Phase II mitigée pour l'éculizumab dans le syndrome de Guillain-Barré

L'anticorps monoclonal éculizumab (Soliris*, Alexion) dirigé contre la protéine du complément C5 n'a pas atteint le critère principal d'évaluation de son efficacité chez des patients à un stade avancé du syndrome de Guillain-Barré, mais les données semblent encourageantes, selon une petite étude de phase II publiée dans The Lancet Neurology.

Malgré les traitements existants (plasmaphérèse et immunoglobulines intraveineuses), beaucoup de patients atteints du syndrome de Guillain-Barré ne se rétablissent pas totalement, soulignent Sonoko Misawa de l'université de Chiba (Japon) et ses collègues.

Alors que de précédentes études ont suggéré un rôle du complément dans les lésions des nerfs périphériques, les chercheurs ont voulu évaluer l'efficacité et la sécurité de l'éculizumab, un anticorps monoclonal humanisé dirigé contre la protéine du complément C5, chez des patients souffrant d'un syndrome de Guillain-Barré à un stade avancé.

Ils ont pour cela inclus un total de 34 patients âgés d'au moins 18 ans atteints d'un syndrome de Guillain-Barré et ne pouvant pas marcher sans aide (score fonctionnel entre 3 et 5 points).

Les patients, qui ont tous reçu un traitement à base d'immunoglobulines (400 mg/kg/jour en intraveineuse pendant 5 jours), ont été randomisés entre l'éculizumab (900 mg, 23 patients) et un placebo (11 patients). L'inhibiteur du complément C5 et le placebo ont été administrés par voie intraveineuse à raison d'une fois par semaine pendant 4 semaines.Le critère principal d'évaluation de l'efficacité du traitement était la proportion de patients ayant récupéré la marche sans aide, présentant un score fonctionnel inférieur ou égal à 2 points. Les chercheurs ont prédéfini pour cela un seuil minimal de réponse, avec une limite basse d'intervalle de confiance à 90% qui soit supérieure à 50%.

A l'inclusion, 30 patients sur 34 présentaient un score fonctionnel de 4 points, soit un handicap très sévère.Après 4 semaines de traitement, la proportion de patients capables de marcher sans aide était de 61% dans le bras éculizumab et de 45% dans le groupe placebo. Le critère principal d'évaluation n'a pas été rempli, dans la mesure où la valeur basse de l'intervalle de confiance à 90% était de 42% dans le groupe éculizumab.

Tous les patients ont présenté des effets indésirables. Trois patients ont présenté des effets secondaires graves, dont 2 dans le bras éculizumab (une anaphylaxie pour l'un et une hémorragie intracrânienne et un abcès pour l'autre). Il n'est pas exclu que ces effets secondaires aient été liés au traitement, précisent les chercheurs.

Les auteurs appellent à ce que des études de plus grande ampleur soient mises en place afin d'évaluer l'efficacité de l'éculizumab dans le syndrome de Guillain-Barré, estimant que leurs résultats préliminaires soulevaient "la possibilité que l'inhibition du complément grâce à l'éculizumab puisse améliorer l'évolution à long terme" de la maladie chez ces patients.
