More kids means more worry. Was it always so?
October 20, 2023
I attended a luncheon recently put on by the alumni club of the small Catholic school that I attended in Dearborn. I sat at a table with fellow members of my graduating class (1965), and one of the topics we discussed was the number of "large families" when we were growing up.
Among friends from my class alone, the Flanigans had 11 children. The Laczynski family had eight. And the O'Reillys had seven.
I had many friends from other classes who also came from mega familes: The Geisingers had 11 children. Another branch of the O'Reilly clan had nine. And the Academy Award for the biggest family of all at St. Alphonsus (which encompassed grades one through 12) went to the Horrigans with 16 kids (including two sets of twins).
There were children everywhere in those days. In fact, I would put the average number of kids per family in my old neighborhood at five. Which is why classrooms at St. Al's had upwards of 60 students when I started grade school in the early '50s. (Today, 35 kids per class is considered practically criminal.)
I guess our parents took it to heart when the Lord said, "Be fruitful and multiply..."
Now, to the point of all this:
On an absolutely gorgeous day earlier this month, my wife and I received a phone call from our neighbors across the street who asked if we'd like to sit on their boat that evening to enjoy some cocktails and conversation.
"Sign us up," we said. The world was looking sunny and bright as we anticipated kicking back and watching boats return from a day out on Lake Michigan. Until, that is, we started getting phone calls from our kids that afternoon.
The first came from one of our daughters, informing us that another surgery would be required if she hoped to have a second child. Then one of our sons called to report that he'd been experiencing panic attacks. By the time we were into our second beer on the boat, a call came from our other daughter, letting us know that her baby had been diagnosed with cellulitis.
Thankfully, we did not receive a call from our other son who lives near Chicago that day.
Like most fathers, I suppose, I started worrying about my kids on the spot. Upon lying down that night I tossed and turned in bed, continuing to worry. And to think deep thoughts.
I thought about how much my wife and I have enjoyed having four children--a large family by today's standards--and how much we unconditionally love them all. And yet, I reasoned as I attempted to relax, if we had decided to have only two kids, perhaps we would experience only half the worry.
Then I started to think about my folks.
They had five children. It's not entirely clear to me how much they worried about my siblings and me. My Dad was a hands-off parent, leaving most of the responsibility for raising us to my stay-at-home Mom. If there was worrying to be done about us kids, I'm sure she shouldered the load. Her greatest worry, quite frankly, was my Dad--every time he drove home drunk from the bar. Which was often.
In those days, we all walked to school. Played outside into the dark. And took the Tireman bus downtown, unchaperoned. If my folks worried about us doing those things, I was oblivious to it. Except for one of my brothers, we all did reasonably well in school. Plus, other than toncilitis, knee operations and some broken bones, we were all healthy. I think it's safe to say that about the only thing my parents really, really worried about was that, when we were teenagers, I or one of my brothers would get some girl pregnant. Or that one of my sisters would get pregnant.
As regards those supersized families back in the hood, I suspect there were times--although I don't know--that some parents may have worried about how they would feed all those mouths. In the case of the Horrigans, Mr. Horrigan died somewhere along the way when practically all of their children were in school. If anyone had a right to worry, certainly it was Mrs. Horrigan. How could she not? And how in the world, I wonder, was she able to raise 16 kids alone? If she had signigicant help, I was oblivious to that, too.
I only know this for sure: It's true that your kids are your kids for life. And that you're likely to worry about them until the day you die.

A man drove through a crowd gathered for a fundraising event Saturday, killing one person and injuring 17, before fatally attacking a woman in a neighboring borough, Pennsylvania authorities said.
The suspect, identified as Adrian Oswaldo Sura Reyes, 24, was arraigned by Magisterial District Judge Doug Brewer on two open counts of criminal homicide, Pennsylvania State Police said in a news release early Sunday.
Reyes, who was from Nescopeck, a small township in Pennsylvania's eastern Luzerne County, “was denied bail and remanded to the Columbia County Correctional Facility,” the release said.
No details were released about the victims.
At an earlier news conference on Saturday evening, state trooper Anthony Petroski said that a vehicle drove through a “community event” in Berwick around 6:15 p.m.
After the crash, the suspect was accused of fatally assaulting a woman in Nescopeck, where he was taken into custody by municipal police, Petroski said.
Police are investigating whether the suspect intentionally drove into the crowd, and have not said whether there was a connection between the suspect and the woman who was killed in Nescopeck.
NBC affiliate WBRE of Wilkes-Barre reported the crowds had gathered at a Berwick benefit event to raise funds for the families of the three children and seven adults who died in an early morning house fire on Aug. 5 in Nescopeck.
Officials have not released the identities of the two people who were killed. Columbia County coroner Jeremy Reese said the woman in the Berwick crash was 50 years old, and from Wilkes-Barre. She was declared dead at the scene.
The 17 injured people were taken to four area hospitals, Petroski said. Their conditions were not immediately available from law enforcement Saturday night.
Mike McMullen, a spokesperson for health care provider Geisinger, said it had been treating 15 patients from the Berwick accident at three of its locations. Five are in critical condition, and three are in fair condition. Seven were treated and released, he said.
Saturday's benefit was being held at the Intoxicology Department bar and restaurant. People had also gathered in an adjacent outdoor space, where various vendors and a basket raffle had been set up under tents. The event also included a water balloon fight and a dunk tank, according to the bar's social media.
“We can never replace these people that were lost, but we can do our best” to support family, bar owner Lauren Hess told NBC affiliate WBRE of Wilkes-Barre at the benefit Saturday.
On Saturday night, the bar posted on Facebook that it would be closed until further notice.
“Today was an absolute tragedy,” the post said. “Please respect our privacy while we grieve and try to process the events that occurred.”
Funerals were set to begin Friday for some of the victims who died in the Aug. 5 blaze, with additional services scheduled for Sunday and Monday.
The deceased included seven adults ranging in age from 19 to 79, two boys ages 5 and 6, and a 7-year-old girl, according to Pennsylvania State Police. All 10 died of smoke inhalation, the coroner determined.
Pennsylvania State Police initially called the case a “complex criminal investigation.” Investigations into the fire and its cause, which has not been determined, are ongoing.
The fire broke out before 3 a.m and first responders reported flames that repelled their efforts to get inside the two-story home and make rescues, authorities said later that day.
The home was burned to the ground, and remains were discovered as authorities went through the rubble.
Asked if the suspect in Saturday’s incidents was also a possible suspect in the Nescopeck fire, Petroski said, “At this time he is not a suspect in that incident.”
Nescopeck is a borough of more than 1,400 people, located about 48 miles southwest of Scranton. Berwick, in Columbia County, has a population of about 10,300 and sits across the Susquehanna River from Nescopeck.

Do Physicians Value Medical Scribes?
Tumblr media
There are nearly as many ways to scribe as there are ways to be a physician. The actual practice of scribing isn't quite as simple to generalize across contexts and specializations. But there are a few things that all of us realize very early on in our training, which essentially boil down to this: scribes are best seen and not heard, dutifully typing findings to the physician's preference but refraining from actively participating in assessment or treatment, to be as non-disruptive as possible. Most studies on the employment of medical scribes have concentrated on their clinical and financial viability, which have received generally favorable reviews. It has been demonstrated that the employment of scribes increases the frequency and quality of patient contacts per hour for both patients and clinicians. Both the RVUs per visit and the RVUs per hour increased for clinicians using scribes in terms of clinical reimbursement. (Medical Scribes, Provider and Patient Experience, and Patient Throughput)
According to recent research published in Health Affairs, doctors now split their time roughly equally between visiting patients and working on computers. The researchers studied data on doctors' time allocation tendencies in more than 31 million electronic health record (EHR) transactions between 2011 and 2014, which 471 primary care clinicians logged. They discovered that, on average, doctors spent 3.08 hours seeing patients and 3.17 hours working at their desks. (Why Geisinger posts negative physician reviews)
The study confirms the conclusions of other studies. According to a study published in the Annals of Internal Medicine last year, doctors only spend 27% of their workday seeing patients and just under 50% of their time on administrative and EHR chores. Physicians are increasingly using medical scribes to handle data input to alleviate the administrative load associated with EHRs and free up time for face-to-face interaction with patients. The American College of Medical Scribe Specialists predicts that by 2020, there will be 100,000 medical scribes working in the United States, up from 15,000 in 2014. (ACMSS). According to the Journal of the American Medical Association, at least 22 businesses were offering scribe services as of April 2015.
(The Rise of the Medical Scribe Industry Implications for the Advancement of Electronic Health Records)
Recommended Reading : AI Medical Scribe Vs In-Person Medical Scribe
A medical scribe has a significant impact and offers several advantages to patients, medical office employees, and clinicians. A medical scribe gains expertise in terminology and coding and can lessen the administrative work required of doctors and office workers. This can reduce the extensive time required to catch up on data entering. A medical scribe frees up a doctor's time, allowing them to devote more time to their patients and employees. A clinical scribe reduces a physician's administrative duties, making the doctor more effective throughout the day. ( Scribes Lessen FPs' Administrative Burden, Researchers Find ) A medical scribe reduces documentation, giving a doctor more time daily to communicate deeply with more patients. Wait times are shortened, patient satisfaction is raised, and income may rise—especially in private practice. As scribes update electronic health records in real-time, they prevent memory lapses, distractions, and confusion that can occur in medicine's fast-paced, sometimes stressful field. This benefits the completeness and accuracy of patient information.
How Has Electronic Health Records (EHR) Lead To The Need For Medical Scribes?
Electronic health records, or EHRs, include much information to handle since they are much more thorough, precise, and particular than the outdated paper charts of the past. EHR was initially introduced to speed up the creation of medical records and lessen the time healthcare professionals had to spend on paperwork. These computerized versions of patient information, which include medical charts, test results, and therapy, are known as electronic medical records. They also include documentation of immunizations, laboratory, and radiological testing, as well as long-term care and prescription records. While complicated, EHR may offer a comprehensive picture of the patient's health, which is crucial for providing quality patient care. The capacity to gather more thorough and precise documentation led to more information gathering rather than a decrease, which has had some unforeseen effects. EHR record overload causes doctors to report less work satisfaction and increased rates of burnout. Patients are also less content. In summary, EHR has fundamentally altered how doctors practice medicine. It has caused a movement toward a more administrative position and away from the amount of time spent on direct patient care. As a result, medical scribes are now required. Medical scribes focus on document retrieval, upkeep, and updating in EHR. Physicians may return to providing direct patient care by giving medical scribes the chores associated with the EHR. (EHR use consuming physicians’ time for patients )
According to research that mixed family and internal medicine and used in-person scribes, labor relative value units (RVUs) rose by 10.5%, and the number of patients seen each hour rose by 8.8%. 3 Additionally, the time doctors spent looking at patients grew by 57% while their time looking at computers reduced by 27%. Compared to the pre-scribe period, the percentage of clinic charts that remained open after the day did not vary considerably. According to other research, scribes increased physician satisfaction in all areas, including time spent charting, time spent with patients, and the quality and correctness of the charts. The scribes' significantly reduced the time required to complete charts. 32.6% of scribes finished in 48 hours, compared to 28.5% of charts created by doctors. In further research examining the impact of scribes on physicians' attitudes and behaviors, most of the physicians' complaints were workflow and administrative issues at the clinic rather than the scribing service itself. There is an apparent reason why there is a rise in the need for medical scribes. Complete and accurate records are required to provide the finest continuum of care possible, but attentive doctors are the other component of the jigsaw. Medical scribes work alongside doctors to improve the efficiency of the healthcare system by allowing doctors to spend more time with patients and less time managing their electronic health records (EHR).
