New research has been published this month on diet and dilated cardiomyopathy. The paper can be accessed here: https://onlinelibrary.wiley.com/doi/abs/10.1111/jvim.16606 Key takeaway: When compared between diet groups, healthy adult dogs eating diets containing pulse legumes (peas, lentils, chickpeas, beans) as main ingredients have lower cardiac function and higher ventricular volume than dogs eating grain-inclusive diets without potatoes or pulses in the first 10 ingredients. Study summary: In this study, researchers sought healthy, owned adult dogs that had been consistently eating the same diet for at least 1 year duration and receiving 90% or more of their daily calories from a single commercial kibble. Diets were divided into "traditional" (grain-containing, no pulse legumes or potatoes in the first 10 ingredients) and "non-traditional" (pulse ingredients as main ingredients). In order to address potential confounding variables, researchers excluded dogs with congenital heart disease, heart murmur, or arrhythmia, as well as several breeds based on their potential predisposition to DCM or taurine-deficiency. In total, 23 dogs matched for age and breed were enrolled into each study group. Dogs accepted into the study received an echocardiogram. The screened dogs had cardiac measurements falling within accepted reference ranges for normal. However, on average, dogs in the non-traditional diet group had reduced systolic function (the ability of the heart to pump out blood to arteries) and larger ventricular volume (the amount of blood filled into the chamber before being pumped out-- excessively high volumes, or volume overload, is a component of heart disease). Researchers note that further study would be needed to determine if increases in ventricular volume precede increases in ventricular measurements. Researchers conclude: "Despite these limitations, results from this study showed that dogs eating nontraditional diets had several echocardiographic variables suggestive of larger left ventricular size and lower myocardial systolic performance as compared to dogs eating traditional diets, although echocardiographic variables for both groups were largely within extant reference intervals.”

I had a pause for summer holydays.
Here’s a representation of a friend that I decided to realize because she told me she has a mitral valve prolapse and associated regurgitation. In this condition some blood goes back from the left ventricle into the left atrium during systole. It is realized by the fact that the mitral valve doesn’t close properly and become leaky. It is reported as a quite frequent condition spanning from the 0,6% to the 15% of the population (nowadays considered realistic around 2,5%). It can be primary (it appears by itself, sometimes you are born with it) or secondary (caused by something else). Between the most common primary mitral valve prolapse causes (that she probably has) we can find fibroelastic deficiency and localised prolapse. In this case there are changes in the valve’s leaflets that make a rupture of the chordae tendinae more probable. In some other cases some of those chordae are simply missing. The heart valves are composed of connective tissue so their consistency is the same of a paper. To be taken in position during systole, to be watertight, the atrio-ventricular valves (mitral and tricuspid) need the chordae tendinae that work as rods. In the case there are problems with the chordae, under the pressure realized by the ventricle contracting, the valve (a leaflet) overturns back, the tight can’t be guaranteed and blood leaks. The symptoms vary greatly depending on the patient. A “mitral valve prolapse syndrome” consisting of a combination of auscultatory and echocardiographic findings, in addition to non-specific symptoms of chest pain, dyspnoea, palpitations, arrhythmias, syncope and anxiety has previously been reported. They depend a lot on the severity of the condition, though and can be totally absent. One associated symptom is usually a wooshing sound, during ausculation, after the first tone, defined holosystolic murmur. The progression and outcomes of MVP are variable. The majority of patients with a mild one have no clinical sequalae but it tend to get worst by aging (one should follow regular checkups as she does). An interesting thing is that mitral valve prolapse has been associated with sudden cardiac death with an incidence of up to 2% per year that is thought to be associated with ventricular arrhythmias. In the case it gets too severe with dangerous or disturbing symproms, the approach is surgical with the substitution or the repair of the valve.
