RSV season was bad as fuck but yesterday was probably the worst I've seen our ED yet. Was promised that we don't hallway board any of our patients but apparently... now we do!!... and I was the hallway RN for the first 4 hours of shift while triage was just randomly dumping patients on me to get them out of the waiting room, ended up with this poor girl in sickle cell pain crisis sitting in a chair for hours waiting for me to put in an IV and give the pain meds that there were already orders for, and two patients that the provider discharged for me, and one patient that I never even saw, because I walked past my "asthma" BPD baby that I got zero report on from triage and was like hm ok, they're not working too hard, maybe the neb worked, WAIT WHY ARE THEY AT 85% ON ROOM AIR and they ended up on high flow and transferred to a different facility's PICU because there are no beds left at my hospital. When I left, 75% of the beds in the ED were boarders admitted and waiting for beds upstairs! It was basically an inpatient unit with some emergency critical care capacity! Aaaaaand now FEMA is about to strip away our emergency funding and overflow space because COVID is no longer a pandemic but an "end-emic" :)))))))))))))))))))

In this investigation, Capparis sepiaria was analysed for its phytochemical constituents qualitatively and quantitatively. The antibacterial property of aqueous, ethanolic and hexane extracts of Capparis sepiaria was studied against different bacteria include Escherichia coli, Proteus mirabilis, Salmonella typhi, Enterobacter aerogenes, and Shigella flexineri. Hexane extract of Capparis sepiaria showed the maximum growth inhibition zone of 20.4±0.2 mmat concentration of 500 mg against Salmonella typhi. Please mention the highest inhibition result including extract name and concentration. The antioxidant effect of those extracts was also studied against α-tocopherol as a control. From the results, Alkaloids, flavonoids, saponins, and tannins were revealed to be present in Capparis sepiaria. Ethanol extract at the concentration of 500 μg/ml showed 57.81% antioxidant activity against 500 μg/ml of α-tocopherol which showed 66.76% as a standard reference. Análisis y Plan de manejo: Hombre de 29 años con sin antecedentes de importancia, con larga estancia hospitalaria secundaria a politrauma por arma de fuego de carga múltiple en región toraco-abdominal el 14/06 y larga estancia hospitalaria extra institucional. Sufrió hemotórax derecho, hemoperitoneo, múltiples laceraciones en ambos lóbulos hepáticos, lesión de colon ascendente, transverso y gástrica única, y trauma pancreático. Como complicaciones ha presentado disfunción multiorganica, lesión renal aguda, pancreatitis necrosante, peritonitis (Candida tubo germinal positivo 21/06 y 6/07 y P. aeruginosa 6/07), fístula pancreática, y fungemia el 21/06. Durante su estancia extrainstitucional requirió por lo menos 10 intervenciones para lavado y drenaje de hemoperitoneo en relación a sangrado, drenaje de colección y peritonitis, corrección de evisceración y omentectomía por necrosis, entre otras. Fue remitido el 9/07 para embolización, por sangrado persistente de origen desconocido, anemizante. Ingresó con bacteriemia por P. aeruginosa sensible de origen en foco abdominal, que depuró y ha requerido múltiples lavados por peritonitis fecal asociada a fístula, con aislamientos polimicrobianos: C. tropicalis sensible, E. coli sensible y P. aeruginosa resistente a carbapenems (10/07); C. tropicalis, E. coli, K. pneumoniae sensibles y P. aeruginosa MDR (por mecanismo no enzimático) (13/07); E. hormaechei, P. aeruginosa MDR, E. faecalis, K. pneumoniae y C. tropicalis (16/07); C. tropicalis y E. coli (19/07, absceso pancreático); C. glabrata sensible a micafungina, K. aerogenes productora de carbapenemasa S solo a amikacina, genta, TMP y tigeciclina (31/07). Último esquema ceftolozano tazobactam + amikacina (doble cubrimiento para P. aeruginosa), ampicilina sulbactam (E. faecalis y anaerobios), fluconazol y ciprofloxacina (ajustado el 4/08). Por última vez fue llevado a cirugía el 7/08 donde se sigue documentando fístula entero atmosférica sin canalización adecuada al exterior, y abundante materia fecal en cavidad; no se tomaron cultivos. Requirio tratamiento en cuidado crítico por sangrado digestivo dependiente de arteria del ciego que necesitó embolización. En las últimas tomografías con persistencia de las colecciones (3 y 11/08), por lo con oco no controlado en el contexto de fístulas no canalizables con abdomen bloqueado, colección persistente en retroperitoneo y aislamiento de gérmenes MDR. El 15/08/ 23 se realizó nuevo drenaje guíado por TAC donde obtienen 5cc de pus espeso de necrosis infectada de donde se aislaron Candida glabrata, K. aerogenes resistente a carbapenems sin detección de resistencia enzimatica sensible a TMP/SMX y en la nueva TAC del 20/08/23 se demostró persistencia de la colección con dren in situ, el 22/08/23 se hizo nuevo drenaje de 10cc de pus (no se ordenaron cultivos) y se dejó nuevo dren para optimizar el drenaje de dicha colección persistente. Venia recibiendo caspofungina y TMPS desde el 17/08/23 y Tigeciclina desde el 18/08/23. Desde el 170/08/23 estaba sin fiebre y con descenso de la PCR. El 23/08/23 se suspendió tigeciclina. Ayer Cirugía indico lavados en marea observandose salida de líquido por herida de laparotomia y adicional tuvo de episodios de fiebre ayer por lo que se decide hemocutlivar por riesgo de bacteriemia y candidemia, ante foco intraabdominal no controlado lo cual condiciona el desarrollo de gérmenes MDR, se comentará con Cirugía general. Atentos a evolución.

Health Canada recently published a consumer product recall for certain laundry products due to the presence of a chemical hazard. On March 31, the government agency shared that people should immediately stop using certain fabric conditioners sold in the country.

"This recall involves The Laundress brand fabric conditioners," advises Health Canada, noting that all lot codes are being recalled. "All recalled products have Laundress New York' printed at the top of the label. The recalled products were sold individually, in kits, or as part of a set. An issue with the products sold by the company is that they can have a chemical impurity known as ethylene oxide, which is a carcinogen.

According to the recall, it can cause "adverse health effects if there is significant and direct long-term exposure." Approximately 13,000 units were sold in Canada, and as of March 28, there were no reports of incidents or injuries in the country.

The recent recall comes in addition to a previous recall announced on December 1, 2022, for "certain lots of Laundress brand laundry detergent, fabric conditioner, and household cleaning products" due to their possibly containing "Burkholderia cepacia complex, Klebsiella aerogenes, and multiple different species of pseudomonas." In that recall, the company said it was aware of 11 people who had become sick with a pseudomonas infection.

A complete list of products being withdrawn by The Laundress can be found here. People who have any of the listed items can request reimbursement from the company via their website.

"After requesting a refund, consumers should dispose of the product by closing the bottle tightly and placing it in the household trash," advises Health Canada. "We look forward to reintroducing The Laundress with products that reaffirm our commitment to the highest standards of consumer safety and quality."
