Blog #4 - Process Safety News - Cronobacter Spread at Similac Factory

In class, we discussed a number of historical events involving process safety hazards that led to disaster, but did not touch upon one important component of process safety that has been an issue for some important chemical processes: biohazard safety. In particular, bacterial contamination is an ongoing and serious problem for plants that manufacture food products, pharmaceuticals, and chemicals intended for consumption. A conspicuous recent example of this hazard is the outbreak of Cronobacter bacteria in the Abbott Laboratories plant in Sturgis, Michigan, which played a major contributing role in the national shortage of infant formula earlier this year, and left many infants sick or dead [1].

Abbott’s Sturgis plant, which manufactures Similac baby formula out of milk solids, vegetable oil, and various minerals and salts, was inspected by the FDA in September 2021, a year delayed because of Covid restrictions. (Rather than OSHA or the Chemical Safety Board, the FDA was the regulatory body tasked with performing safety inspections of the plant, as it concerns a food manufacturing production line). When Cronobacter was found, instead of shutting down the plant or recalling already sold and distributed cans of formula, the FDA chose to simply tell Abbott not to distribute the infected cans of formula, and assumed that Abbott would just fix the contamination problem on their own. Little did the FDA know, this contamination incident was likely a result of a number of ongoing failures of process safety.

According to reports published in Bloomberg Businessweek [1], the FDA inspection revealed a number of safety issues at the Sturgis plant, including employees cross-contaminating bags of ingredients with unwashed hands, cracks and holes in pipes and drying equipment that were in desperate need of repair, and pooled water in areas that should have been dry. The drying towers, which turn the liquid formula into powder, are a critical step in preventing biohazards from impacting the safety of the produced formula: by the point that it reaches the dryer, the liquid formula has undergone a number of steps including pasteurization and homogenization, but is not sterilized. If the dryer does not perform adequate sterilization of the formula, the formula is likely to be contaminated as it is packaged into cans. A whistleblower and former employee at the plant interviewed by Bloomberg claimed that managers emphasized productivity and meeting production goals over safety, and deliberately hid health violations from their superiors and from authorities.

Since the inspection in 2021, at least 100 infants have been infected by Cronobacter after consuming Abbott-branded baby formula [1]; their symptoms have ranged from vomiting and fever to life-threatening secondary infections such as salmonella and meningitis, as infant immune systems are more vulnerable to these infections than adults. At least 7 infants died. Although another inspection of the plant in January 2022 by the FDA found 5 strains of Cronobacter in the plant, and a concurrent internal inspection by Abbott found 20 strains, Abbott denies that there is a link between these infants’ illnesses and direct contamination from their plant, since the strains that infected many of the infants do not match those found in Sturgis. This burden of proof is a major issue that many regulatory agencies face when seeking to improve the safety of this manufacturing process and hold those who violate safety standards accountable for their actions. Because there are likely to be many strains of bacteria in a factory, which may go away on their own or may be obscured from direct inspection, it may not be clear where a certain strain originated. Furthermore, there may not be a homogeneity of contamination within all the particles of formula, making it difficult to prove Cronobacter contamination of a given can of formula, even when contamination is suspected.

Biosafety procedures are a critical component of process safety and risk assessment in a chemical plant that produces any products that will be consumed by humans. Similarly to examples we discussed in class, poorly maintained equipment and safety systems, as well as the lack of emergency response planning and the inadequate training of operators who will be handling critical ingredients, can all contribute to a process becoming unsafe. Because an unsafe baby formula manufacturing process can have wide-ranging consequences, from the death of babies who consume contaminated formula to indirect health risks placed on those who depend on a consistent supply of formula for their medical needs, more safety improvements and regulations will need to be made to prevent this sort of tragedy from happening in the future.
