Pediatric Cardiac Anesthetist Plays Xenoblade Chronicles Definitive Edition: A Medical Perspective On Faced Mechon

I’m a nerd in many ways more than one: when I’m not in the OR taking care of kiddos with heart problems, I play video games to decompress. As I’m a first-time player having the time of my life with XCDE, I find the Faced Mechons fascinating and wanted to dive deeper into concept and biomedical science behind it. (Spoilers and science talk abound below the cut!)

One of the game’s many plot reveals is that Faced Mechons are fueled and powered by the blood of a Homs pilot, and this is how they’re able to exploit the Monado’s inability to harm people. This may seem like a thing of science fiction, but I think it actually has some real-life basis.

I’m referring to cardiopulmonary bypass (CPB).

This process and technology behind CPB is amazing but also wildly complex, so I’ll do my best to break it down in a way that makes sense to people with no medical background.

You may be wondering how open heart surgery is possible if the heart is normally always pumping and moving blood. Wouldn’t all that movement and bloody mess get in the way if you’re trying to fix a bad heart valve or jacked up coronary arteries? Yes it certainly would, and that’s where CPB comes in.

Here’s a very VERY simplified representation of bypass setup:

More or less the process of cardiopulmonary bypass:

-Connect the heart to a bypass machine by surgically inserting cannulas through the aorta (the vessel that lets oxygenated blood go out the heart) and the vena cavas (the vessels that let blood flow into the heart)

-Stop the heart from beating by injection of a cold potassium-rich cardioplegia solution (this appears as a flatline on the EKG monitor during bypass, so this is to be expected, and no one in the OR team is panicking and starting CPR. All that potassium forcibly arrests the heart.)

-Reroute blood to and from the bypass machine circuit so the surgical field is clear, blood gets oxygenated and the body stays perfused

Some caveats, though:

-The tubes and circuit of the bypass machine are foreign material. Any time blood comes into contact with things outside the body, its response is to form clots. This is the same mechanism behind transplant organ rejection. That’s not good at all. Blood needs to flow freely and cleanly without resistance throughout the CPB circuit.

-There’s a drug that makes sure those clots don’t form: an anticoagulant called heparin. Anesthesia gives heparin and measures blood levels to make sure it’s sufficiently anticoagulated before the green light to “go on pump.”

Some questions I asked myself and attempt to reason out here:

Those Face Mechons are obviously at least three times the size of their pilots. The average blood volume in a human/Homs is 4-5 liters. Even if CPB takes pulsatile cardiac output out of the picture, there’s no way they have enough blood to distribute throughout the machine without being bled dry and dying of blood loss. Where’s all the blood coming from?

-I propose that somewhere in the frame is a reservoir of either autologous blood or same-type blood. In real life, autologous blood donation is a method of blood salvage and transfusion: the patient donates a portion of their blood preoperatively for it to be given later in surgery to replenish intraoperative blood loss. I imagine that when they’re not in the field, Seven and other mechanized Homs spend part of their time having a bit of their blood drained over the course of weeks and months for later use. And by same-type blood, I’m referring to the blood transfusion most people typically think of, where the source of blood is from someone else but has the same blood type. This other blood (or allogenic blood, to use the proper clinical term) may come from those people deemed not worthy to become Face Units. Maybe the Mechons grabbed random people off the streets just for blood supply fodder. In the bypass schematic, the reservoir is just another component added to the circuit that allows blood to flow freely between the machine and its pilot.

What about the circuit itself?

-It’s mostly under the skeletal framework of the Mechon, some parts visible (as in the glowing red lines you see in the game). The oxygenator and centrifugal pumps are deeper inside the machine.

Where does the heparinization of blood come in?

-I think it would happen at whatever docking bay the Face Mechons launch from. Maybe the frame is advanced enough to deliver a pre-programmed dose intravenously to its pilot as they board their frame and integrate with it. In real life, if a proper dose of heparin is given to the patient, this preparatory step before going on CPB doesn’t take long at all, only a few minutes.

How do the aortic and venous cannulations fit into this?

-What with the extensive surgery, all the mechanical additions and implants into Seven and the others, it wouldn’t be far-fetched to imagine permanently grafted cannulas that extend past their insertion sites in the heart vessels and connect with tubing from the Mechon frame. If the pilots need to disengage from their machines for whatever reason, the cannulas can just attach on some port on their mechanical bodies and their heart resumes normal pulsatile flow/beating and pumping like it usually would. The cannulas are ready for reattachment with the Mechon frames at any time.

Normally patients on the pump are intubated and under deep anesthesia throughout surgery. If the Homs pilots aren’t anesthetized and intubated, how are they functioning inside the Mechon frames?

-Since the pilots are not having surgery, and of course they’re wide awake and conscious, I’d say they have no need for invasive airways. Some patients need intubation to secure their airway and be hooked to the anesthesia machine, since general anesthesia renders you unable to breathe on your own, so the machine breathes for you. Safe to say that doesn’t apply to the Faced Mechons. I can see them having face masks and nasal cannulas available in the cockpit for enhanced oxygenation, but nothing more than that.

Basically, to boil it all down, I like to think that the Mechon frames function like highly advanced, mobile, weaponized cardiopulmonary bypass machines. It’s a twisted, horrifying mesh of the biological and mechanical.

I hope that this isn’t too over your head and you found it plausible and interesting. Maybe I’m diving too much into this. Oh well, it’s fun to think about. Next I plan to make another post about Seven’s mechanized anatomy and physiology.

a little comic about a headcanon i have about rex's kids of them being all musicians: mio plays the flute, glimmer plays the lute, and mythra's kid the drums (specifically a little hand drum)

This comic takes place after time has resumed after Aionios fell and i thought its possible that everyone may not remember exactly but they're memories may be triggered by something, as we see here with mio remembering noah and miyabi thru her playing the flute and glimmer remembering practicing the lute with A.

Ouroboros kids gender hcs cuz now I'm thinking about it a fair bit

Noah: trans man. of course. his tmasc swag is immense. when he got top surgery, Lanz was the first one to both convince him to do it and congratulate him afterwards. he sees it as one of the best decisions of his life, and loves his scars

Eunie: she's a girl in the most convoluted of ways. best not to ask her about it she barely understands it herself

Lanz: his gender is gymbro. cis dude but with a healthy masculinity. he's also the biggest trans ally you'll ever see

Mio: girl in the most vague way. like if you asked a creature that didn't know anything about gender what a girl was. that's Mio. she prefers to go unlabelled but if she had to, she'd just slap on a demigirl and call it a day

Sena: the kind of gender bimbo girlies have. confident femininity while also being constantly up for experimenting and trying out different expressions. vaguely genderfluid, I think you could say. though she mainly likes sticking to the fem side

Taion: the agender boy of all time!! I think he feels very disconnected from his gender as he sees no use or importance for the concept (also for autism reasons), so he usually completely disregards it. for the simplicity he describes himself as a boy, but really he's just a person trying to get by. though however, a soft masculinity serves as a great comfort for him

Shulk, Fiora, and their family wish everyone a merry Crys-mas! The super embarrassing parents part is optional.

I put some ramblings about my Shulk/Fiora family theories and headcanons under the cut if anyone cares to see them.

Shulk/Fiora kid theories/headcanons in descending order of how seriously I take them:

Nikol - I mean...

Crys - Used to not like this, but I became a strong believer after Future Redeemed because of all the Alvis parallels and what FR reveals about Shulk, especially the Christlike traits, kindly mentor personality, and the silver eyes (which I used to take as proof against the theory). In my HC, he's the firstborn by several years and Ontos-flavored child due to Shulk's lingering connection with Alvis. Since Christmas doesn't exist on Bionis, Crys-mas is what they call his birthday.

Ethel - Wasn't the biggest fan at first, especially since it was mostly tied to Cammuravi being Pyra and Rex's child. But all the deliberate Fiora references with Ethel have me leaning a little more towards her being related than not. In my HC, she's the Pneuma-flavored child, twins with Nikol, and her silver hair comes from Fiora's lingering connection with Meyneth/Galea.

Noah - Very unlikely and I don't actually take it that seriously, but there's just enough connection and thematic parallels to Shulk that it's fun to think about. He's the baby of the bunch and either an adopted orphan or the Logos-flavored child, due to Shulk being explicitly identified as a Logos analogue due to his lingering connection with Zanza. My boring HC for his black hair is he got it from one of Fiora and Dunban's parents, while my crack HC is he got it because Malos hitched himself to Shulk's seed from beyond the grave because Logosbros or something.

Headcanon: so, Klaus made sure everyone in the world was speaking the same language as him, even in Alrest. That's why we never hear about differing languages in any of the games. However, after his death, the worlds' are now evolving, which means stuff like Linguistic Shift starts occurring. It's not noticeable by the time Xenoblade 3 happens, however, the next time the worlds interact, there is a language barrier.

My roommate and I have just been on an absolute journey of a conversation.

It all began on the equipment screen. My roommate was swapping Shulk and Reyn's pants to get the better (unique) gem onto Reyn, and I said the fateful words: "You know, I feel like this has absolutely happened for them before. I don't know when, or why, but Shulk and Reyn have absolutely swapped pants with each other before." And my roommate leapt on it immediately, so now we have a scenario where Reyn notices that the gem on Shulk's pants is better for him than it is for Shulk and is like "hey, hand it over", to which Shulk immediately starts stripping (to the horror of anyone in the immediate area) So then we were like "oh, Shulk and Reyn are absolutely "kiss the homies goodnight" with each other, and that lead into a discussion on the sexuality of all the party members, which shook out as follows:

Shulk: Bi but isn't aware of it during the main game. He has enough trouble realizing Fiora likes him, he is NOT aware of men yet

Reyn: Either straight but a ferocious ally or bi by virtue of living in a barracks for an undefined amount of time. In that case he's either experimented with a guy once and decided it was fine but not for him or loved it and is all in on men

Fiora: Straight (but has kissed a few girls to make sure)

Sharla: Bi, but has deeply reconsidered her attraction to men after witnessing Reyn and Shulk attempt to swap pants in public

Dunban: The only crush he's ever had was on Dickson as a teenager but that's long gone at this point

Riki: Straight, but has definitely experimented in the past

Melia: Aroace, but has kissed Fiora once just to be sure. Thought she had a crush on Shulk but it was actually a squish

And once all of this was established, we finally left the equipment screen with this absolute gem: "I'm so glad we had this conversation. Let's go kill god."

Kevesi and Agnian Heroes living in the City headcanons

-Ethel and Cammuravi make literally everything a competition (who can run the most laps around the entire City, who can eat the most torpedo wraps in one sitting, good ol' fashioned brawls at the wrestling ring, etc)

-Alexandria tried to enroll in university but was turned away for being too young. High school quickly bored her to tears. It begged the university to get Aionios's resident smartass off their hands. The university finally caved in and let her enroll to study business and IT.

-Valdi is everyone's go-to guy for fixing Levnises, of course. He insists on bunking in the tech quarter, forgoing actual furnished rooms in the residential quarters.

-Cammuravi was initially treated as a walking fire hazard until Ethel and the Ouroboros gang convinced people that he will not in fact burn down the entire City, by accident or otherwise.

-Fiona regards the park as her favorite spot in the City. She's the City kids' favorite playmate.

-Isurd is a new member of the City's tabletop and board game club. He got Zeon to join when he mentioned that some games are farming sims.

-Miyabi joins the City street performers, alternating between her flute and fans to entertain folks with music and dance.

-Not wanting to steal Ethel and Cammuravi's thunder as already established sparring instructors, Teach spends much of his time at the university to learn how gentler arts are being taught.

-Zeon is absolutely enthralled by the concept of greenhouses and balcony gardens.

-Ethel's second home is the City library. The senior librarian is practically her adopted mother.

-Juniper spends more time outside the City than in it, patrolling and exploring the wilderness outskirts to their heart's content.

-Ashera is the resident terror among Lost Number recruits and soldiers. She's always showing up to the training facility uninvited, joking about eating them for breakfast, lunch, or dinner, depending on the time of day. Ethel swatting her off like a fly is a daily occurrence. ("Ash please go away, I already fought you yesterday, you're scaring the poor recruits")

-Isurd becomes a regular at the City's most popular massage parlor and acupuncture clinic. It's very hard for Taion to hold a proper conversation with him there when he's emitting satisfied "oohs" and "aahs" every five seconds.

-Despite stating precautions and giving warnings to discourage any derring-do, Juniper is not popular among parents for being a "bad influence" on their children, who want to slide down ziplines willy nilly. They have to limit their sliding to nighttime, when the kids are in bed.

-It was Fiona's idea to grow more flowers around the Remembrance Stones. City folks backed the project and believe the place has become even more lovely and precious for it.

-Valdi has his own names for all the City Automatons. Half the Lost Numbers think it's endearing and the other half think it's downright annoying.

-One day Hollis gathered all of them at the medical facility to sit down for The Talk. Alexandria and Teach took meticulous notes. Zeon, who had just grasped mastering how to grow potatoes, was hopelessly confused. Juniper, a veteran at growing potatoes, was just as confused. Ashera kept making faces like something had died in the room. Valdi nodded along politely, but really couldn't care less about something that's not at all like making Levnises. Fiona and Miyabi thought the whole thing was simply magical. Isurd stared off into space, mulling over that kind of possibility with a certain someone who loves saffronias. Ethel and Cammuravi looked around everywhere but at each other, their faces red as Noah's jacket.

xenoblades writing hcs because I saw a post and it has Inspired me (is this the third post I've made in this format now ??)

Noah - honestly, pretty average. legible and neat, and he can get it down fast. though he misspells things often, mainly putting down the wrong letter. the paper is often covered in smudges from him desperately erasing it.

Mio - she has very nice, flowy writing, that's both legible and easy to get down. of course, she's the one who writes the most often, so she has the most practice. she definitely puts cute little flourishes on letters, like long y's and fancy capitals.

Eunie - she tries to make it neat, but it always falls flat. her writing is a mix of normal and cursive, and often ends up hard to read by accident. she also misspells things often, but instead of erasing her mistakes she just scribbles it out or writes the right letter over top, extra bold.

Taion - he has Annoyingly fancy cursive. like it's basically illegible. he thinks it's perfectly fine but whenever the shows his notes to others they all struggle and give up after a few seconds of trying to read it. (Eunie complained about it so much he got self conscious and changed it. still cursive, but much more legible.)

Lanz - pure chicken scratch, the exact opposite as Taion's but the same level of illegible. Noah and Eunie have gotten used to reading his writing, but Taion nearly had a heart attack when he first saw it (Sena just gave a pitiful smile). he's trying to make it better, but he also just doesn't write things often.

Sena - oh she tries so hard to make it pretty. perfect bubble letters, very rounded in style. she has fun with it but it takes her forever to get anything down since she's making sure it's All Perfect.

Background-less version:

FR Shulk in the style of one of Daisuke Ishiwatari's Baiken drawings, because one-armed swords(wo)men are some of my favorite things.

I like to think that when Shulk initially lost his arm, he trained directly with a copy of Dunban's own sword to get his bearings first. Thus by the time he started using the REX+ again, he had the muscle memory of swinging it like a katana.
