Hiatus Hernia Surgery: Finding Relief from Heartburn and Other Symptoms
Hiatal hernia, a condition where part of your stomach pushes through the diaphragm into your chest, can cause a range of uncomfortable symptoms like heartburn, regurgitation, chest pain, and difficulty swallowing. While lifestyle changes and medication can often manage these symptoms, some individuals find long-term relief through hiatus hernia surgery.
Tumblr media
When is surgery considered?
Surgery is typically recommended when conservative measures fail to provide adequate symptom control. It may also be considered for:
Large hernias: Hernias exceeding 2-3 cm in size are more likely to cause severe symptoms and complications.
Bleeding: In rare cases, hernias can cause bleeding from the stomach lining.
Narrowing of the esophagus: Hiatal hernias can constrict the esophagus, making swallowing difficult.
Twisted hernia: A serious complication where the herniated stomach tissue loses its blood supply.
Types of Hiatus Hernia Surgery:
Two main approaches are used:
Laparoscopic surgery: Minimally invasive procedure involving small incisions and specialized instruments for repair. This is the preferred method due to its quicker recovery time and fewer risks.
Open surgery: Requires a larger incision in the abdomen for direct access to the hernia. This method is usually reserved for complex cases or when laparoscopic surgery is not feasible.
The Surgical Procedure:
The specific technique employed depends on the severity of the hernia and the surgeon's preference. However, some general steps include:
Repositioning the stomach: The surgeon gently pulls the stomach back down into the abdominal cavity.
Repairing the hiatal opening: The diaphragm muscle is strengthened or stitched closed to prevent further herniation.
Fundoplication (optional): Wrapping the upper portion of the stomach around the lower esophagus to create a valve-like mechanism, reducing acid reflux.
Recovery and Outlook:
Most patients recover well from hiatus hernia surgery. Laparoscopic surgery typically involves a shorter hospital stay (2-3 days) and faster recovery time (4-6 weeks) compared to open surgery.
Potential risks and complications:
As with any surgery, there are potential risks involved, including infection, bleeding, and anesthesia-related complications. Specific risks associated with hiatal hernia surgery include difficulty swallowing, gas bloating, and recurrent reflux.

Symptoms of Hernia
Symptoms of Hernia: Common symptoms of a hernia include:
A visible bulge or lump, especially when coughing or straining.
Discomfort or pain, which may worsen with physical activity.
Burning or aching sensation at the site of the bulge.
A feeling of pressure or heaviness in the abdomen.
Nausea or vomiting (in severe cases).
Tumblr media
Diagnosis: A hernia is typically diagnosed through a physical examination by a healthcare professional. The doctor may ask about the symptoms and medical history, and perform a thorough examination of the affected area. In some cases, additional imaging tests such as ultrasound or MRI may be used to confirm the diagnosis.
Surgical Techniques: There are two primary surgical techniques used for hernia repair:
Open Hernia Repair: In this traditional approach, the surgeon makes an incision near the hernia site, pushes the protruding organ or tissue back into place, and reinforces the weakened area with stitches or a mesh patch. The incision is then closed with sutures.
Laparoscopic Hernia Repair: This minimally invasive procedure involves making several small incisions in the abdomen. A laparoscope (a thin, lighted tube with a camera) is inserted through one incision, allowing the Best Laparoscopic Hernia Surgeon in Delhi to view the hernia on a monitor. Surgical instruments are then inserted through the other incisions to repair the hernia using sutures or a mesh.
Recovery: After hernia surgery, the recovery period varies depending on the surgical technique used and individual factors. Generally, the following guidelines apply:
Open Hernia Repair: Recovery time is typically longer, with most individuals returning to normal activities within 2-4 weeks. Strenuous activities may be limited for up to 6 weeks.
Laparoscopic Hernia Repair: Recovery time is often shorter, with most individuals returning to normal activities within 1-2 weeks. Strenuous activities may be limited for 3-4 weeks.
During the recovery phase, it is important to follow the surgeon's instructions regarding wound care, pain management, physical activity restrictions, and dietary recommendations.
Potential Complications: While Hiatus hernia surgery in delhi is generally safe, there are potential complications associated with the procedure, including:
Infection at the surgical site.

Inguinal hernias, in the inner groin area, occur when the intestine protrudes through either of two passages in the lower abdominal wall called inguinal canals. Researchers have estimated that about 27% of men and 3% of women will develop an inguinal hernia at some point in their lives.
Best hernia surgeon in delhi
Femoral hernias, in the upper thigh/outer groin area, occur in deeper passages called femoral canals. They are much less common than inguinal hernias and are far more likely to occur in women than in men. They also present a higher risk of complications if they are not repaired.

Ventral hernias occur though an opening in the muscles of the abdomen. There are three types of ventral hernias:

Epigastric hernias occur above the bellybutton.
Incisional hernias are caused by a weakening of the abdominal muscle resulting from an incision made during a previous abdominal surgery.
Umbilical hernias, which occur near the bellybutton, are most common in newborns, especially those born prematurely. These hernias usually close on their own by the time the child turns 4. Umbilical hernias can be more problematic when they occur in adults.

Hiatal hernias occur when the upper part of the stomach bulges into the chest through a small opening (the hiatus) in the diaphragm. The hiatus allows the esophagus, which carries food from the mouth to the stomach, to pass through the diaphragm.

A common symptom of most hernias is a noticeable lump or bulge, and potentially some discomfort or pain. The lump or bulge may not always be present; for example, it might go away when you lie down. Symptoms may worsen when you are standing, straining, or lifting heavy objects. Most hernias can be confirmed by a doctor during a physical exam, but sometimes imaging is necessary.

Hiatal hernia symptoms are an exception to the general rule, as they do not cause a bulge. But hiatal hernias may cause symptoms such as heartburn, acid reflux, and regurgitation of food or liquids, which are often treated with medication.

What are the options for hernia surgery?

Hiatal hernia symptoms can often be treated with medication, but most other types of hernias require surgical repair, although not always immediately. There are two primary options:

Open hernia surgery. The surgeon makes a cut in the groin to view and repair the hernia. After repairing the hernia, the surgeon uses stitches alone or stitches and a piece of mesh to close the abdominal wall. The mesh is designed to strengthen the weak area of the abdominal wall where the hernia occurred.

Laparoscopic hernia surgery. The surgeon makes several small cuts in the lower abdomen and inserts special tools to view and repair the hernia. The surgeon typically uses a piece of mesh to close and strengthen the abdominal wall. There are some surgeons who opt for robotic repair, which means they sit at a console controlling robotic arms that perform the surgery.

While the use of mesh is predominant and has been shown to help prevent the recurrence of hernias, it also has potential complications, including a risk of chronic pain.

“Despite reduced rates of recurrence, there are situations where the use of surgical mesh for hernia repair may not be recommended,” the FDA advises. “Patients should talk to their surgeons about their specific circumstances and their best options and alternatives for hernia repair.”

What are the alternatives to surgery?
Best hernia surgeon in delhi
“Watchful waiting” is considered a potential alternative to surgery when a hernia is causing minimal or no symptoms. People who delay surgery, especially men with an inguinal hernia, should watch for symptoms and see a doctor regularly.
Best hernia surgeon in delhi
About 70% of men with an inguinal hernia who delay surgery will develop new or worsening symptoms and will need surgery within five years, according to the National Institute of Diabetes and Digestive and Kidney Diseases. One risk of waiting too long is that a larger hernia is more difficult to repair.

Hernia Surgery
The hernia occurs when a weak spot in the muscles or tissues allows an internal organ to protrude. Hernias are divided into several different types, such as inguinal hernias, femoral hernias, umbilical hernias, and hiatus hernias. The treatment for a hernia involves the use of a laparoscopic surgical procedure and general anesthesia, as well as a breathing tube. On the lower abdomen, three half-inch or smaller incisions are made. Hernia repair is performed laparoscopically using a camera called a laparoscope to visualize the hernia damage on a monitor.

Laparoscopic Fundoplication – Say Goodbye to GERD
GERD or Gastro-Esophagal reflux disease, is a condition related to gastroenterology that causes inflammation, pain (heartburn) and other serious complications, like scarring,in the oesophagus. GERD occurs when acid refluxes or regurgitates from the stomach back up into the esophagus. GERD results from a dysfunction in the lower esophagal sphincter or LES, which is a muscle which usually closes off the esophagus from the stomach under normal conditions.
Fundoplication is a standard surgical method for treating GERD. When combined with laparoscopy or laparoscopic surgery, it is called laparoscopic fundoplication.
The Procedure
During the procedure, the fundus, or the part of the stomach closest to the entry of the esophagus is gathered, wrapped and sewn around the lower end of the esophagus and the LES in a process called plication (hence the name fundoplication = fundus + plication). The procedure increases the pressure at the lower end of the esophagus, thus reducing acid reflux. All this is done through five small holes made in the stomach through which the laparoscope and tools can be passed for surgery. Other steps are also frequently taken during fundoplication, which reduce acid reflux. For example, if the patient has hiatal hernia, which 80% of GERD patients do, the contents of the stomach (hernia sac) are pulled down from the chest and the opening in the diaphragm (the hiatus) through which the esophagus passes from the chest into the abdomen sewn so that the stomach remains within the abdomen. The advantage of laparoscopic fundoplication, as opposed to the traditional surgery is a speedier recovery and less post-operative pain. There are super-specialty hospitals in Gurgaon and Delhi NCR, as well as hospitals in Patna and Darbhanga in the Bihar region where this procedure is available.

Laparoscopy for Hiatus Hernia
Laparoscopic Hiatus Hernia Repair with Laparoscopic Nissens Fundoplication (Hiatus Hernia with Gastroesophageal reflux Disease)
A laparoscopic fundoplication is a keyhole procedure to relieve chronic heartburn caused by acid travelling the wrong way into the oesophagus when acid reflux is not controlled by medication & lifestyle changes. This disease is also called Hiatus Hernia with Gastroesophageal reflux. Heartburn is burning feeling that starts from in the middle chest and moves upwards towards neck & throat. In Hiatus Hernia when part of stomach slides into chest cavity causes acid reflux.
Dr Randeep Wadhawan has done hundreds of successful procedures of Laparoscopic Hiatus Hernia repairs with laparoscopic Nissens Fundoplication. In this procedure with the help of Laparoscopy(key hole surgery) the gastroesophageal junction is dissected , the hiatus hernia is repaired with sutures/ mesh depending on the size of the defect in the crura followed by a wrap at the top end of the stomach around the bottom of the oesophagus to form a new valve thus preventing the reflux.
Advantages:
One hour procedure.
2 Days of Hospitalisation.
No long term medication.
Excellent results
Return to work in 3-4 days time.
