In the untitled s8 meanstiel fic (wherein Cas does not "get better" after s7) that it seems less and less like I'm going to write, the way the actual dean/cas/meg sex scene would happen is:
It would come towards the end of at least 60k of character development, which is something to bear in mind.
Castiel and Meg had been sleeping together since before the start of the story. Which Dean hates. And Meg is viciously jealous of Dean. But Cas doesn't see his relationships with them as mutually exclusive at all. And after Dean and Cas do the "I love you"/"I love you too" thing, Meg is still metaphorically clinging to Cas with her teeth. And Dean very begrudgingly puts up with her because Cas cares about her.
(There would be a fair amount of Dean&Meg stuff in the story where it's established that their mutual hatred does not just come from jealousy but also from recognizing something of themselves in the other. And they come to a certain level of understanding. But they're still very antagonistic).
(It's also important that when Dean tells Cas that he loves him, Cas can't really deal with it and kind of freaks out. Because he still feels so overwhelmed with guilt for the leviathan stuff, and polluted and self-loathing. The idea of Dean loving him breaks something in him. It seems unjust. It's wrong. This reaction is cut across by Plot Things happening.)
At a point when most of the plot is done and things are relatively quiet, Dean and Cas are having a moment, which turns into kissing, which starts to turn into more. But Cas becomes overwhelmed again. And Cas wants to keep going, but he's having a hard time, like to the point of shaking and hyperventilating. And this hasn't been something that Dean has been able to bring Cas down from on his own at any point.
So my idea is. Dean goes to find Meg. And he does not say what's going on, he just tells Meg to come with him. And he leads her back to the bedroom hoping that she understands on her own and he doesn't have to say anything. Upon seeing Cas on the bed, half undressed, Meg gives Dean a very questioning look. And all says to Meg, pointedly, and kind of pissy, is something like, "I'm not kissing you."
And anyways, Meg is Cas' emotional support girlfriend when he's having sex with Dean.
