh. "I suppose we now settle for Madeleine to take the crown again."

The viscountess's chambers, Monterisso Palace...

"Elisse." Amalas pleaded. "Please, talk to me."

The young viscountess shook her head. Tears fell silently as she clutched a pillow to her stomach. Her thoughts were too much for her. All the plans she had dreamed of as a little girl were destroyed from one simple examination.

All from thinking Liam had beautiful blue eyes and seemed like a nice man she could easily fall for.

A choked sob burst forth as she doubled over from the pain of it all.

"Elisse." Amalas tried to wrap her arms around her. "I'm so sorry."

Elisse knew her cousin was. She knew Amalas would move heaven and earth for her if she could. Unfortunately, there were some things even a powerful queen could not do.

"We'll call in every specialist we can find." Amalas whispered, gently rocking Elisse. "I refuse to believe--"

"Stop!" Elisse cried out. "Please stop." Her anguished eyes met her cousin's. "I can't continue to hear doctors say that it is impossible."

"It's not." Amalas argued. "I can’t believe you are unable to--"

"I only have one ovary!" Elisse snapped. "It is so damaged from the endometriosis and benign tumors that I'm lucky to have a period every few months." She looked down at her worthless womb. "Even my uterus is filled with fibroids to the point they have warped it beyond repair. Surgery won't help." She wiped angrily at her tears. "No one will marry me now."

"That's not true! You--"

"Amalas, you know what nobles need." Elisse bit out. "They need heirs. They need a woman to fulfill that role. I'm completely obsolete in our world now."

She turned away from her cousin's sudden tears. She didn't want her pity. She didn't want anything except those elusive images of cuddling her own children.

If she could turn her mind off and focus on something else, anything else...

"I'm going to go away for a while."

Amalas jerked upright. "What?"

"I need to be alone." Elisse turned to face her. Her eyes filled with tears when her gaze dropped down to her cousin's ever growing baby bump. "I need to come to terms with all of this."

"Will you back in time for..." An expression of guilt flickered over the queen's face for such a selfish question.

"I'll try." Elisse hedged. "I promise."

She knew Amalas depended on her. Though the two had numerous cousins sprinkled in various titled positions all over Europe, they had always had a special bond that made them more like sisters. Perhaps it was their mothers' doing that made certain they would always have the other close at hand.

But Elisse knew deep down, she would have been close to her cousin without any outside influence. The notorious Queen of Spies distrusted most people, even some of their blood relatives. Yet, she trusted Elisse with everything.

She hated to leave her during her pregnancy, but the viscountess didn’t think she could take being around an expectant mother right now.

Amalas hugged her once more. "I'll arrange your travel. But I insist you take Felix with you as a bodyguard." She waited on Elisse's agreement. "Let me know where and when you wish to leave."

"I don't care where." Elisse whispered. "Just as long as I can leave as soon as possible."

New York City, a month later...

"One nondescript bar as requested." Maxwell announced, holding the door open.

"Steaks for the table!" He yelled out.

"And four whiskeys!" Drake chimed in.

Riley rolled her eyes at Daniel. "You're really going to leave me with these guys?"

"I promise I'll work two of your shifts whenever you want." He bargained.

She let out a tired sigh. "Make it three and we have a deal."

He shook her hand. "You are brutal with negotiations."

She laughed, piled the tumblers of whiskey on her tray, and headed for the bachelor party.

She passed three drinks out. "I thought there were four of you."

"There are." Drake winked at her, causing a blush to form on her cheeks. "Right behind you is number four."

She turned around and blinked.

He's so handsome.

Liam smiled at her. "Pardon me, Miss?"

"Hmm?" She shook herself out of her daze. "Of course."

His smile flashed, making her knees feel slightly weak. "If you would..." He gestured toward his seat that she was blocking.

Her cheeks flared even more with color. "Right. Sorry."

She decided then and there to not allow her eyes to rest on him the remainder of their time here.

A few hours later, Liam approached her.

"Thank you for being so patient with us." His charming smile was a bit bashful. "I don't suppose I could repay such kindness with buying you a drink."

Riley found herself mesmerized by his blue eyes. "Where were you planning on going next?"

"I actually hoped you could help with that." He rubbed the back of his neck. "The guys hoped to go to a nightclub."

Her nose wrinkled. "I say forget the nightclub. There's a beach cove I could show you that is just the place to unwind."

"That sounds perfect." He held his hand out. "Shall we?"

Six months later, Edenbrook Hospital, Boston...

Elisse chewed on her lower lip, wishing she could hear what the group of doctors were saying behind the glass partition.

After taking her file and passing it to a man in surgical scrubs, Dr. Ramsey walked back into her room.

"Ms. Mallin, after further examination," his expression gentled, "I believe your doctors in Monterisso were correct."

Elisse lowered her eyes. "I see."

"I still want to perform the surgeries we discussed." He sat down beside her hospital bed. "The severe pain in your uterus will only increase if we don't remove the fibroids. Dr. Tanka isn’t sure though how it will be after your surgery."

"Whatever you think." She closed her eyes tight. "It doesn't really matter what shape it is in. I suppose we should just remove it altogether."

Her doctor rested his hand over hers. He waited patiently for her to make eye contact.

"We won't know for certain until Dr. Tanka gets in there, but we believe we can save your ovary."

A bitter laugh escaped her lips. "By all means, hold on to that worthless body part."

His brow furrowed. "Elisse, I don't like the thought of you in this state of mind before surgery."

The Dangers of Acetaminophen

Each week, around 23 percent of U.S. adults, or 52 million Americans, use a medication containing acetaminophen, according to the Consumer Healthcare Products Association (1). The ubiquitous use of acetaminophen, both by itself and as a component of over-the-counter and prescribed medications, is a major public health issue. Acetaminophen is generally considered safe when taken as recommended, but the margin between a safe dose and a potentially lethal one is incredibly small. Even when taken as recommended, acetaminophen use can have major side effects. In this article, I’ll discuss the many reasons we should avoid acetaminophen whenever possible and share several safer alternatives for pain relief.

Liver toxicity

Overdose with acetaminophen is the leading cause for calls to poison control centers in the United States, with more than 100,000 instances per year (2). It’s responsible for more than 56,000 emergency room visits and an estimated 458 deaths every year due to acute liver failure. In fact, over half of all acute liver failure cases in the United States are due to acetaminophen overdose (2). Taken over several days, as little as 25 percent above the maximum dose, or just two additional pills a day, has been reported to cause liver damage. This staggered overdosing may be more dangerous than a single large overdose (3).

Think twice before you reach for Tylenol.

Current guidelines recommend taking no more than 4,000 milligrams of acetaminophen daily (4). Yet according to a 2006 study published in the Journal of the American Medical Association, even this dose, taken for four or more days, frequently causes elevated serum alanine aminotransferase (ALT), a marker of liver injury. Moreover, the elevations in ALT often persisted for several days after the acetaminophen was discontinued (5). Concerns about liver toxicity were raised as early as 1977, when an FDA advisory panel said it was “obligatory” to add warning labels about liver damage. The FDA dragged its feet until 2011, when it finally issued a statement urging drug manufacturers to limit the strength of each capsule to 325 milligrams of acetaminophen and mandated warning labels about liver toxicity (4). Acetaminophen toxicity is increased when combined with narcotics like codeine or hydrocodone, when more than the prescribed dose was taken in a 24-hour period, when more than one acetaminophen-containing product is taken at the same time, or when alcohol is consumed while taking acetaminophen (6, 7). How does it cause liver damage? Acetaminophen depletes levels of glutathione, the body’s master antioxidant. Glutathione helps protect cells from damage by free radicals. Interestingly, prompt administration of N-acetylcysteine (NAC), the precursor to glutathione, can prevent mortality from acetaminophen toxicity (8).

Gut health and microbial drug metabolism

Chronic use of acetaminophen doses greater than 2,000 milligrams has been associated with a 3.7 times increased risk of bleeding in the upper gastrointestinal tract (9). Acetaminophen can also cause intestinal permeability. Overdose with acetaminophen causes massive necrosis of the liver tissue, which releases a protein that results in leakage of bacteria from the gut into the bloodstream (10). I’ve written before on the Kresser Institute blog about the role of the gut microbiota in drug metabolism. Interestingly, those with higher levels of certain gut bacterial metabolites may be more susceptible to acetaminophen toxicity (11).

Cardiovascular health, kidney disease, and cancer

Several recent studies have also shed light on acetaminophen’s relation to cardiometabolic health. One study, published just this month, found that those taking opiates (some of which contain acetaminophen) were 95 percent more likely to be obese and 63 percent more likely to have hypertension (12). Of course, we can’t separate out the effects of acetaminophen in these combination drugs. The kidneys are also affected by acetaminophen. Heavy use of acetaminophen is associated with an increased risk for kidney disease. This is especially true when combined with alcohol consumption. One study found that concomitant use of acetaminophen and alcohol resulted in a 2.23 times increased risk for kidney dysfunction (13). Researchers have also found associations with cancer. A 2013 meta-analysis of epidemiological studies found that acetaminophen was associated with a significantly increased risk of kidney cancer (14). A 2011 study of more than 64,000 older men and women found that acetaminophen use was also associated with several different types of blood cancers, including myeloid neoplasms, non-Hodgkin lymphomas, and plasma cell disorders like multiple myeloma (15).

Blunted emotions and empathy

Researchers at Ohio State University wanted to determine if acetaminophen affected emotional processing. They found that participants who took acetaminophen “evaluated unpleasant stimuli less negatively and pleasant stimuli less positively, compared with participants who took a placebo.” In other words, both negative and positive stimuli were less “emotionally arousing” to people who had taken acetaminophen (16). In another double-blind, placebo-controlled experiment led by a different research group, participants rated their perceived pain, personal distress, and empathic concern after reading scenarios about another person’s physical or social pain. Participants who had received acetaminophen beforehand showed less empathy towards others’ pain (17). Together, these studies raise concerns about the broader social side effects of acetaminophen. If a quarter of Americans use this every week, what impact is this having on us as a society?

Autism, ADHD, and brain health

Acetaminophen is currently marketed as safe during pregnancy. However, acetaminophen use may have neurodevelopmental consequences for the fetus. A 2016 study followed more than 2,000 mother–child pairs from the first trimester of pregnancy and performed several tests of behavior when the children were about five years old (18). Forty percent of the children were exposed to acetaminophen in utero. Exposure to acetaminophen was associated with lower attention function and a greater risk for ADHD-like hyperactivity and impulsivity symptoms. In boys, acetaminophen exposure was also related to a greater number of autism spectrum symptoms, and mothers who used acetaminophen more frequently had an even greater chance of having children with autistic-like behavior. The authors even ran the data again, excluding mothers that had chronic illness, urinary tract infections, or fevers during pregnancy, and found the same results, suggesting that it was not illness itself that contributed to the association. Some researchers hypothesize that activation of the cannabinoid system by acetaminophen may be affecting neural development (19). A study in rats found that acetaminophen causes toxicity to neurons even at concentrations too low to cause liver failure (20). It caused the neurons to undergo apoptosis, or programmed cell death.

Severe skin reactions and asthma

Acetaminophen has also been linked to rare but very serious skin reactions such as Stevens-Johnson syndrome and toxic epidermal necrolysis. These reactions can be fatal. Between 1969 and 2012, there were 107 such cases, of which 67 required hospitalization, and 12 people died. In 2013, the FDA issued a warning and mandated that skin reactions be listed on acetaminophen-containing drug labels as a potential adverse effect (21). Exactly how acetaminophen causes these potentially fatal skin reactions is unknown, but particularly alarming is that they can occur even if you’ve taken acetaminophen in the past without any problems. Researchers have also found an association between acetaminophen and asthma. A 2009 systematic review and meta-analysis found that in both adults and children, the risk of asthma increased with prior acetaminophen use. Moreover, prenatal exposure to acetaminophen also increased the risk of asthma in child, possibly by increasing oxidative stress during prepregnancy. (22) Of course, these are only associations, and they don’t prove that acetaminophen is the cause of these problems. Some of these children may have received acetaminophen due to viral respiratory infections that may have also contributed to the development of asthma. Still, the evidence is strong enough that several experts have recommended that acetaminophen use should be avoided in children with asthma or at risk for asthma (23, 24).

Altered reproductive function

Acetaminophen is also an endocrine disruptor. In a 2016 study, rats given acetaminophen had female offspring with fewer eggs, smaller ovaries, and smaller litters when they reached reproductive age. Male offspring had fewer numbers of sperm progenitor cells early in life, but these returned to normal by adulthood. Particularly concerning was the effect it had on the next generation. Granddaughters of the rats given acetaminophen were also found to have smaller ovaries and stunted reproductive function (25). How did this happen? The researchers speculate that it has to do with acetaminophen’s effect on prostaglandins, hormones that are involved in pain and inflammation but are also important in regulating female menstrual cycles and reproduction. In humans, acetaminophen use during pregnancy, especially for durations of more than four weeks, is associated with an increased risk of having male children with an undescended testicle at birth (26). Prolonged exposure to acetaminophen also decreases the amount of testosterone produced by human fetal testes (27), which could have major effects on male development.

Alternatives to acetaminophen

Hopefully, I’ve convinced you to think twice about taking acetaminophen. Ultimately, pain is a sign of a deeper problem, and throwing pills at it is only suppressing symptoms. Eating an anti-inflammatory diet, getting adequate sleep, exercising, getting exposure to sunlight, and managing stress are all part of an effective pain-management strategy. For acute relief, there are some alternative painkillers that are far less toxic and, in many cases, just as effective as acetaminophen. Here are a few that I always keep in my “medicine” cabinet:

Curcumin: a potent anti-inflammatory, curcumin is one of the natural compounds found in turmeric. I recommend 1,500 mg up to three times a day of the Meriva-SR form, which is much more bioavailable than other forms of curcumin. (Note: curcumin is contraindicated if you are taking blood thinners.)

Magnesium: three of the most common reasons for taking acetaminophen over the counter are headaches, muscle pain, and menstrual cramps. For many people, magnesium can provide significant relief from these symptoms, often equivalent to or better than Tylenol. I recommend 400 to 600 mg of the magnesium glycinate form.

Boswellia: another anti-inflammatory, the resin of the Boswellia tree has been used medicinally for centuries. It works via a different mechanism than curcumin, so they are complementary when taken together. I recommend 100 mg up to three times a day in the AKBA form.

Clove oil: works like a charm for toothaches. Mix a few drops of therapeutic-grade clove oil with a spoonful of coconut oil and swish it around the mouth.

CBD oil: also known as cannabidiol, this is the non-psychoactive oil of the cannabis plant that has pain-relieving and anti-inflammatory properties. It does not produce the “high” that THC does, and as of now, it is currently sold legally over the counter. Acupuncture: can be quite effective at modulating local and systemic pain and reducing inflammation.
