Just a reminder that I have an ongoing fic of these two on AO3! Here's the warnings and (brand new!) summary below:
WARNINGS: blood, manipulation, sex, mentions of trauma, violence and gore in some chapters (will be stated in chapter notes), sometimes there will be angst, the idiot gets cared for
Primrose is a druid, tree-hugger, and the de facto leader of their merry little band of tadpooled misfits. She's been completely isolated from society, living alone in the woods for who knows how long, and given her naivety, Astarion figured she was raised by wolves.
Regardless, being a leader means she is a sturdy foundation for himself to latch on to, if he can manipulate her to care for him. She's prone to waxing poetics and altruistic to a fault, the perfect victim for his ministrations – so imagine his surprise when she turns out to be more complex than he gave her credit for.
Why, exactly, has she been hiding from society? Prim carries herself as if she would never do any wrong, but when provoked, she's shockingly deadly. The more time he spends with her, the more he has a suspicion that there's something dark lurking beneath the surface…. Perhaps it's time to find out.
This fic updates weekly! It starts off kind of like sequential drabbles, but has turned into a larger overarching story that I'm excited to share!! 🫶 No joke, I have 60,000+ words for this in the doc right now, I'm just forcing myself to take my time with posting it so I can be consistent lol

So it's understood that the first time Astarion feeds on tav, it is intimate. Not necessarily sexual, but there's a closeness that needs to happen. Tav has to trust him with their life. As their relationship progresses, I wonder if how he feeds changes. The first time he is gentle of course, even cradling tavs head and making them as comfortable as he can. But he still just bites and moves on. But maybe, as time passes, he finds himself stroking tavs hair or rubbing his thumb along their cheek while he feeds. Or taking his time so he can hold them for a while longer. Maybe that's one of the things that makes him realise how hard he's falling for them.

i saw someone on reddit mention this and wanted to see it for myself, so here it is. if you initiate a romance with astarion, but then later pursue someone else, he’ll almost always be catty about it, imply that you have poor taste for choosing someone else over him, and will reject any offer of polyamory on the grounds of either personal standards (ew, you want him to be in a love triangle with who?) or self-preservation (he ain’t fighting lae'zel for your ass lmao).
but if his rival-in-question is karlach…

meanwhile, if you put karlach in the same situation (start out romancing her, then pursue astarion), she’ll also start out quite sweet…

What if your Tav was a recruitable companion, instead of the main character? (contains major spoilers for the game, and for some dark urge runs as well)
General
Where can your Tav be recruited?  Are they first encountered on the Nautiloid, or in the Nautiloid crash region?  Or are they not recruitable until a later act?
Do the other companions have special comments or reactions upon recruiting your Tav?
Does your Tav have any comments or advice when you recruit other companions?
What sort of general actions raise or lower their approval?
Are there any instances where your Tav can permanently leave the party, depending on player character actions?
Do they have any secrets that can be revealed?  What are the prerequisites for this secret coming to light?
Do they have their own personal quest that spans the course of the game?  Can it take different branching paths depending on the choices the Player Character makes?
What do they say when the Player Character asks them to stay in camp?  How about when the Player Character asks them to come adventuring again?
Does your Tav have any escalating conflicts with one of the other companions, like Lae’zel and Shadowheart’s knife-fight?

How do they react to the Player Character taking their first tadpole power?
Will they stay with the Player Character regardless of siding with the goblins or the tieflings, or is it possible for them to leave the party permanently?

How do they react to the Dark Urge killing Alfira?
If romanced, how do they react to the Dark Urge trying to kill them in Act 2?
Romance
Is your Tav a romanceable character?  Are there any specific requirements to romancing them?
Does your Tav need to be flirted with to start the romance, or will they approach the PC themselves if approval is high enough?
Are they a polyamorous or a monogamous option?
Do they have a special romance scene at the tiefling/goblin party?
Does the romance have different branching paths, or just one route to take?
How do they react to the player character breaking up with them, or choosing another character over them?
What questions can Zethino ask the PC about Tav in the Love Test? 
If they’re poly, do they have a reaction to the PC engaging in a relationship with Halsin?
How do they react if the PC has sex with Mizora? The Emperor?  Haarlep?
Will they join in with the PC and the Drow Twins, or no?
What are Tav’s plans for the future?  Do they propose to the PC, or is marriage not something they’re interested in?

It's a rarer option when switching to control his character outside of combat. He quotes Poe directly, saying:
"Villains! Dissemble no more -- I admit the deed! Tear up the floor. Here, here! It is the beating of his hideous hear- oh, no, that's his brain.... Where did I leave that heart?"
You can hear it in this video, around the 16 second mark! (Though I recommend watching the entire thing, as it has some of his sillier one-liners 💜)

I love how Astarion quotes The Tell-Tale Heart every once in a while. It's a rarer line, and initially I thought it was out of place (Neil is very well versed in theater, so I assumed it was a riff from him), but since reading an analysis of the work I think it was pretty purposeful.
The piece is all about fear and paranoia, things we know Astarion is plagued by despite how he might act. Similarly, the narrator of the story also tries to convince the reader that they are not as troubled as they seem. In the end, the narrator is consumed by the beating of the heart of the old man he killed and dismembered, the sound growing louder and louder until in a fit of rage he reveals the body to the police to absolve himself from the persistent beating.
Except the police never heard the heart beat, because it wasn't the old man's heart at all. The narrator was consumed by the sound of his own heart beating more and more rapidly in his chest from fear. He was the owner of the thing that forced him to reveal his true nature, he is the owner of the tell-tale heart.
And what happens with Astarion after you romance him? He realizes over time that, while he tried to deny his feelings and was initially only interested in manipulating you for his own means, he actually has grown to care for you. You have done something to his heart that hasn't happened in centuries, you have made it feel as if it has started beating again.
Therefore, his tell-tale heart leads him to admit his transgressions, which were committed out of fear and paranoia for his safety.
So the line is actually very, very apt. His confession during Act 2 is his own version of "Villains! Dissemble no more! I admit the deed! Tear up the planks! Here, here! It is the beating of his hideous heart!" Except, of course, it is his own heart that he is unearthing for us (and it's not so hideous, after all).
