When we're talking canon Dollrice (like black butler not red butler), it's not hard to believe Maurice thought Doll was a guy, but it'd be funnier if Doll herself also thought Maurice was a girl, and both probably thought that for a while until a third party was like "You do realise Doll is a girl/Maurice is a guy" and you just see them stare at eachother like:

Thing is, Doll is definitely aware of the fact she looks like a guy and sometimes purposely messes with people (Hell, I myself headcanon her as bigender). But I believe Doll, never in her own life would've thought that she would ever get confused over someone else's identity than the usual other way around. Must've been a wild experience.

in long story short: Both Doll and Maurice are so gender-breaking norms that they ended up fooling eachother by pure accident, they're the definition of "Handsome Girl" and "Beautiful Boy".
