Therapeutic options in ectopic pregnancy are as follows:

1.Expectant management

2.Methotrexate

3.Surgery(laparatomy vs laparascopy)

1.expectant management:

Asymptomatic patient

hemodynamically stable

declining B-HCG levels

The patient should be closely followed

2.Metotraxate(single vs multiple dose)with leucovorin

#Indication:

Hemodynamic stability

No severe or persisting abdominal pain

The ability to follow up multiple times

Normal baseline liver and renal function test results.

Gestational sac <4cm

B-HCG<5000miu/ml

No fetal cardiac activity

#Contraindication:

fetal cardiac activity

free fluid

B-HCG >5000miu/ml

Gestational sac >4 cm

Presence of intrauterine pregancy(absolute contra.)

Evidence of tubal rupture(absolute)

Sensitivity to methotrexate(absolute)

#regimen:The multiple-dose regimen for methotrexate consists of the administration of daily IM doses of 1 mg/kg, with leucovorin (folinic acid, which reduces side effects) given on alternating days, until there is a 15% decline in β-HCG over 2 days.

The single-dose regimen consists of 1 dose of methotrexate 50 mg/m2, followed by a repeat β-HCG measurement at day 4, and another dose of methotrexate 50 mg/m2 if the β-HCG has declined less than 15% between days 4 and 7.

Sequence of TB infection in kidney.

Stenosis of calyceal neck or pelvic ureteric junction → Hydronephrosis and pyonephrosis → Perinephric abscess → Kidney replaced by caseous material (puty kidney) → Calcifcation → Autonephrectomy (a calcifed non-functioning kidney, representing end-stage disease).

Autonephrectomy is the fnal result of marked parenchymal fbrosis and obstructive uropathy.
