Bone remodeling is especially significant in oral implantology where the success and longevity of dental implants is highly dependent on the relationship between the implant components and the surrounding oral tissues. The peri-implant bone remodeling occurs when the implant is exposed to the oral environment in a second stage or one stage immediate loading procedure. This bone remodeling which is usually seen around dental implants results in Bone volume reduction. This decrease in bone volume can occur due to two reasons 1. Stress concentration at the Crestal region of dental implant and 2. Localized inflammation of the soft tissue at the implant abutment. Well-healed, osseointegrated Brånemark implants require a moment of torque of over 20 Nom. This is considerably higher when the implants are longer than 13 to 15 mm. Such attempts should not be made in weak bone (Brånemark, 1996) and of course cannot be clinically inserted. As studies by Johansson and Albrektsson (1987) have shown, a torque of 11 Ncm is necessary to remove an implant three weeks after implant placement. This torque is in creased to 68 Nom three months after sur grey, and to 88 Ncm after a healing period of twelve months. Further animal studies with removal torque in the case of healed implants with different surfaces (SLA and Osseotite) showed that the removal torque values for the sandblasted and etched (SLA) implants were considerably higher than in the case of the (Osteotite®) implants, which were only lightly etched (Buser et al., 1998). Further studies were carried out with regard to the removal torque in the case of machine processed, TPS-coated implants and com pared with implants with a SLA surface, which were inserted in poor bone quality of the upper jaw of pigs.
