Insurance: Is there a way to get this covered?
Yesterday 1/25, I contacted my insurer. I'm currently on a private insurance plan through my former employer.
Sterilization
My insurance plan won't cover a second procedure under diagnosis code Z30.2 [Encounter for sterilization]. Their coverage of sterilization under diagnosis code Z30.2 is limited to one procedure per member per lifetime.
Cancer prevention / family history
My first priority was to check if procedure code 58661 [Laparoscopy, surgical; with removal of adnexal structures (partial or total oophorectomy and/or salpingectomy)] could be covered under diagnosis code Z80.41 [Family history of malignant neoplasm of ovary]. The representative I spoke to confirmed that this service has the ability to be covered, but pre-authorization is required. Certain requirements must be met to prove medical necessity. Without sharing too much family/personal info, I'll just say that I don't quite meet the requirements.
I thought this might happen, so I was prepared with a backup option.
Gender dysphoria
My second priority was to check if procedure code 58661 could be covered under diagnosis code F64.9 [gender dysphoria]. The representative confirmed that this service also has the ability to be covered, but as before, pre-authorization is required. Certain requirements must be met to prove medical necessity, and this time, I'd be able to meet the requirements. They're the same requirements I'd have to meet for coverage of hysterectomy under diagnosis code F64.9. Also, I've already been diagnosed with gender dysphoria.
Well, that's not bad! At least it's something.
But since I'll need the same kind of therapist letters either way, I might as well get a hysterectomy.
Moreover, I suspect that this coverage of procedure code 58661 under diagnosis code F64.9 is intended to cover oophorectomy with or without salpingectomy--i.e. removal of ovary, with or without removal of tube--but not salpingectomy alone. I have a feeling that it won't work out in the end. I can't be sure, and maybe it's still worth a shot.

Over a month into discontinuing Lupron
I went to the gynecologist on Thursday
I asked her what she thinks could be causing my issues, and she said there could be endometriosis but in such minute amounts that it doesn't show on a laparoscopy (however the visible amount doesn't correlate to pain levels and someone could show few to no symptoms of endo while having a ton of it in their body), or it could be adenomyosis which can only be diagnosed with a hysterectomy, or it could be primary dysmenorrhea which is where your body makes a bunch of prostaglandins for no reason
I told her about a letter I'm writing to a local hospital in which I complain about their approach to mental health and include a nurse practitioner who used to do my primary care and now works at the hospital in their eating disorders program and who said my issues probably aren't recognized by modern medicine, so I wanted to be able to say "a specialist thinks it's X which is known to occur in Y percent of the population" and have that information to plug into the letter (which is mostly done and just needs me to fill in where I talk about what we suspect is going on) but my gynecologist offered to write a letter of her own -- I said she didn't need to do that but I like that she made the offer
We won't be doing it any time soon bc Alysena is working great so far, but I asked her a couple of questions about hysterectomy and she said I'd be spending the night in the hospital and they'd be taking my cervix
I told her I've been taking only the Alysena without the Estrace or Movisse (since it's just two different sets of estrogen and progesterone) and she said it's correct to do that
Something to keep in mind if you're going to have a laparoscopy -- I found out on Thursday that they did an internal exam during my surgery :/ which is totally legal to do without consent in some places, apparently Ontario is included in that, but I still have complicated feelings about it and will probably be talking about it in my next therapy session bc part of my whole thing is that I have information kept from me and I'm not allowed agency... I didn't tell the gynecologist about these feelings during the appointment but I'll try to tell her next time
Alysena is working well and I have plenty of refills so I'm supposed to come back in 6 months, which is August
I checked the mail today and I got a letter from a gastroenterologist with an appointment date for a consult in June, since my gynecologist told me to have my GP refer me to one

I don’t even need a pad for it. But the cramps are awful.
I had severe endometriosis. I almost lost a kidney, part of my bowel and my bladder had been bisected. I had 4 laparoscopies before I finally said OK to a total hysterectomy. I only wish I would've done it sooner.
Sometimes I get scared that I might have it but I haven’t had any other issues other than really bad cramps during my “period”
