CES 2019: ExactCure sécurise l'usage du médicament grâce à la biomodélisation
La start-up ExactCure a présenté au Consumer Electronics Show (CES), qui se tient jusqu'à vendredi à Las Vegas, une application modélisant l'action d'un médicament sur le "jumeau digital" d'un patient afin de sécuriser la prise de traitements, et travaille déjà avec plusieurs établissements et industriels de santé.
Créé en janvier 2018, ExactCure allie mathématiques, biologie et informatique pour simuler l'effet d'un médicament sur un patient en fonction de ses caractéristiques personnelles, de son état de santé et des autres traitements qu'il a pu prendre dans la journée.
Cette biomodélisation est fondée sur les "modèles pharmacocinétiques des médicaments", qui sont notamment publiés par les industriels pour obtenir des autorisations de mise sur le marché (AMM), a expliqué à APMnews Fabien Astic, cofondateur d'ExactCure avec deux anciens ingénieurs de Dassault Systèmes, Frédéric Dayan et Sylvain Benito.
"Notre savoir-faire consiste à créer des 'métamodèles' sur la base de plusieurs modèles mathématiques existants, et d'y accoler des caractéristiques propres à un patient pour diminuer l'incertitude quant à l'action du médicament", a-t-il résumé, rappelant que "la médication inappropriée tue cinq fois plus que les accidents de la route en France".
Présentée dans une version bêta au CES, l'application d'ExactCure pourra être utilisée par un patient sur ordinateur ou sur smartphone. Elle lui propose de créer son "jumeau digital" en saisissant plusieurs informations comme son poids, sa taille et son âge.
L'utilisateur peut ensuite indiquer les médicaments qu'il compte prendre et leur dosage, et l'application simule la durée de leur action sous la forme d'une horloge colorée du bleu au rouge, les zones rouges indiquant un risque de toxicité.
ExactCure vise un certification de son outil comme dispositif médical (DM). Un brevet sur sa technologie de biomodélisation est en cours de dépôt.
"Optimiser l'action du médicament"
Fabien Astic a mis en avant les multiples usages du dispositif, aussi bien au niveau des patients que des professionnels de santé, médecins et pharmaciens, pour "aider à cibler la fenêtre thérapeutique et optimiser l'action du médicament, en fonction du profil du patient".
L'idée d'ExactCure est notamment née d'un échange entre Frédéric Dayan et le Pr Philippe Beaune, spécialiste de la médecine personnalisée et praticien à l'hôpital européen Georges-Pompidou (HEGP, AP-HP, Paris), aujourd'hui conseiller scientifique de la start-up, qui constatait les difficultés à ajuster ses prescriptions face à des patients souffrant de plusieurs pathologies.
Pour "coller aux besoins des utilisateurs", la version bêta de l'application va être testée cette année dans le cadre de trois projets pilotes menés avec des patients et des professionnels de santé, en visant en priorité les maladies chroniques, a indiqué Fabien Astic.
Le premier concernera un groupe de patients "vieillissants, isolés géographiquement et socialement, et polymédiqués", le deuxième sera mené avec une association de patients souffrant de rhumatismes inflammatoires chroniques, et le troisième avec l'Union des syndicats des pharmaciens d'officine (Uspo) et le Regroupement autonome des généralistes jeunes installés et remplaçants (Reagjir)
"Nous discutons aussi avec des professionnels très motivés dans les hôpitaux", a dit Fabien Astic, expliquant être en discussion avec le CHU de Nice, l'HEGP pour un projet autour des patients atteints d'un cancer, et l'hôpital universitaire de Monastir en Tunisie.
Intérêt des industriels
Fabien Astic a souligné l'intérêt de l'outil pour les industriels pharmaceutiques, qui peuvent ainsi "accompagner la pilule du médicament par un compagnon digital, capable de dire au patient comment l'utiliser".
Un autre axe "très prometteur" de développement est de permettre à l'utilisateur d'indiquer sur son "jumeau digital" comment il se sent après la prise d'un traitement, pour "pouvoir fournir à l'industriel un retour statistique et qualitatif 'en vie réelle' sur l'effet du médicament".
Le modèle économique d'ExactCure repose sur les industriels. "L'utilisateur final est le patient ou le professionnel de santé, mais le client est l'industriel, qui a tout intérêt à ce que son médicament soit mieux utilisé", a expliqué Fabien Astic. Il a fait état de "discussions avancées" avec Sanofi et AstraZeneca.
La start-up est "accélérée" au sein du 3DExperience Lab, incubateur de Dassault Systèmes, avec qui elle est présente au CES de Las Vegas. En plus de sa "visibilité" et de ses "relations avec des investisseurs et potentiels clients", le premier éditeur français fournit à la jeune pousse sa suite logicielle Pipeline Pilot*, qui lui permet d'élaborer ses simulations.
Un secteur "extrêmement dynamique"
Parmi la quinzaine de start-up accompagnées par Dassault Systèmes au CES, près de 40% concernent la santé. On peut citer Biomodex, iLumens et Digital Orthopedics dans la simulation et l'impression 3D, ou Bioserenity, qui fournit des vêtements connectés pour le suivi de maladies chroniques.
Le secteur de la santé est jugé "extrêmement dynamique" par Géraldine Gandveau, responsable marketing du 3DExperience Lab, interrogée par APMnews.
"On sent aujourd'hui un vrai besoin de changer d'approche dans la santé, avec la volonté de soigner de façon plus juste et précise aussi bien grâce à la molécule que grâce au device et à la technologie", a-t-elle analysé.
La start-up ExactCure est pour l'heure financée essentiellement par des aides publiques régionales, nationales et européennes, et devrait boucler une première levée de fonds d'au moins 1 million d'euros en 2019.

Paris-based medical device company Biomodex is opening a factory in the Boston area in the next year to produce replicas of individual patients’ organs using 3D printing.
The company announced Thursday it has raised $15 million for the 4,500-square-foot facility, from European investment firms Idinvest Partners and InnovAllianz.
The synthetic organs are used by doctors to “test drive” procedures on patients, said Carolyn DeVasto, Global Vice President of Commercialization at Biomodex. The companies uses medical images of the organ that is to be operated on, such as a heart with a structural defect or a brain with an aneurysm, to design the replica.
Right now, products are in the testing stages and Biomodex is running independent studies in which surgeons rehearse their procedures on the replica organs before performing the real thing.

“They practice with these printed organs much like they would a cadaver,” DeVasto said, “to give physicians as life like an experience as possible.”
The startup plans to hire 10 to 12 new employees from the US in the next year, with about half expected to work at the Boston facility.
