I'm not sure what exactly is going on with Hutch in season 4 but I'm pretty sure it could be cured by Starsky fucking him straight through one of their mattresses

Starsky and Hutch - Older Guys

After Starsky returns to the force after the events of Sweet Revenge, he takes the Lieutenant’s exam and eventually becomes Captain. Hutch meanwhile quits the force for good and finally finishes medical school, becoming a top Cardiology Consultant. They enjoy their new life together, finally safe with Starsky behind a desk and Hutch treating patients. 

Or so they thought until Starsky is shot during a hostage situation at his own precinct and is then eventually rushed to the hospital where Hutch specialises, barely alive. Hutch despairs as history seems to repeat itself
