What if Patrick Bateman was a vampire? Not sure if he'd be a Cullen but at the same time I can't see him being by himself as he thrives off of attention and acceptance from others. What would his gift be? Would the Cullens even let him stick around if he wanted to?
I mean, he'd be a sad sad man, as you note. However, it's not for the reason you specify.
He'd continue his life of eating prostitutes, homeless people, and his old coworkers and somehow nobody notices. He still goes to work, nobody notices he's not perfectly tan anymore. Somehow, no one notices he's a fucking vampire because they're also not entirely sure which yuppie he is.
The Volturi even crack down on him for breaking the law, but end up murdering his coworker, because they can't tell the difference either because that's the world Patrick Bateman lives in.
Or rather, that's Patrick's gift, to be utterly indistinguishable from his peers in every way. Somehow.
Patrick never escapes Wall Street and certainly never makes it into the Cullens.

What if you woke up one day and you were one of those Wammy orphans? wyd?
I imagine I am dropped out of the running for L extremely early, as most the orphans weren't actively in the competition from what we saw (if you take LABB as canon then that iteration was B and A and then later canonically we have only Near and Mello who are brought into the know/listed as serious contenders, it's unclear but implied that most the other orphans were not in the running).
If I am in the running...
I am not arrogant enough to think I can handle it when I have no evidence Light Yagami is guilty any more than anyone else does. I stay out of it and hope things go according to canon where Near managed to beat Light by the skin of his teeth.

Would Light have had any reaction to having a personal hater? Like he acts so fake there's no way there wasn't at least one person willing to talk shit to his face. He might just have a little tantrum in his head about it due to his ego but I'm really not sure if he could be pushed to being showing anger. Thoughts?
He had one in canon.
L introduces himself in person and Light has to find himself dealing on a personal level with this deeply weird person who is waiting for him to confess to be a murderer.
And honestly, he handled that quite well. Light of course was a murderer and deeply internally flustered by this, but he showed no outward reaction and was only at all interested because L had proven himself at least involved in the investigation.
The point was, if Light wasn't losing his shit then, it's not going to happen.
As it is a normal person? I imagine Light would privately think they're a little bitch and a petty loser, but would hardly consider them worth his time to throw even an tantrum (and this isn't unwarranted as you are talking about someone coming up out of nowhere to tell Light he sucks because he's so cool and talented and FAKE you are a FAKE, Light Yagami, and HAVE YOU EVER THOUGHT ABOUT THE LITTLE PEOPLE, LIGHT YAGAMI?! HAVE YOU?! I BET YOU HAVEN'T BECAUSE YOU'RE FAKE AND A LOOOOOOOOOOOSER) I imagine Light would shrug and continue on his merry way.

Can vampires chew gum? I'm very into gum lore so I thought to myself "maybe if it was unflavored chiclet because they couldn't have that flavored sugar spit concoction it normally makes" but do vampires even have spit? I need your thoughts desperately
Probably not.
The venom seems to dissolve most substances and if they could it'd taste unbearably awful.
It's telling, I think, that we don't see any vampire besides Edward put things that aren't blood in his mouth (and his eating of pizza was very much forcing himself to in order to impress his lady love knowing he'd vomit it later and his eating her tear was... also him vomiting that tear later but having really deep feelings about it.)

What would Death Note look like if Sayu was a menace to society, like a unapologetic asshole who is on a mission to terrorize the world? Obviously she'll get some pushback from her traditional family but would the story really change that much?
Not really, it's not about Sayu.
Light might be less adoring of his little sister than he is in canon and generally view her as a delinquent. (I imagine there's strife between the entire family), but Light purposefully tried to emotionally distance himself from his family. He ultimately failed, however, what this meant was he started spending less time at home and much less time with his sister.
The difference would be in whether Light would allow the notebook to be sent to Mello when Sayu is kidnapped. If he loves her less--maybe Light swaps it for a fake and if Sayu's released then 'hooray' if not oh well. If Sayu dies while abducted or else the notebook doesn't fall into Mello's hands, then Soichiro likely doesn't die as he doesn't make the eye deal and go on a raid of Mello's lair.
This in turn might mean that Light will try to find a way to more surreptitiously eliminate both Near and his own task force as he'll want a way to spare his father while protecting his identity (ultimately, Light was very reluctant to send Soichiro to his death and certainly not cold when Soichiro ended up dying believing Light was innocent after all). This means we might not get the confrontation in the warehouse or else it would go differently.
Generally though, I'm inclined to believe the plot would essentially stay the same.
