Union Minister for Health & Family Welfare Harsh Vardhan says the Centre is coordinating with states to strengthen capacities to stop coronavirus from spreading. In an interview with Prerna Katiyar, the minister spoke about increased screening of passengers at airports and seaports and spreading awareness in rural areas. Edited excerpts:

What key steps are being taken to prevent a coronavirus outbreak? When the news of the outbreak of a disease rising from novel coronavirus (Covid-19) came to us, it was essential for us to review our situation and preparedness for a full-blown response. On January 8 (before WHO declared Covid-19 a public health emergency), we undertook the first review with the help of experts from WHO as well as those from the field of our preparedness as well as tenacity of our response mechanisms to manage any case that might get imported to India.

We also asked states to review their preparedness, identify gaps and strengthen core capacities in surveillance, laboratory support, infection prevention and control, logistics, risk communication and, in particular, hospital preparedness in terms of isolation and ventilator management of critically ill patients.

Screening of passengers started on January 18. Initially, airports in Delhi, Mumbai, Chennai, Kolkata, Bengaluru, Hyderabad and Kochi were covered but subsequently, it has been expanded to 30 airports. As per the evolving situation, initially universal screening was taken up for all passengers coming to India on direct flights from China, South Korea, Japan, Iran, Italy, Hong Kong, Vietnam, Malaysia, Indonesia, Nepal, Thailand and Singapore.

Since March 4, directions have been issued for universal screening for all international passengers coming into India. Signages have been displayed at prominent places in airports and ports, in-flight announcements are being made and self-declaration forms are being filled up by all passengers. As on March 7, 7,675 flights have been screened covering 799,105 passengers. Teams of specialist doctors are stationed at all the airports to ensure effective screening and arrangement for isolation in the attached hospitals.

What about screening of passengers at India’s seaports? Screening of passengers has also been initiated at 12 major seaports and 65 nonmajor ports to identify passengers and crew coming from China and to isolate them in case they are found to be symptomatic. As on March 5, 16,532 people have been screened at the ports. The government has initiated screening at all integrated check posts with bordering countries in collaboration with Uttar Pradesh, Uttarakhand, West Bengal, Sikkim and Bihar as well as Sashastra Seema Bal and Land Port Authority.

In view of the continuing lockdown of the Hubei province in China, we decided to evacuate Indian students and other professionals working in Wuhan and neighbouring cities. Two special Air India flights were operated between Delhi and Wuhan on January 31 and February 1 that brought back 654 passengers, including 647 Indians and 7 Maldivians.

These evacuees were kept in the Army Quarantine Centre in Manesar and ITBP camp in Chhawala. All these evacuees were tested after 14 days and on being found negative, discharged on February 18.

Further, the Indian Air Force on February 26 evacuated 112 people from Wuhan, which included 76 Indians and citizens of Myanmar, Bangladesh, Maldives, China, South Africa, the US and Madagascar. They have all tested negative so far and are stable. The Indian embassy and consulates are also in regular contact with the Indian community in other parts of China and are keeping a constant track of their well-being. Another evacuation was successfully carried out by Air India by bringing back 124 people on February 27. This included five foreign nationals who were aboard the Covid-19-infected cruise ship Diamond Princess from Yokohama, Japan. Even these evacuees have tested negative and are stable.

Regular ‘contact surveillance’ has been initiated across the country for all cases having travel history from all major Covid-19-affected countries and for people having contact with such persons and/or having fever, cough or breathlessness. Through the Integrated Disease Surveillance Network, all such people were tracked and as on March 7, 31,598 people were brought under community surveillance and monitored.

The state surveillance officers, district surveillance officers and rapid response teams of health professionals led by state health secretaries are monitoring all such people on a daily basis. We have issued guidelines to support states on surveillance and contact tracing, surveillance at points of entry, laboratory samples collection, packaging and transport, clinical management protocol and infection prevention and control in healthcare facilities.

What is being done to prevent shortage of critical preventive items? To ensure availability of critical items like personal protective equipment and masks, their exports have been restricted. Risk communication material has been prepared and is being widely disseminated even in regional languages through state governments. Press releases are being issued on a regular basis.

Information about travel advisory and other pertinent issues is also being shared on the social media. Required awareness in community is being ensured through technical briefings by experts on radio and television. Daily press briefings are being held by ministry of health and information is also being shared through social media.

A 24×7 control room is operational with call centre number 011-23978046. So far, around 10,000 calls have been attended, including 667 international calls. The government is in regular touch with WHO to get updates on the evolving scenario and best practices.

What are the steps being taken to spread Covid-19 awareness in villages? We have adopted a two-pronged strategy. We have deputed eight Central teams to rural areas adjoining international borders in UP, Uttarakhand, West Bengal, Sikkim and Bihar to review the activities at border crossings, hold gram sabhas and facilitate awareness and risk communication to communities.

A total of 3,977 gram sabhas have been held and 1157,129 people have been screened at border check posts. For other rural areas, we have sought support of the ministry of panchayati raj to facilitate awareness generation at the community level and have provided them with requisite publicity material. Publicity material has been distributed to states in regional languages also for wider dissemination. We are also utilising the network of National Disaster Management Authority to spread awareness about the disease.

How many additional beds have been set aside for Covid-19 patients? States and Central government hospitals have been advised for assessment of isolation and critical care facilities and adhere to infection prevention and control guidelines. Sufficient isolation beds and supplies have been made available in the tertiary facilities across the country to manage any outbreak. As on date, around 17,500 beds are available at various government facilities. Additional facilities will be made available as and when required. I have also held meetings with leading private hospitals and they have all assured support to tackle the situation. Apart from isolation beds, nearly 14,000 quarantine facilities have been identified so far and more are being identified.

How is the Centre coordinating with states on containing Covid-19? The health ministry is the nodal agency for this mission and we have taken upon the task of coordinating this effort with our sister departments in the central government as well as in states. We are regularly reviewing the emerging situation as well as preparedness with the states and Union territories. Me and the health secretary (Preeti Sudan) have been organising regular meetings with health ministers, health secretaries and other senior officials in states and UTs to review the public health interventions being undertaken and assistance required, if any, from the Government of India. I must commend states for the wonderful efforts they have been making in containing the outbreak of the disease. My team in the health ministry is working 24×7 to ensure no stone is left unturned in containing the spread of this virus.

Do we have enough testing facilities for the disease? National Institute of Virology (NIV) in Pune is the nodal laboratory. As part of the Indian Council of Medical Research¡¦s preparedness for emerging and reemerging infectious diseases, NIV has established capacity for molecular diagnosis of Covid-19. Next generation sequencing has also been established. The facility for collection and testing of clinical samples has been set up in 51 laboratories across the country. Additionally, 56 laboratories have been identified as sample collection centres. The network is being expanded.

Are there any plans to rope in the paramilitary and the army to combat the spread of the virus? At the highest level, our Prime Minister is reviewing the situation on a regular basis with representatives of all Central ministries. Further, a Group of Ministers, chaired by me, has been constituted with participation from ministers of civil aviation, external affairs and shipping and home affairs to facilitate intersectoral coordination at the highest level and discuss pertinent issues. We have held four meetings so far. Senior officials from the ministry of defence, home ministry, department of commerce and department of pharmaceuticals are already represented in the group for speedy decision-making.

The principal secretary to PM has held five meetings so far. Since January 27, the Cabinet secretary has held 15 meetings with different ministries. A technical Joint Monitoring Group under the director general of health services under my ministry has held seven meetings. I have written to various chief ministers and held video conferences with state health ministers. I have also visited the Delhi airport to check field level implementation. My ministry¡¦s joint secretaries have been allotted states to guide and monitor their preparedness.

Similarly, in times of exigencies such as this, capacity building is a must. As you have asked about various agencies of the government that have been roped in to combat this deadly virus, we have built a lot of capacity in the last 45 days. These include training of airport health and immigration officials. State & district surveillance officers have been oriented through video conferences.

Quarantine medical teams have been oriented on patient management and infection prevention and control and state rapid response teams trained for managing high risk pathogens. The NDMA has convened meetings with 20 civil society organizations and 300 NGOs. A national training of trainers was held on March 6. It was attended by 260 health professions from states, ESIC, railways, defence & other ministries, with 1,000 locations logging in. On March 9, state-level trainings are being organised and district-level trainings will be completed by March 20, 2020.

The focus areas of this capacity-building have been epidemiology, surveillance, laboratory support, clinical management, nonpharmaceutical interventions, infection prevention control and risk communication.
