Ed/Izzy/Stede — Ed knocked up Izzy while Stede was gone
Stede returns to the Revenge to find Izzy is pregnant with Ed's baby. The two are breathtakingly unconcerned about the situation, Ed because "Izzy's really responsible, he'll see that everything turns out okay" and Izzy because "babies happen all the time, no sense worrying about a child that's not even here when everyone should be worrying about the work they need to be doing now." Denial? Stupidity? Who knows!
Except it quickly becomes clear that everything is not fine, or even going well. Ed and Izzy avoid each other aside from Ed giving orders and Izzy giving status updates. Izzy is in his fifties, hasn't had time to adjust or/and fully recover from the toe incident, and now is being hindered by pregnancy side effects. The whole crew shares mutual anxiety watching Izzy endanger himself on a daily basis. Everyone is afraid of doing or saying the wrong thing in Blackbeard's presence and invoking his wrath.
Stede has an "Oh my God"/facepalm reaction and makes it his goal to fix everything. Somewhere along the way of trying to prevent Izzy from dying of stupid reasons while pregnant, Stede finds himself with a second boyfriend. (Up to filler if Stede brings along his crew, Lucius is there, Ed and Stede reconcile immediately or it happens slowly, Izzy is trans or a/b/o/ or under a curse.)

Previous Ed/Izzy, Lucius/Pete/Izzy — Trans!Mpreg
Ed got Izzy pregnant (dub con max, not non con), but doesn't really have an attachment to Izzy or the child.
For whatever reason, despite knowing Ed has no interest in being a father, Izzy decides to keep the baby. He doesn't know why, he doesn't know how he's going to accomplish it, but Izzy knows one thing - he wants to be a good parent to his child.
Lucius and Black Pete see a new side to Izzy, as he struggles to work through his emotions, figuring he should at least practice being nice to people. They take him under their wing as the stress of pregnancy takes its toll, and the camaraderie slowly turns into romance.
Lucius and Pete really admire Izzy for all his ability to change. Izzy is obviously grateful to them for their help, but even more than that - he finds he can relax and laugh with Lucius and Pete, that romance doesn't need to feel like life or death. He starts to remember his love of actually sailing, when he listens to how badly Pete longs to be a pirate. He appreciates Lucius's raw honesty and boldness. When the baby comes, Izzy has two men more than happy to stay by his side. Well... Pete may actually be passed out on the floor, but the sentiment remains the same.
Bonuses:
+ Pete is also trans, so he bonds with Izzy over that, and can really empathize with how hard it must be for Izzy to have a baby.
+ Lucius is still poly, maybe has a bit of a side thing with Fang, but very much loves Izzy and Pete. Pete may have done it before, but he's feeling pretty emotionally and romantically fulfilled with Izzy and Lucius.
+ Pete loses his idol worship for Ed when he sees how he treats what any other pirate would die for- a first mate like Izzy Hands.
+ Izzy in labor, waffling between wanting to keep his labor a complete secret from Ed, and also wanting someone to drag Ed in so that Izzy can throw shit at him for making him give birth.

We've been having fun transing the Izzy, but WHAT IF we turned this ship around? Izzy's a woman, actually. Does she know this? How does she figure it out?
Maybe he's angry and repressed, hates that Jim gets to be themselves and no one bats an eyelash. Maybe Ed is also transfem and it drives Izzy mad that his captain is being corrupted like that when they both know that it's wrong and bad (it isn't. Ed's so happy like this, Iz, why can't you understand? Just be happy for me, mate, I know you'll never know what it feels like to finally be the person you always felt you should be but you don't have to be such a dick about it.)
Maybe she actually chills the fuck out a bit when she stops having to be in boymode in front of the crew. She puts on the man when she has to - it's a fuckery, but somehow this one got extended and extended and now she's losing her mind wearing this ill-fitting costume for so long. What makes her break? She accepts that this is her crew now?
I'd love Steddyhands (or Ed/Izzy, especially if you wanna go pre-canon), but gen is also great! Roach/Izzy or Frenchie/Izzy are also amazing options. I love Jim so if you wanna throw them in the mix feel free!

Izzy/Any, Ed/Stede — Ed thinking about having kids, Izzy turns out to be pregnant
Set some time when everyone is reunited on the Revenge...
Ed and Stede are talking in private, Ed talking about Stede's kids. He's met them a few times and they all seem to get along. Wonders aloud if potentially they could adopt them. Stede counters that Mary might have something to say about that. But the conversation goes on with Ed reminiscing. He has considered having kids, and did give it a try multiple times over the years. But Izzy just never took to it.
The conversation peters out but it's not entirely forgotten in the days and weeks later. Stede thinking about his kids but also what is now very much implied about Ed's past with Izzy, and Izzy himself...
Yet, at the same time Stede thinks more on the subject, Izzy is looking horrible. Starts out with frequent bouts of sickness. Then tiredness he can't shake. More frequent moodiness.
It's Stede who clues in first, having seen Mary through both her pregnancies... apparently it wasn't Izzy who was experiencing conceiving issues. Soon it's quite clear that Izzy is pregnant.
Maybe Izzy is aged down? Or Izzy and Ed. And whether Izzy is Trans - or if this a/b/o - is author's choice.
+ Ed takes this as a Sign, even though it's been at least three years since he's slept with Izzy.
+ Ed has commented that Izzy must've had fun the last time they made port, and Izzy lets him believe that. But maybe... this was something that happened on ship. More often than not.
+ Izzy had no reason to think that he could conceive because of all the times he couldn't with Ed. So this comes as a shock to Izzy as much as everyone else.
