What's the difference between a nurse and nurse practitioner?
They are two different "levels" in the nursing hierarchy.
Here's how that hierarchy works in the USA:
At the very beginning rung you have STNAs, or State Tested Nursing Assistants. STNAs have taken a class that lasts in the weeks-months range of time and received a certificate. They have then taken a test to prove that they understand the material. In a job context, STNAs do a lot of the direct patient care- they bathe and feed patients, take vitals, turn patients who cannot move for themselves, etc...
The next rung up is an LPN, or Licensed Practical Nurse. LPNs have gone to a nursing program lasting usually between 1 and 2 years, have received a diploma, and have taken a test called the NCLEX-PN. In a job context, LPNs can do everything an STNA can do, plus they can gather information about a patient's condition, pass medications (except some IV medications) and do many nursing interventions like wound care, placing IVs and catheters, etc... An LPN must work under the license of an RN.
The next rung up is an RN, or Registered Nurse. RNs have either an associates' degree (2-3 years) or a bachelor's degree (4 years) in nursing, which has allowed them to sit for a test called the NCLEX-RN. No matter which education route the nurse takes to get their RN the license is the same and they are allowed to do the same things in a clinical context. In a job context, an RN can do everything an LPN can do, but can give all medications (including IV) and are allowed to also make care decisions LPNs cannot. They also hold an independent license, meaning they can provide nursing level care without needing to be supervised by a higher license-holder.
The next maybe half rung up is a Certified Registered Nurse. These are nurses who have worked in a particular area of nursing (med-surg, psych, ICU, etc...) for a certain number of years and taken classes and a test to say they know a lot about that area of nursing. Now, unlike the nursing license itself, this certification is usually through a private organization instead of the state of practice. Certified Registered Nurses don't have any additional skills they can do, but they do act as a resource for other nurses and may get paid more depending on the organization they work for.
The next rung up is an APRN, or Advanced Practice Registered Nurse. An APRN is a nurse who has a masters degree or higher in nursing, and who, again, has taken a test. Here is where things get complicated. Because 4 different licenses fall under APRN:
Nurse Practitioner (NP). An NP is an APRN who works under a physician's license in some states and hold independent licenses in others. They are able to do a lot of things that doctors can do, including diagnose, treat, and prescribe (what they are able to prescribe depends on the state they are in). NPs specialize in a specific patient population, and they can only work with the type of patient they have specialized in.
Clinical Nurse Specialist (CNS). A CNS is an APRN who acts as a consulting nurse. Basically, they have gotten specific education in a specific area of nursing for a specific population, and they consult with the multidisciplinary team, generally at a hospital, about the nursing care for that type of patient. CNSs typically cannot prescribe medications, but may prescribe nursing interventions.
Certified Registered Nurse Anesthetist (CRNA). A CRNA is an APRN who works as an extension of an anesthesiologist. They provide anesthesia during surgery and procedures.
Certified Nurse Midwife (CNM). A CNM is an APRN who delivers babies and helps mothers and children through the entire maternity and birth process. They may also do things like women's health exams, nutritional counseling, and the like.

Brad brought Bria to the doctor after twenty-four hours. She had a cough, sore throat, runny nose, and was sneezing. Since she was on chemotherapy, his anxiety level was high. While they sat in the waiting room, she wore a mask. He took her hand into his. The doctor would know what to do. She nodded. He could tell she was very tired. She was physically tired and mentally tired of being sick. He had all the compassion in the world for her. It wasn’t fair.
The woman he loved was independent, energetic, had a great sense of humor, loved everyone around her, was selfless, and just an amazing woman. He missed that. She was trying so hard but her body was trying to kill her. He rubbed her back as she coughed into her arm. When her name was called, he got up and wheeled her back into the exam room.
The nurse noted she did not look well. She took off her mask, so she could examine her. Did her chest hurt when she coughed? She nodded and put her hand over her stomach. Did it hurt her stomach? She nodded. Brad had to help her explain her symptoms because she was having trouble talking, due to her sore throat. They started the day before. She had been doing well the previous day. Then, when she woke up, she complained about not feeling well.
It was likely not a cold but the flu. The nurse passed on the information to the doctor, who did a more thorough assessment. She confirmed that she did not have a cold but the flu. The differences were subtle. A cold came on gradually, while the flu happened suddenly. Both were dangerous for patients undergoing chemotherapy because of their weak immune systems.
Did she have nausea or was she throwing up? She shook her head no. Brad mentioned she was more tired than usual. He had her sleep as much as possible. During the day, he was also giving her bottles of water for her throat. Those were good things to do. The doctor was also going to prescribe an antibiotic to help her fight the infection. Was he the only person taking care of her? No, his mother and a friend were also helping out.
He was limiting visitors to those who were healthy. Whoever they lived with also had to be healthy. The doctor agreed that was a great idea to help prevent her from getting more sick. Was he living with her or did he come over to help out? No, he lived with her. She gave him instructions on how to help her beat the flu virus. It sounded like he was doing everything correctly.
There were just more things he probably didn’t think of. She recommended not having any visitors at all, except for the mentioned friend and family member. The more people coming in and out of the house, the more germs there were. Bria nodded showing she understood. Brad also confirmed he understood. Could she be around her two cats? Yes, cats could not make her more sick. However, she she not change their litter box. He did that.
After the appointment, she thanked her by extending her hand. The doctor shook it and told her to feel better. She nodded as Brad thanked her. They then proceeded to go to the pharmacy for her prescription. She put her mask back on before asking for a bottle of water.
At home, he helped her to the couch. He put the blanket over her before going back to the kitchen. She had her bottle of water with her. Garfield jumped up on her lap. Human? She petted him while he head-butted her. Both he and Tiny could see how sick she was, even though they didn’t understand what was wrong. She continued petting him as he sat down on her stomach.
Brad read the instructions on her medication. Take one pill per day with or without food. He would give it to her at dinnertime and then once a day at breakfast. Before coming back to the living room, he grabbed the whiteboard and marker they used for reminders and brought them out to the living room for her to use. He then grabbed his phone and called his parents, as they wanted an update. Garfield purred, hoping it would help her feel better.
He also kneaded her with his paws. She kissed his forehead. Because she couldn’t speak, she couldn’t tell him she loved him like she always did. From his body language, she got the feeling he already knew and he loved her too.
Joyce was thankful to hear from him. He relayed the information given to them by the doctor. They assumed she had a cold, but that wasn’t the case. She reminded him he didn’t know everything. He knew that. Bria fell asleep while they were talking. Garfield laid down beside her. Brad put the blanket over her to keep her warm. He wanted her to get as much sleep as possible.
He had to go back to rehearsals the following day, so Joyce volunteered to stay with her. Thank you. Could she give her a warm bath sometime during the day? It would help a lot with her flu. Yes, she could do that. After finishing the conversation, he called Mike.
“Don’t blame yourself. We didn’t know.”
He nodded. “I’ll try not to. Who’s going to stay with her tomorrow?”
“My mom is coming over around nine. I’ll make breakfast for her and leave it in the refrigerator before I leave. Then, she can eat it whenever she wakes up.”
“That sounds like a good idea. I’ll call the other guys and tell them the news. Remember to take care of yourself, too. Do something for yourself for a few hours while she sleeps.”
He would do that. Thank you. Sometimes he got so involved in taking care of Bria, that he forgot about himself. She would want him to take care of himself. After finishing the call, he checked on her and found her still fast asleep. He went upstairs and grabbed a change of clothes before going into the bathroom. A nice warm bath was calling his name.
The bathroom had a clock, so he could keep track of time. Mike called his parents, Chester, Rob, and Joe to let them know. When Phoenix came over, he let him know as well.
“Mike, breathe. We’re all very scared for her but you need to stop for a moment.”
He inhaled and exhaled. “Thank you.”
“You’re welcome. You’re not going to take away her cancer by stressing yourself out. She wouldn’t want you in the hospital for exhaustion. You or Brad. Right now, the only thing you can do is to let everyone know what’s going on and you did that. I’m taking the phone, so you don’t get obsessed with it.”
“What did I do to deserve you?”
He laughed and put the phone in his pocket. They then kissed before going to the kitchen to make dinner. The bath felt incredible! It was like a vacation! He washed the germs off of his body and just relaxed in the warmth of the water. Yes, he did lose track of time.
He drained the water, rinsed out the bathtub then got dressed. Bria was awake when he went back downstairs. She was sitting up with the blanket on her lap. Asshole. Just kidding. He read what she had written and laughed. How was she doing? She wasn’t as groggy. Other than that, she still felt the same. He was going to make her some chicken noodle soup. After helping her in her wheelchair, he brought her into the kitchen.
