I hate to bring up the pliable bones theory (no, really, I hate it), but like...

Elain's a Seer, and canonically, not every child conceived between a winged faerie and a High Fae female will have wings. Even if Nesta didn't take steps to protect Elain like she normally would and ask the Mother to change the shape of Elain's pelvis as well, knowing that there was something brewing between her sister and Azriel after Solstice party in ACOSF...

What's to stop Elain from training her power and using the magical rhythm method to predict which conception would result in wings vs. which wouldn't, if she wants biological children with an Illyrian badly enough? Ultimately, it's foolish to think it actually matters what her reproductive system is capable of one way or another, but I'd put money down that she can still neatly sidestep the whole mess anyway, widened pelvis or not.
