Dagmar Keller / Martin Wittwer
Whilst looking for inspiration for the type of images I wanted to take in everyday life, I came across a series of photographs Passengers by Dagmar Keller and Martin Wittwer who were drawn to passengers on an old bus at a bus station as they passed through Poland. 
The passengers were behind stained windows that were covered in ice. It gave the appearance of a painting. What I like about these images was that they were unplanned and not staged. What I also liked was how they explored not just the people on the bus, but the surroundings as well. This was the quality in photography that I was looking for. The playfulness and exploration.

Passengers, #21 – This image to me makes me wonder what he is thinking, what emotions he is feeling and where he is going on this cold night. The black of the night makes the viewer focus on the subject directly and because there is ice on the window and the subject cannot be seen clearly, it makes the viewer use their imagination.

Passengers, # 94 – The photographers explored the surroundings of the bus stop. It appears to be a curtain but who knows where it is. I like the way the light hits the curtain and it may seem random in the context of all the other images, but it shows me the playful quality I am after as an influence to my own work.

Rinko Kawauchi
Rinko Kawauchi was born in the early 1970’s in Japan and became interested in photography while studying graphic design. She has a very soft style of photography which she emphasises with soft colours. Her work and aesthetic is to have an almost dream like quality and will mostly shoot ordinary things.
Why her work is of interest to me is that she does not second guess herself and runs by instinct. If she feels compelled to photograph something, she will and not ask herself why. She rarely will include people in her images and is interested in images that move her rather than just look good.
She puts a lot of thought into her compositions and one of her main conventions is juxtaposition. She wants the viewer to imagine what is going on. She also commonly makes use of cropping and prefers to use natural light.
What I found interesting about Rinko is that she does not like to take multiple images and then select the best one later. She will purposely take her time and get the shot she had in mind.

This image follows Rinko Kawauchi’s conventions of cropping and natural light. What I like about it is that it is off centre and the lighting gives it a dream like quality. I like the rule of thirds quality and the juxtaposition quality of the light shining from below where you would think it would come from the moon itself.

I like this image as you would expect there ordinarily to have some other feature to concentrate on but in this case it is just the birds. It feels very serene and makes me imagine what it must be like to be one of those birds. Me imagining this just highlights one of her aims which is to make people use their imaginations.

Lieko Shiga
Born in Japan in 1980, Lieko Shiga has become a rising star in Japanese photography mainly due to her going against the grain of what other Japanese photographers do and  being so expressive. What I find fascinating about her work is that she has no one particular style. She will integrate her own personal experiences and encounters into something surreal and she will do this by any conventions she can find including lighting tricks or double exposure of her images.
She is best known for her Spiral Coast project which was inspired when she was invited to become the towns photographer when she moved to a small community (Kitakama) in Japan. Her community was hit by a tsunami in 2011 and the town was flattened and over half the residents died.
Through her imagery, Shiga managed to capture not only the history of the village from before the tsunami, but also the spirit of the village. She cleverly did this by mainly shooting at night which gave the desired uncertainness. The images had a certain vagueness like it is a memory and she managed this by using a flash that does not reach the whole frame.
The deepest part of her work I find is her concept of photography itself. Shiga feels that photography is more than capturing a moment in time, Photography to her is not the past, present or future, but instead there is just space. There is no time. Shiga views herself as the camera as she bases most of her work on personal experiences. Photography therefore takes on a physical form. This is the sort of aesthetic I want to incorporate into my work. I want my camera to capture the qualities of what I see.

I love the texture of this image. You can almost image cars running up and down this beach and doing wheelies. What fun they must have had. After all, the beach is about fun. This is capture here. This would be my interpretation of this image but when you see the title Blind Folded Pilot it takes on another meaning. Now you can see someone lost and trying to find their way. Ironically this setting was staged by the photographer and she used a stick to make her patterns and lines.

One of Lieko Shiga’s conventions is to place random items next to people she is photographing and this is an example of this. This image is of the chief to her village. Behind the subjects is a massive tree root and she Lieko would never admit how she did it but it appears as though the tree root passes through the body of the chief. the red colouring of the root and over the chief is of significance as well as it would symbolise the blood.

Irina Rozovsky
Irina was born in Russia in 1981 has exhibited work in galleries worldwide and now resides in Georgia, US. Her belief is that the camera is a third eye and believes a photograph can be taken anywhere and anytime. A lot of her work has been based around rootlessness, migration, diaspora and personal versus political freedom this was inspired mainly though her migrating from Russia to America when se was seven years old.
Ten years ago Irina was invited to a friend’s birthday party on the lake in Prospect park in Brooklyn. Irina had been to this park many times, but it was the first time on the lake. Looking back at the park from the boat she was on changed her perspective of the park and the people that visited it. It was as if the world stood still for her and she was compelled to begin a ten-year journey of photographing the people that frequented the park as the park covered many different areas of Brooklyn such as the wealthier side to the poor areas. There were people from all different cultures, religions, and ethnicities in different areas of the park and Irina spent the next 10 years visiting the park and taking photographs and she felt like she got something new and exciting every time she returned. 
She spent her time discussing what she was doing with the people she was photographing, and subjects seemed to fully embrace her vision. She named her project In Plain Air and this has been an inspiration to my latest work. I have always tended to stage my photographs but what I want to do during this assessment is shoot my images in their natural form. The subjects do not need to be glamorous or use props that are in theme. I will find objects that are of interest to me and look for interesting ways to photograph them and in such a way that an otherwise dull and everyday object, can become interesting and a become a piece of art.
Irina Rozovsky I have noticed shoots from different angles which can change people’s perspective on issues. Her projects can run for varying times from a few weeks to several years and she will not move on until she feels she has captured the true essence of what she is there for. This to me shows the connection she has with the project and how she can run on instinct.

There are so many things I like about this image. I like the angle she has captured this from. I like how the reflection on the sand looks like glass. I like the irony of the flowers (as a living thing) are in plastic bottles which kill so many creatures in the sea. This is a statement that I’m sure she wants to get across.

1986 - the first TRUST (The Punk Magazine In W Germany) appears. The German hardcore scene is still in its infancy. Some of you will remember vividly and not be able to suppress a few sentimental tears, for others the seriousness of life had not even begun.
One of the better known bands back then was TU DO HOSPITAL. In the first TRUST there was some kind of report about that. 15 years later I ask Winni, who was then singing at TU DO HOSPITAL, about his memories of then and what his life looks like now….

Hi, first introduce yourself, what's your name? how old are you? What are you doing?
Winni: Hi - I'm Winni, on this planet for 34 years, currently in San Francisco, California, where I work as a graphic designer.
Winni, you used to sing at TU DO HOSPITAL. in the first TRUST there was a report about you and 2 concerts that you played in leutkirch and linz. that was 15 years ago. what memories do you still associate with the band and the “scene” in germany back then?
Winni: I found the scene very exciting at the time. The whole punk rock history seemed to be changing and a lot of new people came into the scene who were very active and redefined punk rock.
I think a lot of kids came along who were very influenced by the US hardcore scene and that brought new momentum to the movement. Suddenly you saw a lot of new people at the concerts who didn't care how “punk” they looked or whether they were laughed at when they went around with their skateboards.
This may all be a very personal experience, but around the mid-80s there was suddenly a lot going on. Many new bands & zines started, more concerts were organized and all of a sudden there was a network across Germany and other countries that made it possible for bands like ours to play everywhere.
At concerts you often met the same people who were very active, whether it was a cow village in Holland or Kreuzberg - at that time you traveled huge distances to see bands and a lot of people in bands were also involved in organizing concerts and wrote for zines. I think to myself that that has not necessarily changed today, only the “scene” is probably much bigger and more professional today compared to back then.
To stay a bit in the past: TU DO HOSPITAL was a band with very personal lyrics that were probably written by you. “You think you know me, but that's not true. you see my clothes and hear what i say, but what i feel is hidden inside me from you. my thoughts change very often, because i'm learning new things every day, but you have this picture of me and you think i'm still like that ”(from“ wrong image ”). what do you think of your old texts today? and to stick with the wrong image, what kind of person is winni wintermeier today?
Winni: I haven't looked back at the old lyrics for a long time and can no longer find the old record to read it all over again. I can still remember a few fragments and I think that many of the things I wrote back then seem a little naive today. On the other hand, much of it is still valid today and some thoughts have not necessarily changed. I can't even describe in a few words what kind of person I am today.
I could try to describe my personality, but it will probably sound like some sappy personal ad. Who I am and what I do in this world is a question that I continue to research with great interest and which may never be able to answer. This is partly reflected in the old texts.
In retrospect, what significance did the band have in your life? why and how did you break up?
Winni: At that time the band was already an important part of my life. As a result, I have had relevant experiences and met many people who have also shaped my life. We broke up because of musical and personal differences without much drama. We just got to a point where it wouldn't have been easy to continue as a band.
And does it still matter today? are you still in contact with your bandmates? do you even have contact with anyone “from the past”?
Winni: It hasn't lost its meaning, but looking back it was just one of the many important experiences I've had in my life. I lost contact with my old colleagues, but recently a friend sent me a CD from the Carnival of Souls, the band from Achim, the old guitarist.
You now live in san francisco and run the one-man company 3AM (www.3am.net). how did your move come about?
Winni: Boredom drove me to San Francisco in 1992. I had studied photography in Germany for two years and that didn't really help me. I just couldn't imagine going through this for 2-3 years. I had visited San Francisco several times and had become a kind of second home.
I knew a lot of people who do interesting things and there was a lot going on in terms of culture. For me it was very inspiring to take the big leap to concentrate on the things that interest me (music, photography, design, art, etc ...)
Do you see a connection between the punk / hardcore ideals that you also promoted in your texts for TU DO HOSPITAL and what you are doing now? “It's ok if you don't like our music because we don't play what you want to hear. if you don't like the things we sing about, listen and try to understand us. I don't mind if you don't like our music. we put our energy into it and like to play it ”(from“ our music ”)“ i feel young and strong, full of energy. I know how to use my strength, I won't hide it. i live to be creative and not to waste my time. I know there is an end, but I'm not afraid of dying. "(from" 1-2-tu-do ")
Winni: Partly yes, I'm a little “punk rock damaged”. I still have many ideals from earlier times that are reflected in my work. With “3 am” I work on a lot of record covers for various small indie / punk rock labels or just for friends for little money.
These are often projects that are very non-commercial and where no one earns anything materially. On the other hand, I also do a lot of things for bands that call themselves punk, but are basically nothing more than any other rock band that want to be popular and successful.
I often have to make compromises so that I can pay my rent. To get to the quotes - I sometimes wish I had the same energy again that I had at the young age of 18 - my body can no longer keep up with what I plan to do in my head, ha, Ha.
With 3AM you do some graphics / layout things for various punk / hardcore things. how did this collaboration come about?
Winni: A friend of mine was working at a small punk label that had just released two records and needed a graphic designer for their new projects. Back then I had designed a couple of CD covers in Germany that I showed them and all of a sudden I got a lot of work.
The label (Fat Wreck Chords) then grew quite quickly and other labels (Hopeless, Side 1, etc ...) saw my stuff and suddenly it became my full-time job. It was quite hectic because I had to teach myself everything and I had hardly any idea how to use a computer.
Can you live on 3AM?
Winni: So far I've been able to make ends meet, but right now I have to look around a little for new work. The budget for a lot of record covers is quite small and for the last two years I have often helped out with a hi-tech magazine that paid me quite well, but they are just going down the drain.
San Francisco has become very expensive and the new internet economy has almost completely destroyed the city, which has made it very difficult for many people to survive here. At the moment I would like to concentrate more on my photography, because I don't feel like sitting in front of the computer all the time.
Do you actually see your work as political? do you try to make certain statements in what you do? or is that more "art for art`s sake"?
Winni: My graphic arts less, although I often work for bands that are politically oriented in their lyrics. Some of my photos are more political because I am interested in documenting the influence people have on their environment. That suggests more in the direction of photojournalism and as an artist I wouldn't necessarily describe myself because this term is not important to me.
I think that with 3am I will try to create a free space in which I can work for people who have similar points of view and who work outside the big system and that is politically motivated.
what motivates you these days?
Winni: The urge to explore life. I find inspiration for this everywhere, whether it's music, art, literature or the old man sitting around the corner in my café.
What will you do if we repeat this interview in 15 years?
Winni: I don't know - I still haven't decided on a big plan for the future. I'll be able to tell you that in 15 years.

Rockman.EXE Episode 47 Review.

Let the Wacky Races parody begin!
We start the episode with car related shots, something that looks like a photo shooting with Aki-chan, more car related close ups and a scene of Iceman being taken away from Aki by Glyde.

A race is taking place today! Shots of the drivers getting their cars ready along with Rush getting chained to one of the cars for some reason, and Rockman putting on the goggles we saw earlier along with his battle mask.

We see the different “cars” at the starting line as the race begins!

Yes, that is in fact Magicman’s arm shoveling coal into a train engine.
Its is reveal to be a Net Mobile Grand Prix hosted by Yaito since the commentator is one of her maids. Just then, a newcomer drives in at the last second.

Blues joins the brawl! I mean race.
After the title card, Yaito’s maid tells us the name of each car and their respective drivers.

Rockman is driving the only real race car, Gutsman is racing in a tank, Roll’s car looks sentient and Glide is wearing gloves over his gloves.

Mania-Obsessive, the Ex-WWW Navis are driving a train, Net agents are driving a manly submarine and Yaito’s maid just straight up insults Blues. Talk about an objective commentator.

This is basically Mario Karts, so we all know they’re gonna attack eachother, be realistic woman!
The winner, aside from the title, will also receive a kiss from the CGI race queen.

I can only imagine those are the lyrics of a new cheezy song of hers.

This is something I didn’t like from the dub, we all know that Roll has a crush on Rockman, but it is not okay to make her jealous of other girls? The dub change the dialogue to Roll complaining about Rush not being in the car with her, as if she didn’t know that she has him strapped to the car’s front. It’s actually cute how Roll wants to win the race to avoid having Aki kiss him.
But Rockman couldn’t care less about the kiss because all he wants is to win no matter the battle, as he then gives us a flashback from a few days earlier at the Maha Ichiban where Yaito is telling everyone (including Higure for some reason) about the race.

I love that little gag with Netto, eating curry is always a priority for him. Also, why is one of Hello Kitty’s friends on the wall? Or... maybe its just a pig.
Yaito then shows them a new line of Navi cars with Numberman as the model.

The idea behind these cars is that Navis will be able to drive cars through the internet, which is pretty stupid considering that there are no highways in Internet city and Navis can easily travel fast plug-in style. But we all know that the real reason is to make Blues look cool in a future episode and have a Wacky Races parody.
In order to advertize the cars, Yaito wants to hold a race and asks Netto and the others to take part, they accept, but the Ex-WWW overhears them and ask to join as well for the publicity for their restaurant. And for some reason Commander Beef and the rest of the Net Agents arrive and decide to enter as well to foil WWW’s idea. And Yaito being so selfish doesn’t give a crap about the WWW’s intention like she did in the last episode and just accepts everyone.

But it DOES bother you when Enzan joins the race at the last second, you self centered big headed hypocrite! You are tied in with Rush for the second spot on my list of most hated characters from this season! (You should all know who number one is).
In the cyberworld, Glyde asks Iceman to be his co-pilot and Roll invites Rush along before noticing something wrong with him. Turns out the Cutman brothers are back and they made a paper cut out of a flea to spy on Rockman’s group. They plan to take out Rockman during the race because they still think he’s evil and now they seem to have a crush on Roll all of a sudden.

Makes sense that Glyde asks to team up with Iceman, because I don’t believe there is a car small enough for him.
The flashback ends, making me wonder if the Cutmen scene was part of Rockman’s memory, and we return to the race. We also see the thoughts of the Net Agents taking part as well.

Funny you ask that, Miyuki, because we then cut to Madoi wondering why the WWW is driving a train. Mahajarama just happens to have a thing for trains.

We cut to Rockman who just now finds out that Blues is in the race, I mean come on, Netto didn’t tell him?

Gutsman manages to pass Rockman... On a tank! How?
Gutsman gets in front of Numberman and decides to bend the rules by attacking the car, sending it flying and forcing Numberman to log out after it blows up leaving an emoji face Higure-san.

The cars enter a tunnel where Iceman decides to freeze the road causing Gutsman to drift out of control.

However, Roll-chan is too close, but she avoids crashing into Gutsman by quickly drifting along with him. Worried about Roll’s safety, Gutsman decides to destroy his own car forcing him to log-out.

Here is another line changed in the dub. It’s a fact in both versions that Gutsman has a crush on Roll and he even sacrificed himself in episode 24 to protect her. Here, the dub had him saying that he was getting dizzy and he destroys his own car because he couldn’t take it anymore. I guess the dub wanted Roll to be portrayed more as an equal than having a man sacrificing himself for her for no real reason.
After this we see the Cutman brothers waiting outside of the tunnel where they make a giant paper cut out of a canon. Netto cheers for Rockman, but just then the WWW train pushes his car off balance making him slide out of control before cutting to commercials.

We come back from commercials with Netto saving Rockman by changing his tires, with a little warning about using those tires in public roads that was skipped in the dub. Rockman regains control by driving on the walls of the tunnel and the race continues with Higure-san as a new commentator.

The insult of the day is “pain in the ass” because this is the third time we hear that in this episode. Or the sub was too lazy to think of another insult.
The cars exit the tunnel where the Cutmen are waiting for Rockman with the canon, but for some reason they just stand there. I guess the cars are so fast that they didn’t get the chance to shoot?

The line in the dub was just as pathetic. ¬ -¬
The next obstacle is a draw bridge opening, Rockman and Blues manage to jump to the other side without problems, Roll-chan has some kind of air bag under her car that allows her to bounce to the other side, but the manly submarine can only do what it does best, dive.

So the Net agents are out and now its time for the WWW to upgrade their train from a steam powered locomotive to an electrical train thanks to the Count and Elecman.

I like how Coloredman and Heatman are waving their hands in the air like they don’t even care. XD
Time for the Cutmen’s third attemp to destroy Rockman, by planting something on his car using a manhole they created out of paper. It looks like they forgot cut some stairs because Cutman Jiro (Eyebrows) is using the rest of the Cutmen to give him leverage. The fat cutman at the bottom gets a supper boost of strenght causing Jiro to get run over by Rockman’s car. They ask about what happened to the paper cut out, which turns out to be a time bomb, and they assume that it successfuly stuck to Rockman’s car making them feel victorious and making up a new excuse for last time about Roll-chan being in danger too. What’s this sudden crush for Roll?!

“It was at this moment that Cutman Jiro realized... He f*!ked up.”
We return to the race with the WWW Navis in first place and attacking the other cars, they eventually take out the Battle Bus much for Yaito’s maid’s dismay.

Boo-hoo, maybe now you  can concentrate in being an objective commentator.
The WWW feels confident, but Enzan, Netto and Meiru power up their Navi’s vehicles giving them a boost of speed to pass the WWW train.

As the WWW Navis are shocked by Rockman and the others passing them, the Otoko-Maru raises from the water and returns to the race just to get in the way of the Wakahage taking both teams out.

This was just a hilarious accident, but the dub made it look like the Navis themselves did it on purpose to get the WWW out of the race. That would actually make sense, Sharkman, Woodman and Skullman sacrificing themselves for their main objective fits with their characters, after all we don’t see their reactions inside the submarine. They like “Screw this race, we are taking the WWW with us!” ;)
Cue the final lap music of Mario Karts, or in this case, Rockman’s victory music since we have reach the third and final lap of the race with Rockman, Blues and Roll-chan racing eachother.
We see the Cutmen waiting at the finish line as Rockman and Blues focus on eachother and both of them try to make a move in a corner, but they seem to forget that Roll is behind them because they open up a gap for her to move on to first place.

And the seventh panel was ruined by a layering mistake. ¬ . ¬
Roll is now in the lead, but Rockman and Blues are not happy with this so it’s time for their Netops to send in their last accessory chips to transform their vehicles, with Meiru doing the same to keep her advantage over them.

Roll’s corny car just became a freaking rocket plane with wheels!
As Rock and Blues try to catch up with her, we check back with the Cutman brothers who discover a tiny problem with their plan of blowing up Rockman.

That little Cutman is an idiot. Turns out the bomb was stuck to Jiro’s back all along, probably after he got ran over by Rockman’s car, the rest of the Cutman panic trying to take it off with no avail.
The three Navis are almost at the finish line, Roll is holding her ground, until one of her tires suddenly blows up for whatever reason causing her to spin out of control, Rockman and Blues take advantage of this to pass her.

Roll’s car finally crashes and blows up sending Rush flying and forcig Roll to log out leaving behind a tear.

Yeap, that is a tear drop breaking. The dub kept this in because I remember wondering what that was. I inicially thought it was like a sweat drop, but after watching the original version, I realized that was a tear because Roll was heartbroken knowing that she failed to stop Rockman from winning and getting kissed by Aki-chan. AWWWWW.
So now it’s down to Rockman and Blues! Who will become the champion?

Hmmm... Considering that motorcycles don’t normally take part in car races, this is gonna be a tough call.
Anyway, the race is over and I want my fireworks!

Woo hoo! Let’s celebrate because this is the last time we see the Cutman brothers. (Until the Beast saga, but that’s another story.)
As they are waiting for the results, Blues raises his index finger to the sky making everyone believe that he is declaring himself the winner, but he surrprises them by pointing at Rockman and declaring him the victor.

But this moment of good sportmanship is ruined by the photo finish that shows that the winner of the race.... Is Rush.

I dont know why they cutted out Blues’s reaction in the dub, it was funny. Or do they think Blues is too cool for this?
Yes, Rush is the winner because he crossed the finish line first after Roll’s car blew up, so I guess in a way, Roll got her wish. :3
And the episode ends with everyone booing as Aki-chan kisses an unconscious Rush and Netto breaking the picture with his face.

My thoughts?
Before I forget again, here is a fun fact! Cutman Jiro and the original Cutman were voiced by the same person who voiced Bubbleman in the rest of the anime, Chiyako Shibahara. It was until the Beast saga when the Cutman Brothers and Cutman himself return for an episode where I realized it was the same voice, the only difference being that they don’t say “Puku” all the time.
Wacky Races was an old Hanna-Barbera show from the 80′s that was very popular in a lot of countries, specially in Japan for many animes would include an episode based on the concept. Whether it was a direct parody or just inspired by it, this episode is clearly one of those examples. Sure, it might also be a reference to the Megaman Battle and Chase game that was released in 1997, but that could’ve been inspired by Wacky Races too.
How can I tell if this was directly inspired by the Wacky Races cartoon?

Because Roll-chan’s car is an obvious parody of Penelope Pitstop’s car from the show, with the car wearing mascara an all. And I think there was also a tank in the race.
All and all, this was a very fun episode to watch, it wasn’t ridiculous like the baseball one, this one was written into the story to actually make sense. Remember my criterias for episode 43? The episode introduced something new that fits in with the anime’s universe, and Blues’s bike will make a comeback in a future episode.

The 9/11 Memorial was created in remembrance of the nearly 3,000 victims of the attacks of September 11.  The memorial consists of twin reflecting pools that sit in the footprints of where the Twin Towers once stood.  The bronze panels that surround the Memorial pools are engraved with the names of every person who died in the 2001 attacks as well as the World Trade Center bombing of 1993.  
The Memorial Plaza is home to the “Survivor Tree”, a Callery pear tree that survived the terrorist attacks at the World Trade Center.  In October of 2001, the tree was found at Ground Zero severely damaged and was removed for rehabilitation and recovery.  The tree was returned to the Memorial in 2010 after making a complete recovery.  The tree stands as a reminder of survival and resilience.
The Memorial Museum opened in May of 2014.  The Memorial and the Museum combined occupy eight acres of land.  The museum houses 23,000 images, 10,300 artifacts, 500 hours of movies, and 1,970 oral histories.  It is divided into three parts: historical, memorial, and foundation hall exhibits.  The historical division explains events that led up to the attacks; the memorial section humanizes the event and immortalizes the impact on humanity; the foundation hall provides opportunities for hope and reflection.

Physicians will be allowed to bill Medicare for discussing terminal diagnoses, hospice care and other issues, to give patients better control over their last months.
It didn’t take a hard sell to convince Joe Brannigan that he needed hospice care.
The longtime former legislator and executive director of Shalom House, a nonprofit mental health agency, had been a vocal advocate for end-of-life care, and he helped to raise money to build the Gosnell Memorial Hospice House in Scarborough.

“They really know what they’re doing,” said Claire Brannigan, his wife of 47 years. “I was impressed with the level of care they provided. It was a very good experience for us.”
Despite the widely acknowledged benefits of hospice care, however, only 46 percent of the 2.1 million Medicare patients who died in 2013 used their hospice benefits, according to the U.S. Department of Health and Human Services.
Some people are unaware of hospice care, or they think they don’t qualify, or they avoid it because they see it as giving up rather than taking charge. Sometimes, however, physicians fail to inform their patients about end-of-life options because they’re unfamiliar with hospice care, or they’re focused on curative care, or they want to avoid a difficult conversation.
To help Medicare beneficiaries better understand and navigate their end-of-life options, starting in January, physicians will be able to bill Medicare for advance care planning discussions they have with patients. Under a new rule from the Centers for Medicare & Medicaid Services, doctors will be reimbursed about $80 for half-hour conversations with patients who have a terminal diagnosis on topics such as preparing a living will, designating a health care proxy, choosing a hospice provider and other end-of-life decisions.
“It’s about ensuring that people get the care they want,” said Dr. Mark Wrona, a palliative and hospice care expert at New England Cancer Specialists in Scarborough. “It sends a message that it’s important to start having these conversations. And if physicians aren’t comfortable having these conversations, they need to learn how or put their patients in touch with people who can.”
EXPERT CARE, LESS EXPENSIVE
Wrona and other health care providers acknowledge there’s a financial benefit for Medicare in the face of a rapidly growing senior population.
Medicare spending on hospice care increased from $2.9 billion in 2000 to $15.1 billion in 2014, but it’s still three to 14 times cheaper than acute care provided in a hospital. Hospice care costs $162 per day at home or $722 per day at the hospice house, while the typical inpatient hospital stay in Maine cost $2,238 per day in 2013, according to CMS.
“It’s a bit cynical to think lack of reimbursement is the only reason some physicians aren’t having these conversations,” said Dr. Thomas Keating, medical director at Hospice of Southern Maine. “It will definitely encourage them. It will be interesting to see how much. Hopefully it will lead to better care.”
Opponents of the new rule worry that unscrupulous physicians might “railroad” patients into receiving less care, Keating said.
“But really, it’s an opportunity to provide information and document a patient’s wishes,” Keating said. “It’s supposed to be an impartial conversation about options.”
Patients often want to hear their physician’s opinion about their prognosis, Keating said, and it’s their responsibility to share that information, even if it’s a difficult conversation. Unfortunately, he said, most doctors have had little training in end-of-life care, in part because the focus of medical school is largely on curing illness.
Given the pending Medicare billing change, Hospice of Southern Maine is considering ways that it can provide educational opportunities for physicians who want to learn more about end-of-life care and how to talk about it with patients.
“The conversations are certainly not easy, and there will need to be additional education for some physicians,” said Daryl Cady, CEO of Hospice of Southern Maine. “But end-of-life discussions need to be part of the life cycle. We need to give patients and families much more control at this very important time of their lives.”
SPENDING TIME TOGETHER
The Brannigans embraced that control. The assistance of hospice workers freed Claire, a retired nurse, to be a present, supportive partner through the last three months of Joe’s life. She used the free time to take a shower, do household chores or go to the pharmacy. A licensed social worker before he retired, Joe Brannigan chatted with the hospice workers and pressed them to share what they knew about what lay ahead.
“He wanted to know what to expect and they were able to talk with him about it,” his wife, 70, recalled. “They had real expertise, not only about his (terminal) diagnosis, but also about the services hospice provides. And they never seemed to be in a rush.”
Keen to give his wife a break, Joe Brannigan spent one week at Gosnell Memorial Hospice House in so-called respite care, which is covered by Medicare. During that week, Claire Brannigan had lunch with friends and went to a movie, but mostly she spent time at the hospice house with her husband. They read newspapers each day and chatted. After serving more than 27 years in the Maine Legislature as a representative and a senator, Joe Brannigan still loved to talk politics with his wife.
“We were able to just be together and enjoy each other,” Claire Brannigan said. “It’s so much better to go through the process with awareness and appreciation than to avoid talking about it.”
