The textbook series of events usually involves: your cervix getting softer, thinner, and opening contractions starting and growing stronger and closer together your water breaking Your doctor may start checking how you’re progressing at each prenatal checkup during your last trimester. When might you go into labor if your doctor tells you you’re already 1 centimeter dilated? Here’s what to expect. What does dilation mean? Your cervix is the passageway from the uterus to the vagina. During pregnancy, hormones in your body cause many changes. One change is that the mucus gets thicker in the opening of the cervix, causing a plug. This prevents bacteria and other pathogens from reaching the developing baby. Your cervix typically remains long and closed (around 3 to 4 centimeters in length) until you get closer to delivery day. During the first stage of labor, your cervix will start to open (dilate) and thin out (efface) to allow your baby to move through your birth canal. Dilation starts at 1 centimeter (less than 1/2 inch) and goes all the way to 10 centimeters before there’s enough space to push your baby into the world. Dilation and labor You may have no signs or symptoms that your cervix has started to dilate or efface. Sometimes, the only way you’ll know is if your doctor examines your cervix at a routine appointment late in your pregnancy, or if you have an ultrasound. The cervix of first-time moms may remain long and closed until delivery day. Moms who’ve had a baby before may be dilated for weeks leading up to their delivery day. Contractions help the cervix dilate and efface from the beginning stages to the full 10 centimeters. Still, you may be dilated slightly without noticeable contractions. Other signs of labor Being 1 centimeter dilated doesn’t necessarily mean you’ll go into labor today, tomorrow, or even a week from now — even if you’re close to your due date. Fortunately, there are other signs you can look out for that might indicate your baby is on their way into the world. Lightening You may have heard that your baby will drop close to your due date. This process is called lightening. It describes when your baby starts to settle lower in your pelvis to prepare for delivery. Lightening may happen in the weeks, days, or hours before you go into labor. Mucous plug Your cervix protects your baby during pregnancy, and this includes your mucous plug. As your cervix begins to dilate, bits and pieces of the plug may start to fall out. You may notice mucus on your underwear when you use the restroom. The color can range from clear, to pink, to blood-tinged. Labor may happen the day you see your mucous plug, or several days later. Contractions If you feel your belly tighten and release, you may be experiencing practice contractions (Braxton-Hicks), or the real deal. The key is to time whatever tightening you’re feeling. Time if they’re coming randomly, or in regular intervals (every 5, 10, or 12 minutes, for example). Usually, if these contractions are infrequent and painless, they’re practice contractions. Read more about Braxton-Hicks vs. real contractions. If they grow stronger, longer, and closer together and are accompanied by cramping, it’s a good idea to let your doctor know what’s going on. You may also feel contractions start in your back and wrap around your abdomen. Rupture of membranes One of the more classic labor signs is your water breaking. If this happens, you may experience a large gush, or a trickle of fluid. The liquid is typically clear and odorless. It’s important to call your doctor if you suspect your waters have broken. Take note of how much fluid you experienced and any secondary symptoms (contractions, pain, bleeding) you have. When to call your doctor Preterm labor (before 37 weeks) If you’re experiencing bleeding or leaking of fluid at any point in your pregnancy, call your doctor or midwife right away. Also call your doctor if you’re having frequent contractions, pelvic pressure, or other signs of labor weeks (or months) ahead of your due date. Term labor (37 weeks or more) Let your doctor know about any symptoms of labor you’re experiencing. Contact your doctor if you think you may have dilated early (for example, if you lose your mucous plug or have bloody discharge). There are risks to trying at-home induction techniques. The biggest risk would be if you’re preterm and your baby isn’t head-down. Induction techniques to break your water may carry a risk of your baby’s umbilical cord slipping out before their head. This is a life-threatening emergency called cord prolapse. What to do in an emergency Call 911 if your water breaks at home and you notice any bright red bleeding, or a dark brown color in your water. I’ve technically got about 4 weeks to go, but considering that my son arrived 3 weeks early, we are prepared for anything at this point. (Although I have to admit, I’m hoping this one takes his or her sweet time… I’m in no rush.) Because I’m delivering this baby in a new city this time around, I’m working with a new doula and I thought I’d share the tips to prepare for labor that she shared with me at our most recent appointment. Many of these tips and supplements are ones I used during my last pregnancy (and I’d say they were effective!), but some of them are new to me, too. Note: This post is not a substitute for medical advice nor is it a prescription. You should always consult with your own doctor before making any changes to your diet or lifestyle. Around the 36 to 37 week mark, here’s what I add to my routine to help encourage a better labor and delivery experience. Other than eating dates, walking, and squatting, which should be safe at any time during an uneventful pregnancy, I wouldn’t start taking these extra supplements any earlier than 36 weeks just to make sure you don’t accidentally encourage preterm labor. (None of these supplements should be capable of inducing labor before you’re ready, but I prefer to stay on the safe side!) Dates. Research has shown that women who consume 6 dates per day for 4 weeks prior to their estimated due date are more likely to go into spontaneous labor, have a shorter first stage of labor, and have a higher cervical dilation upon admission to the hospital when compared to women who did not consume any dates. [source] These Date Energy Balls, Raw Brownie Bites, and Peanut Butter “Cookie” Larabars are all a great way to increase your date consumption! Red Raspberry Leaf Tea. Red raspberry leaf tea is thought to help balance hormones and tone the uterus, possibly because it’s high in vitamins A, B, C, and E, along with important minerals like magnesium, potassium, and calcium. According to this study, red raspberry leaf tea has been shown to shorten the duration of labor and may reduce the need for medical interventions, such as the need for c-sections, forceps, and vacuum births. I’ve heard anywhere from 3 to 4 cups of tea per day is recommended in the third trimester, and I’m currently using this brand to make brewed iced tea. Alfalfa. This is something that I did not take during my first pregnancy, but my doula and many midwives recommend it because it naturally contains high levels of vitamin K, which is essential for blood clotting– and therefore it might help to prevent postpartum hemorrhaging. If you’d prefer to skip a supplement, vitamin K can be found in dark leafy greens like kale, spinach, and collard greens, and you can also brew alfalfa as a tea. Stevia. This recommendation was a shocker to me, as many of you know my negative experience with stevia in the past. According to my doula, she has seen women increase their amniotic fluid rather quickly by adding a drop of pure stevia to each glass of water they drink, and in some cases, they’ve been able to avoid induction due to low amniotic fluid using this method. I’ve only been able to find anecdotal evidence regarding this information, so I can’t say for certain that it’s proven, but I suppose it would be worth a shot if you’re worried about low fluid levels. It’s not currently a concern for my own pregnancy, so I’m not taking it, but I thought I’d pass along the tip. (Do be sure to look for pure stevia, as many brands have added ingredients that are not ideal.) Squats. Squats are one of the best exercises you can do to prepare for labor, as they can create more room for your baby to enter the birth canal. The American Pregnancy Association recommends squatting during labor because it can open your pelvic outlet by 10 percent! I did my fair share of squats using Physique 57 workouts during my last pregnancy, and my doula recommends aiming for 300 squats a day(!) if you want an easier birthing experience. Foods that are fried, highly processed foods, sugar and baked items are better avoided. - Avoid: Alcohol Please go virgin when it comes to alcoholic drinks while on your period (yes, even if you had a really difficult day). While alcohol may reduce cramps temporarily, there are too many negative effects for you to ignore—despite the temporary relief it may give you. It can cause your period to become irregular and also increase estrogen levels in the body, worsening period symptoms. Estrogen is one of the two main female sex hormones is responsible for keeping a girl's menstrual cycle regular. It also keeps the cholestrol and blood sugar levels in check, and is therefore extremely important for a problem-free period. Just one more consequence if you’re not convinced: its diuretic nature increases your water retention levels. What this means is that there is an excess of fluid build-up in your body, which makes you appear more bloated. Tip: Try a watered-down lassi or some coconut water, it will keep you hydrated and are recommended period foods. - Avoid: Caffeinated drinks Turn down the volume of coffee—one of the main drinks you need to avoid during your period. This really helps, because caffeine is a stimuant, which leads to a fluctuation in your hormones, and that is something you definitely want to avoid during your period. It increases blood pressure and heart rate leading to tension and anxiety attacks and in turn, worsening the PMS symptoms. Caffeine also decreases your body's ability to absorb iron, which affects your energy levels and could increase PMS symptoms. Tip: Have green tea, tomato and carrot juice, or any type of soup instead. These are some of the foods to eat during a period to avoid cramps. - Avoid: Processed foods Frozen foods, fast foods, bacon, pickles, canned soups, papad etc. come with unknown ingredients and preservatives that aren’t the best foods during your period. They can interfere with hormones and amplify PMS discomfort. Processed foods are also a huge source of sodium, which is a major cause of bloating and water retention, and is best avoided during and a few days before your period. Since we are constantly exposed to a lot of varieties of foods, it is hard to determine if a particular food is processed or not. But to make things easier, here's a great tip - always read the ingredients on the package; if there are more than five ingredients mentioned, then it would be better to avoid buying it, as it is more than likely to be processed. Tip: Have homemade meals, lots of salads and veggies, and heart and tummy-warming foods like khichadi and porridge. - Avoid: Foods high in fats Fatty foods influence hormones, lead to cramps and can make you feel gassy. Given the fact that your skin is already sensitive during these days, incorrect food choices during your period can make this issue worse and cause it to become even more dry and dehydrated. Additionally, foods that are rich in saturated fat content increase the production of a particular chemical called prostaglandins, which are notorious for alleviating period pain. Ideally, you should try and avoid fatty meats, burgers and cream-based desserts. Tip: Here's what to eat during periods instead - whole wheat bread, dal chawal, porridge, salmon and lean meats, etc. - Avoid: Dairy products This might come as a surprise to you but dairy products like milk, cream and cheese are some of the foods to avoid during periods. A new form of the coronavirus is spreading rapidly in New York City, that may weaken the effectiveness of vaccines, two teams of researchers have found. The new variant, called B.1.526, first appeared in samples collected in the city in November. By the middle of this month, it accounted for about one in four viral sequences appearing in a database shared by scientists. One study of the new variant, led by a group at Caltech, was posted online on Tuesday. The other, by researchers at Columbia University, is not yet public. Neither study has been vetted by peer review nor published in a scientific journal. But the consistent results suggest that the variant’s spread is real, experts said. “It’s not particularly happy news,” said Michel Nussenzweig, an immunologist at Rockefeller University who was not involved in the new research. “But just knowing about it is good because then we can perhaps do something about it.” Dr. Nussenzweig said he was more worried about the variant in New York than the one quickly spreading in California. Yet another contagious new variant, discovered in Britain, now accounts for about 2,000 cases in 45 states. It is expected to become the most prevalent form of the coronavirus in the United States by the end of March. Researchers have been scrutinizing the genetic material of the virus to see how it might be changing. They examine genetic sequences of virus taken from a small proportion of infected people to chart the emergence of new versions. The Caltech researchers discovered the rise in B.1.526 by scanning for mutations in hundreds of thousands of viral genetic sequences in a database called GISAID. “There was a pattern that was recurring, and a group of isolates concentrated in the New York region that I hadn’t seen,” said Anthony West, a computational biologist at Caltech. He and his colleagues found two versions of the coronavirus increasing in frequency: one with the E484K mutation seen in South Africa and Brazil, which is thought to help the virus partially dodge the vaccines; and another with a mutation called S477N, which may affect how tightly the virus binds to human cells. By mid-February, the two together accounted for about 27 percent of New York City viral sequences deposited into the database, Dr. West said. (For the moment, both are grouped together as B.1.526.) The Columbia University researchers took a different approach. They sequenced 1,142 samples from patients at their medical center. They found that 12 percent of people with the coronavirus had been infected with the variant that contains the mutation E484K. Patients infected with virus carrying that mutation were about six years older on average and more likely to have been hospitalized. While the majority of patients were found in neighborhoods close to the hospital — particularly Washington Heights and Inwood — there were several other cases scattered throughout the metropolitan area, said Dr. David Ho, director of the Aaron Diamond AIDS Research Center. “We see cases in Westchester, in the Bronx and Queens, the lower part of Manhattan and in Brooklyn,” Dr. Ho said. “So it seems to be widespread. It’s not a single outbreak.” The team also identified six cases of the variant that pummeled Britain, two infections with a variant identified in Brazil, and one case of the variant that took over in South Africa. The latter two had not been reported in New York City before, Dr. Ho said. The university investigators have alerted the authorities in New York State and in the city, as well as the Centers for Disease Control and Prevention, Dr. Ho said. He and his colleagues plan to sequence about 100 viral genetic samples a day to monitor the variants’ rise. Other experts said the sudden appearance of coronavirus variants was worrying. “Given the involvement of E484K or S477N, combined with the fact that the New York region has a lot of standing immunity from the spring wave, this is definitely one to watch,” said Kristian Andersen, a virologist at the Scripps Research Institute in San Diego, who was not involved in the new research efforts.
