CHATELAINE -- What You Need To Know About Pelvic Floor Dysfunction
BY STEPHANIE FERGUSON (FJHI '24) -- The morning after I gave birth to my first daughter, in 2017, I eased myself out of bed and felt a sudden gush of fluid. I panicked. I’m an ob/gyn, and my first thought was that I was having a postpartum hemorrhage. It could be fatal. I could bleed to death. READ MORE.

One of the isolating things about certain disabilities is that you can't talk about the "gross" aspects so when you start to deal with it, you aren’t prepared. I have EDS (still in the diagnosis process after 5-6 years but multiple doctors said I probably have it and my cousin is already diagnosed with H-EDS) and I knew about the likelihood of digestive issues but nobody mentioned the increased risk of prolapses due to the connective tissue being wonky. So then, when at 17, when I got first developed a minor rectal prolapse, I thought it was normal... Now, at 20, I feel a feeling of pressure and inability to completely empty my bladder all at once (no pain and increased urge to urinate) and I thought it was a UTI for almost 3 months before realizing it was probably a bladder prolapse/a vaginal prolapse. And I can't really tell anyone except my doctor and mom. I can say I feel like shit because my knee subluxed or my joint pain is flaring up, but I can't exactly explain why I need extra bathroom breaks (other than my IBS, which doesn’t help matters).
Roughly 40-50% of people AFAB will experience pelvic organ prolapses (POP) in their lifetime and no one is prepared or aware of what it feels like, so you just ignore the pain and are too embarrassed to talk about it

Common Challenges with the Pelvic Floor During Pregnancy
Though pregnancy is a natural and miraculous experience, it can also lead to various pelvic floor challenges due to the physiological transformations that occur within the body. Some of the prevalent issues encountered during pregnancy include:
1. Urinary Incontinence: The increased pressure on the bladder and pelvic floor muscles often results in urinary leakage, particularly stress urinary incontinence. Additionally, some expectant mothers may face urge incontinence, characterized by sudden and intense urges to urinate.
2. Pelvic Organ Prolapse: Pelvic organ prolapse arises when the pelvic floor muscles and connective tissues weaken, causing pelvic organs to descend into the vaginal canal. Symptoms may include sensations of heaviness or pressure in the pelvic area, vaginal bulging or protrusion, urinary challenges, and discomfort during sexual intercourse.
3. Pelvic Discomfort: Pelvic pain is a frequent concern during pregnancy, stemming from various factors such as altered posture, increased pressure on pelvic structures, hormonal fluctuations, and musculoskeletal imbalances. Conditions like symphysis pubis dysfunction (SPD), sacroiliac joint dysfunction (SIJD), and round ligament pain commonly contribute to pelvic discomfort during pregnancy.
4. Sexual Challenges: Changes in hormone levels, pelvic floor function, and body image during pregnancy can lead to sexual dysfunction for some women. Symptoms may include discomfort or pain during intercourse, reduced libido, and difficulty achieving orgasm. Addressing pelvic floor issues and maintaining open communication with healthcare providers or pelvic floor specialists can aid in enhancing sexual function during pregnancy and postpartum.
You can get in touch with Dr Firuza Parikh, who is India' s best gynecologist and currently practices at HN Reliance Hospital in Mumbai, to help you have a smooth pregnancy journey and also manage any problems associated with the pelvic floor health.

Native tissue repair: This type of POP repair device does not use mesh. Instead, the surgeon uses sutures to repair and tighten the weakened muscles and tissues that support the pelvic organs.
The type of POP repair device that is right for you will depend on your individual circumstances and the severity of your prolapse. In some cases, a combination of a transvaginal mesh and native tissue repair may be used.
Benefits of POP Repair Devices:
Relieve symptoms of POP: POP can cause a variety of symptoms, such as urinary incontinence, bowel incontinence, and pain during intercourse. POP repair devices can help to relieve these symptoms.
Improve quality of life: POP can have a significant impact on a woman's quality of life. POP repair devices can help women to regain their independence and participate in activities that they may have had to avoid due to their prolapse.
Prevent further prolapse: POP can recur without treatment. POP repair devices can help to prevent further prolapse from occurring.
Regional Analysis:
North America is expected to hold the largest share of the global POP repair device market throughout the forecast period. This dominance is due to factors such as high prevalence of POP in North America, well-established healthcare infrastructure, and early adoption of new technologies.

Pelvic Organ Prolapse Repair Market: Global Demand Analysis & Opportunity Outlook 2036
Research Nester’s recent market research analysis on “Pelvic Organ Prolapse Repair Market: Global Demand Analysis & Opportunity Outlook 2036” delivers a detailed competitors analysis and a detailed overview of the global pelvic organ prolapse repair market in terms of market segmentation by treatment type, product type, end-user, application, and by region.
Growing Adoption Minimally Invasive Procedures to Promote Global Market Share of Pelvic Organ Prolapse Repair
The global pelvic organ prolapse repair market is estimated to grow majorly on account of the increasing popularity of minimally invasive procedures. Patients and doctors alike are beginning to favor minimally invasive surgery more and more as they use smaller incisions, inflict less trauma on tissues, minimize blood loss, and shorten hospital stays when compared to traditional surgery. This is expected to significantly drive market growth as laparoscopic prolapse repair is a minimally invasive surgical technique that utilizes a video camera to guide multiple tiny incisions for reconstruction of the pelvic floor.
Some of the major growth factors and challenges that are associated with the growth of the global pelvic organ prolapse repair market are:
Growth Drivers:
Surge in Number of Pregnancies across the globe
Rising Awareness Regarding Reproductive Health
Challenges:
The complications associated with Pelvic Organ Prolapse Repair, and the high cost of the treatment procedures are some of the major factors anticipated to hamper the global market size of pelvic organ prolapse repair. Problems with vaginal repairs for pelvic organ prolapse have included bleeding, infection, and transient nerve or muscle injury which can cause pelvic pain during sexual activity and cause discomfort.

By treatment type, the global pelvic organ prolapse repair market is segmented into surgical, and non-surgical. The non-surgical segment is to garner the highest revenue by the end of 2036 by growing at a significant CAGR over the forecast period. The most common non-surgical treatment for Pelvic Organ Prolapse (POP) is a pessary fitting, that provides support after a prolapse by creating a helpful blockage inside the vagina that happens when the uterine or vaginal walls weaken.
Furthermore, pelvic floor physical therapy, can help the muscles function properly, lessen the symptoms of pelvic floor dysfunction, and help with issues such as incontinence, uncomfortable sex, and other problems. For example, Kegel exercises, also known as pelvic floor muscle training, can also be used to treat overactive bladders and other pelvic floor problems.
Besides this, a surgical procedure such as colporrhaphy, sometimes referred to as vaginoplasty, is a surgical technique used to cure anterior (front) vaginal prolapse and to fix any defects in the vaginal wall that may be producing uncomfortable symptoms to aid in returning the pelvic organs to their natural place.
Additionally, by application, the uterine prolapse is poised to garner a significant share. Uterine prolapse, a common condition that can occur with aging and cause the uterus to sag out of position can be treated by both surgical and nonsurgical methods such as laparoscopic procedures, and Kegel exercises.
By region, the Asia Pacific pelvic organ prolapse repair market is to generate the highest revenue by the end of 2036. This growth is anticipated by a higher burden of uterine prolapse in the region. According to the study, those from lower-income backgrounds are more likely to experience uterine prolapse, which is known to create psychological and physical issues that lower patients' quality of life. For instance, In North India, the prevalence of uterine prolapse is more than 7%, and the incidence is around 19% in Eastern India.
Besides this, nulliparous prolapse accounts for more than 1.5% of genital prolapse in India; in young women who have given birth, this frequency rises to around 6%, making India one of the highest worldwide.
As a result of all of these factors, the surgical sling techniques are extensively utilized in India which are developed by renowned Indian gynecologists, that customize treatment for each patient, especially those who want to maintain their reproductive function.

What causes pelvic organ prolapse?
Tumblr media
Pelvic organ prolapse (POP) is primarily caused by the weakening or stretching of the pelvic floor muscles and tissues that support the pelvic organs. Several factors contribute to this weakening, leading to the descent of pelvic organs into the vaginal space. Common causes include:
Childbirth: The act of giving birth, especially through vaginal delivery, can stretch and weaken the pelvic floor muscles and supportive tissues. The risk increases with the number of vaginal deliveries.
Aging: The natural aging process can lead to a gradual loss of elasticity and strength in the pelvic tissues, making them more prone to prolapse.
Hormonal Changes: Reduced levels of estrogen during menopause can contribute to the weakening of pelvic muscles and tissues, increasing the risk of prolapse.
Chronic Conditions: Conditions that cause increased intra-abdominal pressure, such as chronic constipation, chronic coughing (due to conditions like chronic bronchitis or smoking), and obesity, can strain the pelvic floor and contribute to prolapse.
Genetics: There may be a genetic predisposition to pelvic organ prolapse. If a woman's mother or sisters have experienced prolapse, she may have an increased risk.
Hysterectomy: Surgical removal of the uterus (hysterectomy) can alter pelvic anatomy and increase the risk of prolapse, especially if other supporting structures are weakened during the procedure.
Connective Tissue Disorders: Conditions that affect the strength and integrity of connective tissues, such as Ehlers-Danlos syndrome or Marfan syndrome, can contribute to pelvic organ prolapse.
Heavy Lifting and Straining: Regularly engaging in heavy lifting or activities that involve repetitive straining can contribute to pelvic floor muscle weakness and increase the risk of prolapse.
Obesity: Excess body weight can put additional stress on the pelvic floor muscles, contributing to their weakening and increasing the likelihood of pelvic organ descent.
It's essential to note that pelvic organ prolapse is a multifactorial condition, often influenced by a combination of these factors. Additionally, some women may be more predisposed to prolapse based on their individual health history and genetic factors. If someone experiences symptoms suggestive of pelvic organ prolapse, consultation with a healthcare professional, such as a gynecologist or urogynecologist, is crucial for an accurate diagnosis and appropriate management.
