Polly update! Polly did great for her operation! The vet who performed the surgery said it was a bit tricky, but they were able to repair the fracture with a plate. Polly will hang out at the clinic for another day or two to properly manage her pain, and to be sure she is up and standing. Clinic staff tell us she’s eating well, and she’s much more relaxed than she was when she first arrived.   Scientists and clinicians from across the globe shared insights into the therapeutic potential of cannabis in London last week. We explore some of their findings here.  The International Congress on Clinical Trials on Cannabis (CT-CANN23) took place in London on 15–16 February, inviting experts from several continents to showcase their contributions to the growing evidence base for the therapeutic use of cannabis. The event saw leading scientists, clinicians and industry professionals explore ways of improving current research strategies while discussing new discoveries and future avenues in the rapidly evolving field. Discussions over the course of the two days focused on clinical methodologies, regulatory issues and pharmacological considerations, as well as advice on the practicalities of prescribing, with support from the Medical Cannabis Clinicians Society (MCCS). Findings covered a broad spectrum of emerging areas of cannabis research from cancer and chronic pain to anxiety, PTSD and even inflammation associated with tooth loss. Cannabis researcher and Harvard University neuropsychiatrist Dr Staci Gruber shared some initial findings from the world’s first clinical trial of a full-spectrum, high-CBD product for anxiety, which she conducted as part of the Marijuana Investigation for Neuroscientific Discovery (MIND) programme.  In this open-label trial, participants were given a dose of oil containing 10mg CBD and 0.26mg THC, three times a day for four weeks. Their outcomes were measured using two standardised self-reporting questionnaires, plus the clinician-administered Hamilton Anxiety Scale (HAM-A).

McLean Hospital Cognitive and Clinical Neuroimaging Core and Marijuana Investigations for Neuroscientific Discovery (MIND), and Dept of Psychiatry at Harvard Medical School, USA The product was ‘well tolerated’ with no adverse effects or intoxication reported. Analysis revealed ‘significantly improved’ ratings of anxiety, as well as ratings of mood, sleep and quality of life. Dr Gruber also noted ‘stable or improved performance on tasks of executive function’. A double-blind placebo-controlled phase is now underway, alongside clinical trials exploring the effects of CBD on anxiety in patients with glioblastoma and in bipolar disorder. Observational data collected through the MIND programme, which Dr Gruber founded in 2014, informed the study and has paved the way for several other clinical trials, she explained. “Each type of study design has strengths and weaknesses and they generally complement each other,” Dr Gruber said. “We need real-world data, from real-world patients, taking real-world products. These data provide important insights into what specific products could be efficacious for specific conditions and indications, and should be targeted in clinical trials.”

Credit: Dr Staci Gruber, MIND Programme Cannabis in dentistry: inflammation and tooth loss Periodontitis is a severe gum infection that can lead to tooth loss as well as other serious health complications. There is currently no cure, but the disease can be managed through regular maintenance. Researchers based in the Czech Republic have been exploring whether CBD could help reduce the inflammation associated with the condition, alongside its potential benefits and drawbacks in the field of dentistry. In this placebo-controlled double-blind study, patients were split into two groups and given toothpaste and dental gel – with or without CBD – while a comparison group was treated just with a Corsodyl dental product. A statistically significant difference was observed between the first two groups, with the authors summarising: “A statistically significant improvement in gingival indices after 56 days of CBD application in patients with diagnosed periodontitis, with no adverse effects, is reported.” Brain cancer: increasing survival in glioblastoma? Researchers in Australia have been exploring the effects of medicinal cannabis oil in patients with glioblastoma multiforme (GBM) – one of the most aggressive forms of brain cancer. There is a significant need for new interventions, with only 5% of patients surviving more than five years after their diagnosis. Scientists at Southern Cross University, led by Dr Janet Schloss, carried out a double-blind RCT on a group of patients aged 18 and over. One group was given a 2.25ml daily dose of oil containing a 1:1 THC (10.35mg) to CBD (10.8mg) ratio, while patients in the second were given daily doses of 1.8ml containing 4:1 THC (27mg) to CBD (6.8mg).

Dr Janet Schloss, National Centre for Naturopathic Medicine at Southern Cross University, Australia At the end of the 12-week study period, 11% of patients had a reduction in disease, 34% were stable and 16% saw a ‘mild enhancement’ of their condition. Improvements in physical symptoms, sleep and overall ‘contentment with quality of life’ were deemed to be ‘statistically significant’. The researchers said: “We believe this study provides good evidence that medical cannabis administered to this patient population is safe and well tolerated and can provide symptomatic relief to these patients. “Our study data suggests that cannabis can be helpful for many symptoms affecting QoL … especially sleep disturbance.” In a post-trial follow-up, 47% of patients were still alive, with some having survived up to seven-and-a-half years after diagnosis. Dr Schloss and colleagues concluded: “A single nightly low dose of THC-containing medicinal cannabis was well tolerated in inoperable and recurrent GBM and AA3 patients and significantly improved sleep, functional well-being and QOL. “Medicinal cannabis may have the potential to increase survival in patients with high-grade gliomas.” Veterans: PTSD, insomnia and TBI Dr Celeste Thirwell, director of the Sleep Wake Awareness Programme (SWAP) in Canada, has treated over 6,000 veterans across the country. In her clinic she uses medicinal cannabis and a range of other non-invasive treatments to treat primarily pain, PTSD, insomnia and traumatic brain injury (TBI). A total of 22 veterans took part in a pilot study to explore the effects of cannabis on these symptoms and its ability to reduce patients’ reliance on the use of multiple pharmaceutical medications (known as polypharmacy). Patients were using an average daily dose of 6g over a period of more than a year. According to Thirwell, most started with a CBD oil and added THC flower, vapes and other forms of administration, depending on their symptoms and activity throughout the day. Results showed that after cannabis treatment, the veterans’ scores on a standardised PTSD checklist had reduced from an average of 67.9 to 34.5 (a score of 50 is considered significant for PTSD). They also reported improvements in sleep, anxiety and chronic pain. The number of daily medications that patients were taking also reduced from an average of eight to an average of three and, as a result, so did the negative side-effects of those medications, which include gastrointestinal issues, brain fog, erectile dysfunction, sedation, increased anxiety and suicide ideation. Speaking at the conference, Dr Thirwell said: “What I learned from my work with veterans is that we were able to improve their sleep, decrease their PTSD symptoms so they can function… help their TBI symptoms, improve pain management so they become more mobile… and most importantly, we help their personal and family relationships. We’re saving families with the cannabis treatment that we’re using.” Headache disorders: cluster headaches Despite some research linking migraine to a deficiency of the endocannabinoid system (ECS), according to Israeli researcher Dr Amnon Monek, to date, there is little high-quality evidence for the use of cannabis to treat this type of headache disorder. Cluster headaches, however, are a different story. Only around 1.4% of the population experience cluster headaches and they generally affect more males than females. An attack can last up to 60 minutes, and someone experiencing a flare-up may have up to eight attacks every day, with symptoms including severe pain, tearing, a runny nose and reddening of the eyes. Around 5%-10% of patients will develop chronic cluster headache, where the symptoms will affect them on a daily basis.

Dr Amnon Mosek, The Headache and Facial Pain Clinic, Sourasky Medical Center, Israel Several years ago, Dr Monek conducted a retrospective study on 18 patients with chronic cluster headaches who had failed to respond to all other medications. The average length of time they had been experiencing symptoms was nine years, with an average of up to six attacks every day. Patients sourced their own cannabis and consumed an average of 1g a day, with smoking recorded as the preferred mode of administration. According to Dr Monek, at a follow-up appointment around half of the patients had not responded to the treatment, but those who did had seen almost all of their pain reduced. CES Alpha Stim Therapy Device

The CES Alpha Stim Therapy Device is a non-invasive device that helps relieve symptoms of depression and anxiety. It works through mild electrical stimulation to calm overactive areas of the brain, such as the limbic system. In a recent study, CES applied to earlobes decreased Hamilton Anxiety Rating Scale (HAMRS) scores by 40.4%.
This therapy is approved by the Food and Drug Administration (FDA) to treat insomnia, pain, and depression. The device is available on prescription and can be administered in the comfort of your own home. Unlike traditional electroshock treatments, which are administered under general anesthesia, Alpha-Stim is relatively free of side effects.
The device is used by a mental health professional to trigger certain neurological activities. While the exact mechanism behind the device is unclear, experts believe that the current may travel to the brain in a diffuse manner. One theory suggests that alternating microcurrents may stimulate afferent branches of cranial nerves, such as the glossopharyngeal and facial nerves. Another theory says that the current may interfere with high- and low-frequency noise.
CES is different from electroconvulsive therapy in that it is intended to interrupt ongoing cortical activity. Instead of using a large electrical current, it transmits tiny microcurrents through handheld electrodes. These devices have been used by mental health professionals for years.
Various studies have shown that Alpha-Stim is effective at treating depression. Some studies have found that nine out of ten patients experience significant relief after one treatment. Other reports show that Alpha-Stim has been found to reduce the intensity of pain and headaches. There is also a report of CES calming down the amygdala, an area of the brain involved in emotional processing.
CES has been studied in monkeys and humans, and is believed to produce a calming effect on the central nervous system. Alpha EEG waves slowed after CES in the monkeys. Itil et al., published a study in the Journal of Pain and Symptom Management. They found that CES reduced the frequency of the alpha EEG wave by more than 0.5Hz.
Similarly, a Cochrane review of randomized trials of CES for chronic pain uncovered positive outcomes for CES. However, the results are insufficient to establish whether the improvements in net health outcome are clinically significant.
The primary symptom in a pilot study was a reduction in generalized anxiety disorder. In this study, participants were randomly assigned to receive a sham CES or an active CES. During the scan, participants were instructed to keep their eyes closed and not think about anything. After each scan, they completed a validated questionnaire that assessed their symptoms. Compared to the sham group, the active group did not have any changes in their symptoms.

The parlor door eased open, then tapped closed again. Footsteps padded across the carpet towards the back of the room. Hamilton remained still in his bed, his eyes closed, uninterested in his visitor. The heavy curtains over the windows swished open suddenly, their metal rings skittering against the curtain rod, and bright daylight pressed against his eyelids.
“Close them,” he said roughly, shifting his head fractionally towards his intruder.
“Sorry.” The curtains closed again. “Do you have headache, honey?”
“Eliza?” He squinted in the dim light, and could just make out his wife’s figure coming around the bed.
“When’s the last time you turned?” She began easing pillows gently beneath his back without waiting for his answer. He gritted his teeth against the spasms shooting up his spine from the movement. Slowly, with the support of four pillows, he found himself lying on his side.
She crawled over the bed when she’d finished, and laid down before him, her nose almost touching his. She smelled of fresh cut grass, of flowers and sunshine, so very alive. Smiling slightly, she said, “Hi.”
“Hi.” He placed his hand on her waist, the cotton of her dress soft and warm under his fingers. “You came back.”
Hurt flickered over her features. “Of course I came back.”
He hadn’t meant to sound so surprised; he knew her too well to truly suspect she’d abandon him. And he knew she’d felt guilty for taking time to go out to the Grange with the little ones, no matter how he’d tried to ease her conscience. He didn’t know how to tell her without hurting her that the clinical, detached hands of a stranger were often so much more bearable than her familiar, tender touch.
The more he’d come back to himself, the more humiliated he’d been by her care. For weeks, she’d dutifully fed him, checked him for bedsores, cleaned his soiled sheets, and washed him with damp rags. Whenever she’d noticed him withdrawing, she’d insisted, “I’ve seen you poorly a thousand times before. This isn’t any different.”
But it was different, he wanted to respond. So very different.  
He tried to smile. “How was the Grange?”
“The gardens are still blooming,” she said, voice lilting upwards in enticement. “You could come see them. We could pack the wagon with lots of pillows to keep you comfortable, and I’ll hold your hand the whole way. Some fresh air would do you good.”
He shook his head.
“The children miss you.”
“I miss them.” He did, terribly, but he couldn’t bear for them to see him like this, a pale shadow of himself. “But I can’t.”
She sighed, detecting the note of finality in his answer. Adjusting her head on the pillow beside him, she reached out, her fingers carding through his sheared-short hair. “This is new.”
Hosack had agreed to cut his hair for him while she’d been away. He hated how tangled, sweaty, and unkempt it had become. It’s not the shortest he’s ever worn it—he’d shaved his head entirely once when he was twenty to combat the lice plaguing his whole unit—but it’s the shortest Eliza has ever seen it.
“Seemed easier,” he said. “What do you think?”
“I like it. Very handsome,” she said, her fingers exploring the back of his head to take in the feel of the short strands.
The admiring look in her eyes made him want to roll over, kiss her, make love to her, sweet and slow. A wave of grief rushed over him at the impossibility of something that had once been so natural. He closed his eyes again.
“How are you feeling?”
He fought not to scoff at the question. His lower body lacked all sensation. The parts of his torso he could still feel felt wrong, his organs all rearranged, the indelible fingerprint of the doctors’ work inside him. His back was in a state of almost constant spasm, his muscles confused as they tried to latch on to his shattered spine. The fever had dropped below boiling some weeks ago, but persisted in an intermittent fashion, leaving him itchy, exhausted, and foul tempered. He now had all his mental faculties to experience the pain to its full intensity, just as Hosack was recommending he start easing up on the constant doses of laudanum.
“My ribs are healed,” he reported, his only good news. “You can hug me now, if you’re careful.”
Her arms immediately wound around his upper torso, squeezing him against her for a long moment. She made a contented sound in the back of her throat. “I missed this.”
“Mm,” he hummed in agreement, hugging her in return.
When she eased back, she gave him a scrutinizing look. “You’re so thin. Do you have any appetite yet?”
“No.”
“Do you not feel well?”
“No,” he said again, clinging on to the ambiguity of the double negative. He doesn’t feel nauseated, exactly, not like before. Nothing seemed to agree with him, though, and less he ate, the less he was forced to make use of the strip of cloth everyone around him was too polite to call a diaper. He greedily guarded these last scraps of his dignity.
Her nose bumped his as she leaned in to press a kiss to his lips. “Will you try to eat something? For me, please? I’ll get you anything you want. Ice cream?”
He smiled. “Maybe some soup.”
“And bread?” she pressed.
“And bread,” he agreed.
She laid there for a silent moment, her beautiful dark eyes studying his face, then she cupped his jaw gently. “Can I tell you something?”
“Anything.”
“I wanted to go the Grange. Not just for the children. For me.” Her words carry the air of a confession. “It hurts me to watch you hurting like this, knowing there’s nothing I can do to make you better. I feel so useless.”
He hugged her close again. “You do make it better. You make it bearable.”
In the dark, stale air of the parlor, a prisoner in his own body, he sometimes almost believed he was in hell. The laudanum bottle by his bedside, just within reach, had been a source of sore temptation during her two week absence: a few mouthfuls too many might bring sweet release from the torment his life had become. Only thoughts of how devastated and guilt-ridden Eliza would be kept him fighting, her constant pleas for him to stay ringing in his ears. He couldn’t bear to leave her, however much he might think it best for her to leave him.
She gave a hitching breath against him. “I’m sorry I left.”
“I understand,” he assured her, kissing her temple. “I don’t blame you for needing a break.  I feel like such a burden. We’ve gone from husband and wife to invalid and nursemaid.”
“That’s not true,” she said, pushing at his shoulder so she could look at him. “You’re still my husband.”
He grunted, skeptical.
“Alexander.” Her tone went sharp. “You’re my husband. The love of my life. We vowed to love each other through sickness and health, through good times and trying times. It is my privilege to take care of you.”
“If you say so.”
“I do.” She kissed him again, more firmly. “I love you.”
“I love you, too.”
She nodded, as though something had been decided. “I’ll go get you that soup.”
When she returned, she helped turn him onto his back and propped him up into a mostly seated position, the soup bowl balanced on a tray laid over his lap. He managed half the bowl and a full dinner roll before he had to stop, his stomach rebelling at the sudden influx of food. She replaced the tray with a basin and rubbed his back.
“Deep breaths,” she encouraged. “Try to keep it down.”
He managed to hold on to the food, the queasiness tamping down to a heavy feeling in his gut, but the hunched position he’d assumed while battling the intense nausea sent his back into painful spasms. Tears gathered in his eyes as his muscles clenched and seized, unrelenting. A moan fell past his lips.
“What do you need?” she asked. “What can I do?”
“Down,” he gasped, hands clutched into fists around the sheet beneath him.
She pulled the pillows out from behind him quickly, lowering him down flat onto the mattress. “Do you want more laudanum?”
“Yes.” It did little to help the spasms, but sometimes it made him sleep through the worst of the pain. The bitter taste of the drops filled his mouth a second later, and he swallowed gratefully. He could feel her fingers brushing over his hair.
He slept.

The curtains swished open again, pulling him out of his light doze. He blinked in the brilliant afternoon light, and winced at the flood of noise from outside as the window slid open. A cacophony of smells rushed in with the warm breeze. He brought a hand to his temple, shielding his eyes. “Ow. My head.”
“Doctor Hosack says the headache is from too much laudanum and not enough food,” Eliza said, voice cool. The good doctor must have lectured her about his supposed overuse of the pain medication while he’d been asleep.
“Whatever the reason, it still hurts.”

He stared up at her, unmoved.
“Want me to prop you up?”
“I don’t want to see him.”
“Here,” she said, lifting him gently to ease some pillows behind his back. “That’s better.”
“Eliza,” he insisted.
She ignored him. Crossing the room, she opened the door and waved Troup inside. “He’s ready for you.” Hamilton gave her a hard look, but she only smiled at him in response.
“Thank you, Mrs. H,” Troup said as he limped past her, the tap of his cane muffled on the carpet.
“I’ll leave you gentlemen to your conversation,” she said, tapping the door closed behind her.
Troup settled his bulk into the chair next to the bed where Eliza had held vigil in the early days of his injury. The lingering annoyance from his wishes being ignored faded at the sight of his old college roommate. Deep down, he knew it wasn’t really Eliza or Troup who had annoyed him in the first place, only the pain and the intermittent fever making him testy.
“You look better than the last time I saw you,” Hamilton said.

“Are you in a lot of pain?” Troup asked, eyes softening.
He nodded. “It’s better than it was at first, but….”
Troup reached out and took his hand, squeezing it twice in sympathy. “I like the haircut,” he said, trying for levity.
“Thanks,” Hamilton said, chuckling.
“I’m glad you agreed to see me. No one’s seen hide nor hair of you since you left Bayard’s. We’ve all been worried sick.”
Hamilton only smiled weakly in response. He hadn’t wanted to see anyone outside Eliza, Hosack, and the hired help who’d tended to him when Eliza wasn’t home. An audience of well-wishers would do little to improve his suffering, he’d reasoned.
“Morris has a present for you, by the way, if you feel up to having him by sometime soon. He had a chair made up for you, from the same people who made his wooden leg. He showed me when I stopped by Morrisania a week ago. You’ll love it. Very sleek and stylish.”
“A chair?”
“A wheelchair,” Troup clarified. “For when you’re feeling stronger, of course.”
He hadn’t actually much considered life outside this room without the use of his legs. Of course a wheelchair would be necessary. He still couldn’t picture himself using one. “That was…very kind of him.”
“Mm. Have you heard who’s in town?” Troup asked, jumping topics easily.
“No.” He hadn’t looked at a newspaper for nearly two months now.
“Our esteemed President.” Troup grinned.
“Jefferson? In New York? Why?”
“Well, he hasn’t exactly taken me into his confidence, but word is he’s here to see you.”
“Me?” His head reeled. Outside the misery of this little room, the world had continued spinning, it seemed. He felt off kilter at being thrust back into it with so little notice. “What does he want with me?”
“Say what you will about Jefferson, but he’s an astute politician, and visiting you is the politically expedient thing to do.”
Hamilton rubbed his fingers over his temple, trying to ease the dull throbbing. “How you do you figure that?”
“The outcome of the duel is his worst nightmare. We have all the benefit of your martyrdom without the detriment of your loss. He’s worried about the elections in November. I think he’s hoping that a visit to you here in New York can undo some of the damage. Stop the bleeding, as it were.”
He really hated Thomas Jefferson. Even the idea of having to deal with politics in his condition exhausted him. “I can’t believe him.”
“I know. And I doubt he’ll stop being a thorn in your side any time soon. Everyone loves you right now. You’ve never been such a threat to him.”
“What, does he think I’m going to challenge him for the presidency from my sickbed?”
“Do you want to? You might win.”
“No.” If he’d ever needed further proof that democratic decision-making was flawed, this was it. “That’s ridiculous. I didn’t have a snowballs chance in hell of being president two months ago. Taking a bullet to the gut hasn’t improved my qualifications for the job in any way. The Presidency isn’t supposed to be a popularity contest.”
“And yet….”
Hamilton growled low in his throat, annoyed again.
“Sorry to be the bearer of bad news.” Troup looked more amused than sorry.
He grunted. “I suppose it’s better to be prepared.”
Troup nodded.
"Have you seen Burr?" Hamilton asked. He couldn't say why--base curiosity perhaps. No one had spoken his name since that fateful day, as though he were the boogeyman, like to jump out from the shadows if his name were uttered too many times.
The question seemed to startle Troup. "What?"
"I know you're friends with him. Have you seen him, since...."
"Briefly. He was in a hurry to get away. New York and New Jersey were battling over who had the right to indict him, and for what. New Jersey opted for attempted murder." Troup sounded frustrated by the charge, but then he paused, studying him, wondering if he had said too much.
"The indictments are ludicrous," Hamilton said, trying to ease him. "If they're going to proceed, they may as well indict me as well."
"They're not going to do that. It would be political suicide," Troup said. Clearing his throat, he added, "I hate that he hurt you, Ham. You know that, right?"
"I know. I'm not asking anyone to choose between us."
Troup nodded again, and seemed to consider his next statement carefully. “Do you mind if I ask you a personal question?”
“Greatly,” Hamilton replied, deadpan. “How dare you, my dearest old friend, presume to ask me a personal question.”
Troup laughed, but sobered quickly. “How are you fixed for money?”
Hamilton sighed. “I don’t know. General Schuyler offered to deed over some land to Eliza, I think,” he said. He had only a vague memory of the conversation.
But that wouldn’t do for very long.
He hadn’t actually thought about money much, either, over the past several weeks. He would need to go back to work, sooner rather than later, he realized. At least he could still practice law from a wheelchair. The idea of it still didn’t seem real. Keeping himself isolated from the world had helped hold all these concerns at bay: money, work, politics. One visit had brought them all rushing to the forefront.
“A few of us have been talking. We’re going to take up a collection, just to help you make ends meet, until you’re back on your—” Troup stopped, catching himself at the last minute.
“Feet?” Hamilton supplied.
Troup met his eyes. “Let us do this for you, Ham. You need help, and we’re all happy to give it. I’d hate to see you and Eliza lose the Grange.”
“I’d hate that, too.” He didn’t like the idea of charity, but he wasn’t exactly in a position to refuse. He reached out for Troup’s hand again. “Thank you, Robert.”
“A lot of people love you, Ham. Myself included. Thank the good Lord that you’re still with us. I can’t imagine….” Troup’s throat sounded tight as he trailed off, and his face crumpled.
Hamilton squeezed his hand. “You don’t have to imagine. I’m right here.”
They fell into a companionable silence.
After a few minutes, Eliza poked her head in the door. “Everything going all right in here?”
“We’re fine,” Hamilton said softly.
“I should go,” Troup said. “You need your rest. I’ll come back soon, though?”
Hamilton nodded. “I’d like that.”
Eliza showed Troup out, then came back to the room, tilting her head to the side, the picture of innocence. “Did you have a nice visit?”
“You did that on purpose.”
“Did what?”
“Fresh air, sunshine, a visitor. You’re trying to bring me back to the land of the living.”
She smiled a little. “Is it working?”
“All too well.”
She crossed the room and slid the window shut once again. She left the curtains partly open, though, letting the sunshine continue to brighten the room. “I wanted to remind you that you’re still you in there. You’re not just a burden, or an invalid I’m obligated to look after. It wasn’t too overwhelming, was it?”
“No,” he assured her.
“Good.” She leaned down to adjust his pillows. “Do you want to lie back, or turn over?”
“I’ll stay sitting up for a little while longer,” he decided. The piercing twinge in his back was still there, painful as ever, but it wasn’t yet in an all-out spasm.
“Ready for more food?”
He grumbled lightly. “I already ate.”
“It is customary to eat more than once a day, sweetheart. Especially when you’re skinny as a rake. More soup?”
He considered a moment, then smiled. “How about that ice cream?” He wasn’t sure it would sit with him any better than the soup, but he was willing to try for Eliza. “I could a use treat after the news Troup brought me.”
She looked positively delighted, though she asked, “What news was that?”
“Apparently Mr. Jefferson is intending to pay us a visit.”
Her face fell. “You’re kidding.”
“I wish.”  
She sighed, then shook her head, and smiled again. “Well, I’m not happy with Mr. Jefferson, but I won’t begrudge anything that gives you an appetite. I’ll go get started on your ice cream. Vanilla?”
“Yes, please.”
She kissed him, her lips lingering lovingly against his, before slipping from the room. Alone, he sat propped against the pillows, his hand braced against the tender, aching scar on his abdomen, mind whirling. There was so much to be done.

Summary: Elizabeth Hamilton, with a wonderful career, loving husband, and a child, is satisfied in life. How does she handle it when her husband who could never be satisfied pulls the rug out from under her, destroying everything she held close to her heart?
Eliza Hamilton stayed as far away as she could from politics, making her the silent member of every family gathering, and mandatory social event that she had to attend with her husband. She kept to herself, shoving food in her mouth whenever it was expected of her to give an opinion, or excusing herself with a pretend call from work, initiated by her sister Peggy at her request. 
She has always been able to politely decline to give an opinion on her family's political doings when in the company of others. Even her social media was void of any political statement, be it her own, a retweet, or even a like. The most political she has even been in public eye was her celebrating the newly introduced healthcare bill, a tweet she drafted very carefully along with Angelica, to be free from any word that could cause controversy. 
Since childhood, she built for herself, a very distinct identity beyond her highly politically involved family. She loved that she was the black sheep in her family of politicians and those who aspired to be one. Growing up with her father in the Senate, mother in the federal court, and sisters who both aspired to be the first female president of the country, she still wished to be a doctor. 
After decades of successfully avoiding being the talk in the political world, she was finally in the spotlight on 25th August 2017, still silent, not just out of choice, but shock. The only sound in the room came from her son who had strapped his barbie to an airplane, running around her office with the toys lifted over his head. 
"I'm sorry," Peggy said, taking her hand in hers and giving it a squeeze. 
If she had been thinking clearly, she wouldn't have looked his name up on google. Headlines, all from credible news organisations, just minutes from each other, were on her screen. Every word in the article she opened looked like they were competing to see who'd tear her apart first. 
The Reynolds Affair- An Act of Political Sacrifice? 
Treasury Secretary Alexander Hamilton has released a statement, revealing shocking details of his year-long affair with Maria Reynolds in response to accusations against him for embezzling government funds. 
He was formerly accused of having colluded with James Reynolds, a convict, for seeking financial gain by appropriating the government's reserves. In an incredibly detailed response to reports of the accusation, Hamilton writes that he did have connections with Reynolds, but that his rivals had hypothesized unacceptably, the nature of his crimes. 
"My real crime is an amorous connection with his wife, Maria Reynolds, for a considerable time with his knowledge. I had frequent meetings with her, most of them at my own residence when my family was away," he writes. 
The affair began in the summer of 2016 when Hamilton says he was lured into a trap by Maria Reynolds who pretended to be a victim of domestic abuse in need of financial help. He transferred an amount of $750 to her account. 
The affair continued for weeks before Hamilton received an e-mail from her husband James, who accused him of ruining his family. He proceeded to threaten the Secretary to reveal the details of his misconduct to his wife of eleven years, Elizabeth Schuyler Hamilton and 'requested' a small payment.
 Hamilton writes, "It was only then that I knew they were both conspiring together to extort money from me. I paid a sum of $31,000 in different instances. I have a clear record that proves me innocent. I never spent a cent that wasn't mine. I have reasons for shame, but treason will never be one of them. I have revealed every bit of information that the people of this country have the right to know. Any further information is only for myself and my family to discuss." 
When asked whether it was worth revealing scandalous information regarding his personal life to free himself from speculations of treason, Hamilton called it "An Act of Political Sacrifice." 
She slammed the laptop shut, and got up swiftly, sending her chair backward to hit the wall behind her. Little Phil, startled by the noise, ceased playing to look at his mom and aunt. Eliza managed a smile to convince him and mostly herself that everything was alright, but her pretense crumbled quickly when a tear slipped through, making her pain known. 
A loud knock on the door grabbed her attention. A man in scrubs entered, fidgeting with his pen. 
"Dr. Hamilton, shall I call in the next patient?" 
"What?" She raised her eyebrow in confusion. 
"Miss Schuyler asked me to push back the appointments for an hour," he reverted his eyes to a glaring Peggy. 
She sighed, rubbing her temples. 
"Peggy, could you take Phil to mom's? Tell her I'll pick him up at 9 tops." She knew her kid enough to know he would ask too many questions she wouldn't be able to handle now. It would also serve as an excuse for her to go to her parents rather than her own place. 
"Are you sure? You could come with us," Peggy persuaded. 
"I have a job, Margarita," she said before signaling her colleague to send in the next patient. 
"Call me if you need me, alright?" Peggy picked her nephew up, answering his questions about taking him to the duck pond near Grandma Schuyler's home.
 Eliza washed her face thoroughly as though it would wipe off the shame it held, and then her hands before being seated in her chair, putting up a friendly smile for the scared kid who anticipated an injection. 
It was 10 when Eliza had no patients lefts to attend. On a normal day, she would ask Terry to stop granting appointments after 7 so that she could go home to her husband and baby. The thought of her baby gave her a heartache. She had never gone as long without calling up her mother to check on him. He must be asleep. Her mother hadn’t even called the front desk to ask when she would be back. Her phone was locked up in her table drawer as always and she didn’t have the heart to get it out when she heard a string of notifications.
In an empty building with nobody to put up a brave face for, Eliza finally broke down in a corner of her office. Her knees were drawn closer to her body, her hair a complete mess, she looked exactly the way the photographer stood by her window wanted her to.

Desperate Kiwis are queuing at hospital pain clinics because they can’t afford the dentist - and health bosses are not backing free dental care.

The position comes amidst growing concern among health workers about people who can’t afford hefty dentist’ bills and live in chronic pain as a result, with some resorting to gruesome “DIY dentistry”.
There are long queues at hospital pain clinics for temporary fillings for teeth extractions, people turning up at emergency departments because of dental pain.
Members discussed whether the health boards should call on the government to provide subsidised or free dentistry.
Waitemata DHB chief executive, Dr dale Bramley, told the meeting the Ministry of Health was “already considering how the dental service could be included in the public health service, and that the DHB boards should support”.
A resolution was passed by the committee recommending the DHBs “support comprehensive dental care access for all New Zealanders as part of the public health system”
Grant Pollard, the ministry’s group manager for population health, denied any current work on possible changes to publicly-funded dental care.
Any more to provide more subsidised or free dental care would carry a huge cost. Currently, about $198 million a year is spent on oral health services, with most covering universal services for children and teenagers.

Hamilton dentist Dr Assil Russell launched a petition for subsidy for dental care for at-need people and an increase in the age for free dental care to 20 years.
Russell’s charity Receive a Smile has treated almost 10,000 patients free of charge, including those with bad infections, and broken and rotting teeth.
Currently, Government-funded dental work for adults is limited to emergency care for pain and infection relief, and work and income grants for those on a low income and who are in serious pain and need urgent treatment. Dental care is also given to a small number of hospital inpatient and work needed after accidents are funded by ACC.
The NZ Dental Association wants more subsidised care for people on low incomes - but not universal access saying such a step would likely cost more than the annual budget of large DHBs

In New Zealand wealth determines dental health 
Inequalities in untreated tooth decay are wider in New Zealand than in Australia, Canada or the United States.
The latest NZ Health Survey run annually by the MOH reported that cost prevents an estimated 15 per cent of NZ adults (just over half million) from seeing a GP about a medical problem and nearly 7 per cent (around a quarter million) from filling a prescription. For dentistry, cost stopped 44 per cent of adults (around 1.6 million) from accessing care. In the young adult age group, this was over half the population.
Adult dental health care is a neglected health policy issue
The risk for dental problems is determined in early childhood and can be intergenerational 
Those born into disadvantaged families go on to have greater rates of tooth decay as adults.
Young adulthood is when incomes are often at their lowest but untreated dental decay is at its highest
Conventional wisdom has it that the best approach to reducing inequalities is to educate the public but that doesn't work. To address social inequalities we need to address the underlying structural issues and ensure timely access to quality dental care
In dentistry inequalities in the delivery of dental care services are greater than inequalities in the disease itself.
The government spends around NZ$16 billion annually on health and spends around a quarter of a billion dollars a year on dental care.
NZ’s dental care system demonstrates why we can’t rely on market forces to equitably distribute population health services. The cost of introducing universal coverage for dental care would be high but doesn't mean the idea should not be considered or variation of it.
Prevention - it isn't just about brushing teeth and water fluoridation. NZ needs to increase spend on preventative dental care to save on the high cost of dental interventions.
Cardiovascular disease, diabetes and obesity have many of the same causes as dental disease. Stricter limits on the marketing and sale of sugary drinks and lollies, better regulation of “hidden sugars” and continued efforts for tobacco control would have benefits beyond dental health.
Dentistry is poorly integrated with other areas of health care. Neither is it funded like other areas of health care.
Dental surgery is expensive just like any other form of surgery. Accessible dental care can’t happen without funding.
First steps towards the goal that all New Zealanders are able to access affordable dental care should be population-level prevention and ensuring that disadvantaged groups can access emergency dental care.

The Association says that countries like Australia are “miles ahead” in terms of utilising the extended skills of dental and oral health therapists.

There is no shortage of dental and oral health therapy graduates in New Zealand and if we are to retain these graduates in our country then they need to be provided with the opportunity to extend their respective scopes. In the latest, NZ Health Survey around half of the people surveyed (53%) said they only went to the dentist for problems or did not go at all. Maori, Pacific people, those on low incomes and people with mental health problems often miss out on dental care. People in rural areas without dentists also struggle. Late last year I was in a rural healthcare centre a couple of hours north of Gisborne when a young woman with severe dental pain turned up. She had managed to get an appointment with a dentist in Gisborne but had no transport so couldn’t get to the appointment
In a recent paper in the Journal of the Royal Society of New Zealand, John Broughton and his colleagues wrote about the dental health of Maori with mental health problems. They interviewed 32 people and asked about their oral health and its impact on their lives, using a standard questionnaire that has been used in many studies. People reported that their oral health was causing them psychological and physical pain and disability and affecting their social interactions.
…. Not surprisingly, getting all this necessary dental work done had a big impact on participants. They experienced lower levels of pain, discomfort, and disability, and it is likely that the treatment led to increased mental as well as physical health.
The study makes clear that there is a very large burden of serious untreated dental problems in the community, and suggests that people with mental health problems should be a priority group for receiving dental care. The authors point out that Maori ways of thinking about health stress the connections between physical, mental, family and spiritual health. In this case, it is really clear that poor physical health, like not having many (or any) teeth, or having pain or infection from bad teeth, are likely to lead to other health and social problems.

Rates of tooth decay in New Zealand far exceed rates overseas
Northland's lack of fluoridation in its water supply and New Zealand's "minimal" regulation around the availability and promotion of sugary foods and drinks also played a role, they said. 
President of the New Zealand Dental Association, Bill O'Connor said Northland and Auckland's dental services were "overwhelmed".
The findings reinforced the association's position particularly around the role sugary drinks and water fluoridation played in oral health, he said

Revive A Smile is a New Zealand based dental charity established in 2011 by Dr Assil Russeull and run by a team of dental health professionals from all over Aotearoa.
Revive a Smile aims to reduce oral health disparities experienced by disadvantaged and impoverished Kiwis. 
The project works by providing much needed free dental treatment education and oral health products (such as health packs, including toothbrushes, floss and mouth wash.
Revive a Smile provides dental care and various members and groups of the community including homeless persons, victims of domestic violence, refugee migrants, low-income adults and the elderly. It aims to empower kiwis to take control of their oral health by treating toothache and developing personal skills, enabling people to become, healthy, contributing members of society.
Founded on Dr Russell’s philosophy that “caring creates change”

For today’s poster I used the word I got from class: chicken for my poster. I wanted to try hand drawing it as opposed to doing it digitally as I really enjoyed it from Monday’s class the quick sketching of ideas. I played cut the pictures out then played around trying out different compositions before scanning it and editing it in photoshop. After doing the reading through my poster I wanted to show the key idea of “perspective being an asset”. In my poster I show two chicken one that is white the other that is of colour, the white chicken is able is free ranged and lives a life outside of a cage and is able to receive food. In comparison the chicken who is coloured is a chicken that is caged can only watch as the other eats.

Available for adoption: Betsy! AGE AT ARRIVAL: Approximately one year ARRIVAL DATE: May 25th, 2020 GENDER: Spayed Female ADOPTION FEE: $150 NOTES: Betsy was found as a stray with a broken leg. Hamilton Animal Services was called and they took Betsy to their vet where her leg was wrapped up and she was put on pain medication. After Betsy’s stray time was done and no owner came forward to claim her, HAS reached out to us for help. We transported Betsy to see Dr. O’Brien at Village Cat Clinic, and the leg was given a thorough exam. The break was so severe, our options were; try to repair it with several expensive surgeries, or, amputate the leg. There was no guarantee the surgeries would be successful, and this extensive treatment would be a lot for this poor girl to go through for the possibility that it won’t work. After much discussion with Dr. O’Brien, it was decided the best thing for Betsy would be to remove the leg. Betsy had the leg removal and spay surgery a few days after her rescue, and everything went great. Big thank you to Dr. Kornya and all the staff at The Cat Clinic for taking such great care of Betsy. She has now recovered in her foster home, after a few more vet visits when she managed to get some sutures out despite her cone! Betsy is still as happy and purry as she was before. She’s a sweet, sweet kitty and now that she has the go-ahead from her veterinary team, Betsy is ready to find her forever home! Here’s a note from Betsy’s foster family: “Betsy is such a sweet little soul! She’s friendly, gentle, affectionate and was an absolute champ getting through her leg amputation and weeks of cone-wearing. She loves pets, loves to play and roll around and purr. She’s fast on three legs and still figuring out stairs and jumping up onto things. She’s been hanging out with our kitty for the past week or so and they play together as well so it looks like she wouldn’t mind having kitty friends. Fixing leaking roof- my dad and shifu Gun were helping me to fixed clinic roof. I learned a lot yesterday. We had a quotation of 4000 dollars for doing the same work. It cost me less than $250 dollar to fixed it. There are a lot of things we could do it ourselves. That is why I keep doing live video to show you guys how to do some acupressure points to manage pain at home.
