Tumblr media
There is something so inherently nasty about cigarettes.
The unnatural smoke that burns your eyes, the chemicals that collect under your fingernails, and the smell that manages to leave an everlasting scent on your clothes.
You were never a smoker– somehow managing to avoid the advances of the punk outcasts trying to sell their self-rolled cigs in the back of your highschool parking lot for a dollar each. Sure, there was the typical uncle who seemed, and smelled, like he went through two or three packs a day. The faded voice of a family friend warning the children of the dangers of “the cancer stick,” and that smoking one was equivalent to signing your soul away to the devil.
A scoff leaves you, smirking as you free said cancer stick from its confinement. You were never one to heed the advice from strangers who believed they knew you better than you knew yourself anyway. Bringing it up to your lips and quickly lighting the end, basking in the warmth the small flame brings to battle the chill of the Chicago air.
It's not as though you didn’t know the risks that the habit came with, you are not ignorant to science and health officials; but as you inhale the first hit and can practically swim in the warmth of the filtered tobacco as it fills your lungs, you damn the professionals and all their holier and wiser than thou bullshit. But as you go for a second drag, the door to the alleyway opens and you’re greeted with unruly blonde hair, light blue eyes, and the face of a man who looks like he got the shit kicked out of him.
Because he has, you think, blowing smoke from your nose at the thought. Ever since the transition from The Beef to The Bear, things in and out of the kitchen have gotten easier, but that doesn't mean a headache doesn’t follow. Signing up to work in a kitchen comes with its ups and downs, mostly downs. But those scarce highs are filled with such intense feelings of euphoria, that it is the true addiction that should be studied.
Carmy walks towards you, quick rushed steps, leaning on the wall next to you, close enough to ensure that your arms are touching. A sigh leaving his lips as he rests his weight on the wall, raking a hand through his hair only to continue to drag it down his face. You can see him turn towards you from your peripheral, but you’re looking forward because looking at him means kitchen talk and no matter how long you’ve known Carmy you know that every break talk will just lead to him ranting and raving and you're on a smoke break for a reason and–
The cigarette is plucked from your lips, fingers decorated with SOU disappear with your cigarette just as quickly as they appear, bringing it up to their owners lips for one hit, a second, before he’s placing the stick back exactly where he stole it from.
To say you’re surprised would be a lie. This isn’t the first nor will it be the last time Carmy does this. Hell, he’s the whole reason why you kissed your lungs away in the first place.
You’ve known Carmy for a few years now, having met at that bastard of a restaurant in New York. You weren’t even supposed to be there, having worked at a restaurant adjacent to it, but they were low on staff and the GMs were close enough to send their chefs back and forth when need be.
It was moments before dinner service was supposed to begin, every chef taking last minute precautions to ensure they don't get chewed out by the newly established CDC, Carmen Berzatto. You don’t even know what he looks like yet, the kitchen is doused in pure silence that even asking someone what he looks like seems like a distraction worthy of a mental breakdown from a fellow chef. Even though your check didn’t come from this place, you prepared your station as well as you would in your own restaurant because that’s what being professional means; treating anywhere you cooked with the most respect.
Stepping foot outside and leaning against the wall, you began digging through your pockets for your phone, cursing to yourself when you realized you left it next to your station.
“Hey, uh, I got an extra smoke if you want,” says a voice coming from your right. Turning in its direction, you find a long, blonde-haired man sitting on a milk crate. A cigarette is dangling from his fingers, the smoke swirling dangerously close to his eyes before he brings the cigarette back to his lips, your eyes skimming on the tattoos that decorate his arms and biceps.
“Uh, I’m sorry what,” you question back, having forgotten the original prompt said by him.
“A smoke,” he holds out a carton of cigarettes towards you, “that’s what you're looking for right?” The box is white but decoded with a strip of blue running through the center. The look he gives you is so inviting, but there's only one problem:
You don’t smoke.
Not once has a cigarette grazed your lips. Not once have you been possessed by the ghost of defiance and inhaled the breath of the devil. Not once have you been wrapped in the haze of smoke.
But the look of desperation that’s hidden behind his eyes, the subtle look asking to not be left alone in the back alley of the world’s best restaurant, is enough for you to reach out and grasp your one way ticket to demise–and oh how right you were. How could one assume that a measly little cigarette would alter the rest of your life.
The physics of it seemed easy enough: inhale and then exhale, breath in and then breath out, anybody can do it. So you take the cigarette out of the box, and lean back on the wall, inspecting it like it would sprout legs and run away.
“Hey, uh, do you have–,” the flame of a lighter is already being cupped by his hand. You bend over, close enough to this man to smell the left over nicotine mixed with the atmosphere of the kitchen. He doesn’t look away, mesmerized by the way your eyes drift to the flame to ensure the end of the cigarette is lit, the slight tilt of your head towards the heat. Even when you blink back up to him he doesn’t look away, he’s almost afraid to breathe in this moment, worried it’ll be another thing he manages to fuck up.
But then you're inhaling and–
“Holy shit are you alright,” there’s a hand on your back, patting with a gentle force with the hopes of expelling your coughing fit. “Here, have some water,” he hands you his container of water, because what kitchen has bottled water?
Taking a sip, you contemplate a universe where you can save this situation. How does one manage to fuck up this badly? All of the movies make it look so easy, but the burning of your lungs say otherwise. But the warm hand on your back doesn’t move once you stop coughing, and you turn to see worried eyes meet your own. A beat passes, then two, then a scoff leaves your lips as you shake your head in disbelief.
“Sorry, I uh,” you scramble for something, anything, to save your pride, your dignity. Here is this incredibly attractive man willing to give you a small piece of his world, and you spat it back out in his face. He must be thinking the worst demeaning thoughts, because what chef isn’t thinking in the worst way possible? Here is some person who can’t even inhale properly, what makes them think they can handle the smoke in the kitchen? Coughing up a storm all because of what, one drag of a cigarette and the chef needs to tap out–
“No it's okay, I know these ones taste bad as shit, but they’re the only pack I had on me,” he rubs the back of his neck with his free hand (the other is resting still on your back, not that either of you noticed), “I normally have this other brand, y’know a little sweeter and not as bitter and uh, yeah sorry about that,” he trails off, looking sheepish at the thought of giving out a shitty cigarette brand.
You are given two choices now: one, you can lie and agree that the brand is shit, keeping a small amount of pride and dignity, or two, come clean and admit to this total stranger that this is the first time you’ve held a cigarette and you only agreed because he looked pretty.
A former option has never looked more inviting.
So you lie, you lie out of your ass and agree that the brand is shit and that you have to get back to your station. Packing in a joke about how fucking insane the CDC apparently is and that you’re glad to only be here for the one night. You wish him luck for the night, he gives a small chuckle and wishes you luck as well.
It was five minutes later that someone pointed out that the CDC just walked in from the back and you realize that he was the same man whose cigarette you coughed up.
But that was years ago, and now here you are, with the same CDC behind his new restaurant, a now shared cigarette between your lips. You followed Carmy throughout his time in New York, you followed him to his brother’s sandwich shop, and you will follow him throughout his new endeavors at The Bear. Following him wasn’t always easy, if anything there are more lows than highs, but it’s the small moments like these that make everything worth it.
“You wanna know something funny,” he asks, stealing the cigarette again.
“What?”
“This is the same brand I had you smoke the first time we met.”
Pulling the pack out of your pocket, you let out a hum of acknowledgement, “holy shit you’re right,” the blue stripe around the box stands out against your palm.
You turn to look at him for the first time since he’s stood next to you, backs against the harsh brick of the building.
He’s already staring, a knowing smirk growing across his face, “Thought you hated that brand?”
Stealing the cigarette back, you let out a last puff of smoke, “Only hated it cause you were the one to give it to me.” You finish the cigarette, throwing it onto the concrete and stomping it out, “Come on Berzatto, this place won’t run itself,” you call out with a small wave thrown over your head, walking back towards the kitchen.
Carmy laughs, knowing that you hate the story of how you two met. He can’t help but tease ever since he found out he gave you your first cigarette by accident. You didn’t know anything about different brands, just that you found the man giving you one attractive. Carmy only knows this after taking you home after a drunken night with Sydney, you babbling about anything and everything that it took him a few hours to put the whole story together.
Of course he feels bad at certain times, such as watching you pat yourself down for a smoke only to find that you finished your last pack the other day. But Carmy is always there to give you one of his, whether it be his last one or not, only if you two can share it with a small moment together outside.
And so he walks back inside, looking forward to the next smoke break, and the one after that, until his lungs couldn’t handle anymore, only to keep going if yours haven't given out yet.

⚔️ 𝗡𝗲𝘄 𝗶𝘁𝗲𝗺! Mostly Invisible Wand Wand, legendary ___ This wand is completely invisible. While holding it, you can use an action to cast the invisibility spell from it, targeting yourself only. The spell ends early if you are no longer holding the wand. For the duration, the wand becomes visible. Its appearance changes each time; roll a d12 and use the table below to determine its appearance. Alternatively, the GM can decide how the wand appears. d12 | Wand Appearance — 1 | A glowing twig with a leaf and caterpillar on the end. The stick emits dim light out to a range of 10 feet. 2 | A salami. 3 | An opulent goblet seemingly filled with a never-ending supply of wine. The wine is illusory, but somehow still stains clothing. 4 | A children's paper fan on a stick. The fan is always spinning, as if by a stiff breeze. 5 | A sparkler that never fizzles out. It sheds bright light out to a range of 5 feet and dim light for another 5 feet. 6 | A collapsed fishing pole with a still-flopping fish affixed to the hook. 7 | A polished silver trumpet that emits a loud note every minute. The sound is audible out to a range of 100 feet. 8 | A brilliant pink wooden dowel with 8-foot-long purple ribbons trailing from its tip. Glitter streams behind it whenever it moves. 9 | A long tobacco pipe that constantly emits a thin cloud of smoke behind it. 10 | A silver knitting needle tethered to a yarn string and tangled ball. 11 | A snake that is very obviously confused. Its bites are harmless, although it hisses whenever another creature comes within 10 feet of it. 12 | A quill that scribbles a line of ink through the air for the duration. The ink hangs in the air, defying gravity, as if it were scrawled on a piece of paper ___ ✨ Patrons get huge perks! Decompensated cirrhosis
Albumin, INR, and platelets tell you about decompensated cirrhosis
Get MELD score (tells you about prognosis) and Child Pugh score (estimates cirrhosis severity). MELD-Na is a new version of the MELD score that is better than the MELD and includes Na+; it's based on whether pt goes for dialysis twice weekly, Cr, bilirubin, INR, and sodium. MELD-NA tells you the severity of end stage liver disease. Pts who present with bleeding varices or ascites have decompensated cirrhosis and worse prognosis. So calculate the MELD score to educate the pt about his/her prognosis. Child-Pugh is based on total bilirubin, albumin, INR, Ascites, Encephalopathy.
Maddrey's discriminant function suggests which patients with alcoholic hepatitis have a poor prognosis and would benefit from steroids. It's based on PT and total bilirubin.
Cirrhosis is decompensated if the pt has varices or ascites.
Pts who present with bleeding have 50% mortality in 6 months.
All cirrhotics should get an EGD to evaluate for presence of varices.
In pts with ascites, calculate the SAAG score, which tells you whether the ascites is due to portal HTN.
Patients with SAAG greater than or equal to 1.1 gm/dL is considered as having high SAAG, indicating the presence of portal hypertension, while those with SAAG less than 1.1 gm/dL are considered as having low SAAG, indicating the absence of portal hypertension.
This is an example of a GI note for a pt who came in with decompensated cirrhosis:
Pt is a ----- w/ a PMH of hepatitis C, EtOH abuse, tobacco use, and IVDU who was admitted for acute decompensated liver cirrhosis w/ascites. 1. Acute decompensated liver cirrhosis w/ascites New diagnosis of liver cirrhosis likely 2/2 EtOH/hepatitis C. MELD score = 21, indicating 19.6% estimated 3-month mortality. Notable symptoms include scleral icterus and abdominal ascites. Low concern for SBP in the absence of fever, leukocytosis, or peritonitis. Initial labs include hemoglobin 14.5, platelet count 113, INR 1.6, PTT 29.2, total bilirubin 8.3, AST 125, ALT 58, albumin 2.7, total protein 9.3. No evidence of anemia. Parsaesophageal varices suggested on CT imaging require eventual EGD for further evaluation.
Ceruloplasmin and AFP normal. Iron studies and alpha-1 antitrypsin indicative of inflammation. Additional labs pending. 4L ascitic fluid removed during 9/7 paracentesis. PMN less than 250, which is not indicative of SBP. Ascites albumin pending for SAAG calculation. RUQ U/S demonstrates slowed portal vein flow, though no evidence of masses or thrombosis.
-acute hepatitis panel, ANA (anti-Nuclear Antibody), ASMA (Anti-Smooth Muscle Antibody), AMA (antimitochondrial), SPEP (Serum Protein Electrophoresis) pending -less than 2 g daily sodium restriction -ascites fluid analysis w/ cell count, albumin, and protein for calculation of SAAG -ammonia level pending -confirm vaccination status of HAV, HBV, influenza, Pneumovax, Prevnar -avoid NSAIDs + counsel for alcohol cessation -steroids for EtOH liver disease contraindicated due unclear hepatitis C status -MRI abdomen for evaluation of portal vein thrombosis/masses -100 mg spironolactone + 40mg lasix qd
2. EtOH use Hx of significant alcohol use over the past 6 years w/o reported hospitalization. Reported taper from 3-4 glasses of vodka daily to several ounces daily; last drink 3 days ago. No withdrawal symptoms at this time. EtOH level less than 3 on admission. Suspected primary etiology of liver cirrhosis.
-management of potential withdrawal symptoms per primary team (CIWA protocol without ativan) -cessation counseling strongly recommended in setting of liver cirrhosis -potential options for medication assisted therapy include acamprosate, naltrexone, baclofen, or gabapentin -nutrition therapy including multivitamin, thiamine, and folate -recommend nutrition consult
3. Hepatitis C Reported hx of hepatitis C not previously treated, unknown genotype. Last seen by GI prior to COVID.
-HCV viral load + genotype pending -outpatient GI f/u for further management
4. Paraesophageal varices on CT Confirm CT findings w/ EGD prior to considering initiation of beta-blocker. -outpatient EGD, unless concern for GI bleed or masses seen on abdominal MRI
5. Family hx of colon cancer
Sister diagnosed w/colon cancer. Pt has not had colonoscopy to date. -outpatient colonoscopy recommended

The growing global demand for areca nut can be ascribed to its numerous health advantages. Areca nut has been shown to boost cognitive performance, decrease inflammation, and help digestion. It has also been shown to have anti-cancer, anti-diabetic, and anti-microbial qualities, making it a popular element in herbal treatments and supplements.
However, the areca nut industry faces several problems, including the harmful influence of areca nut on health, notably its link to mouth cancer. The World Health Organization has categorised areca nut as a carcinogen, causing its usage to diminish in some areas. Also, the growing availability of alternatives such as nicotine gum and electronic cigarettes has influenced areca nut demand.
The worldwide areca nut market is expanding steadily, owing to rising demand from the food and beverage industries as well as increased awareness of its health advantages. The usage of areca nut in traditional medicine, mainly in India and China, drives demand for the product.
Geographical Analysis:
Because of the highest consumption of areca in India, Pakistan, and other nations, Asia-Pacific dominates the areca nut market. Areca nuts are mostly utilised in the production of pan, pan masala, and chewing tobacco, which are primarily eaten in the Asia-Pacific area. The region's areca nut consumption is being boosted by rapid population expansion and increased demand for chewing tobacco.
Chewing tobacco is consumed by a huge population due to its good effects such as mouth freshener, germ-killing, digestion aid, mood booster, oral cleansers, and tension relievers. Chewing tobacco is made with crushed areca nut, tobacco, lime, and catechu flavour ingredient. Technological and media advertising, as well as excessive marketing of chewing tobacco in low-cost sachets and compact eye-catching packages, have massively increased the goods' sales area thus, raising demand.

The table above (Table 1), shows the demographic variables. We can see that the heavy cannabis users showed significantly higher cannabis and nicotine dependence scores compared to the healthy controls. This is reflected by higher CUDIT and FTQ scores for the CB group compared to the HC group at both baseline and follow-up points. As shown, the CUDIT scores appeared stable throughout time but the FTQ scores were higher at the follow-up point in both groups. All except one subject in the CB group smoked their cannabis with tobacco. 2 subjects in the HC group used cannabis 20-25 times in their life, while the rest used cannabis less than 5 times. The AUDIT scores, representing alcohol problem severity, increased over time as well; however, comparing the groups to each other, the changes were not significant. The difference in consumed alcohol at baseline was not significant at baseline (p=.23)  however, it was at follow-up (p=0.02).
GM volume group comparisons:
The results showed a cluster with larger grey matter volume in the CB group when compared to the HC group in the cerebellum, which was significant at baseline but not at follow-up. This cluster was also not significant after the inclusion of control covariates except for the frequency of alcohol use. The results also did not show any group differences in other ROI or at the whole-brain level at both time periods.
GM volume associations with cannabis use:
Tumblr media
The results showed a significant inverse correlation between amygdala volume and cannabis-related problems. There was a significant negative correlation at baseline between CUDIT scores and grey matter volume in a cluster in the left amygdala. However, the cluster was no longer significant after including control covariates except for the frequency of alcohol use. There was a significant negative correlation between the grams per week and grey matter volume in a cluster comprising the amygdala and hippocampus at follow-up, which remained significant after controlling for the severity of nicotine use, the severity of alcohol use, and frequency of alcohol use and without subjects with comorbid substance or psychiatric disorders. The whole brain analysis showed a significant negative correlation between the amount of grams weekly and grey matter volume in a cluster in the superior temporal gyrus, which was no longer significant after control. The linear regression analysis found no significant correlation between the CUDIT score and cannabis use measures and any of the ROIs or whole-brain analyses.

A new seed has been discovered that could have dire consequences for the environment. The seed, which has been dubbed the "direful seed," has the potential to wreak havoc on ecosystems and agricultural systems.
The direful seed was discovered by researchers at the University of California, Riverside. The seed is a member of the genus Nicotiana, which includes tobacco plants. The direful seed is a mutant of the tobacco plant, and it produces a chemical that is toxic to many other plants.
The direful seed's chemical can kill other plants by causing them to produce too much of a hormone called auxin. Auxin is a plant growth hormone that helps plants grow and develop. However, too much auxin can cause plants to become deformed and die.
The direful seed's chemical has the potential to cause widespread damage to ecosystems and agricultural systems. The chemical could kill off important plant species, and it could also contaminate food crops.
The direful seed is not yet available to the general public, and it is not known how it will be used. However, the discovery of the seed highlights the importance of GM (genetically modified) crop research.
The direful seed is a reminder of the potential risks of GM crops. GM crops are designed to be resistant to herbicides and pests, but they can also have unintended consequences. The direful seed is a reminder that we need to be cautious when introducing new GM crops into the environment.

Still Day 18
5.30pm
Belle Meade Plantation was a short easy drive from the hotel, so with a hearty Hampton breakfast in our tums, we were happily on our way.
We had decided to forgo a tour of the House and instead opted for the Journey to Jubilee Tour, which as the title suggests is the story of the enslaved at this Plantation.
A lot of people think, me included, that Plantations were all about tobacco and cotton but this is not so, Belle Mead bought and sold of thoroughbred horses.  The Harding’s owned this 8200 acre site and they owned 136 slaves and had been wealthy landowners for generations. In their heads they were good honest folk and didn’t think they were doing anything wrong.
The owning of slaves was a sign of wealth and so that everyone could see this wealth, slave homes would be built within viewing distance of guests.
There is evidence that most big houses had Refectory's, these were rooms built into the house where “ fresh” slaves were taken and broken until they did their masters bidding.  The enslaved at Belle Meade were skilled men and women, the type of skill needed to look after race horses, even though they weren’t in the field, their working day was just as harsh beginning at daybreak and ending at sunset with only every other Sunday off.
Young boys between the ages of 8 and 12 were trained to be jockeys, their weight kept down by lack of food.
Suddenly there is a Civil War, Union soldiers march into the Plantation and they tell you, you are free to go! Would you go? Would you leave everything you have ever know ?
Imagine this scene, you are an enslaved person, perhaps you have a husband , wife, children perhaps and have been indoctrinated from birth to do your masters bidding.
The nearest town of Nashville is six miles away, the only way there, across the open terrain is by cart,  you have never even set foot outside the Plantation.
Inside the Plantation you have shelter and a modicum of food, perhaps a little safety.
The guide asked us all this question and with the exception of one we all voted to stay.  The winning of the war by the Union didn’t really set the African American people free at all, yes they became “ employed” but with meagre wages and with very heavy fines for the slightest misdeed.
I left the Plantation with a heavy heart if I’m honest, the tour was called  Journey to Jubilee, but there was none such freedom for the enslaved  of Belle Meade.
Now we were very lucky with our next stop, it isn’t usually open to the public on a Monday, but a very lovely lady messaged me to say that “ yes , because we were outta state” they would open. I think we are as outta state as they come !  What a slow journey, first of all the SatNav took us to the GM motor plant and we ended up, somehow in their factory parking lot, we managed to WAZE our way out of that, then we found ourself on an interminable road with a maximum speed of between 35 and 65 MPH, lumbering through those little towns that have no rhyme or reason at all, where you find a cemetery next to an American version of Halfords, were cables are slung from one pole to another and traffic lights hanging tentatively from them
Then, if it couldn’t get more diverse we had to cross Wilson Dam to get onto the other side of the river.
Mile upon mile on an open three lane road, going no faster than 45 miles per hour is the most boring thing in the world. This journey took so much longer than it should have, but we eventually drove up the drive of the Belle Mont Mansion in Tuscumbia Alabama and a very kind lady by the name of Stephanie came out to meet us.
The Belle Mont is tiny in comparison to the other Houses we have been to, but it’s unusual because the rooms are all on one floor, with just a small attic room on the floor above.
Steph took us around the house, gave us a brief history of it as well. I was very surprised to hear that the Native American nation were still around in Alabama until the late 1800′s.
A lovely story is that the local Cherokee people had never seen glass and as the sun set on the Mansion, the panes in the windows would change colour and shimmer and sparkle. The evenings would find the Cherokee families on the grass outside of the house staring in wonder at this “magic”.
We are now settled in for the night in Florence AL
