La Casa de Papel | 3x03 - 48 meters under ground
The love of my life named her Cynthia back when we were told it would be a girl. But then - (...) So that was it! From Cynthia... Cynthi... Cincy - nati. Cincinnati.
It's pure logic. He's named after a city, like his family. This family.
Cincinnati.
Right?

McDonald's Restaurants by Magnitude of Mountain Backdrop / Rut
by u/Gigitoe
Eating a Big Mac® while gazing at Big Mountains is an experience that millions of Americans can relate to. But little do we ask ourselves, which McDonald's restaurants have the biggest, baddest mountain backdrop?
So I set out to answer this question, using rut, a topographic metric that does particularly well at identifying cities with badass mountain backdrops. (For more info, check out this Mercury News article by John Metcalfe, or my research paper)
Roughly speaking, this is how rut works:
The higher the mountains rise above a restaurant, the more impressive they appear, and the greater the rut.
The more steeply the mountains rise above a restaurant, the more impressive they appear, and the greater the rut.
Here's a tier list that shows the rut of the ruttiest McDonald's restaurant in various cities. The cutoffs are rather arbitrary, so please don't poison me with a Grimace Shake if your city doesn't get the tier you like :)
S tier - rut > 400 m (i'm lovin' it. would visit just for mountain views)
Palm Springs, CA (647 m) | Weed, CA (624 m) | Springville, UT (609 m) | Lone Pine, CA (521 m) | Rancho Cucamonga, CA (503 m) | Kailua Kona, HI (432 m) | Salt Lake City, UT (404 m)
A tier - rut between 200 to 400 m (impressive, but probably wouldn't visit just for mountain views)
Colorado Springs, CO (383 m) | Gatlinburg, TN (273 m) | Tacoma, WA (267 m) | Tucson, AZ (246 m) | Juneau, AK (223 m) | Tucson, AZ (209 m) | Albuquerque, NM (215 m) | Las Vegas, NV (209 m)
B tier - rut between 100 and 200 m (mountains nearby, or distant big mountains, or very distant huge mountains)
Anchorage, AK (192 m) | Seattle, WA (185 m) | Manchester, VT (166 m) | El Paso, TX (160 m) | Los Angeles, CA (153 m) | Portland, OR (144 m) | Denver, CO (126 m) | Boise, ID (106 m)
C tier - rut between 50 and 100 m (big hills nearby, or distant mountains, or very distant big mountains)
Rutland, VT (99 m) | San Diego, CA (84 m) | Asheville, NC (75 m) | Middlesboro, KY (70 m) | Phoenix, AZ (67 m) | Roanoke, VA (46 m)
D tier - rut between 25 and 50 m (hills nearby, or distant big hills, or very distant mountains)
Billings, MT (40 m) | Rapid City, SD (37 m) | Dalton, GA (38 m) | Hot Springs, AR (37 m) | Pittsburgh, PA (33 m) | San Francisco, CA (30 m) | Portsmouth, OH (26 m) | Greenville, SC (26 m) | Huntsville, AL (25 m)
E tier - rut between 10 and 25 m (small hills nearby, or hills in the distance, or very distant big hills)
Syracuse, NY (21 m) | Concord, NH (20 m) | Duluth, MN (20 m) | New Haven, CT (14 m) | Cincinnati, OH (13 m) | Wausau, WI (13 m) | Portland, ME (10 m)
F tier - rut between 0 and 10 m (flatter than a patty)
Nashville, TN (8.5 m) | New York City (4.3 m) | Kansas City (2.8 m) | Miami, FL (1.7 m) | Houston (1.6 m), TX | Chicago, IL (0.6 m)
For more locations, here's a spreadsheet with the rut of every McDonald's on this map.
----------------
Note: a rut of X doesn't mean the surrounding mountains rise a height of X above the restaurant. Instead, a rut of X means that a restaurant's mountain backdrop is as impressive as a restaurant at the base of a vertical cliff of height X. The less steeply the surrounding mountains rise above the restaurant, the lower the rut.
Also note: rut only considers rise above surroundings (relative height differences and angle of elevation). it does not consider absolute elevation, nor the "aesthetics" of a mountain, nor visibility (or lack thereof) due to weather or smog.
Attributions: Gavin Rehkemper (locations), FABDEM (elevation model), Google Earth Engine (calculations), ESRI (basemap), Kai Xu (rut metric and map)
If you like rut, you'll probably like its older brother jut even more. Jut measures how impressive, spectacular, or badass a mountain is—considering both its height above surroundings and steepness. If you want to find the most impressive mountains near you or worldwide, you may find the link above to be useful.
Let us know if you have any questions or comments—I'm happy to address them!

MONSTER ENERGY OUTBREAK TOUR ANNOUNCES “AMPLIFIED” EVENT SERIES
Tumblr media
2023 has been a year of firsts for the Monster Energy Outbreak Tour, fresh off their first nationwide Latin tour with rising Trap/Reggaeton sensation, Mariah Angeliq, today they've announced the debut of Monster Energy Outbreak Tour: AMPLIFIED, an extension of the popular, genre-spanning tour series.
OUTBREAK TOUR: AMPLIFIED TICKETS HERE
youtube
'AMPLIFIED' will feature 3 bonafide dance music headliners - GHASTLY , BOOGIE T.RIO x Manic Focus, and LEVEL UP as part of a singular EDM tour series which will give fans the opportunity to win amplified experiences - ranging from VIP passes to watch the show side stage and pre-show soundcheck access to personal meet & greets with individual photos, autographed merchandise, and more!
Multi-genre artist GHASTLY, known for his continuously evolving musical sound and discography that spans a multitude of genres, burst onto the scene as a solo artist when he produced the high-profile collaboration with Lil Jon and Mija, titled “Crank It,” on Skrillex’s world-famous record label, OWSLA. In 2022, GHASTLY released his highly anticipated sophomore album, Haunted Haus which showcased his unique take on both house and future bass, and debuted in the top 5 Dance albums on Apple Music with smash hits including “Smoke” with Madalen Duke and “Fangs” with ALRT.
BOOGIE T. RIO is a live band that consists of Boogie T on vocals/guitar/keys, longtime musical cohort Frank 'Animal Drums' Castro on drums, and New Orleans-based musical virtuoso Andriu ‘Yano’ Yanovski on keyboard and synth bass. Lead vocalist Boogie T has risen to prominence in the bass music world as a producer and a touring DJ, performing at festivals such as Lollapalooza, Bonnaroo, Coachella, EDC, Electric Forest, and many more. BOOGIE T. RIO is the next phase in the evolution of Boogie T, with the band dropping their debut “Three For All” EP and first live performances in Summer 2018, followed by their “3Fall EP”, and subsequent 50+ city national tour in 2020.
The Manic Focus Band is the live instrument fusion of electronic music by John “JmaC” McCarten, a Denver-based producer with roots in funk, soul, and new-era hip-hop. A multi-dimensional artist with a rowdy style that spans across genres, Manic Focus fuses soulful blues with heavy-hitting bass, creating a tone that’s entirely his own. The Band include instrumentalist and producer Marvel Years on guitar joined with the legendary percussion of Colby Buckler (Marvel Years Trio, Emancipator Ensemble). JmaC and Manic Focus have had some incredible live performances at some of the most popular festivals and venues including Lollapalooza, Hulaween, North Coast Music Festival, Electric Forest, Bonnaroo, The Gorge Amphitheater in WA and numerous Red Rocks performances supporting Zhu, Emancipator, Pretty Lights and more.
Originally a battle DJ studying under the legendary Rob Swift, LEVEL UP later dove into music production after discovering dubstep. Since then, she has established herself as an artist at the crossroads of blending heavy and future bass, with her own flips of classic dance and emo anthems. Her live performances have captivated audiences at legendary venues such as Red Rocks, Lollapalooza, Electric Daisy Carnival Las Vegas, Bonnaroo, Lost Lands, Beyond Wonderland, Electric Forest, and Bass Canyon, to name a few.
Since launching in 2011, Monster Energy’s Outbreak Tour has been shaping music culture and breaking new artists, while being involved with many of today’s biggest names in music across Hip Hop, EDM, Country, Rock, Latin, and Comedy.

Geography. Pick the local team. If you are in say Miami, congratulations you can be a Dolphins fan and if you want to support teams across several levels, then the Dolphins for the NFL, Miami Hurricane among the power college programs and Florida International Panthers in the so-called Group of 5. If you are in an area not near an NFL team pick a Reggie White And Jalen Hurts Philadelphia Eagles City Skyline Signatures Shirt college and could even find enjoyment following a small regional team that plays in Division II or III or Division I FCS. Aesthetic reasons. You like the dark blue and orange combo of the Denver Broncos then that can be your team (also opens up the Boise State Broncos in college football). I’m a Denver Broncos fan and Kansas City is a Reggie White And Jalen Hurts Philadelphia Eagles City Skyline Signatures Shirt rival but I have to admit I like their home uniform. Like red and black? That gives you the Atlanta Falcons in the NFL, Texas Tech and Arkansas State and Cincinnati just off the top of my head. I don’t like the University of Texas but I happen to think their road uniform is one of the best in college football.

"We're looking at this tour as a huge opportunity, because for the first time we get to do exactly what we want. We’re planning on bringing out extra musicians, we're going to have costume changes and an intermission and circus acts and things like that. We're going to have stage props. We already have a giant light-up windmill.” – Ryan to MTV

ABOUT THE SUPPORT BANDS:
The Hush Sound was a support band throughout the whole tour. They opened for Panic during the 2008 Honda Civic Tour too. Ryan had stumbled across The Hush Sound online in 2005 and passed their purevolume info along to Pete to help them get signed.
OK Go was on this tour until June 22nd and then The Dresden Dolls took over on June 27th. OK Go was double booked for their last 3 shows, so they had to leave early. They posted that the guys in P!ATD were “intensely awesome to an extraordinary degree.”
Lovedrug stepped in for the shows at Orlando, Myrtle Beach, and Norfolk since OK Go had to leave.
Billboard said “Another sign Panic! At The Disco is distancing itself from the punk-pop archetype is its choice of cabaret rock act the Dresden Dolls as the opener for its current tour.”
here’s a bit more info on what happened with The Dresden Dolls & P!ATD fans.
MISC DETAILS ABOUT THIS TOUR:
what to even call this tour: the summer 2006 tour didn’t have a fancy name since it was the band’s first national headlining tour. It was usually just some variation of “the tour,” “the Panic tour,” etc. When the band was overseas later they sometimes referred to it as “the American tour,” and then “the summer tour” became more frequent by November once the Nothing Rhymes With Circus tour existed. I’ve seen a few people call this the Lucent Dossier tour, but I think that could get confusing since those performers were also with P!ATD in the UK in late October.
Jon was 20, Ryan & Brendon were 19, and Spencer was 18.
most shows for this tour sold out quickly and were moved to larger & larger venues that were still being upgraded as the tour progressed.
fans usually said the whole show was close to 4 hours long (including the opening bands).
a quote I liked about the summer tour + another one.
the reviews, fan feedback, & overall response to the summer tour.
shirts at the merch table were $20-$25 and hoodies were $40-$45 depending on what city you were in.
Robb Jibson did a lot of the band’s lighting during the pre-split years, so I just felt the need to point out his presence on this tour.
A few fans started freaking out that they spotted Matt Cortez (Frank Iero’s guitar tech) at shows. Apparently MCR was taking a short break and P!ATD used some of their crew for this tour. I’m pretty sure that’s how they got Robb Jibson too.
the guys did a little chant/cheer thing as a group before shows this season.
there were stories of bras being thrown onstage and girls & women flashing the band.
here’s more about what the crowds were like this summer.
The band started doing meet & greets before shows during this tour (they’d done some signings before but those were more like promos for stores).
The band had hired personal security now – this was their first tour with Zack.
Here’s more about what fans were like this summer.
Eric, Bartram, the Lucent Dossier performers, and people from the opening bands were able to chat with fans after many shows.
Spencer was so incredibly responsible and had managed the business aspect of the early band. Obviously other people handled a lot of the band’s business now, but it sounded like Spencer was still the one the crew would go to with questions. He was also the one who had to write the band’s public statements for news outlets.
Dream Rockwell and Amanda Palmer both mentioned that the band’s macho crew members were difficult to deal with. A couple fans also mentioned negative comments that the crew made to them about the band’s music. It sounded like some of the crew might have treated Brendon like an annoying kid as well.
there was no encore, which disappointed some fans… but the band did take a bow (here are more of the pictures).
here’s a magazine ad.
Brendon was still stage diving during Build God at some shows... here’s another decent video.
The band put their own money into this tour, so it sounded like most of the income they actually kept came from the merch sales. That was part of the Brent drama... the guys kept emphasizing that if he’d paid any attention to the business side of the band, he’d know that this summer was not going to be a big financial success for them. Spencer said “We are basically breaking even with the amount of money it is costing us to put on the show that we want to put on.”
ABOUT THE MUSIC ASPECT OF THE SHOWS:
Brendon was still playing the piano for some songs, but the band had also hired Eric Ronick as a touring keyboard player (and misc percussion & whatever was needed). That meant that Ryan didn’t need to play the piano part in There’s A Good Reason anymore.
Bartram Nason was the new touring cellist who played his own personal cello with an electric pickup during this tour. He also helped with misc percussion.
Jon helped out with the keyboard too.
here’s a video of a soundcheck.
the band often did a variation on the whole “I don’t smoke!” thing during There’s A Good Reason. They usually did the “S.E.X.” part for that same song too.
Forgive Durden had a banjo when they opened for P!ATD that spring... and now Ryan had a banjo.
Ryan had been taking voice lessons for a while now and said Brendon had been helping him too. All of his hard work was evident as his singing had been getting steadily better through each tour so far. By this summer he was SO good! The huge improvement in less than a year was so incredibly inspiring. Ryan really threw himself into whatever he did. Spencer & Brendon had improved tremendously as well. The summer tour was 10 months after the band played their first show, so they could’ve easily been unprepared to pull off a national tour... but Spencer, Ryan, and Brendon had basically hit the ground running and worked so hard to perform at the level their band had jumped to. And thankfully Jon was a really good musician and already had experience touring.
the band was trying to create a full show using songs from only one album, so they added two covers: Karma Police by Radiohead and Tonight, Tonight by the Smashing Pumpkins.
in a 2007 interview Ryan talked about the Tonight Tonight cover and said “we hear Billy Corgan is a fan, which is quite a thrill.”
the songs were mostly played in a similar order to how they appeared on the CD. Ryan told OnMilwaukee:
“We kind of looked at this record as two halves. The first half was much more rock derivative and electronic music, and the second half we started exploring more organic instruments and more organic kinds of songwriting, a little bit of influences from jazz and Latin music, and just using more theatrical elements in those songs. So we would play our tours like that. We would sort of set it up as two different shows. It wasn't completely in sequence but it was always the first half and then the second half.”
ABOUT THE AESTHETIC:
here’s more about the costumes.
this is the tour that felt like Moulin Rouge! (here’s what it reminded me of). The band said they were going for an "early France, circa 1900" feeling.
old fashioned piano music played from speakers while the crew set up after The Hush Sound’s set.
the giant windmill didn’t always fit into smaller venues.
the set design was inspired a bit by the Tonight, Tonight video.
Spencer had his light up drum set!
Brendon told Dose Magazine: "We wanted our live show to be kind of like a Broadway play in a sense. You have music being played and performance going on or a little skit.”
About the makeup: Lucent Dossier wore makeup onstage and started doing the band’s makeup as well. It looked like that really helped Ryan step into a role and come out of his shell a bit during this tour. Here’s a general idea of Ryan’s makeup this summer (he started doing his own makeup eventually during this tour). Brendon seemed to like wearing makeup, but didn’t do anything too elaborate on his own until later that year. The same thing applies to Spencer... he seemed to like the makeup that Dream Rockwell did, but he didn’t get really into doing his own makeup each night until October 2006 (the 6 red looks in this post are from late 2006, but the others are from the summer tour). Jon did not seem to appreciate the makeup lol. He wore it on the first night of the tour, but in general he usually decided to go without it.
eventually the band’s website said “tour makeup provided by our friends over at M.A.C. cosmetics” this summer and the guys tried to mention that in interviews sometimes too.
fans said there was a smoke machine at some shows.
an Absolutepunk update about the summer tour said “Panic! at the Disco's live set now includes blinking lights, dancers, a costume change, and a skinnier drummer.”
Ryan later told a German magazine that people who grow up in Vegas feel committed to do something bigger because everything there is brighter, louder, more colorful etc. One review of the summer 2006 tour said that P!ATD pulled off everything Motley Crue had been trying to do with their circus tour (and that review came before the Nothing Rhymes With Circus Tour even existed ha).
LUCENT DOSSIER
Dream Rockwell from Lucent Dossier did their costumes and directed the performance aspect of the tour. She and Dusty also did the lapdance, which Dream said was supposed to be “kind of showing the unglamorous side of it.”
There were 3 performers at a time, but 5 overall – here’s more info about who’s who.
The Lucent Dossier performers shared a bus with some of the crew.
here’s a great video from Roger with highlights from the summer tour, Lollapalooza, and a short bit from October.
Dream posted on the Lucent Dossier tribe:
“As for the boys in the band, they’re great…. for 18 year olds they are incredibly wise, attentive and business minded. They don’t drink or smoke, they don’t party or bring groupies back stage (at least not so far!). It’s as easy to have deep conversations with them as it is to laugh and joke around. I’m very impressed with how they conduct themselves and we’ve had lots of fun hanging with them.”
Roger Fojas from Lucent Dossier was so much fun because he got really involved with the band & fans and seemed to care a ton about what he was working on. Here’s part of his myspace post from late July (I added links that seemed relevant):
it's funny because I'm usually a photographer when I'm not a circus ringleader clown dancer extraordinaire (and yes... of course I've been shooting crazy mad photographs of my experiences with Panic!)... but on tour I'm on the other end of the camera ... on stage everyone is being photographed from the audience point of view (and then posted on blogs and video streams across the internet)... then here in Denver there are all these proffessional video cameras being set up everywhere (seems like there'll be an upcoming concert video of our show in the works)... and even Amanda and Bryan of the Dresden Dolls are making this faux music video they are at war with the Panic! boys..... it's a funny concept where both bands are scheming at destroying the members of the other band.
THE LIVE IN DENVER DVD:
Yes, the dvd is fun. But there were almost 50 shows scheduled for the summer tour. Denver was only one single night… every picture from the summer tour is not “Live in Denver.”
The dvd came out over 3 months after this tour ended (here’s the box set). It was nice to have such clear footage, but at the same time the band didn’t really feel like themselves. The audio on the dvd was mixed by the producer of AFYCSO, so it also doesn’t sound totally like the normal band that summer either (this is just my own opinion). Also, Brendon wasn’t as relaxed as he normally was. We’d heard that he was also nervous for the Astoria show that was filmed during P!ATD’s first headlining tour and that the band had been much more fun to watch on other nights in that tour when he could loosen up more. So I don’t watch the dvd nearly as much as regular youtube videos because those feel more like the season I remember. I do love how Ryan was extra animated at the Denver show, though!! (He ended up winning MVP that night).
OTHER STUFF:
An excerpt from a summer 2006 Pollstar interview with Scott Nagelberg (the band’s manager) and Andrew Simon (their booking agent):
"If they had gone out with just the four of them, just playing in front of a banner behind them or nothing, they wouldn't be as happy," Nagelberg said. "Even though the tour would be successful and they would potentially come home with more money, it's about feeling comfortable and bringing the show they want to bring, and that's worth more to them than anything else.” Both Simon and Nagelberg used the phrase "raising the bar" in describing the band's live set, and both agreed that they'd never seen a band rise through the ranks so quickly. "It's moved faster than anybody possibly could have imagined," Nagelberg said. "It's lightning in a bottle, and it's a matter of working and taking their great ideas and turning them into something feasible and into a reality.” "There's not a lot of bands out there right now at their level who are putting so much back into their show," Simon said. "It's a real spectacle.”
Spencer had been talking about his increasingly ambitious plans for a potential national headlining tour for a while. He really had an incredible imagination for the visually creative side of the band (but he also had the ambition & work ethic to pull it off). Spencer told Big Cheese Magazine in January 2006:
“We had ideas to do a headlining tour, to make it a little more theatrical, a little more than a lot of bands do. We’re going to try and step it up... I don’t know if we’d ever get to the point of performing in arenas and stadiums, but I remember reading that one Pink Floyd stage set was so in-depth they had to have two made and would hopscotch them between venues. It’s like, who would even think to do anything like that anymore? Bands have got too lazy. They’ll go onstage and maybe shoot some confetti into the crowd. That’s about it.”
It sounded like that Pink Floyd inspiration was considered for the summer tour. Spencer told Kerrang that they “were thinking about having two different stage sets at one time but it was just impossible.” Ryan added: “we had to scale back our ambitions a little because we weren’t thinking about what was physically possible. It was all getting quite extravagant.”
AbsolutePunk asked Spencer “Do you ever think that the theatrics you put into your live show all of the sudden will make people think you’re a ‘gimmick’ band?” Spencer replied:
“We switched when we were able to headline and control the overall show. When you’re an opening band, you have a very short set and limited stage room. You’re also not making enough to pay for gas, let alone a crew and production so it doesn’t make sense to half ass it. I don’t get why trying to do the best show possible for our fans makes us a ‘gimmick’ band. The album doesn’t wear makeup. If you liked the album before we had make up on. Why would it suddenly become worse?”
Kerrang gave the Toronto show the highest possible rating and said in their review:
“The band, meanwhile, are on fire. While frontman Brendon Urie struts and flirts in the middle of it all, Panic! are so slick it seems impossible that they’ve been playing live for less than a year… the fact that they’ve just one album to draw on doesn’t matter in the slightest, such is the audio visual overload onstage. And having the balls to pad the set with covers of Radiohead’s ‘Karma Police,’ and Smashing Pumpkin’s ‘Tonight Tonight’ and pulling it off has to be applauded. … It’s the antithesis of punk rock – glossy, glitzy, pure showbiz, impossible to recreate in your garage. Panic! are throwing down the gauntlet, challenging others to bring showmanship back to live music, but for now they’re alone in their ambitions. One of their songs insists that lying is the most fun a girl can have without taking her clothes off. They’re wrong – this spectacle just might hold that title.”

Pandemic pushes expansion of ‘hospital-at-home' treatment
(AP) - As hospitals care for people with COVID-19 and try to keep others from catching the virus, more patients are opting to be treated where they feel safest: at home.
Across the U.S., “hospital at home” programs are taking off amid the pandemic, thanks to communications technology, portable medical equipment and teams of doctors, nurses, X-ray techs and paramedics. That’s reducing strains on medical centers and easing patients’ fears.
The programs represent a small slice of the roughly 35 million U.S. hospitalizations each year, but they are growing fast with boosts from Medicare and private health insurers. Like telemedicine, the concept stands to become more popular with consumers hooked on home delivery and other Internet-connected conveniences.
Eligible patients typically are acutely ill with — but don’t need round-the-clock intensive care for — common conditions including chronic heart failure, respiratory ailments, diabetes complications, infections and even COVID-19.
They are linked to 24/7 command centers via video and monitoring devices that send their vital signs. They get several daily home visits from a dedicated medical team. Just like in a hospital, they can press an emergency button any time for instant help.
Research on such programs around the world over the past 25 years shows patients recover faster, have fewer complications and are more satisfied, while costs can be a third lower.
Doctors, hospital officials and patients tout other advantages: People get more rest sleeping in their own bed. They can eat what they want, start moving around quicker and go outside for fresh air. They’re less likely to fall in their familiar surroundings, where they have support from family and even pets.
“I would recommend it in a heartbeat for anybody to be able to stay at home,” said William Merry, who received care for pneumonia in July at his Ipswich, Massachusetts, home. “There was never any problem. Never.”
Merry, who had endured an uncomfortable hospital stay six years ago, refused another one when antibiotic pills didn’t help and his temperature hit 103. So his doctor arranged care through Boston-based Medically Home.
Merry and wife Linda, a retired nurse, said they were amazed at how quickly the service transformed their dining room into a mini-hospital room. Technicians set up medical equipment, gave them supplies and oxygen tanks, then explained how everything worked.
That eased their stress, as did regular video calls with a doctor. They got daily schedules listing planned medical staff visits, blood draws, tests, IV medicine administration and other care, she said.
“I think it’s really important,” she cautioned, “that the person has somebody that’s able to be at home.”
Dr. Bruce Leff, a geriatrics professor at Johns Hopkins School of Medicine and a home hospital pioneer, did pilot studies years ago. He found benefits for elderly patients who, as he said, were otherwise “basically going to get crushed by the hospital” due to risks of developing blood clots and infections, losing mobility and developing delirium.
Even before the coronavirus pandemic emerged earlier this year, some hospitals were considering at-home-care programs to absorb temporary patient spikes — and avoid the high cost of new buildings.
It’s unknown exactly how many U.S. programs exist, but when COVID-19 struck, some institutions rushed to sign up with Medically Home and similar services.
Nashville, Tennessee-based Contessa Health, which serves 14 hospitals in six states, says it’s adding two more hospitals shortly and is negotiating potential contracts for about 20 more. Patient volume has jumped 140% since last year and it’s added care for patients “admitted” from urgent care and cancer clinics.
Another company, DispatchHealth, previously focused mainly on preventing ER visits by rushing paramedics to provide diagnostic testing, medication and other care at patients’ homes or elsewhere. The Denver-based company says it has 200-plus contracts with insurers in 19 U.S. markets to treat seriously ill and injured people at home. It piloted a hospital-at-home program in November, already has programs running in three cities and plans rapid expansion.
Some hospitals have mounted their own at-home programs. In late March, eight of the Atrium Health system’s 36 hospitals in the Carolinas and Georgia began one for COVID-19 patients who don’t need intensive care. It’s already treated about 11,000 people.
Meanwhile, hospitals with existing programs are seeing far more patients choose at-home care.
In New York, the Mount Sinai at Home program went from handling 10 patients a month to up to 30, said its director, Dr. Linda DeCherrie. The program has since added a twist in which patients start care inside the hospital, then finish at home. “Everybody we offered it to said yes,” said DeCherrie.
DeCherrie said the hospital-at-home model has been used on a small scale in the U.S. since the mid-1990s, but it was held back because traditional Medicare and some insurance plans either didn’t cover such treatment at all or didn’t reimburse for the full cost of care.
But when the pandemic struck, the Centers for Medicare and Medicaid Services temporarily let hospitals bill for care outside their walls, including in patients’ homes. Many private insurers also are covering in-home hospital care during the pandemic. Hospital groups and others want Congress to make those changes permanent, at the same rates as in-hospital care.
Raphael Rakowski, co-founder of 4-year-old Medically Home, said the number of patients treated this July is up tenfold from July 2019.
“Our business is exploding because of COVID,” he said.
It now treats patients for 10 hospitals and one physicians’ group in five states, including two that were set up soon after the pandemic hit: Boston’s Tufts Medical Center and Adventist Health’s West Coast hospitals. Two Mayo Clinic hospitals joined this summer. Medically Home should be operating in 12 states by early 2021, Rakowski predicts.
He says some patients are offered at-home care after being examined in an emergency room. In other cases, doctors arrange the care for patients getting cancer treatment, those with a sudden illness, some about to get surgery, or homebound patients with dangerous complications.
The Veterans Health Administration operates 12 hospital-at-home programs. Last year, they served 1,120 veterans. More vets are using the program during the pandemic, said Dayna Cooper, head of agency’s home-based programs. One of the busiest, in San Antonio, saw a 90% jump in veterans treated this March through June versus last year.
Another four of the agency’s 170 hospitals are working to start programs. Cooper said studies of the programs in Cincinnati and Honolulu found they cut costs by 29% to 38%, without differences in survival or hospital readmissions.
While interest in the programs has skyrocketed, whether in-home hospital care blossoms after the pandemic largely depends on whether government and private insurers continue to cover it at profitable prices.
If they don’t, Johns Hopkins’ Leff said: “I think most hospitals will go back to normal.”

During the Week
After the game Sutton praised Lock for changing a play call from the sidelines that led to a Bronco touchdown. This is the kind of story I want to hear about the new quarterback. Joe Flacco was the first quarterback of the season and he just took his marching orders without question, until he was relieved of his duties. Perhaps Lock heard Flacco’s message about bad play calling costing the team victories and took a mental note. Brandon Allen was the next quarterback to play for the team and he was able to scramble. Hopefully Lock noticed Allen took fewer hits and opened up the offense by moving outside the pocket.
The problem though and this has happened in almost every Bronco game this season is the hard headed, defensive, controlling, conservative play calling coming from the sidelines. In the first quarter, Lock was making plays and the team looked great. In the second quarter, the staff started to exercise control and the play calling became extremely conservative and the Broncos play went downhill. This lasted till the end of the game.
Members of the media jumped all over the attendance numbers. There were 19,000 no shows. These are people that paid for their tickets but didn’t go to the game. It was noticeable on the TV. Personally, I don’t really enjoy going to the stadium. Everything you buy is a complete rip off. Security is annoying. The Broncos prefer you use e-tickets which means bringing a ‘smart’ phone to the game. I didn’t even bring up the fact that concession stands in the Denver area have terrible rankings, which I pointed out in one of my posts last season maybe. Basically health inspectors found rats in food storage areas. For those reasons and many more, football is best enjoyed at home on your couch.
Von Miller was back in the news. He didn’t play on Sunday and sports talk speculated on his value to the team. The point being made was that you can have a losing season with or without him. People were questioning the value of spending a lot of money on a pass rush when you haven’t even found your franchise quarterback. Von Miller’s mom even gave Elway flack about that during the Vance Joseph era. Von’s Halloween party of 2018 reentered the news because Von’s personal photographer has decided to sue x-Bronco quarterback Chad Kelly over injuries suffered during a fight at the party.
During the game, there was an ad by the Broncos showcasing the Broncos offensive line serving food to the homeless. A pretty standard pr move by the NFL during the holidays. I’m mentioning it because it was of the Broncos offensive line. Garret Bolles was not in the ad.
Last week I forgot to mention that the next generation of the Bowlen family, Britney, was given a new title/position with the Broncos. I didn’t mention it because the title sounded like corporate gamesmanship to me. The title was fancy, but to me, it sounded like she would be in charge of parking outside the stadium. I’m bringing this up because there was some talk about the contracts of Elway and Ellis coming up in two years and some think Brittany was put on the “fast track” to take over the team. The big assumption is that the contracts of Elway and Ellis will not be renewed. Who will replace them? Nobody knows.
